The Role of Military Culture: Military Values and Perceived Stigma as Predictors of Psychological Help-Seeking Propensity

Despite the relatively high availability of free or low-cost psychological care many service members and veterans choose not to seek services (Vogt et al., 2014). This paper aimed to investigate whether military culture moderates the relationship between stigma and help-seeking intentions among service members. Prior work has suggested that service member underutilization of services may be the result of military culture and its emphasis on placing the mission above personal needs but this finding has not been clearly demonstrated in the literature. The findings of this study revealed that high commitment to military values coupled with heightened perceptions of stigma may play an important role in a service member’s decision to seek out psychological treatment. These results lend support to the effects of commitment to core military values and perceived stigma on psychological help-seeking intentions and demonstrate the need to consider these factors when tailoring psychological interventions for military personnel.

Além disso, a mesma se propõe a identificar e compreender os aspectos técnicos desta modalidade de acompanhamento, realizando uma reflexão teórica e técnica a partir do referencial psicanalítico. A metodologia utilizada foi o estudo de caso. Foram apresentados e discutidos os relatos de dois casos clínicos, de João e Luciana, que receberam atendimento psicológico domiciliar por seis meses. O desenvolvimento tecnológico contemporâneo permite a detecção precoce de malformações fetais, tais como a anencefalia, que inviabilizam a sobrevivência do bebê. Tal constatação implica imediata revelação diagnóstica aos pais, que deverão decidir pela continuidade ou interrupção da gravidez. O presente trabalho tem como objetivo investigar a experiência emocional de casais que se deparam com o diagnóstico de anencefalia fetal, tendo em vista trazer subsídios para um melhor atendimento psicológico. A investigação organizou-se como pesquisa qualitativa, com método psicanalítico, estruturada ao redor de procedimentos investigativos de acesso, registro, interpretação e interlocuções reflexivas sobre atendimentos clínicos. A partir de sessões com casais parentais, realizadas ao longo de oito anos, foram elaboradas duas narrativas transferenciais ficcionais, que preservam elementos essenciais da dramática em pauta. Uma das narrativas aborda uma situação de opção por interrupção gestacional, enquanto a outra focaliza uma decisão de continuidade. Os procedimentos interpretativos permitiram a \"criação/encontro\" dos seguintes campos de sentido afetivo-emocional ou inconscientes relativos: É um pesadelo?, Quem ou o quê está aí?, É preciso decidir. O quadro geral aponta que grande parte do trabalho clínico tem lugar num campo bastante singular, É preciso decidir, que se define pela urgência de tomada de decisão relativa à eventual interrupção de processos vitais. O campo É um pesadelo? aponta para o fato deste tipo de revelação diagnóstica, derivada do uso de uma tecnologia, que detecta problemas que não estão sendo vivenciados como sinais ou sintomas físicos, gerar muito frequentemente reações dissociativas, cujo manejo torna-se, assim, clinicamente indispensável. O outro campo, Quem ou o quê está aí?, assume uma posição de centralidade, nesta clínica, na medida em que porta consigo uma interrogação radical acerca do estatuto ontológico do feto, vivido como um bebê ou como um não-bebê. This paper focuses on the investigation of the emotional experience that couples have when facing a fetal anencephaly diagnosis, bringing instruments for a better psychological care. From the sessions with couples, during eight years, two transferential fictional narratives, that preserve essential elements of drama at stake, were created. One of the narratives approaches a situation of choice for pregnancy interruption, while the other aims at the decision to keep it. The interpretative procedures allowed the \"creation / finding\" of the following fields of affective-emotional sense or relative unconscious: Is it a nightmare?, Who or what is there?, We have to make a decision. Policejní stres se zaměřením na genderové rozdíly / Police Stress and Gender Differences

This diploma thesis concerns on very current and important topic, which is the stress of police officers. The text deals in particular with the possible causes that can give rise to stress among police officers and with the differences between men and women in the case of police stress. The third chapter also summarizes the possibilities of psychological support at the Police of the Czech Republic. The thesis also includes research that has a quantitative and qualitative part. The aim of this research is to find out which situations are the most important sources of stress for police officers and whether these resources vary depending on the sex of the police officer. Quantitative research was conducted using the Work Stress Inventory (IPSS) on a sample of 116 respondents. It has been found that the most significant stressor is the excessive amount of administrative activity for police officers. The research also showed a significantly higher level of police stress among female police officers. The qualitative survey was conducted using a semi-structured interview. The sample was six respondents. The interview was conducted mainly to determine possible interpretations of the results of the previous quantitative survey.

