Furthermore, delivery may be associated with a decrease in pelvic floor muscle (PFM) strength. Objectives: to compare PFM strength in women who delivered vaginally or by cesarean section and nulliparae, investigate the factors associated with PFM strength and demonstrate a correlation between measurements of PFM strength obtained by vaginal digital examination and by perineometer. Methods: a cross-sectional study was conducted, including 31 women following vaginal delivery, 30 following cesarean section and 30 nulliparae. PFM strength was investigated by vaginal digital examination (modified Oxford grading system) and by perineometer. Multiple linear regression analysis with adjustment for covariables was used to compare the mean PFM strength and identify factors associated with PFM strength. The Kendall test was applied to investigate the correlation between vaginal digital examination and a perineometer. Results: the mean age of the participants who delivered vaginally, underwent cesarean section and those who were nulliparous was 32.3±5.8 years, 30.5±5.4 years and 27.2±5.9 years (p<0.01), respectively. The mean PFM strength of women undergoing vaginal delivery and cesarean section was 25.6±14.5cmH2O and 39.6±22.0cmH2O (p<0.01, adjusted for covariables), respectively. A correlation between measurements of PFM strength obtained by vaginal digital examination and perineometer device was observed (tau=0.82; p<0.01). Non-white race/ethnicity was negatively associated with PFM strength (coefficient: -10.2424; p=0.02). Conclusions: women with a history of vaginal delivery had a lower PFM strength than those delivering by cesarean section. Non-white race/ethnicity negatively affected PFM strength. Our data suggest that vaginal digital examination may be applied in clinical practice because of its expressive correlation with a perineometer. / O parto pode determinar modificações do assoalho pélvico, que podem ocasionar disfunções tais como incontinência urinária e fecal e prolapso de órgãos pélvicos. Além disso, o parto pode associar-se à diminuição da força muscular perineal (FMP). Objetivos: comparar a FMP em mulheres submetidas ao parto vaginal, à cesárea e nulíparas, investigar os fatores associados à FMP e verificar a correlação entre as medidas da FMP obtidas por meio do exame vaginal digital e pelo perineômetro. Métodos: conduziu-se estudo de corte transversal, incluindo 31 mulheres pós-parto vaginal, 30 pós-cesárea e 30 nulíparas. A FMP foi investigada por meio do exame vaginal digital (escala modificada de Oxford) e pelo perineômetro. Utilizou-se a análise de regressão linear múltipla com ajuste por covariáveis para comparar a média da FMP e identificar os fatores associados à FMP. Aplicou-se o teste de Kendall para investigar a correlação entre o exame vaginal digital e o perineômetro. Resultados: a média de idade das participantes submetidas ao parto vaginal, à cesárea e das nulíparas foi de 32,3±5,8anos, 30,5±5,4 anos e 27,2±5,9 anos (p<0,01), respectivamente. A média da FMP de mulheres submetidas ao parto vaginal e à cesárea foi de 25,6±14,5cmH2O e de 39,6±22,0cmH2O (p<0,01, ajustado por covariáveis), respectivamente. Verificou-se correlação entre as medidas da FMP obtidas por meio do exame vaginal digital e pelo perineômetro (tau=0,82; p<0,01). Conclusões: mulheres com antecedente de parto vaginal apresentaram menor FMP quando comparada àquelas submetidas à cesárea. Raça/etnia não branca afetou negativamente a força muscular do assoalho pélvico. Nossos dados sugerem que o exame vaginal digital pode ser aplicado na prática clínica, uma vez que apresentou expressiva correlação com o perineômetro.

