Optimizing Femoral Head Preparation in Hip Resurfacing Arthroplasty

Hip resurfacing is an alternative to total hip arthroplasty for the young and active patient likely to outlive traditional means of hip joint replacement. The acetabular cup is implanted in much the same fashion as an uncemented total hip arthroplasty, however, implantation of the femoral component is unique to hip resurfacing, presenting both distinct benefits and limitations. Hip resurfacing spares much of the proximal femur including the femoral neck and portions of the femoral head. This may be advantageous if the patient requires revision surgery; however, preservation of the femoral neck bears with it the risk of femoral neck fracture. The exact mechanism of neck fracture is not fully understood. Avoiding potential fracture risks is vital to ensuring optimal patient outcomes. The current work investigated mechanical femoral head preparatory factors that may predispose to femoral neck fracture. Intra-operative computer navigation is emerging as the gold-standard in orthopaedic care. In hip resurfacing, navigation may improve the surgeon’s ability to optimally implant the resurfacing prosthesis; however, much of this technology is still in its infancy and requires investigation into the accuracy and repeatability of this peri-operative tool. Pre-operative planning can assist the surgeon in optimally determining the size and position of the resurfacing components, specifically in reference to the patient’s unique anatomy, prior to performing the operation. This may aid in correct implant selection and provide a basis on which to conduct intra-operative navigation. However, the accuracy and repeatability of pre-operative planning for hip resurfacing has not yet been established. Thus, this body of work looked to establish a clear methodology for pre-operative planning, intra-operative computer navigation and surgical technique in order to optimize preparation of the femoral head, ultimately reducing the risk of femoral neck fracture in hip resurfacing.

The application of 3D Printing in reconstructive surgery

Thesis (MScEng (Industrial Engineering))--University of Stellenbosch, 2010. / ENGLISH ABSTRACT: As part of a growing trend in the medical industry of patient specific solutions, a need arises for means and methods that could grant surgeons the ability to improve their pre-operative planning, and help streamline their intra-operative proceedings relative to each individual patient. A suitable solution has emerged in the form of Additive Fabrication. Most of the traditional layer manufacturing technologies have been considered to be too expensive for medical application, and could not always be justified. However, more cost effective technologies, such as 3D Printing, have recently come to the scene and definitely require a fresh re-consideration for medical applications. In this report the research results are presented that look at the applications of 3D Printing in various fields of reconstructive surgery. Based on a variety of case studies the outcome strongly suggests that 3D Printing might become part of standard protocol in medical practice in the near future. / AFRIKAANSE OPSOMMING: Tans beweeg die mediese veld al hoe meer in die rigting van pasiënt uniekheid. Dit beteken dat behandeling begin weg beweeg van standaard prosedures en soveel moontlik aagepas word om aan te pas by elke unieke pasiënt. As deel hiervan ontstaan die behoefte by chirurge om hul operasies ook beter te beplan spesifiek tot elke individu, en sodoende te verseker dat die prosedures in teater so glad moontlik verloop. Daar is reeds tegnologië in die vorm van Addidatiewe Vervaardiging wat hierdie probleem aanspreek. Tot op hede was die finansiële implikasies vir meeste van die onderskeie tegnologië ‘n struikelblok wanneer dit kom by mediese toepassings. Tog, danksy meer koste effektiewe tegnologie soos 3D Drukwerk, is dit die moeite werd om weer op nuut te kyk na die moontlikhede wat die tegnologie kan bied. In hierdie verslag word daar gekyk na die verskillende toepassings van 3D Drukwerk in die veld van rekonstruktiewe chirurgie. Op grond van die resultate verkry vanaf ‘n wye verskeidenheid gevalle studies word die gevolgtrekking gemaak dat bekostigbare tegnologie soos 3D Drukwerk ‘n baie goeie kans het om in die nabye toekoms deel te word van standaard prosedure in die mediese praktyk.

