Consumo de la comunicación publicitaria de la marca Domestika en YouTube durante la pandemia

El presente artículo, propone un análisis de los componentes que constituyen la decisión de compra en consumidores de 20 a 30 años a través de contenidos publicitarios pre-roll en la plataforma de YouTube, enlazados a la empresa Domestika, con el objetivo de establecer un conocimiento centrado en los consumidores del servicio digital mencionado. De la misma forma, se estudia la situación en el estado temporal de pandemia, determinando los efectos y cambios desarrollados a nivel educacional en el mundo frente a esta realidad, y la aparición del E-Learning como nueva forma necesaria de aprendizaje. Por otra parte, se destacan las distintas posibilidades que ofrece YouTube, como medio digital para los contenidos publicitarios de las marcas. En adición, se resalta dentro del eje de investigación, la exploración de la importancia esencial que representan las estrategias digitales comerciales, para el desarrollo favorable de las marcas. En relación con todo lo expuesto con anterioridad, se ha llegado a efectuar una metodología a través de la investigación de distintos artículos y fuentes relevantes, en los cuales se ha basado el presente trabajo. / This article proposes an analysis of the components that constitute the purchase decision in consumers aged 20 to 30 years through pre-roll advertising content on the YouTube platform, linked to the Domestika company, with the aim of establishing knowledge focused on the consumers of the digital service. In the same way, the situation in the temporary state of pandemic is studied, determining the effects and changes developed at the educational level in the world in the face of this reality, and the appearance of E-Learning as a new necessary form of learning. On the other hand, the different possibilities offered by YouTube stand out as a digital medium for brand advertising content. In addition, the exploration of the essential importance that commercial digital strategies represent for the favorable development of brands is highlighted within the research axis. Telehealth for Contraceptive Care During the Initial Months of the COVID-19 Pandemic at Local Health Departments in 2 US States: A Mixed-Methods Approach

OBJECTIVES: This study examined implementation of telehealth for contraceptive care among health departments (HDs) in 2 Southern US states with centralized/largely centralized governance structures during the early phase of the COVID-19 pandemic. Sustaining access to contraceptive care for underserved communities during public health emergencies is critical. Identifying facilitators and barriers to adaptive service provision helps inform state-level decision making and has implications for public health policy and practice, particularly in states with centralized HD governance. DESIGN: Mixed-methods study including a survey of HD clinic administrators and key informant interviews with clinic- and system-level staff in 2 states conducted in 2020. SETTING: Health department clinics in 2 Southern US states. PARTICIPANTS: Clinic administrators (survey) and clinic- and system-level respondents (key informant interviews). Participation in the research was voluntary and de-identified. MAIN OUTCOME MEASURES: (1) Telehealth implementation for contraceptive care assessed by survey and measured by the percentage of clinics reporting telehealth service provision during the pandemic; and (2) facilitators and barriers to telehealth implementation for contraceptive care assessed by key informant interviews. For survey data, bivariate differences between the states in telehealth implementation for contraceptive care were assessed using χ2 and Fisher exact tests. Interview transcripts were coded, with emphasis on interrater reliability and consensus coding, and analyzed for emerging themes. RESULTS: A majority of HD clinics in both states (60% in state 1 and 81% in state 2) reported a decrease in contraceptive care patient volume during March-June 2020 compared with the average volume in 2019. More HD clinics in state 1 than in state 2 implemented telehealth for contraceptive services, including contraceptive counseling, initial and refill hormonal contraception, emergency contraception and sexually transmitted infection care, and reported facilitators of telehealth. Medicaid reimbursement was a predominant facilitator of telehealth, whereas lack of implementation policies and procedures and reduced staffing capacity were predominant barriers. Electronic infrastructure and technology also played a role. CONCLUSIONS: Implementation of telehealth for contraceptive services varied between state HD agencies in the early phase of the pandemic. Medicaid reimbursement policy and directives from HD agency leadership are key to telehealth service provision among HDs in centralized states.