The research focus was on the meaning given by clients to their experiences. The researcher constituted a dialogical relationship with participants and, by doing so was able to access the meanings of their experiences and to communicate them through comprehensive narratives inspired by the ideas of Walter Benjamin. Narrative emerged as a proper methodological strategy to present and analyze patients experiences on family therapy as it was told to the researcher in a clinical setting. After all the families were interviewed a narrative synthesis was written with the main meanings of their experiences as they were understood by the researcher. These interpretations pointed a way that made possible the proposition of some guidelines for the clinical practice with families in institutional settings: 1) the therapy may work even if only two family members take part in it; 2) an easy and neutral attitude by the therapist is positive for the clients; 3) the therapeutical conversation is a model for the family in other contexts; 4) the reflexive conversation between the therapists or between a therapist and a family member is a model for changes in the family dynamics; 5) the couple or any other part of the family may be counseled apart, as a way to deal with specific issues that may influence other family members; 6) the families appreciates the conversations where they can speak, hear and be heard; 7) Even the persons who did not volunteer to go to therapy saw it as an opportunity for positive changes; 8) The pre-settled number of therapy sessions may be changed in a process where the decision is shared with the patients. / Este estudo buscou apreender e interpretar, fenomenologicamente, elementos significativos da experi?ncia vivida por pessoas que conclu?ram um processo de terapia de fam?lia. O foco da investiga??o localizou-se em como os clientes significam a experi?ncia vivida. Foi desenvolvido como uma pesquisa fenomenol?gica na qual o pesquisador constituiu uma rela??o dial?gica com os participantes, a partir da qual apreendeu significados e os comunicou sob a forma de narrativas compreensivas. A narrativa, inspirada nas id?ias de Walter Benjamin, emergiu como uma estrat?gia metodol?gica adequada para apresentar e analisar as experi?ncias de pessoas atendidas em terapia de fam?lia, relatadas ao pesquisador em uma situa??o cl?nica. The investigation was characterized as an exploratory qualitative research of phenomenological inspiration. Individual encounters with nine psychologists ocurred from April to September of 2013. As a strategy for the participants experience based on the researcher's impressions. A narrative-synthesis emerged from all the individual narratives pointing to interpretative elements relatd to the studied phenomenon: (1) being exposed to constant threats to the their physical and psychological integrity and living with risk becomes an inherent part of this professional practice; (2) belonging to an organization that manages the team's actions allows a certain structure for their interventions and also offers support and care to the professionals; (3) psychological care develops in harmony eith the specificities and demands of the situation, developing from attitudes of empathy and acceptance, as well as from the authenticity of the psychologist himself; (4) psychologists revealed feelings of sefrealization as a result of participating in extreme situations, and a desire to remain in a state of readiness for action. (5) working in extreme situations is a fertile and peculiar opportunity of learning. These results revealed the meaning of the psychological practice in extreme situations as human gesture of casting oneself in the direction of others as a mission to care for and facilitate the clinical situation. / Esta pesquisa objetivou compreender e interpretar a experi?ncia de psic?logos que atuaram em diversos contextos considerados como situa??es extremas, incluindo as denominadas abruptas - desastres ambientais e acidentes a?reos - e as cr?nicas - conflitos armados, p?s-guerras e epidemias. Caracterizou-se como pesquisa explorat?ria de inspira??o fenomenol?gica. Foram realizados encontros individuais com nove psoc?logos no per?odo de abril a setembro de 2013. Como estrat?gia para a an?lise dos encontros foram elaboradas narrativas que desvelaram os significados da experi?ncia dos participantes a partir da compreens?o da pesquisadora. Impact d’une prise en charge par la relaxation psychothérapique sur l’ajustement émotionnel, la qualité de vie et l’image du corps de patients atteints d’un cancer hématologique, hospitalisés en secteur stérile pour une greffe de moelle osseuse / Impact of psychotherapeutic relaxation interventions on emotional adjustment, quality of life and body image in hematologic cancer patients hospitalized in a sterile hospital's area for a bone marrow transplantation