A associação desses fatores induz à deficiência motora global e à inatividade física. O objetivo do estudo foi avaliar o perfil dos candidatos à transplante de fígado acompanhados no Gastrocentro - UNICAMP, bem como os efeitos de um programa de intervenção respiratória fisioterapêutica nesta população. Foram avaliados a PImáx e PEmáx por manovacuometria, atividade elétrica muscular do reto abdominal e diafragma por eletromiografia de superfície e anotados os valores de RMS quando solicitado uma respiração forçada a cada três segundos. A função pulmonar foi avaliada pela espirometria e a qualidade de vida pela aplicação do questionário SF-36. Participaram do estudo do perfil 62 pacientes, 74,2% do sexo masculino, idade 55,5±9,5 anos e MELD corrigido 18,6±5,1. A maioria com diagnóstico de cirrose por HVC (24,2%). Mais da metade (56,4%) eram ex-tabagistas, 72,6% ex-etilistas, 46,8% relataram dispneia, e a ascite esteve presente em 40,3% dos indivíduos. Houve diferença significante (p=0,017) entre a PImáx mensurada e PImáx predita para idade e gênero. Os pacientes apresentaram em média valores acima de 80% do predito para as variáveis obtidas por meio da espirometria e baixa pontuação em todos os domínios do questionário SF-36. Dos 62 pacientes, foram randomizados 37 pacientes para constituírem os grupos controle e intervenção, sendo que 23 participaram do grupo controle e 14 do grupo intervenção. As avaliações foram realizadas antes da intervenção, e após três meses desta. Os exercícios foram supervisionados à distância, mensalmente sempre pelo mesmo observador. A intervenção consistiu de um manual ilustrativo e explicativo a ser seguido em casa, com os exercícios de respiração diafragmática; exercício isométrico diafragmático, Threshold IMT®; elevação de membros superiores com bastão e fortalecimento dos abdominais. Todos os exercícios foram realizados em três séries, 15 repetições. Para treinamento com o Threshold IMT® foi utilizada uma carga de 23 a 40 cmH2O, ajustada para cada paciente de acordo com a sua tolerância. Durante os três meses, o indivíduo treinou com a mesma carga estabelecida na avaliação inicial. Houve aumento significativo (p=0,017) da PImáx no grupo controle e grupo intervenção após três meses; bem como nos domínios do SF-36, Estado Geral de Saúde (p=0,019) e Saúde Mental (p=0,004). O RMS do diafragma diminuiu no grupo intervenção (p=0,001) e houve aumento do escore de Capacidade Funcional (p=0,006) no tempo final, comparado ao grupo controle. Concluindo, o estudo do perfil dos pacientes em lista de espera para transplante de fígado revelou que eles apresentavam força muscular inspiratória diminuída em comparação ao predito para idade e gênero, função pulmonar preservada e a qualidade de vida prejudicada. A intervenção proposta foi capaz de melhorar a força muscular inspiratória dos pacientes, e consequentemente, reduzir a atividade elétrica do diafragma. Além disso, o grupo intervenção apresentou melhora na qualidade de vida, principalmente no escore Capacidade Funcional do SF-36 / Abstract: Candidates for liver transplantation may have malnutrition, fatigue, loss of muscle mass and function. The combination of these factors leads to global motor impairment and physical inactivity. The aim of the study was to evaluate the profile of candidates for liver transplantation followed at Gastrocentro - UNICAMP, and evaluate the effects of a program of respiratory physiotherapy intervention in this population. MIP and MEP were assessed by manometer, electrical muscle activity of the rectus abdominis and diaphragm was measured by surface electromyography, the RMS values were noted every three seconds, in this period the patient made forced breath. The pulmonary function was evaluated by spirometry and the SF-36 questionnaire was applied. The study of profile evaluated 62 patients, 74,2% males, age 55,5±9,5 and corrected MELD 18,6±5,1. Cirrhosis due to HCV was found in most patients (24,2%). More than half of the patients (56,4%) were former smokers, 72,6% were ex-alcoholics, 46,8% reported dyspnea, and ascites was present in 40,3% of patients. There was a significant difference (p=0,017) between the measured MIP and MIP predicted for age and gender. Patients had a mean values above 80% of predicted for the variables obtained by spirometry and low scores in all domains of the SF-36. Of the 62 patients, 37 patients were randomized to constitute the control and intervention groups, of which 23 participated in the control group and 14 in the intervention group. Evaluations were performed before the intervention, and three months after that. The exercises were supervised from a distance, monthly by the same observer. The evaluations were conducted before the intervention, and after three months of it. The intervention consisted of an explanatory and illustrative manual to be followed at home with diaphragmatic breathing exercises, diaphragmatic isometric exercise, Threshold IMT®, lifting upper with bat and strengthening the abdominals. All