Efeito analgésico da morfina e xilazina associadas à ropivacaína pela via epidural em cadelas submetidas à mastectomia unilateral total

Este estudo teve por objetivo investigar os efeitos analgésicos trans e pós-operatórios da administração epidural da ropivacaína associada à morfina e/ou xilazina em cadelas submetidas a mastectomia unilateral total. Poucos são os relatos na literatura sobre uso de epidural mastectomia radical. Foram utilizadas 23 cadelas, pre-medicadas com acepromazina (0,02 mg/kg IM) e morfina (0,3 mg/kg IM) , e anestesiadas com propofol e posteriormente isofluorano associado à anestesia epidural com um dos três tratamentos: grupo RM (n=7), ropivacaína com morfina (0,75 e 0,1 mg/kg respectivamente); grupo RX (n=8), ropivacaína com xilazina (0,75 e 0,1 mg/kg respectivamente); e grupo RMX (n=7), ropivacaína associada à morfina e xilazina nas mesmas doses. O volume padronizado final dos tratamentos epidurais foi de 0,35 mL/kg ao serem diluídos com solução salina. Foram avaliados parâmetros cardiovasculares e respiratórios em seis momentos: Basal (durante a anestesia inalatória, antes da anestesia epidural), pós-epidural (30 minutos após tratamento epidural), T1, T2 e T3 (durante incisão da pele e divulsão do tecido dos terços caudal, médio e cranial da cadeia mamária, respectivamente) e T4 (rafia). Os escores de dor (escalas de Glasgow e EAVID) e necessidade de resgates analgésicos no pós-operatório foram avaliados durante 24 horas Houve menor requerimento de isofluorano nos animais dos grupos RX (1,1±0,2% em T3 e T4) e RMX (0,9 a 1,0±0,2% em T2, T3 e T4) versus o RM (1,3±0,2%). Nos grupos RX e RMX, houve diminuição da frequência cardíaca (RX no pós-epidural e em RMX no pós-epidural e T3) quando comparada com o basal mas não houve variações das pressões arteriais intra-grupo nem entre os grupos. O resgate pós-operatório foi necessário em 85,7%, 14,3% e 25,0% dos animais dos grupos RX, RM e RMX respectivamente. Os escores de dor foram menores nas primeiras quatro horas de pós-operatório no grupo RMX versus RX. Os animais do grupo RX demoraram mais para se manter em posição quadrupedal que o grupo RM (335 ± 82 versus 113 ± 69 minutos respectivamente) e teve maiores tempos de bloqueio motor (299 ± 90 versus 195 ± 37 minutos respectivamente). Quando administrados por via epidural, os três tratamentos fornecem analgesia trans-operatória aceitável para cadelas submetidas à mastectomia unilateral total, com mínimos efeitos cardiovasculares. A analgesia pós-operatória é mais prolongada nos protocolos de anestesia que têm morfina na sua composição (RM e RMX). Os protocolos com xilazina podem reduzir o requerimento de isofluorano, mas podem retardar o tempo de deambulação das cadelas. / This study aimed to investigate the intra and postoperative analgesic effects of epidural administration of ropivacaine combined with morphine and/or xylazine in bitches undergoing total unilateral mastectomy. There are few reports in the literature on the use of epidural in radical mastectomy. Twenty-three bitches were pre-medicated with acepromazine (0.02 mg/kg IM) and morphine (0.3 mg/kg IM) and anesthetized with propofol and isoflurane associated with epidural anesthesia with one of three treatments: RM group (n = 7), ropivacaine with morphine (0.75 and 0.1 mg/kg respectively); RX group (n = 8), ropivacaine with xylazine (0.75 and 0.1 mg/kg respectively); and RMX group (n = 7), ropivacaine combined with morphine and xylazine at the same doses. The final standarized volume of the epidural treatments was 0.35 mL/kg when diluted with saline solution. Cardiovascular and respiratory parameters were evaluated in six moments: Basal (during inhalational anesthesia, before epidural anesthesia), post-epidural (30 minutes after epidural treatment), T1, T2 and T3 (skin incision and divulsion of the caudal, middle and cranial thirds of the mammary chain, respectively) and T4 (raffia). Pain scores (Glasgow scales and EAVIDs) and the need for postoperative analgesic rescues were evaluated for 24 hours The intraoperative requirement for isofliurane was lower in the RX (1.1±0.2% in T3 and T4) and RMX (0.9 to 1.0±0.2% in T2, T3 and T4) groups versus RM group (1.3±0.2%). Heart rate decreased bellow baseline in the RX and RMX groups, (post-epidural in RX group and post-epidural and T3 in RMX group) when compared to baseline but there was no significant change in blood pressure within any group or among groups. Postoperative analgesic rescue was necessary in 85.7%, 14.3% and 25.0% of the animals of the RX, RM and RMX groups, respectively. Pain scores were significantly lower from 1 to 4 hours postoperatively in the RMX versus RX group. Time to standing was longer in the RX group than the RM group (335 ± 82 versus 113 ± 69 minutes, respectively) and duration of motor block was also longer in the RX compared to the RM group (299 ± 90 versus 195 ± 37 minutes respectively). When administered epidurally, all three treatments provide acceptable intraoperative analgesia for bitches undergoing total unilateral mastectomy with minimal cardiovascular adverse effects. Postoperative analgesia is longer in protocols containing morphine (RM and RMX groups). Protocols containing xylazine may reduce the intraoperative requirement of isoflurane, but may prolong the time until ambulation of the dogs.

