<p>Peripheral neuropathy (PN) is a common neurological complication occurring in the asymptomatic and symptomatic stages of human immune deficiency virus (HIV) infection. The pain and other symptoms caused by PN can impair functional ability and limit physical activity that could affect quality of life (QoL). Additionally, studies done on quality of life of people living with HIV/AIDS have shown that, HIV-related neurological syndromes, including PN, significantly reduce QoL. The aim of this study was to determine the prevalence of peripheral neuropathy amongst and the quality of life of adults living with HIV/AIDS attending the out-patient clinic at Rutongo Hospital in Rulindo District in Rwanda.</p>

Orientador: Paulo Câmara Marques Pereira / Resumo: A lipodistrofia associada ao HIV (LAHIV) é caracterizada pela redistribuição de gordura corporal, sendo uma das consequências da introdução da terapia antirretroviral (TARV) e relacionada ao aumento do risco para o desenvolvimento de doenças cardiovasculares. Seu diagnóstico é subjetivo e classificado em três tipos, de acordo com a região corporal em que há perda e/ou acúmulo de gordura, a saber, lipoatrofia, lipohipertrofia e lipodistrofia mista, acompanhada ou não de alterações metabólicas. O objetivo do presente trabalho foi verificar a associação dos diferentes subtipos de LAHIV em pessoas vivendo com HIV/aids com os componentes da síndrome metabólica e composição corporal. Foram avaliados dados clínicos, imunológicos, metabólicos, antropométricos e composição corporal de 40 pessoas vivendo com HIV/aids em acompanhamento ambulatorial e uso regular de TARV, de ambos os sexos, com diagnóstico clínico de LAHIV. Os principais achados do estudo foram maiores alterações de perfil lipídico entre as mulheres. Não houve diferença estatística entre os subtipos para o perfil metabólico. As principais alterações foram identificadas no grupo lipohipertrofia, que tiveram maiores valores de porcentual de gordura corporal total, área de gordura visceral (AGV), índice de massa corpórea (IMC) e circunferências abdominal e do pescoço em relação aos outros dois grupos. Magister Artium - MA / Due to tremendous stigma attached to HIV/AIDS, revelation of HIV positive sero-status of an individual has become a significant risk in communities of South Africa (Kalichman el al., 2003;Deacon et al., 2004; Kalichman et al., 2005, Simbayi et al., 2007). Several researchers have argued that HIV/AIDS stigma poses severe problems which include that it delays HIV testing;stops people living with HIV and AIDS (PLWHA) from seeking care; inhibits incorporation of prevention behaviours; increases violence against HIV-positive people; and extends beyond PLWHA to families, providers and volunteers. HIV/AIDS stigma is widespread, and it is widely accepted that it does not only reflect but also is exacerbated by co-existing stigmas related to poverty, race, gender, substance use, and sexual behaviour (Parker et al., 2002; Parker & Aggleton, 2003; Holloway, Seaton, Taylor, 2004).This study aimed to understand whether HIV/AIDS as a social construct and those living with HIV/AIDS are understood and responded to differently by males and females and those previously classified by the apartheid divisive policy as African and Coloured participants in the context of their daily encounters. Hence, the study investigated whether there is a relationship between race, gender and HIV/AIDS stigma among participants and whether this relationship is mediated by age, educational level, and participants’ household situation. In addition, the study explored whether gendered stigmatization is subscribed to by participants, while it further sought to assess the extent to which participants were exposed to HIV and AIDS; and whether there were gender and racial differences with respect to participants’ perceptions about PLWHA.Finally, the study scrutinized participants’ perceived levels where they believe interventions to eliminate stigma could be prioritized. This study utilized a survey questionnaire drawn up on the basis of qualitative findings in earlier studies on stigma and HIV/AIDS in both ‘African’ and ‘coloured’ communities. Two hundred participants were recruited through a convenience sampling method in the Mitchell’s Plain area of Cape Town, South Africa. The sample was stratified by ‘race’ and gender with the majority between the age of 35 and 49 years; 50.3 % of the respondents in the sample were males, while 49.7 % were females. The majority of the respondents were Africans of Xhosa speaking descent(49.2 %) and coloureds (48.2 %) with a small proportion of whites (2.5 %). All standard ethical procedures for research with human participants were adhered to and the project was registered with the UWC Research & Ethics Committee. All participants responded on the basis of informed consent and consent forms were signed to confirm ethical assurances. Confidentiality of the data was observed and the data was kept in a locked up and secure place for a period of five years after the study. Completed survey questionnaires were coded, and analyzed quantitatively using the Statistical Package for Social Sciences version 17.0 (SPSS) and SAS.Inferential statistics showed highly significant gender differences in participants’ personal stigmatization. More so, personal stigma attitudes were more likely to be found among older males and with means to support their households. Descriptive results showed these older males were less likely to know someone with HIV. The study acknowledged that HIV and AIDS as well as PLWHA are socially constructed and intersected with existing social inequalities on difference and hence, recommends that interventions to address HIV/AIDS stigma need to take cognizance of the contexts in which it occurs. It seems important to address de-stigmatization efforts at coloured families since descriptive results reflect some denial that HIV is a challenge and/or that PLWHA are stigmatized in families and community at large. Gendered constructions of stigma, while impacting on both men and women (since both appear to be stigmatized differently), are clearly still salient and it is recommended that efforts be continually made to raise the way in which gender, class, racialised and other differences of power are being played in community responses to HIV and those living with HIV/AIDS.

