Paternal Exposure to Ionizing Radiation in Ontario Uranium Miners and Risk of Congenital Anomaly in Offspring: A Record Linkage Case-control Study

Objective: To determine if paternal preconception exposure to ionizing radiation through uranium mining increases the risk of congenital anomaly (CA) in offspring. Methods: A population-based matched case-control study was conducted. Cases were infants with CAs recorded in the Canadian Congenital Anomalies Surveillance System and born alive in Ontario 1979-86 (ICD-9 codes 740-759); controls were liveborn infants without CAs identified from Ontario birth certificates and individually matched to cases (case-control file {CCF}). Exposed fathers were identified through the linkage of the CCF to the Mining Master File or the National Dose Registry file, which include those who worked in Ontario uranium mines 1952-1986. For men who linked with a case or control child, radon, gamma and total gonadal doses were estimated for three preconception periods: entire, 3-months and 6-months. Odds ratios were estimated using conditional logistic regression. Results: Linkage of 28,991 uranium miners and 40,482 case-control pairs of fathers and offspring in the CCF identified 431 discordant pairs. There was no evidence of increased risk of a child having a CA if the father was ever a uranium miner before conception of the child (OR=0.89, 95% CI=0.74–1.08). Since gamma radiation (especially during the 6-month preconception period) is more biologically relevant to gonads than radon, further analyses were performed on 117 discordant pairs where data on gamma exposures were available. When ever/never miner, exposed to gamma (yes/no), and gamma dose-response variables were all in the model, there was no ever/never miner effect (OR=1.20, 95% CI=0.85–1.69, p-value=0.30), an inverse association for exposure to gamma (OR=0.42, 95% CI=0.25–0.71, p-value=0.001), but most importantly, there was no statistically significant dose-response relationship between gamma dose during the 6-month preconception period and all CAs (OR=1.15 per loge {mSv+0.01}, 95% CI=0.83–1.59, p-value=0.40). Similarly, no dose-response relationship was observed for exposure to gamma radiation in the 3-month preconception period, or for radon or total gonadal radiation in the 3- or 6-month preconception periods. Conclusion: There was no increased risk of a CA among liveborn children of Ontario uranium miners who were exposed to radon, gamma or total radiation during the 3- or 6-month periods before conception.

Αξιολόγηση των επιπέδων της AFP και β-hCG στον ορό της εγκύου κατά το β' τρίμηνο τησς κύησης με σκοπό την πρόβλεψη δυσμενούς έκβασης της κύησης

Σκοπός της παρούσας προοπτικής µελέτης είναι η διερεύνηση της σχέσης µεταξύ των επιπέδων της α-φετοπρωτείνης και της β-hCG στον ορό της µητέρας κατά το β' τρίµηνο της κύησης και της δυσµενούς έκβασης της κύησης σε τυχαίο δείγµα εγκύων γυναικών της Νοτιο-Δυτικής Ελλάδος. Υλικό - Μέθοδος: 130 υγιείς γυναίκες ελληνικής καταγωγής µε αυτόµατη έναρξη της εγκυµοσύνης µελετήθηκαν για τα επίπεδα της AFP και της β-hCG στον ορό τους µεταξύ της 13ης-24ης εβδοµάδας της κύησης και παρακολουθήθηκαν για δυσµενή έκβαση της εγκυµοσύνης. Επίπεδα AFP καθώς και επίπεδα β-hCG στον ορό της εγκύου 2 φορές µεγαλύτερα του µέσου όρου για την ηλικία της κύησης θεωρήθηκαν µη φυσιολογικά. Η στατιστική ανάλυση έγινε µε τη δοκιµασία Pearson's x2. Αποτέλεσµα: Αυξηµένα επίπεδα AFP στον ορό της µητέρας βρέθηκαν σε 27 από τις 130 γυναίκες που µελετήθηκαν (20,7%). Μεταξύ αυτών µόνο 4 γυναίκες (14,8%) ανέπτυξαν επιπλοκές της κύησης. Επίσης αυξηµένα επίπεδα β-hCG βρέθηκαν σε 14 από τις 130 γυναίκες που µελετήθηκαν (10,77%). Όµως µεταξύ αυτών καµία γυναίκα δεν ανέπτυξε επιπλοκές κατά τη διάρκεια της παρούσας κύησης. Συµπέρασµα: Ένας πολυπαραγοντικός έλεγχος της λειτουργίας του πλακούντα κατά το β' τρίµηνο της εγκυµοσύνης µε Doppler των µητριαίων αρτηριών, µε έλεγχο της µορφολογίας του πλακούντα, µε έλεγχο των επιπέδων της AFP και της β-hCG στον ορό της εγκύου δυνατό να µας επιτρέψει την αναγνώριση εκείνων των εγκύων µε αυξηµένο κίνδυνο να αναπτύξουν σοβαρή πλακουντιακή ανεπάρκεια και επιπλοκές της κύησης. / In our prospective study, we investigated the association between midtrimester maternal serum AFP (ms-AFP) and midtrimester maternal serum β-hCG (ms-β- hCG) levels and adverse pregnancy outcome, in the