exercises were performed in three sets, 15 repetitions. For training with Threshold IMT®, a load 23-40 cmH2O adjusted for each patient according to their tolerance was used. During the three months, the subject trained with the same charge established in the initial evaluation. A significant increase of MIP (p=0,017) was found in bouth groups after three months; as well as in domains of SF-36, General Health (p=0,019) and Mental Health (p=0,004). The RMS of the diaphragm decreased in the intervention group (p=0,001) and the score of Functional Capacity increased (p=0,006) at the end of time, compared to the control group. In conclusion, the study of the profile of patients on the waiting list for liver transplantation revealed that they had decreased inspiratory muscle strength compared to the predicted for age and gender, lung function preserved and impaired quality of life. The proposed intervention was able to improve inspiratory muscle strength of patients, and hence reduce the electrical activity of the diaphragm. This thesis was performed at the research centre CINECA in Bologna, Italy. An interface with six degrees of freedom, 6DOF, to be used in a virtual environ- ment for the positioning of medical components was developed in co-operation with IOR, one of the most important orthopaedic hospitals in Italy. The main reason for doing this was to find out whether or not a virtual en-viron-ment and 6DOF interaction could make the pre-operative planning of an ope-ration more efficient compared to other techniques. Is it easy to posi-tion an object using stereovision and a 6DOF tracker tool? Further-more, the interface might also be used in other applications and areas in the future. Described is the development of an interaction class especially constructed for the use of a tracking tool called a stylus pen. This tool takes advantage of all 6DOF, i.e. it recognises movements in the x, y and z directions and likewise the orientation of the tool around the three axis. Moreover, an application which uses the interaction class was created in order to evaluate its usefulness. The application enables the user to load, save and position objects within a virtual environment. The result of this evaluation is then described and discussed. In the evaluations it was shown that the stylus pen with 6DOF is an intuitive in-ter-action tool which works well for positioning. The stereovision also seems to further improve the users ability to position objects. However, the created interaction class needs to be further developed before itcan be implemented in a pre-operative planning tool.

Another aim is to point to potential danger which could endanger the patient while irrational using of the dietary supplement before the medical interventions (interactions of dietary supplement, side effects of dietary supplement). Methodology: The questionnaire research was conducted in order to obtain the data. It was counducted between 9th May 2017 till 30th June 2018 at 9 departments of the University Hospital in Hradec Králové. In total 332 patients in the time before surgery were asked and 257 completed questionnaires were analysed. The main purpose of the present study to examine the relationship between psychological preparation, attitudes toward hospital and health professionals, social support, ways of coping and sociodemographic variables with preoperative anxiety, post-intervention anxiety and post-operation anxiety in children anticipating an operation in child surgery clinic. In addition, the relationship between the anxiety level of child and anxiety level of mother was examined. The sample consisted of sixty children and their mothers. Data was collected utilizing the state form of the State-Trait Anxiety Inventory for iv Children (STAI-C), Attitudes Toward Hospital and Health Professionals Scale, Social Support Scale, KIDCOPE, and the state form of the State-Trait Anxiety Inventory (STAI) for mothers. Results of variance analysis showed that there was a significant decrease in anxiety scores from pre-operation, post-intervention to post-operation periods in treatment groups (information/ information together with mother/ interview). However, no significant difference was found among the control group. Children who received information alone and children who were informed together with mothers and children who were merely interviewed were found to demonstrate lower levels of anxiety than children in control group in post-intervention period. Children in the pre-operation period, were found to have less positive attitude than post-operation period in information group. Children who received information alone or children informing together with mothers, reported using positive coping and blame &amp / anger more than children in the mere interview and the control group. Also children in control group had significantly higher scores on avoidance subscale than other three treatment groups. The results were discussed within the context of relevant literature.