Les virus respiratoires induisent de nombreuses et fréquentes pathologies en pédiatrie avec une morbidité importante. Ces virus sont bien connus car étudiés depuis longtemps mais ils sont en évolution constante. L’apparition des antibiotiques, des antiviraux, des soins intensifs, de la vaccination, les connaissances sur l’hygiène ont transformé l’impact de ces virus sur les populations humaines. Nos modes de vie principalement citadins favorisent la diffusion virale locale par le regroupement de presque tous les enfants dans des crèches ou des écoles. De même l’explosion récente des moyens de transport en particulier aérien qui ont reliés physiquement tous les pays du monde entre eux et qui accentuent la diffusion virale mondiale. Faut-il appliquer les mêmes raisonnements dans tous les pays du monde pour lutter contre ces infections ? Cette thèse apporte de nouvelles connaissances sur ce sujet en se focalisant sur les virus influenza et le virus respiratoire syncytial :1) Pendant la pandémie H1N1 au Canada, la mise en évidence de l’augmentation des hospitalisations des enfants asthmatiques, paradoxalement moins souvent ventilés durant leurs séjours en réanimation pédiatrique.2) L’intérêt de la vaccination contre le virus pandémique H1N1 pour diminuer le recours à la ventilation assistée chez les enfants hospitalisés en réanimation pédiatrique.3) Le Canada n’a pas connu de 3ème vague pandémique en raison d’une campagne vaccinale massive qui bien que tardive, a été efficace même 1 an plus tard.(4) La saisonnalité de la bronchiolite dans les régions tropicales est très différente de celle retrouvée dans les pays tempérés et nécessite de réajuster les recommandations de prise en charge à l’aune des données localesEn décrivant la cinétique et l’impact de la pandémie grippale de 2009 sur les enfants hospitalisés en soins intensifs pédiatriques au Canada d’Octobre 2009 à Mars 2011, en comparant cette cohorte à une cohorte similaire en France Hexagonale, et en décrivant 2 épidémies de VRS en Martinique en 2007 et 2008 pour confronter ces données avec celles décrites dans les pays tempérés, ce travail illustre combien nos connaissances doivent toujours être remises en question du fait de l’évolution du climat, de l’évolution de la société et de l’évolution des connaissances médicales / Respiratory viruses are responsible for much pediatric pathology with significant morbidity. These viruses are well known for a long time but are subject to constant changes. The development of antibiotics, antivirals, intensive care, vaccination, knowledge on hygiene has modified the impact of these viruses on human populations. Our predominantly urban lifestyles support local viral spread by bringing almost all children together in nurseries or schools. In the same way, the large use of modern transport facilities especially air transport (which connect all continents between them) facilitate the world viral spread. In this new environment, should we apply the same medical reasoning all over the world to fight against these infections? This thesis brings new knowledge on this topic, focusing on influenza virus and syncytial respiratory virus:1) During Influenza A(H1N1)pdm09, hospitalizations of children with asthma increase, but they were least often to be ventilated during their pediatric intensive care stay.2) The value of vaccination against the pandemic virus to reduce the use of assisted ventilation in children hospitalized in Pediatric Intensive Care Unit.3) Canada did not experience a third pandemic wave in 2010 because of a massive vaccination campaign that, even late, was effective one year later.4) The bronchiolitis season in the tropics is different from what we know in temperate countries. It is mandatory to adjust management recommendations in the light of regional data.By describing the kinetics and impact of the 2009 influenza pandemic on children in pediatric intensive care in Canada from October 2009 to March 2011, comparing this cohort to a similar cohort in France, and comparing 2 epidemics of VRS in Martinique (French West Indies) in 2007 and 2008 with those that have been carried out in temperate countries, this thesis is an example why we have to constantly question our knowledge because of climate change, change in society and medical knowledge evolution.