Métodos: Foi realizado um ensaio clínico prospectivo, randomizado e cego, composto por trinta mulheres com EM e STUI alocadas aleatoriamente em um dos três grupos e foram tratadas durante 12 semanas: Grupo I: treinamento dos músculos do assoalho pélvico (MAP) com biofeedback eletromiográfico (EMG) e eletroestimulação placebo (GI, n= 10), Grupo II: treinamento dos MAP com biofeedback EMG e eletroestimulação intravaginal (GII, n= 10), Grupo III: treinamento dos MAP com biofeedback EMG e eletroestimulação transcutânea do nervo tibial (GIII, n= 10). As avaliações, realizadas antes e após o tratamento, foram: teste do absorvente de 24 horas, diário miccional de três dias, função dos MAP de acordo com o esquema PERFECT, tônus e habilidade de relaxamento dos MAP, flexibilidade da abertura vaginal e estudo urodinâmico que avaliou a capacidade cistométrica máxima, complacência da bexiga, amplitude máxima da hiperatividade detrusora, fluxo máximo (Qmax), pressão do detrusor no Qmax e volume residual pós-miccional. Foram utilizados os questionários: OAB-V8, ICIQ-SF, Qualiveen e, o questionário de sexualidade, FSFI. Resultados: Após o tratamento, todos os grupos apresentaram redução no peso dos absorventes e redução dos episódios de urgência e urge incontinência. Apresentaram melhora em todos os domínios do esquema PERFECT, diminuição da pontuação dos questionários OAB-V8, ICIQ-SF e aumento na pontuação dos domínios excitação, lubrificação vaginal, satisfação no escore total do questionário FSFI. GII melhorou significantemente quando comparado à GI e GIII em relação ao tônus, flexibilidade e capacidade de relaxamento dos MAP e na pontuação do questionário OAB-V8. Conclusão: Os resultados sugerem que o treinamento dos MAP isoladamente ou em combinação com a eletroestimulação transcutânea do nervo tibial ou eletroestimulação intravaginal é eficaz no tratamento de STUI e função sexual de mulheres com EM, sendo que a combinação do treinamento dos MAP com a eletroestimulação intravaginal oferece vantagens adicionais na redução do tônus muscular e STUI / Abstract: Objectives: The aim of this study is to compare two methods of electrotherapy, most used in clinical practice for the treatment of lower urinary tract symptoms (LUTS), the transcutaneous tibial nerve electrostimulation and intravaginal electrostimulation in women with multiple sclerosis (MS) and its influence on sexual function and Quality of Life (QOL) of these women. Methods: A prospective, randomized, blinded clinical trial was carried out. Thirty women with MS and LUTS were randomly allocated into one of three groups and received treatment for 12 weeks: Group I: pelvic floor muscle training (PFMT) with electromyographic (EMG) biofeedback and sham electrostimulation (GI, n=10), Group II: PFMT with EMG biofeedback and intravaginal electrostimulation (GII, n=10), Group III: PFMT with EMG biofeedback and transcutaneous tibial nerve stimulation (GIII, n=10). Assessments, performed before and after the treatment, included: 24 hours pad test, three days bladder diary, PFM functioning according to the PERFECT scheme, tone and ability to relax PFM, flexibility of vaginal opening and maximum cystometric capacity, bladder compliance, maximum amplitude of detrusor overactivity, maximum flow rate (Qmax), detrusor pressure at Qmax and post-void residual volume outcomes of urodynamic study. The questionnaires included: OAB-V8, ICIQ-SF, Qualiveen and, the questionnaire of sexuality, FSFI. Results: After treatment, all groups showed a reduction in pad weight and reduced episodes of urgency and urge incontinence. They also showed improvements in all domains of the PERFECT scheme, decreased scores of OAB-V8 and ICIQ-SF questionnaires and increased scores of arousal, vaginal lubrication, satisfaction and total score domains of FSFI questionnaire. GII was significantly improved when compared to GI and GIII related to tone, flexibility and relaxation of PFM and, also, in the score of OAB-V8 questionnaire. Conclusion: The results suggest that PFMT alone or in combination with intravaginal electrostimulation or transcutaneous tibial nerve electrostimulation is effective in the treatment of LUTS and sexual function in MS patients, with the combination of PFMT and intravaginal electrostimulation offering some advantage in the reduction of muscle tone and LUTS / Doutorado / Fisiopatologia Cirúrgica / Doutora em Ciências

Impact d’un programme d’exercices des muscles du plancher pelvien sur les signes et symptômes du syndrome génito-urinaire de la ménopause chez la femme post-ménopausée