Avaliação dos efeitos da anestesia peridural torácica sobre as alterações miocárdicas associadas à morte encefálica: estudo experimental / Assessment of the effects of thoracic epidural anesthesia on myocardial changes associated with brain death: an experimental study

INTRODUÇÃO: Atualmente, a maior limitação ao transplante cardíaco, em todo o mundo, é a escassez de doadores cujo número está sempre aquém do número de pacientes portadores de doença cardíaca terminal, e esse hiato fica ainda maior pelo fato de cerca de 25 % dos corações doados não serem utilizados, por estarem acometidos por acentuada disfunção. A descarga catecolaminérgica associada à morte encefálica poderia ser um dos elementos implicados nesta disfunção. Assim, intervenções terapêuticas com intuito de minimizar o exacerbado estímulo simpático visam, em última instância, ampliar a oferta de órgãos para o transplante. OBJETIVOS: Investigar a hipótese de que a anestesia peridural torácica seja capaz de bloquear a tempestade autonômica inerente à morte encefálica por hipertensão intracraniana aguda, minimizando as alterações hemodinâmicas, reduzindo a resposta inflamatória e, por conseguinte, melhorando a condição do enxerto. MÉTODOS: Ratos Wistar machos (250-350 g) anestesiados (isoflurano 5 %) e monitorados continuamente para o registro da pressão arterial média foram submetidos à inserção de cateter no espaço peridural em nível torácico e, em seguida, submetidos à morte encefálica por hipertensão intracraniana aguda pela xxi insuflação de um cateter de Fogarty® introduzido por trepanação. Os animais (n = 28) foram alocados em 4 grupos: grupo salina - infusão de 20 uL de solução salina pelo cateter peridural antes da indução da morte encefálica; grupo bupi-pré - infusão de 20 uL de solução de bupivacaína a 0,5 % pelo cateter peridural, antes da indução da morte encefálica; grupo bupi-20 - infusão de 20 uL de solução de bupivacaína a 0,5 % pelo cateter peridural 20 minutos após a indução da morte encefálica; grupo bupi-60 - infusão de 20 ?L de solução de bupivacaína a 0,5 % pelo cateter peridural 60 minutos após a indução da morte encefálica. Após 6 horas os animais foram submetidos à eutanásia por exsanguinação. Foram realizadas dosagens séricas e no tecido cardíaco das citocinas interleucina (IL)-1beta e fator de necrose tumoral (TNF)-alfa pelo método de ELISA. As moléculas de adesão endoteliais, vascular adhesion molecule (VCAM)-1 e intercellular adhesion molecule (ICAM)-1, proteínas envolvidas no processo de apoptose, Bcl-2 e caspase-3, e a alfa-actina foram avaliadas no tecido miocárdico por técnica imunohistoquímica. Cortes longitudinais do coração foram corados com hematoxilina/eosina e avaliados quanto à presença de edema, infiltrado leucocitário e congestão vascular. Contagens totais de leucócitos circulantes foram realizadas antes da indução da morte encefálica e 3 e 6 horas após. RESULTADOS: Logo após a insuflação do cateter de Fogarty® todos animais evoluíram com sinais clínicos de morte encefálica: midríase bilateral fixa e ausência de reflexo corneano. O aumento súbito e efêmero da pressão arterial foi observado em todos os animais, à exceção do grupo que recebeu a bupivacaína previamente à indução da morte encefálica (grupo bupi-pré) (p<0,05). Houve acentuada e progressiva leucopenia em todos os grupos. As citocinas IL-1beta, TNF-alfa, tanto no soro quanto no miocárdio, não apresentaram diferenças significativas entre os grupos. As moléculas de adesão VCAM-1 e ICAM-1, as proteínas Bcl-2 e caspase-3, a alfa-actina, bem como a análise histológica do miocárdio não apresentaram diferenças significativas entre os grupos. CONCLUSÕES: A anestesia peridural torácica foi efetiva em bloquear o pico hipertensivo associado à morte encefálica. Contudo, tal bloqueio não se correlacionou a alterações na concentração de citocinas, expressão de moléculas de adesão, expressão de proteínas envolvidas com o processo apoptótico e