Dependency in the Clinical Ecology Patient

Dependency is defined as authentic or pathological and is seen as a component important to the treatment of patients with chronic illness. It is hypothesized that a significant portion of ecology patients will meet the criteria for pathological dependence and differ on psychological and physiological parameters from those who do not. This study strongly supports the first two hypotheses but does not find that the two groups differ physiologically. One hundred eleven variables are surveyed. Fifty-two show significant differences between the groups and 29 are significant at greater than the .0001 level. A discriminant analysis was used to determine the least number of orthogonal variables that best discriminate between the groups. These are MMPI Scales 8, 3, subscale Ma2, employment status, and early childhood illness.

Minding the body : questions of embodiment and the practice of psychoanalytic psychotherapy.

It is well understood that psychoanalysis began with Freud’s encounter with hysteria and his work with illnesses of the mind which manifested in bodily terms. However, despite its close connection to the body and the understanding that psychoanalytic theory and practice develop hand-in-hand, psychological conflict that expresses itself in physical terms and more especially the role of the two bodies in the therapy room has received relatively little attention. The topic of this research project is captured in its title: “Minding the Body”, and the four journal articles it presents interrogate the relationship between the mind and body of both the patient and therapist. The thesis begins with two published papers which focus on the body of the patient, rehearsing and extending the psychoanalytic theory of bodily psychopathology and the implications that the different understandings of the relationship between body and mind in different forms of psychosoma have for clinical interventions. The second two papers examine what the analyst’s interpretation of her somatic responses to the patient, and the patient’s engagement with the analyst’s body, can reveal about the dynamics of the therapeutic dyad. The project concludes with a discussion of the clinical implications of a greater focus on the two bodies in the room, suggesting that the techniques developed to make sense of the patient’s physical symptoms can be usefully applied to decode the somatic countertransference as it manifests in a particular therapeutic dyad. That process, coupled with an awareness of the patient’s engagement with the therapist’s body, can create conditions under which the analyst’s body may become an analytic object and this can add significantly to the analytic repertoire.

Despite being recognized as a pathology by the World Health Organization it fits in a different profile about what is a disease, due the fact of being named as a Non-transmissible Chronic Disease, a concept that demands a rethink about what would be health and illness towards a condition that deteriorates the human being, both physically and mentally, although in a slow manner and not an acute one. The rate in which obesity has reached epidemic proportions in several countries also leads to a deep consideration about the historical and cultural aspects that brought it to the current prevalence in several places of the planet nowadays. By becoming the object of study of several fields of the human knowledge, obesity ends up producing a permanent focus of tension between the several branches of science that study it, since each of them highlight one or another factor of the many that compose it. The proposal of this research that is presented throughout the text seeks to unite relevant information regarding obesity that belong to many areas of the scientific knowledge, gathering them in a way to produce a questioning about the status of obesity towards Psychoanalysis. When questioning the possibility of obesity to become an object of study of the psychoanalytical research the need for metapsychological foundation arises that would allow representing it both within the theory and practice in Psychoanalysis. The search for concepts that could explain the psychological suffering of the obese brought to the text a study about the founding of the feeding function, as of the feeling of satiety and other early body sensations, and the possibility that a perversion in the usage of such sensations may be used in the way that obese use them to handle the daily challenges that are brought upon their psyche. The obese body also carries a questioning about the notion of body in Psychoanalysis that could be useful in its understanding, as in which treatment methods would prove to be useful in such specific cases. The contribution of Hilde Bruch, psychoanalyst, is presented as one of the keystones upon which this text could be written, specially by suggesting the existence of two types of obesity, reactive and development, and also a specific therapeutic approach to both cases. Through the description of two clinical cases the obesity types and their treatment are portrayed, which aims to corroborate the metapsychological description that uses the actual neurosis model as a suggested concept to explain the psychological suffering of the obese, marked by an anxiety that lacks symbols to express itself. The ultimate proposal of this text is that obesity may be an important point of convergence for the dialogue between many scientific fields, including Psychoanalysis, that can produce rich contributions in future studies, especially if an interdisciplinary dialogue is pursued / A obesidade é uma das condições que afetam a saúde humana de forma especialmente intensa no século XXI. Apesar de ser reconhecida como uma patologia pela Organização Mundial da Saúde ela se encaixa em um perfil diferenciado do que é uma doença, por se tratar de uma Doença Crônica Não Transmissível, conceito que obriga o repensar sobre o que seria a saúde e a doença perante uma condição que deteriora o ser humano, tanto física quanto psiquicamente, porém de forma lenta e não aguda. A maneira como a obesidade tem alcançado proporções epidêmicas em diversos países também leva a uma profunda consideração sobre os aspectos históricos e culturais que a trouxeram ao patamar de prevalência em diversos pontos do planeta na atualidade. Ao se constituir como objeto de estudo de diversos campos do conhecimento humano, a obesidade acaba por produzir um foco permanente de tensão entre os diversos ramos científicos que a estudam, pois cada um privilegia um ou outro aspecto dos muitos fatores que a compõem. A proposta de pesquisa apresentada no texto busca unir informações pertinentes sobre a obesidade que pertencem a várias áreas do saber científico, reunindo-as de forma a produzir um questionamento sobre qual seria o estatuto da obesidade perante a Psicanálise. Ao questionar a possibilidade de a obesidade ser um objeto de estudo da pesquisa psicanalítica surge a necessidade de encontrar bases metapsicológicas que possam representá-la dentro da teoria e da prática em Psicanálise. A busca por conceitos que possam explicar o sofrimento psíquico do obeso trouxe ao texto um estudo sobre a instituição da função alimentar, assim como da sensação de saciedade e outras sensações corporais primitivas, e da possibilidade para que ocorra uma perversão no uso de tais funções na maneira como os obesos as utilizam para lidar com os desafios impostos diariamente ao seu psiquismo. O corpo obeso também traz consigo um questionamento sobre qual noção de corpo em Psicanálise poderia ser útil para sua compreensão, assim como qual proposta de tratamento seria útil para tais casos específicos. A contribuição da psicanalista Hilde Bruch é apresentada como um dos pilares sobre o qual o texto pôde ser construído, em especial ao propor a existência de dois tipos de obesidade, a reativa e a de desenvolvimento, e ainda uma metodologia terapêutica específica para ambas. Através da descrição de dois casos clínicos os tipos de obesidade e seu tratamento são ilustrados, o que almeja corroborar a descrição metapsicológica que utiliza o modelo da neurose atual como proposta explicativa para o sofrimento psíquico do obeso, marcado por uma angústia que carece de símbolos para se expressar. A proposta última do texto é que a obesidade pode ser um ponto de encontro importante para o diálogo entre as diversas disciplinas científicas, incluindo a Psicanálise, o que pode gerar contribuições férteis em estudos futuros, em especial se conseguirem empreender um diálogo interdisciplinar