Le syndrome génito-urinaire de la ménopause (SGUM) engendre des symptômes importants et des impacts considérables sur la qualité de vie et la fonction sexuelle des femmes post-ménopausées. Les traitements actuels sont efficaces pour soulager un grand nombre de femmes. Toutefois, dans certains cas, ils ne pas soulagent pas tous les symptômes, peuvent entraîner des effets indésirables et être contre-indiqués. Il apparaît alors nécessaire d’identifier des traitements alternatifs ou complémentaires pour le SGUM. Ce projet avait pour but d’approfondir les connaissances sur l’utilisation d’un programme d’exercices des muscles du plancher pelvien (EMPP) pour le traitement du SGUM chez des femmes post-ménopausées. Les objectifs principaux étaient: 1) évaluer les qualités psychométriques d’outils de mesure nécessaires à l’étude du SGUM, 2) étudier la faisabilité et les effets d’un programme d’EMPP auprès de femmes post-ménopausées atteintes du SGUM et 3) investiguer les mécanismes d’action potentiels des EMPP sur le SGUM. Trois projets distincts ont été élaborés dans le cadre de cette thèse. Le premier a permis d’explorer les effets des EMPP sur le SGUM au moyen de l’étude du cas d’une femme post-ménopausée. Suite à l’intervention, les signes et symptômes du SGUM du cas à l’étude se sont améliorés, en plus de leurs impacts sur la qualité de vie et la fonction sexuelle. À la lumière de ces résultats, des hypothèses sur les mécanismes d’action responsables des effets positifs ont été émises, dont une était l’amélioration de la vascularisation vulvovaginale. Le deuxième projet visait donc à évaluer la fidélité de la mesure de la vascularisation d’artères liées aux tissus vulvovaginaux, soit les artères pudendale interne et dorsale du clitoris, à l’aide de l’échographie Doppler. Les résultats recueillis auprès de 20 femmes sans condition urogynécologique ont appuyé la fidélité test-retest de ces mesures. Pour le troisième projet, 32 femmes post-ménopausées atteintes du SGUM ont participé à deux évaluations pré-intervention, un programme d’EMPP de 12 semaines et une évaluation post-intervention. Les évaluations pré-intervention ont permis de confirmer les qualités psychométriques d’outils de mesure du SGUM (Most Bothersome Symptom Approach et Atrophy Symptom questionnaire) en plus de confirmer la fidélité test-retest de la mesure de la vascularisation des artères pudendale interne et dorsale du clitoris à l’aide de l’échographie Doppler chez cette population. Suite à l’intervention, les résultats supportaient la faisabilité d’un programme d’EMPP auprès de femmes post-ménopausées atteintes du SGUM, de par des taux élevés de ii participation à l’intervention et de complétion de l’étude. Les résultats supportaient également l’effet positif de cette intervention en démontrant une diminution significative de la sévérité des symptômes du SGUM, de certains signes du SGUM et de leurs impacts sur leur qualité de vie et leur fonction sexuelle. Les résultats de cette étude suggèrent également l’augmentation de la vascularisation vulvovaginale, l’amélioration de la capacité de relâchement des MPP et l’augmentation de l’élasticité des tissus vulvovaginaux comme mécanismes d’actions potentiels d’un programme d’EMPP chez les femmes post-ménopausées atteintes du SGUM. Des études ultérieures devront confirmer nos résultats. / Genitourinary syndrome of menopause (GSM) causes significant symptoms and considerable impacts on the quality of life and sexual function of postmenopausal women. Current treatments are effective in alleviating the symptoms of many women. However, in some cases, they do not relieve all symptoms, and may cause side effects and be contraindicated. In these cases, it is necessary to identify alternative or complimentary treatments for GSM. The purpose of this project was to increase knowledge on the use of a pelvic floor muscle exercise (PFME) program for the treatment of GSM in postmenopausal women. The main objectives were to: 1) evaluate the psychometric qualities of the measurement tools needed to study GSM, 2) study the feasibility and effects of a PFME program in postmenopausal women with GSM, and 3) investigate the potential mechanisms of action of PFME on GSM. Three separate projects were developed for this thesis. The first project was a case study involving a postmenopausal woman, which provided an exploration of the effects of PPME on GSM. In this case study, the woman’s signs and symptoms of GSM improved, as well as her quality of life and sexual function. Considering these results, hypotheses were proposed on the mechanisms of action responsible for the positive effects of PPME on GMS, one of which was the improvement of vulvovaginal vascularization. The second project aimed to assess the accuracy of the measurement of the vascularization of arteries related to vulvovaginal tissues, namely the internal pudendal and dorsal arteries of the clitoris, using Doppler ultrasound. Results from 20 women without urogynecological conditions supported the test-retest reliability of these measurements. For the third project, 32 postmenopausal women with GSM participated in two pre-intervention assessments, a 12-week PFME program, and a post-intervention assessment. The pre-intervention assessments confirmed the psychometric qualities of GSM measurement tools (the Most Bothersome Symptom Approach and the Atrophy Symptom Questionnaire) and the test-retest reliability of the measurement of the vascularization of the internal pudendal and dorsal clitoral artery in this population using Doppler ultrasound. Following the intervention, the results supported the feasibility of a PFME program in postmenopausal women with GSM, due to high rates of participation in the intervention and completion of the study. The results also supported the positive effect of this intervention by demonstrating a significant reduction in the severity of GSM symptoms, certain signs of GSM, and their impact on their quality of life and sexual function. Finally, the results of this study suggest an increase in vulvovaginal vascularization, an improvement in the ability to relax the pelvic floor muscles and an increase in the elasticity of vulvovaginal tissues as potential mechanisms of action of a PFME program in postmenopausal women with GSM. Further studies are needed to confirm these findings.