South-Western Greek population. Materials and Methods: 130 healthy Greek women with spontaneous pregnancies, investigated for ms-AFP levels between 13th-24th weeks of gestation and followed for adverse pregnancy outcome. AFP levels > 2,0 multiples αnd β-hCG levels>2,0 multiples of the median value for gestation, were considered abnormal. Statistical analysis was performed by Pearson's chi-square test. Results: Elevated ms-AFP levels were detected in a total of 27 from the 130 women studied (20,7%). Among them, only 4 women (14,8%) developed pregnancy complications. Elevated ms-b-hCG levels were detected in total of 14 from the 130 women studied (10,77%). Νοne of them developed pregnancy complications. Conclusion: Multiparameter testing of placental function in the mid-trimester uterine artery Doppler, placental morphology, ms-AFP and ms-hCG screening may allow us to identify women with increased risk to develop severe placental insufficiency and pregnancy complications.

Práticas obstétricas e influência do tipo de parto em resultados neonatais e maternos em Sergipe / Obstetric practices and the influence of mode of delivery on neonatal and maternal outcomes in Sergipe

Background: Brazil has high frequency of inappropriate obstetric practices and cesarean sections. This procedure may be associated with increased maternal and perinatal morbidity. Objective: to describe practices and interventions used during labor and childbirth and factors associated with such practices and to evaluate the impact of mode of delivery in breastfeeding incentive practices and in neonatal and maternal complications. Methods: A Cohort study was conducted, between june 2015 and april 2017, at the 11 maternity hospitals in Sergipe with 768 puerperal women, interviewed in the first 24 hours after delivery, 45-60 days and 6-8 months after delivery. The associations between good practices and interventions used during labor and delivery with exposure variables were described by simple frequencies, percentages, crude and adjusted odds ratios (ORA) with the confidence interval and the association between breastfeeding incentive practices, neonatal and maternal, both short term and late complications and the exposure variables were evaluated by the relative risk (95% IC) and the Fisher exact test. Results: were fed 10.6% of women and 27.8% moved during labor; non-pharmacological measures for pain relief were performed in 26.1%; the partograph was filled in 39.4% of the charts and the companion was present in 40.6% of deliveries. Oxytocin, amniotomy and analgesia occurred in 59.1%, 49.3% and 4.2% of women, respectively. The delivery occurred in the lithotomy position in 95.2% of the cases, there was episiotomy in 43.9% and Kristeller's maneuver in 31.7%. The factors most associated with cesarean section were the private health sector (ORA = 4.27,95% CI: 2.44-7.47), had higher education (ORA = 4.54,95%CI 2.56 -8.3) and high obstetric risk (ORA = 1.9,95%CI: 1.31-2.74). Private-sector users had a greater presence of the companion (ORA = 2.12,95% CI:1.18-3.79) and analgesia (ORA = 4.96,95% CI:1.7-14.5). The C-section delivery resulted in less skin-to-skin contact immediately after delivery (intrapartum c-section: RR=0.18;95%CI:0.1-0.31 and elective c-section: RR=0.36;95%CI:0.27-0.47) and less breastfeeding within one hour of birth (intrapartum C-section: RR=0.43;95%CI:0.29-0.63 and elective C-section: RR=0.44;95%CI:0.33-0.59). Newborns from elective c-section were less frequently breastfed in the delivery room (RR=0.42;95%CI:0.2-0.88) and stayed less in rooming- in (RR=0.85;95%CI:0.77-0.95). Women who were submitted to intrapartum c-section had greater risk of early complications (RR=1.3;95%CI:1.04-1.64; p=0.037) and sexual dysfunction (RR=1.68;95%CI:1.14-2.48; p=0.027). There was no difference in the frequency of neonatal complications, urinary incontinence and depression according to the mode of delivery. Conclusions: good obstetric practices are poorly performed and unnecessary interventions are frequent, and the factors most associated with cesarean delivery have been the private health sector, higher schooling and high obstetric risk. C-section was negatively associated to breastfeeding incentive practices. C-section after labor increased the risk of early maternal complications and sexual dysfunction, six to eight months after delivery. / Introdução: No Brasil, verifica-se elevada frequência de práticas obstétricas inadequadas e de cesáreas. Este procedimento pode associar-se a aumento de risco de morbidade materna e neonatal. Objetivo: descrever as práticas utilizadas durante o trabalho de parto e parto e fatores associados e avaliar práticas de incentivo à amamentação, complicações neonatais e maternas precoces e tardias segundo tipo de parto. Pacientes e Métodos: estudo tipo coorte, no período de junho de 2015 a abril e 2016, nas 11 maternidades de Sergipe, com 768 puérperas entrevistadas após 6h do parto, 45 a 60 dias e 6 a 8 meses após o parto e análise de dados do prontuário das puérperas e dos recém-nascidos. As associações entre as boas práticas e intervenções utilizadas durante o trabalho de parto e parto com as variáveis de exposição foram descritas em frequências simples, percentuais, razões de chances brutas (OR) e ajustadas (ORA) com o intervalo de confiança e as associações entre as práticas de incentivo à amamentação, as complicações neonatais e maternas precoces e tardias e as variáveis de exposição foram descrias por risco relativo (IC=95%) e pelo teste exato de Fisher. Resultados: alimentaram-se 10,6% das mulheres e 27,8% movimentaram-se durante o trabalho de parto; medidas não farmacológicas para alívio da dor foram realizadas em 26,1%; o partograma estava preenchido em 39,4% dos prontuários e o acompanhante esteve presente em 40,6% dos partos. Ocitocina, amniotomia e analgesia ocorreram em 59,1%, 49,3% e 4,2% das mulheres, respectivamente. O parto ocorreu na posição de litotomia em 95,2% dos casos, houve episiotomia em 43,9% e manobra de Kristeller em 31,7%. Os fatores mais associados à cesárea foram ser do setor privado de saúde (ORA=4,27;95%CI:2,44-7,47), ter maior escolaridade (ORA=4,54;95%CI:2,56-8,3) e alto risco obstétrico (ORA=1,9;95%CI:1,31-2,74). Usuárias do setor privado tiveram maior presença do acompanhante (ORA=2,12;95%CI:1,18-3,79) e analgesia (ORA=4,96;95%CI: 1,7-14,5). Os recém-nascidos de puérperas que se submeteram a cesárea tiveram menor frequência de contato pele a pele com suas mães imediatamente após o parto (cesárea intraparto: RR=0,18;95%CI:0,1-0,31 e cesárea eletiva: RR=0,36;95%CI:0,27-0,47) e mamaram menos na primeira hora de vida (cesárea intraparto: RR=0,43;95%CI:0,29-0,63 e cesárea eletiva: RR=0,44; 95%CI:0,33-0,59). Recém-nascidos de cesárea eletiva foram menos frequentemente colocados para mamar na sala de parto (RR=0,42;95%CI:0,2-0,88) e ficaram em menor frequência em alojamento conjunto (RR=0,85;95%CI:0,77-0,95). As mulheres submetidas a cesárea intraparto tiveram maior risco de complicações precoces (RR=1,3;95%CI:1,04-1,64; p=0,037) e de disfunção sexual (RR=1,68;95%CI:1,14-2,48; p=0,027). Não houve diferença nas frequências de complicações neonatais, incontinência urinária e de depressão segundo tipo de parto. Conclusões: boas práticas obstétricas são pouco utilizadas e intervenções desnecessárias são frequentes e os fatores mais associados à operação cesariana foram ser do setor privado de saúde, ter maior escolaridade e alto risco obstétrico. A cesárea associou-se negativamente às práticas de incentivo à amamentação. A cesárea após trabalho de parto associou-se a maior risco de complicações maternas precoces e a disfunção sexual seis a oito meses pós-parto. / São Cristóvão, SE

INTRODUÇÃO: O antecedente de parto prematuro espontâneo em gestação anterior é considerado o principal e mais importante fator de risco clínico para prematuridade, principal causa de morbidade e mortalidade neonatal. Cerca de 25% das pacientes que tiveram parto prematuro apresentarão recorrência. A prevenção secundária consiste na pesquisa de marcadores de maior risco, com o intuito de instituir medidas terapêuticas apropriadas e de evitar tratamentos desnecessários. A hipótese do presente estudo é a de que existe correlação entre os resultados da avaliação do colo uterino e do teste para proteína-1 fosforilada ligada ao fator de crescimento insulina-símile (phIGFBP-1) e que a utilização de ambos em associação possa predizer a ocorrência de parto prematuro com maior sensibilidade. OBJETIVOS: Averiguar a utilidade da medida do comprimento do colo uterino e do teste para phIGFBP-1 na predição do parto prematuro antes de 37 e de 34 semanas, a existência de relação dos testes entre si, o melhor valor de corte da medida