alfa-actina, e alterações histológicas e do leucograma. Portanto, a tempestade autonômica não parecer ser a responsável pela ativação da resposta inflamatória e, em última instância, pela disfunção miocárdica associada à morte encefálica / BACKGROUND: Currently, the main limitation to cardiac transplantation, worldwide, is the shortage of donors whose number is always smaller than the number of patients with terminal heart disease, and this gap is even greater because about 25 % of donated hearts are not used due to severe dysfunction of unknown cause. Brain-death associated catecholaminergic storm may be implicated in this dysfunction. Thus, therapeutic interventions aiming to reduce the sympathetic stimulation result, ultimately, in an increase in the number of organs for transplantation. OBJECTIVES: To investigate the hypothesis that thoracic epidural anesthesia is capable of blocking the sympathetic discharge inherent to brain death, by acute intracranial hypertension, minimizing hemodynamic changes, and reducing the inflammatory response improving, therefore, the graft outcome. METHODS: Male Wistar rats (250 - 350 g) anesthetized (5 % isoflurane) and continuously monitored to record mean arterial pressure, underwent insertion of a catheter into the epidural space, at the thoracic level. Brain death was induced by acute intracranial hypertension by inflating an intracranially inserted Fogarty catheter. The animals (n = 28) were divided into 4 groups: saline group - infusion of 20 uL of saline through the epidural catheter before induction of brain death; pre-bup group - infusion of 20 uL of bupivacaine through epidural catheter before induction of brain death; bup-20 group - infusion of 20 uL of bupivacaine through epidural catheter 20 min after induction of brain death; bup-60 group - infusion of 20 uL of bupivacaine through epidural catheter 60 min after induction of brain death. After 6 h the animals were exsanguinated. Serum and cardiac tissue concentrations of cytokines, interleukin (IL)-1beta and tumor necrosis factor (TNF)-alfa, were performed by ELISA. The endothelial adhesion molecules, vascular adhesion molecule (VCAM)-1 and intercellular adhesion molecule (ICAM)-1, proteins involved in apoptosis, Bcl-2 and caspase-3, and ?-actin were evaluated in myocardial tissue by immunohistochemistry. Longitudinal sections of the heart were stained with hematoxylin/eosin and evaluated for the presence of edema, vascular congestion and leukocyte infiltration. White blood cell counts were obtained prior to induction of brain death, 3 and 6 h thereafter. RESULTS: Clinical signs of brain death, fixed dilated pupils and absence of corneal reflex, were observed immediately after catheter insuflation. The sudden increase in mean arterial pressure was observed in all animals except in those receiving bupivacaine prior to brain death induction (pre-bup) (p<0.05). There was a marked and progressive leukopenia in all groups. Cytokine levels, IL-1beta and TNF-alfa, in serum and cardiac tissue, showed no significant differences among groups. Adhesion molecules, VCAM-1 and ICAM-1, the proteins, Bcl-2, caspase-3 and ?-actin, and the histological analysis of the myocardium showed no significant differences among groups. CONCLUSIONS: The thoracic epidural anesthesia was effective to block the hypertensive peak associated with brain death. However, this blockage does not correlate to changes in the levels of cytokines, expression of adhesion molecules and expression of apoptosis-linked proteins, and alfa-actin. Furthermore, no changes in histological analysis and white blood cell counts were observed. The autonomic storm does not seem to be responsible for the activation of the inflammatory response and, ultimately, for the myocardial dysfunction associated with brain death