Asociación entre disfunción del suelo pélvico y la ocurrencia de la infección del tracto urinario entre mujeres embarazadas en Lima, Perú / Pelvic floor dysfunction and the occurrence of urinary tract infection among pregnant women in Lima, Peru

Introducción: Al tener una disfunción del suelo pélvico aumentan las probabilidades de desarrollar incontinencia urinaria o prolapso genital. Estas condiciones, a su vez, podrían generar una infección del tracto urinario (ITU). En la población de mujeres embarazadas, la disfunción del suelo pélvico ocurre principalmente por la carga de peso del vientre, el cual podría ser una causa de la ITU. Objetivo: Determinar la asociación entre la disfunción del suelo pélvico y la ocurrencia de la ITU en gestantes. Métodos: Estudio observacional de tipo transversal analítico. La población fueron gestantes entre 18 a 35 años de 5 centros de salud en Lima. La disfunción del suelo pélvico se evaluó con el cuestionario The Pelvic Floor Inventories Leiden (PeLFIs) dividiéndose en 3 componentes: sensación de flacidez, patrón de orina e incontinencia urinaria. La ITU se midió incluyendo una sección en el cuestionario abarcando 3 preguntas. Las variables edad, actividad física, ocupación y peso fueron incluidas en la sección de preguntas generales dentro del cuestionario. Resultados: En total fueron 146 gestantes encuestadas que cumplieron criterios de selección. La prevalencia de ITU durante el embarazo fue de 42.5%. Las gestantes con ITU durante el embarazo tienen un puntaje mayor en los componentes de sensación de flacidez (p=0.036), patrón de orina (p=0.001) e incontinencia urinaria (p=0.011). Conclusión: Luego de ajustar las variables confusoras se determinó que existe asociación entre los 3 componentes de disfunción del suelo pélvico y la ITU, ya que la mayoría de las gestantes tuvieron tratamientos contra la ITU para no perjudicar su embarazo. / Introduction: Upon occurrence of a pelvic floor dysfunction, the chances of developing urinary incontinence or vaginal prolapse increase. Such conditions, in turn, may lead to a urinary tract infection (UTI). In pregnant women, the pelvic floor dysfunction mainly occurs due to the weight of the womb, which could be a cause of the UTI. Objective: To determine the association between the pelvic floor dysfunction and the occurrence of UTI in pregnant women. Methods: Observational research of analytical cross-sectional type. The population included pregnant women between 18 to 35 years old from 5 health centers of Lima. The pelvic floor dysfunction was assessed through the questionnaire “The Pelvic Floor Inventories Leiden” (PelFIs), and it was divided into three components: vaginal swelling, urination pattern, and urinary incontinence. The UTI was measured based on three questions in a section of the questionnaire. Variables such as age, physical activity, occupation, and weight were included in the section of general questions within the questionnaire. Results: A total of 146 pregnant women surveyed met the selection criteria. The prevalence of UTI during pregnancy was 42.5 %. The pregnant women suffering from UTI during the pregnancy period show a higher score in the components of vaginal swelling (p=0.036), urination pattern (p=0.001), and urinary incontinence (p=0.011). Conclusion: After adjusting the confounding variables, it was determined that there is no association between the three components of the pelvic floor dysfunction and the UTI since most of the pregnant women followed treatments against UTI to avoid any harm during pregnancy. / Tesis