MÉTODOS: Cento e vinte pacientes (AR, n=41; EA, n=57 e artrite psoriática - AP, n=22) em uso de agentes anti-TNF (monoclonal, n=94 e receptor solúvel, n=26) foram comparados com 116 pacientes com artrite inflamatórias em uso de DMARDs e 117 controles saudáveis. Soroproteção (SP), soroconversão (SC), médias geométricas dos títulos (MGTs), fator de aumento (FI) das MGT e eventos adversos foram avaliados 21 dias após a vacinação. RESULTADOS: Após a imunização, as taxas de SC (58,2% vs 74,3%, p=0,017) foram significativamente menores nos pacientes com espondiloartrites que receberam a terapia anti-TNF, enquanto nenhuma diferença foi observada em pacientes com AR que recebem esta terapia, em comparação com controles saudáveis (p=0,067). Pacientes com espondiloartrites que receberam anticorpos monoclonais (infliximabe/adalimumabe) tiveram uma taxa de SC significativamente menor em comparação com controles saudáveis (51,6% vs. 74,3%, p=0,002) ou para aqueles em uso de DMARDs (51,6% vs. 74,7%, p=0,005), por sua vez não houve diferença para pacientes em uso de etanercepte (86,7% vs. 74,3%, p=0,091). Uma análise dos pacientes com espondiloartrites que apresentaram SC e os que não apresentaram SC revelou que o primeiro grupo teve maior média de idade (p=0,003), maior frequência de anti-TNF (p=0,031) e anticorpos monoclonais (p=0,001), e uma menor frequência de metotrexate (p=0,028). Na regressão logística multivariada, apenas a idade avançada (p=0,015) e tratamento anticorpos monoclonais (p=0,023) permaneceram fatores importantes para a não SC em pacientes com espondiloartrites. Seroprotection (SP), seroconversion (SC), geometric mean titre (GMT), factor increase (FI) in GMT and adverse events were evaluated 21 days after vaccination. RESULTS: After immunisation, SC rates (58.2% vs. 74.3%, p=0.017) were significantly lower in spondyloarthritis patients receiving anti-TNF therapy, whereas no difference was observed in RA patients receiving this therapy compared to healthy controls (p=0.067). Spondyloarthritis patients receiving monoclonal antibodies (infliximab/adalimumab) had a significantly lower seroconversion rate compared to healthy controls (51.6% vs. 74.3%, p=0.002) or to those under DMARDs (51.6% vs. 74.7%, p=0.005), whereas no difference was observed for patients under etanercept (86.7% vs. 74.3%, p=0.091). In multivariate logistic regression, only older age (p=0.015) and monoclonal antibodies treatment (p=0.023) remained significant factors for nonseroconversion in spondyloarthritis patients. CONCLUSIONS: This study revealed a distinct disease pattern of immune response to the pandemic influenza vaccine in inflammatory arthritis patients receiving anti-TNF agents, illustrated by a reduced immunogenicity solely in spondyloarthritis patients using monoclonal antibodies

A Framework for Analyzing and Optimizing Regional Bio-Emergency Response Plans

The presence of naturally occurring and man-made public health threats necessitate the design and implementation of mitigation strategies, such that adequate response is provided in a timely manner. Since multiple variables, such as geographic properties, resource constraints, and government mandated time-frames must be accounted for, computational methods provide the necessary tools to develop contingency response plans while respecting underlying data and assumptions. A typical response scenario involves the placement of points of dispensing (PODs) in the affected geographic region to supply vaccines or medications to the general public. Computational tools aid in the analysis of such response plans, as well as in the strategic placement of PODs, such that feasible response scenarios can be developed. Due to the sensitivity of bio-emergency response plans, geographic information, such as POD locations, must be kept confidential. The generation of synthetic geographic regions allows for the development of emergency response plans on non-sensitive data, as well as for the study of the effects of single geographic parameters. Further, synthetic representations of geographic regions allow for results to be published and evaluated by the scientific community. This dissertation presents methodology for the analysis of bio-emergency response plans, methods for plan optimization, as well as methodology for the generation of synthetic geographic regions.

Computational Methods to Optimize High-Consequence Variants of the Vehicle Routing Problem for Relief Networks in Humanitarian Logistics

Optimization of relief networks in humanitarian logistics often exemplifies the need for solutions that are feasible given a hard constraint on time. For instance, the distribution of medical countermeasures immediately following a biological disaster event must be completed within a short time-frame. When these supplies are not distributed within the maximum time allowed, the severity of the disaster is quickly exacerbated. Therefore emergency response plans that fail to facilitate the transportation of these supplies in the time allowed are simply not acceptable. As a result, all optimization solutions that fail to satisfy this criterion would be deemed infeasible. This creates a conflict with the priority optimization objective in most variants of the generic vehicle routing problem (VRP). Instead of efficiently maximizing usage of vehicle resources available to construct a feasible solution, these variants ordinarily prioritize the construction of a minimum cost set of vehicle routes. Research presented in this dissertation focuses on the design and analysis of efficient computational methods for optimizing high-consequence variants of the VRP for relief networks. The conflict between prioritizing the minimization of the number of vehicles required or the minimization of total travel time is demonstrated. The optimization of the time and capacity constraints in the context of minimizing the required vehicles are independently examined. An efficient meta-heuristic algorithm based on a continuous spatial partitioning scheme is presented for constructing a minimized set of vehicle routes in practical instances of the VRP that include critically high-cost penalties. Multiple optimization priority strategies that extend this algorithm are examined and compared in a large-scale bio-emergency case study. The algorithms designed from this research are implemented and integrated into an existing computational framework that is currently used by public health officials. These computational tools enhance an emergency response planner's ability to derive a set of vehicle routes specifically optimized for the delivery of resources to dispensing facilities in the event of a bio-emergency.