A cirurgia bariátrica consiste em uma alternativa de tratamento cirúrgico da obesidade mórbida, permitindo uma perda de peso expressiva e duradoura com consequente melhoria das comorbidades associadas. A obesidade está associada a alterações da mecânica ventilatória e constitui um fator de risco para o desenvolvimento de complicações pulmonares no pós-operatório de cirurgias abdominais. Objetivo: Verificar se os testes de função respiratória, de força muscular respiratória e de exercícios são capazes de predizer complicações no pós-operatório em pacientes submetidos à cirurgia bariátrica. Método: Foram avaliados 174 pacientes (81% mulheres) submetidos à cirurgia bariátrica, no Hospital das Clínicas da Faculdade de Medicina de Botucatu. Todos os pacientes foram submetidos à medidas antropométricas, testes de espirometria, medida de pico de fluxo expiratório, manovacuometria, teste de caminhada de seis minutos e teste de escada. Os dados foram colhidos no período pré-operatório e correlacionados com as complicações pós-operatórias, incluindo as complicações cardiopulmonares e cirúrgicas. Resultados: A média de idade foi de 41,59 ± 10,70 anos. O índice de massa corpórea variou de 35,44 a 62,79 Kg/m2, com média de 46,74 ± 6,35 Kg/m2. The bariatric surgery consists of an alternative for surgical treatment of morbid obesity, thus allowing significant and lasting weight loss with consequent improvement of the related comorbidities. Obesity is associated with the changes in the ventilatory mechanics and poses a risk factor for the development of pulmonary complications in the post-operative period of abdominal surgeries. Objective: Check whether the respiratory function tests, the respiratory muscle strength and the exercises are able to foresee post-operative complications in patients undergoing bariatric surgery. Methodo: 174 patients (81% women) undergoing bariatric surgery were evaluated at the Clinical Hospital of the Botucatu School of Medicine. All the patients were submitted to anthropometric measures, spyrometry tests, the measurement of peak expiratory flow, manovacuometry, a 6-minute walking test and stair-climbing test. Assim, a avaliação e a terapia nutricional implementadas durante o perioperatório são de fundamental importância para que os indivíduos apresentem a melhor resposta possível a injúria decorrente da cirurgia, associada ao menor período de recuperação, de preferência sem complicações. A avaliação nutricional se mostra complexa a medida que leva em consideração vários fatores já que modernamente o conceito de desnutrição é amplo e avalia também o estado de inflamação sistêmico e não apenas o IMC. Em tempos nos quais temos cada vez mais pacientes obesos, porém desnutridos, uma avaliação ampla e multifatorial é imprescindível. Höftfrakturer är en vanlig ortopedisk orsak till sjukhusvård hos äldre. Att vara äldre och drabbas av höftfraktur är förknippat med hög risk för död. Stora fysiologiska avvikelser preoperativt samt försenad operation är kopplat till ökad risk för komplikationer och död. Äldre, sköra patienter har små marginaler och kan i samband med operation av höftfraktur behöva intensivvård. Det har uppmärksammats på ett sjukhus i Västra Götaland att denna patientgrupp vid ankomst till preoperativ avdelning ibland ej bedöms vara i operabelt skick, vilket medför ett förehavande där patienten förs till postoperativa intensivvårdsavdelningen för optimering inför operation. Syftet med studien är att undersöka hur det postoperativa förloppet hos äldre patienter med akuthöftfraktur påverkas efter införande av preoperativ optimering, stöttat av intensivvårdssjuksköterskan, på vårdavdelning. Studien har en kvantitativ ansats med en kombination av kvantitativ retroperspektiv registerstudie och en kvantitativ prospektiv interventionsstudie. Sammanställningen påvisar en minskning av den totala vårdtiden från 9 dagar till 6 dagar i median. Sammanfattningsvis betonas att intensivvårssjuksköterskan tillsammans med övriga professioner i teamet runt patienten har betydelse för att förbättra den äldre patientens möjlighet att återhämta sig efter akut höftfraktur.