Pelvic Floor Muscle Training in Management of Postpartum Pelvic Floor Dysfunctions: A Literature Review

Women can face a wide range of pelvic floor dysfunctions following pregnancy, ranging from urinary incontinence to pelvic pain. Unfortunately, these problems are not routinely checked for in postpartum check-ups and women do not always bring it to the physician’s attention. Strengthening of the pelvic floor muscles may be able to help women prevent these disorders and improve these women’s lifestyles. The purpose of this thesis was to review and analyze different trials to determine if different pelvic floor dysfunctions (urinary incontinence, sexual dysfunction, and pelvic girdle pain) can be treated using pelvic floor muscle training in the postpartum. After reviewing the literature, it was determined that Pelvic floor muscle training may be effective in treating Urinary incontinence, but there is a lack of research to state that it helps treat sexual dysfunction and pelvic pain. Pelvic floor muscle training is a conservative non-invasive treatment and very simple for women to do on their own, therefore more research should be performed to see if this can be a simple fix to a plethora of problems women face in the postpartum.

Étude comparative de la morphologie du plancher pelvien des femmes âgées continentes et avec incontinence urinaire

But: Cette étude a pour but de comparer : a)la morphologie du plancher pelvien (PP), du col vésical et du sphincter urogénital strié (SUS) par IRM et b) la fonction du PP par palpation digitale (PERFECT scheme) chez les femmes âgées continentes ou avec incontinence urinaire à l’effort (IUE) et mixte (IUM). Méthode: Les femmes ont appris à contracter correctement leur PP et la fonction de leur PP a été évaluée. Une séance d’IRM dynamique 3T a suivi. Résultats: 66 femmes ont participé à l’étude. Les groupes étaient similaires en âge, IMC, nombre d’accouchements vaginaux et d’hystérectomie. La validité et la fidélité des différentes mesures anatomiques utilisées ont été confirmées au début de cette étude. Afin de contrôler l’effet potentiel de la taille du bassin sur les autres paramètres, les femmes ont été appariées par la longueur de leur inlet pelvien. Les femmes avec IUM ont un PP plus bas et un support des organes pelviens plus faible, selon leurs ligne M, angle LPC/Ligne H et hauteur de la jonction urétro-vésicale (UV). Les femmes avec IUE ont un PP similaire à celui des continentes, mais présentent plus d’ouverture du col vésical et un angle UV postérieur plus large au repos que les autres groupes. Il n’y a aucune différence de morphologie du SUS entre les groupes. De plus, selon les résultats du PERFECT scheme, les femmes avec IU ont une force du PP plus faible que les continentes. Les femmes avec IUM montrent aussi une faible élévation des muscles du PP à la contraction. Les femmes avec IUE ont, quant à elle, un problème de coordination à la toux. Conclusion: Les déficits causant l’IUE et l’IUM sont différents, mais supportent tous le rationnel des exercices du PP pour le traitement de l’IUE et l’IUM. Ces résultats supportent le besoin de traitements de rééducation spécifiques aux déficits de chacun des types d’IU. / Aims: The study’s aim was to compare: a) pelvic floor muscle (PFM), bladder neck and urethral sphincter morphologies using MRI and b) PFM function using digital palpation (PERFECT scheme) in continent women, women with stress (SUI) and mixed (MUI) urinary incontinence. Method: Women were taught how to perform PFM contractions correctly and their PFM function was assessed, then each woman completed a dynamic 3T MRI session. Results: 66 women participated in the study. Groups were similar for age, BMI, vaginal deliveries and hysterectomies. Validity and reliability of the different anatomical measures used has been confirmed at the beginning of the study. To control for the potential effect of pelvic size on study parameters, women were matched based on pelvic inlet length. MUI women seemed to have lower PFM resting position and pelvic organ support at rest, based on their M-Line, PCL/H-Line angle and urethrovesical (UV) junction height. However, SUI women seemed to have a PFM morphology similar to that of continent women, but presented a greater occurrence of bladder neck funnelling and a larger posterior UV angle. There were no differences in urethral sphincter morphology between the 3 groups. Functionally, as shown by the PERFECT scheme results, both UI groups had poorer PFM strength on MVC then continent women. The MUI group also showed poor PFM elevation on contraction. Conversely, women with SUI had a timing problem on cough. Conclusion: The deficits in women with SUI and MUI appear to be very different. Notwithstanding, they all support the rationale for PFM exercise treatment in older women with SUI and MUI. However, the findings suggest the need for rehabilitation treatments specificity for each UI type.