nÃo hà / A s(+)cetamina à o isÃmero levÃgiro da cetamina, antagonista do receptor NMDA para glutamato que està envolvido na gÃnese e manutenÃÃo do processo doloroso. A analgesia multimodal consiste na utilizaÃÃo de combinaÃÃo de fÃrmacos objetivando controle adequado da dor com reduÃÃo dos efeitos adversos. O objetivo deste estudo foi avaliar a eficiÃncia da s(+)cetamina isoladamente e da associaÃÃo morfina/cetamina comparadas à morfina isoladamente combinadas a mistura enantiomÃrica de bupivacaÃna (R75l25%) na anestesia peridural e analgesia pÃs-operatÃria em pacientes submetidas à histerectomia abdominal. Foi realizado estudo prospectivo, duplo cego e aleatÃrio, com aprovaÃÃo do Comità de Ãtica e Pesquisa da Universidade Federal do CearÃ. Participaram do estudo 36 pacientes ASA I ou II com idade de 20 a 60 anos submetidas à histerectomia abdominal com anestesia peridural. As pacientes foram alocadas em trÃs grupos: Grupo 1 - Grupo Cetamina (GC): administraÃÃo de mistura enantiomÃtrica (R75-S25) de bupivacaÃna associada à s(+)cetamina; Grupo 2 â Grupo Morfina (GM): administraÃÃo de mistura enantiomÃtrica (R75-S25) de bupivacaÃna associada à morfina e Grupo 3 - Grupo Cetamina/Morfina(GCM): administraÃÃo de mistura enantiomÃtrica (R75-S25) de bupivacaÃna associada à morfina e s(+)cetamina. Foram avaliados nÃvel de bloqueio motor e sensitivo, grau de sedaÃÃo e parÃmetros hemodinÃmicos: pressÃo arterial e frequÃncia cardÃaca a cada 15 minutos durante a cirurgia. No perÃodo pÃs-operatÃrio foi avaliado o consumo de analgÃsicos em 6 e 24 horas, alÃm da incidÃncia de nÃuseas, vÃmitos e prurido. A anÃlise estatÃstica foi realizada utilizando os softwares graphpad prisma 4.0 e Excel 2007. NÃo houve diferenÃa entre a idade, tempo cirÃrgico e o estado fÃsico (ASA) entre os grupos (p<0,05). A frequÃncia cardÃaca e pressÃo arterial mantiveram-se dentro dos valores estabelecidos como normal sem variaÃÃo significativa entre os grupos. A avaliaÃÃo da incidÃncia de efeitos adversos (nÃuseas, vÃmitos e prurido) nÃo foi diferente entre os grupos. A analgesia pÃs-operatÃria avaliada por consumo de analgÃsicos nas primeiras 6 horas nÃo foi diferente entre os grupos. Houve maior grau de bloqueio motor no grupo Cetamorf no tempo T15. Houve conversÃo para anestesia geral em 4 pacientes por falha de bloqueio, nos grupos cetamina-morfina (02 pacientes) e morfina (02 pacientes). Os dados sugerem que a adiÃÃo de s(+)cetamina e morfina nas doses avaliadas à segura, eficaz e permite a reduÃÃo de 50% na dose da morfina epidural mantendo-se o perfil de controle de dor no pÃs operatÃrio. No entanto, nÃo se verificou reduÃÃo da incidÃncia de nÃuseas, vÃmitos e prurido. / The association of drugs with different mechanisms of action in the dorsal horn of the spinal cord decreases postoperative pain, with a reduction in the incidence of side effects. The aim of this study was to evaluate some intraoperative parameters as well as postoperative analgesia and sedation by epidural morphine, S(+)ketamine and S(+) ketamine- morphine associated with Bupivacaine Enantiomeric Mixture (R75L25%) for abdominal hysterectomy. In this prospective, randomized, and double-blinded clinical trial, the efficacy and safety of the administration of epidural S(+)ketamine alone or with morphine were compared with epidural morphine alone (control group) for efficacy and safety comparisons after abdominal hysterectomy. 36 female patients, physical status ASA I and II, participated in this study. These patients were randomly allocated to one of the three treatment groups for having the following drugs administered epidurally: 1. Ketamine Group - Bupivacaine Enantiomeric Mixture (R75L25%) associated with S(+) ketamine (0.4 mg.kg-1); 2. Ketamine-Morphine Group - Bupivacaine Enantiomeric Mixture (R75L25%) associated S(+) ketamine (0.4 mg.kg-1) and morphine (1 mg) 3. Morphine Group, Bupivacaine Enantiomeric Mixture (R75L25%) was associated with morphine (2mg). During the intraoperative period the parameters analyzed were: blood pressure, heart rate, motor blockade level, sensitive level, intraoperative use of