Introduction : La prise en charge psychologique de patients pendant le processus de greffe demoelle osseuse pour un cancer hématologique fait partie intégrante du parcoursthérapeutique. Cette étude a deux objectifs : (1) tester l’impact d’une prise en charge par larelaxation psychothérapique, Méthode Sapir à court et à moyen terme, auprès de cettepopulation (2) savoir si des variables dispositionnelles (alexithymie, optimisme) et desvariables transactionnelles (stratégies de coping, soutien social perçu) peuvent influencer cetimpact.Méthode : 39 patients ont été inclus. Le groupé expérimental (N=20) a bénéficié d’une priseen charge par la relaxation psychothérapique tandis que le groupe contrôle (N = 19) a eu unsuivi psychologique traditionnel. Les patients ont été évalués 4 jours avant la greffe (T1),puis 45 jours (T2) et 100 jours (T3) après la greffe. Les variables dispositionnelles(alexithymie, optimisme), les variables transactionnelles (coping, soutien social perçu) et lesissues à prédire (état émotionnel, image du corps et qualité de vie) ont été mesurées.Résultats : A T3, les patients du groupe expérimental sont significativement moins anxieux(p<0,001), éprouvent une meilleure satisfaction corporelle (p<0,01) et ont une meilleureQDV physique (p≤0,001) qu’à T1. Chez les patients du groupe contrôle aucun changementsignificatif n’est observé avec le temps. Par ailleurs, aucune différence significative entre lesdeux groupes n’a été trouvée aux trois temps de l’étude. A T3, les patients alexithymiques dugroupe expérimental sont significativement moins dépressifs (p<0,05), ont une meilleureQDV cognitive (p<0,01) et émotionnelle (p<0,01) qu’à T1.A T3, les patients les plusoptimistes du groupe expérimental rapportent une meilleure QDV physique (p<0,05) etémotionnelle (p<0,05) qu’à T1.Chez les patients du groupe contrôle aucune modération del’alexithymie et de l’optimisme sur l’ajustement au cancer n’est observée. Enfin, l’effet demédiation du coping et du soutien social perçu n’a pas été démontré dans cette étude.Conclusion : La prise en charge par la relaxation psychothérapique a une place auprès decette population. L’illustration de deux cas cliniques vient confirmer ces propos. Cependant,au regard du faible nombre de sujets, il convient de contre-valider ces résultats auprès d’unepopulation plus large. / Introduction: The psychological care of patients during the process of bone marrowtransplantation for a hematological cancer is part of the therapeutic process. This study hastwo objectives: (1) to test at a short and medium term the impact of psychotherapeuticrelaxation, Sapir’s method, on the psychological adjustment of patients with hematologiccancer hospitalized at a sterile hospital’s room for bone marrow transplantation; (2) toexamine weather dispositional variables (alexithymia, optimism) and transactional variables(coping, perceived social support) can influence this impact.Method: 39 patients were included. The experimental group (N=20) received a care ofpsychotherapeutic relaxation while the control group received a traditional psychologicalfollow-up. Patients were evaluated 4 days before transplantation (T1), 45 days aftertransplantation (T2) and 100 days post-transplantation (T3), respectively. Personalityvariables (alexithymia, optimism), adjustment of cancer variables (emotional state, bodyimage, quality of life) and transactional variables (coping, perceived social support) weremeasured.Results: The patients in the experimental group at T3 are significantly less anxious(p<0.001), experienced a better body satisfaction (p<0.01) and a better physical quality oflife (p≤0.001) than at T1. In patients of the group control, no significant positive or negativemovements are observed by the time on studied variables. Moreover, no significantdifferences are found between two groups on three times of study. The alexithymic patientsof the experimental group are significantly less depressed at T3 (p<0.01) and experienced abetter cognitive (0.01) and emotional (p<0.01) quality of life. The most optimistic patients ofthe experimental group have a better physical (p<0.05) and emotional (p<0.05) quality oflife. In patients of the control group, no moderation of alexithymia and optimism is observedon cancer adjustment. Finally, the mediating effect of coping and perceived social support isnot demonstrated in this study.Conclusion: The psychotherapeutic relaxation has an important place to this population. Thisis confirmed by the illustration of two cases. Finally, due to the small number of subjectsincluded, these results require validation on a larger population.