Winnicott about the psychosomatic relationship. Thus, this study will allow the reader to get in touch with several aspects of Multiple Sclerosis as it history, etiology, diagnosis, treatments, prognostics and psychological. This study also focus on the basic concepts of matureness theory, elaborated by Winnicott, reflecting about his patients suffering from psychosomatic illness. Not only are the Winnicott´s patients important to try to understand the perturbations of the psycho-soma in the multiple sclerosis as the revision of Jacqueline du Pré´s biography, which died due the aggravation of the illness. The confrontation of her personal history with the theory elaborated by Winnicott helps to illustrated what have been studied in this paper. After these analyses, it is considered the idea what in the base of all psychosomatic disorder there is a split. So we can infer that split is also one of the important points of the Multiple Sclerosis, that way the geneses of the disease may be related to gaps on the process of psycho-soma integration. Finally, the necessity of more studies on that subject is essentially to help those patients to have a better life quality. At last, the main objective of this research is considered achieved as it opened a new door of possibilities to try to understand of the multiple sclerosis / Esta dissertação busca construir um trajeto que possibilite discutir a Esclerose Múltipla no hall das doenças psicossomáticas tomando como referencial a relação psicossomática, abordada sob a ótica de D. W. Winnicott. Assim, realiza-se um estudo que permite ao leitor conhecer de forma ampla a Esclerose Múltipla detendo-se em seus 4 aspectos históricos; sua etiologia; diagnóstico; tratamentos; prognósticos; aspectos psicológicos. Este estudo se preocupa, ainda, em abordar os conceitos fundamentais da teoria do amadurecimento, elaborada por Winnicott e refletir sobre os pacientes com transtorno psicossomático atendidos por ele. Além dos pacientes de Winnicott, para analisar a esclerose múltipla, recorre-se ao relato da vida da personalidade pública Jacqueline du Pré, que morreu em decorrência de agravamento da doença e à confrontação da sua história tanto com a teoria preconizada por Winnicott quanto com o que foi estudado até então. Após essas análises, considera-se a idéia de que na base de todo transtorno psicossomático está uma cisão. Sugere-se que a cisão é uma forte característica da Esclerose Múltipla. Nota-se que há características semelhantes a outros transtornos psicossomáticos abordados pro Winnicott, considerando os indícios de que a gênese da doença possa ter uma origem em questões relativas à integração da psique-soma. Por fim, é ressaltada a necessidade de estudos mais aprofundados sobre o tema, sendo o principal o objetivo do trabalho atingido, que é a abertura de uma nova porta para a compreensão da esclerose múltipla

Forskningsfrågan var vilken inverkan KASAM, organisationsengagemang och inställning till ackordslönesystemet har på psykosomatisk stress hos lagermedarbetare. Detta har empiriskt undersökts genom att medarbetarna (n = 99) har fått besvara enkäter i en av Sveriges största distributionsenheter för detaljhandel. Genom en multipel regressionsanalys påvisades en signifikant modell för psykosomatisk stress där KASAM och ackordsinställning tillsammans förklarade upp till hälften av den totala variansen.</p>