Após a disponibilização dos antirretrovirais (ARV), a sexualidade dos portadores de HIV/Aids passou a ser mais freqüentemente estudada, especialmente com enfoque na prevenção e no risco. Objetivos: Descrever a vida sexual de portadores de HIV/Aids e identificar as associações da piora na vida sexual após o diagnóstico de HIV com variáveis relativas às características socioeconômicas, à vida sexual, ao uso do ARV e dos serviços de saúde. Métodos: Os dados utilizados procedem de dois estudos transversais com 250 homens e 729 mulheres portadores de HIV/Aids (amostras consecutivas), realizados em dois centros de referência para o HIV/Aids da cidade de São Paulo, entre setembro de 1999 e fevereiro de 2002.A resposta para a questão sobre a mudança na vida sexual, após o diagnóstico de HIV/Aids de cada questionário, foi avaliada e categorizada como piora ou não. A comparação foi realizada entre os grupos de homens e mulheres em relação à piora na vida sexual após o diagnóstico de HIV. Fatores associados foram estimados por regressão múltipla de Poisson. Os testes foram conduzidos em nível de significância de 5 por cento. Resultados: Os homens relataram mais mudança negativa que as mulheres 59 por cento e 41 por cento, respectivamente (p<0,001). Para mulheres, os fatores associados às mudanças negativas na vida sexual, após o diagnóstico, foram: passar por dor ou desconforto físico no atendimento recebido no serviço de saúde, não ser atendida por enfermeiro, dificuldade de falar com ginecologista sobre a vida sexual, desejo de ter filhos/estar grávida, ausência de desejo de ter filhos e abstinência sexual. Fatores negativamente associados foram: renda entre 2 e 4 salários mínimos, via sexual de infecção para o HIV. Para homens, os fatores associados a mudanças negativas na vida sexual após o diagnóstico foram: desemprego e facilidade moderada de falar com psicólogo sobre a vida sexual. Fatores negativamente associados foram: número de parcerias sexuais durante a vida (pelo menos 6) e uso prévio de maconha. The tests were conducted with a 5 per cent significance level. Results: Men reported more negative changes in sexual life after diagnosis than women 59 per cent and 41 per cent, respectively (p<0.001). Associated factors for women were: experiencing pain or physical discomfort when being attended by healthcare services, not being attended by a nurse, difficulty in talking about sexual life with the gynecologist; desire to have children/being pregnant, absence of desire to have children and sexual abstinence. Negatively associated factors were: monthly income of 2-4 minimum wages, sexual infection for HIV. In men, the associated factors were: unemployment, moderate easiness to talk to a psychologist about sexual life. Negatively associated factors were; sexual partnership through life (at least 6), previous or current use of marijuana. Conclusions: The results seem to point out to a high programmatic vulnerability of women regarding the outcome, as well as individual and social vulnerabilities of men and women, that accompany specific gender scripts

10pt / font-family: &quot / Times New Roman&quot / ,&quot / serif&quot / ">Background: The care and treatment for people with HIV/AIDS (PLWHA) at the Kagera regional hospital began in February 2005. It is important to evaluate quality of life (QOL) of highly active anti-retroviral therapy (HAART) recipients periodical to assess HAART&rsquo / s effectiveness. This study aimed to assess the health-related QOL (HRQOL) of HAART recipients and determine factors associated with it, two years after the programme had started at the kagera regional hospital in Tanzania. Results: The HRQOL of HAART recipients was generally lower than that of the general population. However, the general health perceptions (p=0.191) and mental health (p=0.161) means score of HAART recipients were not different from those of the Tanzanian general population. About 89% of the HAART recipients reported better health than in the previous years. Chronic diseases were associated with lower HRQOL of HAART recipients independently of other factors. HAART recipients with chronic diseases co-morbidity were more likely to score below the general population&rsquo / s mean score for mental health (p=0.007). While the effect of chronic diseases on HRQOL among those who recorded CD4+ cell increase was negative (OR=13.6,95%CI=3.7 / 49.9), there was no effect on those who did not have such an increase. 