This research aimed to grasp, phenomenologically, the experience of the physical therapy trainees regarding their relationship with outpatients at a university clinic. It is an exploratory qualitative research, phenomenologically inspired. Eight individual encounters were carried out with undergraduates who are in their seventh, ninth and tenth semesters. After each of the encounters, the researcher wrote a first-person narrative, based on her impressions of the meaning of the lived experience of the participants. A narrative synthesis was then developed in order to capture the meaning of the experience as such. The analysis made it possible to conclude that trainees strive to offer a treatment that includes, not only technique but also attitudes of respect, warmth and welcome. Caring for patients with neurological problems, especially those involving progressive degeneration, is signified by the interns as unrewarding since recovery is impossible and verbal communication is difficult. They also feel distressed, as they perceive themselves emotionally involved with the patients, despite the attempt to maintain a strictly professional approach. On the other hand, in a broad way, this relationship is referred to as an opportunity for new learnings and psychological maturity with a potential for growth both in personal and professional life. / Inspirada no modelo biom?dico de sa?de, a pr?tica da fisioterapia possui caracter?sticas espec?ficas no que se refere ao atendimento aos pacientes do ponto de vista psicol?gico. O contato f?sico e a frequ?ncia das sess?es acarretam o desenvolvimento de um relacionamento interpessoal que esta pesquisa objetivou apreender fenomenologicamente a partir da experi?ncia de alunos de uma faculdade de Fisioterapia. Foram realizados encontros individuais com oito graduandos que cursavam o s?timo, nono e d?cimo per?odos do Curso com foco em suas atividades como estagi?rios no ambulat?rio de uma cl?nica universit?ria. Ap?s cada um desses encontros, a pesquisadora construiu uma narrativa a partir de sua compreens?o sobre os significados da experi?ncia para o participante. Ap?s concluir esta fase, elaborou uma narrativa-s?ntese contendo o conjunto de significados relatados pelos participantes a fim de interpretar o sentido da experi?ncia como tal. Este processo de an?lise possibilitou concluir que os alunos empenham-se em oferecer um tratamento que inclua, al?m das t?cnicas, atitudes de respeito, cordialidade e acolhimento. Desenvolvem uma vis?o do paciente como pessoa e n?o apenas como caso cl?nico, embora tenham sido treinados para um desempenho t?cnico. Por outro lado, sentem-se angustiados ao se perceberem envolvidos emocionalmente, a despeito da inten??o inicial de manterem uma postura estritamente profissional. Atender pessoas que apresentam problemas neurol?gicos, em especial aqueles que implicam progressiva degenera??o, ? significado pelos participantes como pouco gratificante, em fun??o da impossibilidade de recupera??o dos movimentos e da dificuldade de comunica??o verbal. Consideram que os atendimentos possibilitam uma aprendizagem necess?ria e importante que promove amadurecimento psicol?gico com potencial transformador, tanto para a vida pessoal quanto profissional.