The purpose of this study was to examine psychosomaticsymptomsissuesamongst children and adolescents between the ages of 0-18 years and to clarify the underlying factors.Method: Literature study based on 10 scientific articles.Results: Psychosomatic issues amongst children and adolescents, due to stress, were common in Europe and Scandinavia. The symptoms varied between gender and age. The underlying factors were mainly school environmental issues, home conditions, socio-economic situations and disputes with teachers and peers. The symptoms varied from headache, gastro intestinal problems and insomnia to eating disorders, depression and pounding heart.               Conclusion: The future of children’s psychosomatic health depends on interventions on all levels of society since they are affected by everything from school environment and home conditions to unemployment and recession. / Syftet med denna studie var att belysa psykosomatiska symptom hos barn och ungdomar i åldrarna 0-18 år samt tydliggöra bakomliggande orsaker.Metod: Litteraturstudie baserat på 10 stycken vetenskapliga artiklar.Resultat: Barn i Europa, främst Skandinavien, upplevde olika psykosomatiska symptom till följd av stressande händelser och dessa symptom varierade även mellan ålder och kön. Skolmiljön var en stor utlösande faktor, likväl som hemförhållanden, socioekonomisk situation och osämja med lärare, jämnåriga och familjemedlemmar. Symptomen var allt från huvudvärk, mag- tarmproblem och sömnsvårigheter till nedstämdhet, ätstörningar och hjärtklappning.Slutsats: Barns framtida psykosomatiska hälsa är beroende av insatser på alla nivåer i samhället, då de påverkas av allt från skolmiljöerna de befinner sig i och hemförhållanden, till arbetslöshet och lågkonjunktur.

Anger expression and blood pressure : the influence of self- consciousness

The validity of the Self-Consciousness Scale (SCS) for use with adolescents was investigated. Also, a path model of blood pressure based on the cognitive social learning theory was tested using sets of biological (i.e., family history of hypertension and age), psychological (i.e., public self-consciousness, private self-consciousness, suppressed anger and outwardly expressed anger), and lifestyle (i.e., relative weight and smoking) variables.Subjects were 169 female and 124 male adolescents from the southeastern United States. Parents provided information about the subjects' family history of hypertension and health. Subjects completed the SCS and Anger Expression Scale and a health questionnaire. Subjects' blood pressures, weights, and heights were assessed. Data from all subjects were used for the SCS analyses. Data from 36 subjects who reportedly had health problems or used drugs with cardiovascular effects were excluded from the path model analyses.The SCS data were factor analyzed. Based on the initial analysis, four items were excluded from the data. The subsequent factor analysis suggested that the SCS is a valid measure of the dimensions of self-consciousness in adolescents. Test-retest reliabilities and internal consistencies of the SCS showed reasonable reliability. Comparisons of the SCS scores between college students and adolescents and between female and male adolescents were made.The path model was tested separately for males and females on both systolic blood pressure (SBP) and diastolic blood pressure (DBP), using hierarchical multiple regression analyses of sets. Although the variables collectively explained a significant amount of variance in DBP and SBP for both sexes, few had significant direct and total effects on DBP and SBP, and none had indirect effects on DBP or SBP. Sex differences emerged in the variables' effects on DBP and SBP and the variables' effects on other variables. DBP and SBP increased as relative weight increased for both sexes. No other variables influenced SBP or DBP for males or SBP for females. Females with a positive family history of hypertension had higher DBPs. Females' DBPs decreased as private self-consciousness increased. Musiken för ungdomar : En fenomenologisk studie om musikens påverkan på ungdomars mående / The music for youths : A phenomenological study about music’s impact on youths’ wellbeing.

The purpose of this study is to find out what effect and meaning the youths’ describe that  music has for their wellbeing. The idea is to examine how students studying music at the esthetic program describe that they perceive that practicing music and listening to different types of music affect their wellbeing, with focus on psychosomatic and psychic symptoms. The point is also to examine why the youths think that music can affect their feelings and wellbeing and why they perceive that music is important to them. The study is qualitative and starts from a life world phenomenological perspective. Music is regarded as a phenomenon in this study. The study’s data was collected through a survey which was performed at three different gymnasiums in southern Sweden. The result presents that music listening and music practicing can reduce many of the youths’ psychosomatic and psychic symptoms – mainly by music affecting the youths’ feelings in different ways, making them feel good, creating fellowship and acting as an escape route from reality. Two types of music that can make the youths feel better is calm music and happy music. It also appears that music can increase psychic and psychosomatic symptoms in a way that different types of music can increase symptoms such as anxiety, worry, sleep difficulties and stress. Music described as sad is a type of music that is mentioned making these symptoms worse. The music is important for the youths in different ways. Examples given are that it’s a part of them as individuals, that it works as a support and that it can affect their wellbeing and feelings. The result also presents why the youths think that music can­­­ affect their wellbeing and feelings. The primary point that emerges is that they describe that there is a connection between humans, memories and feelings; that music can affect and amplify peoples’ feelings and memories. In the discussion it appears that music could be used in order to decrease the youths’ psychosomatic and psychic symptoms.