The Time Before a Crisis : An evaluation of Region Östergötland’s pre-crisis phase before the COVID-19 pandemic

During the spring of 2020, COVID-19 rapidly started to spread within Sweden, causing a crisis that affected the entire society, and in particular, the health care sector. What makes the COVID-19 pandemic a unique crisis is that it did not appear as a complete surprise when it reached Sweden. Partly because pandemics are expected to happen and therefore anticipated, partly because reports of COVID-19 and its effects were available before the disease caused a crisis in Sweden. This makes the phase before the crisis, known as the pre-crisis phase, quite long. For this study, it is argued that the pre-crisis phase encompasses actions taken before COVID-19 was known, predominantly in the form of the development of pandemic preparedness plans, as well as the actions taken when COVID-19 was known before it reached Sweden. However, due to the decentralisation of the Swedish political system, the twenty-one regions being responsible for the health care sector, and the uneven geographical transmission of the virus, the pre-crisis phase most likely differed across the country. In this study, it is the pre-crisis phase of Region Östergötland which has been examined and evaluated as a part of the Center for Disaster Medicine and Traumatology’s general evaluation of the COVID-19 pandemic in Östergötland. The study has aimed to answer the research questions related to how the pre-crisis phase was conducted, and how three different theories can contribute to the understanding of the pre-crisis phase. The theories used were Conflicting Objectives Perspective, High Reliability Organisation, and Resilience Engineering. To answer the research questions, semi-structured interviews with eight employees of Region Östergötland involved in the pre-crisis phase were conducted. The interviews were analysed using the thematic analysis approach. The thematic analysis showed that the pre-crisis phase was mainly characterised by organisational changes within the regular organisational structures, different challenges predominately related to acquiring personal protective equipment, a lack of information from the national agencies, and a slight underestimation. These themes were used to answer the first research question. To answer the second research question, the aforementioned theories were applied to the themes. This application showed that there are aspects of the three theories that can contribute to the understanding of the pre-crisis phase. These aspects can also be used during future pre-crisis phases. Furthermore, based on the contributions of the three theories and the thematic analysis, a new model was developed which can be used to describe the different phases of the pre-crisis phase. The application of the theories and the new model resulted in general guidelines highlighting the important processes during a pre-crisis phase, including the processes of monitoring, anticipating, planning, and preparing.

Evidence and Implications of the Affordable Care Act for Racial/Ethnic Disparities in Diabetes Health During and Beyond the Pandemic

Amid the global pandemic, it becomes more apparent that diabetes is a pressing health concern because racial/ethnic minorities with underlying diabetes conditions have been disproportionately affected. The study proposes a multiyear examination to document the role of the Affordable Care Act (ACA) in racial/ethnic disparities in diabetes health. Using the Behavioral Risk Factor Surveillance System from 2011 to 2019, the study with a pre-post design investigated changes in access to care and diabetes health among non-White minorities compared with Whites before and after the ACA by conducting multivariable linear regression, with state-fixed effects and robust standard errors. Compared with Whites, racial/ethnic minorities showed significant improvements in health insurance coverage, having a personal doctor, and not seeing a doctor because of cost. Blacks (3.2% points,  ≤ 0.000), Hispanics (1.6% points,  = 0.001), and "other" racial/ethnic group (1.5% points,  = 0.003) experienced a greater increase in diagnosed prediabetes than Whites, whereas no and small differences were found in diagnosed diabetes and obesity, respectively. The yearly comparisons of changes in diagnosed prediabetes showed that Blacks compared with Whites had a growing increase from 1.2% points ( = 0.001) in 2014 to 3.3% points ( = 0.001) in 2019. Insurance coverage has declined after 2016, and obesity had an increasing trend across race/ethnicity. The ACA had a positive role in improving access to care and identifying those at risk for diabetes to a larger extent among racial/ethnic minorities. However, the policy impacts have been diminishing in recent years. Continued efforts are needed for sustained policy effects.