vasoconstrictor and sedation level. The time interval between each dada collection was 15 minutes. In the postoperative period, analgesia were evaluated using analogue visual scale 2h, 6h and 24h after the end of the surgery as well as the total amount of analgesics drugs requirement during the first 24 postoperative hours. Values were analyzed statistically using GraphPad Prisma 4.0 and Excel 2007. There were no differences between the three groups with respect to age, sex, weight, or duration of the surgical procedures (p<0,05). No differences were found between the groups during intraoperative analysis related to blood pressure, heart rate, Ramsay scores, vasoconstrictor use, and sensitive blockade level. Bromageâs scores were lower in the morpine/s+ketamine group during the first fifteen minutes analysis. Sedation scores were similar in both groups. The epidural blockade alone was not enough for surgical anesthesia resulting in conversion to general anesthesia in 4 patients who belong to Ketamine-morphine (02 patients) and Morphine (02 patients) groups, respectively. None of the patients in either group developed respiratory depression. Other side effects, such as pruritus, nausea, and vomiting, were also similar in both groups. The addiction of s(+) ketamine was safety and efficient to Bupivacaine Enantiomeric Mixture (R75L25%) in comparison with morphine.

Efeito analgésico da morfina e xilazina associadas à ropivacaína pela via epidural em cadelas submetidas à mastectomia unilateral total

Este estudo teve por objetivo investigar os efeitos analgésicos trans e pós-operatórios da administração epidural da ropivacaína associada à morfina e/ou xilazina em cadelas submetidas a mastectomia unilateral total. Poucos são os relatos na literatura sobre uso de epidural mastectomia radical. Foram utilizadas 23 cadelas, pre-medicadas com acepromazina (0,02 mg/kg IM) e morfina (0,3 mg/kg IM) , e anestesiadas com propofol e posteriormente isofluorano associado à anestesia epidural com um dos três tratamentos: grupo RM (n=7), ropivacaína com morfina (0,75 e 0,1 mg/kg respectivamente); grupo RX (n=8), ropivacaína com xilazina (0,75 e 0,1 mg/kg respectivamente); e grupo RMX (n=7), ropivacaína associada à morfina e xilazina nas mesmas doses. O volume padronizado final dos tratamentos epidurais foi de 0,35 mL/kg ao serem diluídos com solução salina. Foram avaliados parâmetros cardiovasculares e respiratórios em seis momentos: Basal (durante a anestesia inalatória, antes da anestesia epidural), pós-epidural (30 minutos após tratamento epidural), T1, T2 e T3 (durante incisão da pele e divulsão do tecido dos terços caudal, médio e cranial da cadeia mamária, respectivamente) e T4 (rafia). Os escores de dor (escalas de Glasgow e EAVID) e necessidade de resgates analgésicos no pós-operatório foram avaliados durante 24 horas Houve menor requerimento de isofluorano nos animais dos grupos RX (1,1±0,2% em T3 e T4) e RMX (0,9 a 1,0±0,2% em T2, T3 e T4) versus o RM (1,3±0,2%). Nos grupos RX e RMX, houve diminuição da frequência cardíaca (RX no pós-epidural e em RMX no pós-epidural e T3) quando comparada com o basal mas não houve variações das pressões arteriais intra-grupo nem entre os grupos. O resgate pós-operatório foi necessário em 85,7%, 14,3% e 25,0% dos animais dos grupos RX, RM e RMX respectivamente. Os escores de dor foram menores nas primeiras quatro horas de pós-operatório no grupo RMX versus RX. Os animais do grupo RX demoraram mais para se manter em posição quadrupedal que o grupo RM (335 ± 82 versus 113 ± 69 minutos respectivamente) e teve maiores tempos de bloqueio motor (299 ± 90 versus 195 ± 37 minutos respectivamente). Quando administrados por via epidural, os três tratamentos fornecem analgesia trans-operatória aceitável para cadelas submetidas à mastectomia unilateral total, com mínimos efeitos cardiovasculares. A analgesia pós-operatória é mais prolongada nos protocolos de anestesia que têm morfina na sua