10pt / font-family: &quot / Times New Roman&quot / ,&quot / serif&quot / ">Background: The care and treatment for people with HIV/AIDS (PLWHA) at the Kagera regional hospital began in February 2005. It is important to evaluate quality of life (QOL) of highly active anti-retroviral therapy (HAART) recipients periodical to assess HAART&rsquo / s effectiveness. This study aimed to assess the health-related QOL (HRQOL) of HAART recipients and determine factors associated with it, two years after the programme had started at the kagera regional hospital in Tanzania. Results: The HRQOL of HAART recipients was generally lower than that of the general population. However, the general health perceptions (p=0.191) and mental health (p=0.161) means score of HAART recipients were not different from those of the Tanzanian general population. About 89% of the HAART recipients reported better health than in the previous years. Chronic diseases were associated with lower HRQOL of HAART recipients independently of other factors. HAART recipients with chronic diseases co-morbidity were more likely to score below the general population&rsquo / s mean score for mental health (p=0.007). While the effect of chronic diseases on HRQOL among those who recorded CD4+ cell increase was negative (OR=13.6,95%CI=3.7 / 49.9), there was no effect on those who did not have such an increase. Either by strands designated as 'scientific' or not, the matter raised polemics, controversies, but also caused to science to put on demand urgent studies on the treatment of this disease. In Brazil, the arrival of innovative antiretroviral therapies from the second half of the 1990’s as well as the requirement for free distribution by the public health system, contributed undoubtedly to increase longevity and quality of life of people with HIV. Overcome this phase, the epidemic maintains its trajectory and other demands, among them, adherence to drug therapies, were outlined as mandatory elements for the successful treatment of this syndrome. In this study, it was intended to historically rescue the deployment of public services care for HIV / AIDS in Ponta Grossa - PR, from the point of view of multidisciplinary teams involved. Moreover, it was emphasized the historical trajectory of the management of these patients during the arrival of the National Humanization Policy (PNH). And how these professionals involved have assimilated this police of the Unified Health System to develop promotion strategies for adherence to antiretroviral therapies. The methodology proposed in this research was conducted through a qualitative analysis of the subject throught exploratory research. Using elements such as: a literature review, documentary research through newspapers, magazines and movies on the HIV/AIDS issues; and interviews with professionals who acted and act in the treatment of patients with HIV for further analysis of content. The results obtained from this research demonstrated the underspending of PNH as supportive tool to adherence of antiretroviral therapies in the researched county. / Desde o advento da AIDS nos anos 1980, muito se discutiu a respeito desta síndrome. Seja por vertentes designadas como ‘científicas’ ou não, o assunto suscitou polêmicas, controvérsias, mas também, provocou a Ciência a colocar em demanda de urgência estudos sobre o tratamento dessa enfermidade. No Brasil, a chegada de terapias antirretrovirais inovadoras a partir da segunda metade dos anos 1990, bem como a obrigatoriedade de sua distribuição gratuita pelo sistema público de saúde, contribuiu, indiscutivelmente, para o incremento na longevidade e qualidade de vida dos portadores de HIV. Superada essa fase, a epidemia mantém sua trajetória e outras demandas, dentre elas, a adesão às terapias medicamentosas, delinearam-se como elementos imperativos para o sucesso terapêutico sobre esta síndrome. Nesta pesquisa, pretendeu-se resgatar historicamente a implantação dos serviços públicos de atendimento para portadores de HIV/AIDS em Ponta Grossa – PR, sob o ponto de vista das equipes multidisciplinares envolvidas. Destacou-se a trajetória histórica da gestão desses pacientes à luz da chegada da Política Nacional de Humanização (PNH). Buscou-se, também, verificar como os profissionais envolvidos assimilaram essa política no Sistema Único de Saúde para desenvolver estratégias de promoção à adesão às terapias antirretrovirais. A metodologia proposta nessa pesquisa foi realizada através da análise qualitativa do tema por meio de pesquisa exploratória. Utilizando-se de elementos como: a revisão teórica, a pesquisa documental através de jornais, revistas e filmes sobre o tema HIV/AIDS; e entrevistas com profissionais que atuaram e atuam no tratamento de pacientes portadores de HIV, para posterior análise de conteúdo. O resultado obtido com essa pesquisa demonstrou a subutilização da PNH como ferramenta incentivadora da adesão as terapias antirretrovirais no município de estudo.