do colo em diferentes idades gestacionais e a melhor época de realização de cada um dos exames. MÉTODO: Foram compilados e submetidos a análise secundária os dados de 101 gestantes com antecedente de parto prematuro atendidas no Setor de Baixo Peso Fetal da Clínica Obstétrica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, entre 2003 e 2008. A medida do comprimento cervical e o teste para phIGFBP-1 foram realizados a cada três semanas, entre 24 e 34 semanas de gestação, e comparados com o desfecho de parto prematuro e nascimento com 34 semanas ou menos, e o melhor valor de corte do colo uterino foi estabelecido por meio de curva de características operacionais. RESULTADOS: Das 101 gestações estudadas, 25 (24,8%) terminaram em parto prematuro, das quais 12 (11,9%) ocorreram com 34 semanas ou menos. As idades gestacionais médias de avaliação foram de 24, 27, 30 e 33 semanas, e os valores de corte do colo uterino foram de 22, 21, 20 e 16 mm, respectivamente. A medida do comprimento do colo apresentou maior sensibilidade (cerca de 70%) e foi capaz de predizer o parto prematuro em todas as avaliações. O teste para phIGFBP-1 não foi útil com 24 semanas, porém foi capaz de detectar de forma independente o risco de prematuridade com 27, com 30 e com 33 semanas. Houve associação estatística dos exames entre si, de forma que o comprimento cervical médio foi menor em gestantes com teste positivo para phIGFBP-1. A associação dos exames elevou a sensibilidade e o valor preditivo negativo de forma significativa. CONCLUSÕES: A medida do comprimento do colo pela ultrassonografia transvaginal constitui bom marcador de risco para parto prematuro com 24 semanas, e o teste para phIGFBP-1 é útil após 27 semanas. About 25% of these patients will deliver prematurely again. Secondary prevention consists in the search for markers of increased risk, in order to institute appropriate therapeutic actions and to avoid unnecessary treatments. The hypothesis of this study is that there is a correlation between the results of the evaluation of the cervix and the test for phosphorylated insulin-like growth factor binding protein-1 (phIGFBP-1) and that the use of both in combination can predict the occurrence of preterm delivery with higher sensitivity. OBJECTIVES: To investigate the usefulness of the measurement of the cervical length and phIGFBP-1 rapid test in the prediction of preterm birth before 37 and 34 weeks, the existence of a relationship between the tests themselves, the best cutoff value of cervical length measurement at different gestational ages and the best time to carry out each of the exams. METHODS: Data of 101 women with previous preterm birth assisted at the Obstetrical Clinic of the Hospital das Clínicas, Faculty of Medicine, University of São Paulo between 2003 and 2008 were collected and subjected to secondary analysis. The measurement of cervical length and the phIGFBP-1 test were performed every three weeks, between 24 and 34 weeks gestation, and compared with the outcome of premature birth before 37 and 34 weeks, and the best cutoff value of the cervix was determined by receiver operator characteristic curves. RESULTS: Of the 101 pregnancies studied, 25 (24.8%) ended in preterm birth, of which 12 (11.9%) occurred at 34 weeks or less. The mean gestational age in each evaluation was 24, 27, 30 and 33 weeks, and the cutoff of the cervix were 22, 21, 20 and 16 millimeters, respectively. The measurement of cervical length showed the highest sensitivity (approximately 70%) and was able to predict preterm birth in all evaluations. The phIGFBP-1 test was not useful at 24 weeks, but was able to independently detect the risk of prematurity at 27, 30 and 33 weeks. Statistical association between the exams was observed, so that the mean cervical length was lower in pregnant women testing positive for phIGFBP-1. The combination of both tests significantly increased the sensitivity and negative predictive value. CONCLUSIONS: The measurement of cervical length by transvaginal ultrasound is a good marker of risk for preterm delivery at 24 weeks, and the test for phIGFBP-1 is useful after 27 weeks. The association of the two tests is valuable and shows high sensitivity and high negative predictive value in women at high risk for spontaneous preterm birth, when the first is preformed with 24 weeks, and the second with 27 weeks