composição (RM e RMX). Os protocolos com xilazina podem reduzir o requerimento de isofluorano, mas podem retardar o tempo de deambulação das cadelas. / This study aimed to investigate the intra and postoperative analgesic effects of epidural administration of ropivacaine combined with morphine and/or xylazine in bitches undergoing total unilateral mastectomy. There are few reports in the literature on the use of epidural in radical mastectomy. Twenty-three bitches were pre-medicated with acepromazine (0.02 mg/kg IM) and morphine (0.3 mg/kg IM) and anesthetized with propofol and isoflurane associated with epidural anesthesia with one of three treatments: RM group (n = 7), ropivacaine with morphine (0.75 and 0.1 mg/kg respectively); RX group (n = 8), ropivacaine with xylazine (0.75 and 0.1 mg/kg respectively); and RMX group (n = 7), ropivacaine combined with morphine and xylazine at the same doses. The final standarized volume of the epidural treatments was 0.35 mL/kg when diluted with saline solution. Cardiovascular and respiratory parameters were evaluated in six moments: Basal (during inhalational anesthesia, before epidural anesthesia), post-epidural (30 minutes after epidural treatment), T1, T2 and T3 (skin incision and divulsion of the caudal, middle and cranial thirds of the mammary chain, respectively) and T4 (raffia). Pain scores (Glasgow scales and EAVIDs) and the need for postoperative analgesic rescues were evaluated for 24 hours The intraoperative requirement for isofliurane was lower in the RX (1.1±0.2% in T3 and T4) and RMX (0.9 to 1.0±0.2% in T2, T3 and T4) groups versus RM group (1.3±0.2%). Heart rate decreased bellow baseline in the RX and RMX groups, (post-epidural in RX group and post-epidural and T3 in RMX group) when compared to baseline but there was no significant change in blood pressure within any group or among groups. Postoperative analgesic rescue was necessary in 85.7%, 14.3% and 25.0% of the animals of the RX, RM and RMX groups, respectively. Pain scores were significantly lower from 1 to 4 hours postoperatively in the RMX versus RX group. Time to standing was longer in the RX group than the RM group (335 ± 82 versus 113 ± 69 minutes, respectively) and duration of motor block was also longer in the RX compared to the RM group (299 ± 90 versus 195 ± 37 minutes respectively). When administered epidurally, all three treatments provide acceptable intraoperative analgesia for bitches undergoing total unilateral mastectomy with minimal cardiovascular adverse effects. Postoperative analgesia is longer in protocols containing morphine (RM and RMX groups). Protocols containing xylazine may reduce the intraoperative requirement of isoflurane, but may prolong the time until ambulation of the dogs.

The purpose of the study was to evaluate the maternal satisfaction after the experience of spinal anaesthesia for Caesarean section in one public hospital of Gauteng Province in South Africa. Data were collected by administering a questionnaire which included the demographic profile and satisfaction score in terms of perioperative elements to 82 women in their postoperative period. Analysis of data was performed by The Statistical Package for the Social Sciences (SPSS) version 13 and Excel 2010 was used for tabular and graphical illustrations. The findings revealed that the majority of women studied were black (95,1%), not married (73,2%) and unemployed (64,6%). The levels of maternal satisfaction in terms of intraoperative, postoperative and other perioperative elements were high, at 94,4%, 90,3% and 85,4% respectively. The lowest percentage (76,8%) was found for the maternal satisfaction about the preoperative explanation. The overall level of maternal satisfaction in this study was 86,7%. The majority of women (82,9%) would opt for spinal anaesthesia in future, while 6,1% would not accept it and 11,0% were not sure if they would opt for it or not. / Health Studies / M.A. (Public Health)