La validité et la fidélité des différentes mesures anatomiques utilisées ont été confirmées au début de cette étude. Afin de contrôler l’effet potentiel de la taille du bassin sur les autres paramètres, les femmes ont été appariées par la longueur de leur inlet pelvien. Les femmes avec IUM ont un PP plus bas et un support des organes pelviens plus faible, selon leurs ligne M, angle LPC/Ligne H et hauteur de la jonction urétro-vésicale (UV). Les femmes avec IUE ont un PP similaire à celui des continentes, mais présentent plus d’ouverture du col vésical et un angle UV postérieur plus large au repos que les autres groupes. Il n’y a aucune différence de morphologie du SUS entre les groupes. De plus, selon les résultats du PERFECT scheme, les femmes avec IU ont une force du PP plus faible que les continentes. Les femmes avec IUM montrent aussi une faible élévation des muscles du PP à la contraction. Les femmes avec IUE ont, quant à elle, un problème de coordination à la toux. Conclusion: Les déficits causant l’IUE et l’IUM sont différents, mais supportent tous le rationnel des exercices du PP pour le traitement de l’IUE et l’IUM. Ces résultats supportent le besoin de traitements de rééducation spécifiques aux déficits de chacun des types d’IU. / Aims: The study’s aim was to compare: a) pelvic floor muscle (PFM), bladder neck and urethral sphincter morphologies using MRI and b) PFM function using digital palpation (PERFECT scheme) in continent women, women with stress (SUI) and mixed (MUI) urinary incontinence. Method: Women were taught how to perform PFM contractions correctly and their PFM function was assessed, then each woman completed a dynamic 3T MRI session. Results: 66 women participated in the study. Groups were similar for age, BMI, vaginal deliveries and hysterectomies. Validity and reliability of the different anatomical measures used has been confirmed at the beginning of the study. To control for the potential effect of pelvic size on study parameters, women were matched based on pelvic inlet length. MUI women seemed to have lower PFM resting position and pelvic organ support at rest, based on their M-Line, PCL/H-Line angle and urethrovesical (UV) junction height. However, SUI women seemed to have a PFM morphology similar to that of continent women, but presented a greater occurrence of bladder neck funnelling and a larger posterior UV angle. There were no differences in urethral sphincter morphology between the 3 groups. Functionally, as shown by the PERFECT scheme results, both UI groups had poorer PFM strength on MVC then continent women. The MUI group also showed poor PFM elevation on contraction. Conversely, women with SUI had a timing problem on cough. Conclusion: The deficits in women with SUI and MUI appear to be very different. Notwithstanding, they all support the rationale for PFM exercise treatment in older women with SUI and MUI. However, the findings suggest the need for rehabilitation treatments specificity for each UI type.

Obstetric interventions and pelvic floor disorders - Abstract One of the principal objectives of obstetric interventions in the second stage of labor is prevention of pelvic floor trauma and associated pelvic floor disorders. The most commonly used and also most frequently discussed interventions are manual perineal protection and episiotomy. The majority of pelvic floor disorders are of subjective nature. Therefore, tools for objectification and severity quantification of these disorders are the key to reliable comparison and identification of the most effective interventions. The aim of the dissertation was to find the most effective modification of manual perineal protection, to compare two frequently used types of episiotomy regarding all pelvic floor disorders and finally to find a consensus on the most suitable instrument for anal incontinence severity assessment. Our studies concerning manual perineal protection experimentally described the direction and extent of perineal deformation during vaginal delivery. The subsequent studies on biomechanical model demonstrated that the most effective method of manual perineal protection in peak perineal strain reduction is when the fingers are placed on the perineum 6 cm laterally and 2 cm ventrally from the posterior commissure at both sides and are...
