An investigation of the relationship between therapist competence and client outcome

This research project was a descriptive study conducted at a clinical psychology practicum center which investigated the relationship between supervisory ratings of student therapist competency and respective client outcome. Cases were assigned to one of four outcome categories on the basis of six outcome criteria. These included: client and therapist assessment of change in presenting complaints and adaptive functioning, client satisfaction with treatment, type of termination, and session attendance. Clinical faculty supervisors completed subjective competency ratings of student-therapists and also rated the difficulty of client cases. A semi-partial correlation was calculated between competency and outcome scores with the effects of case difficulty partialled out of the outcome scores. It was hypothesized that a significant positive semi-partial correlation would emerge. Forty-eight client cases that were month period were included in the study. seen during a six Also, 14 student therapists belonging to four practicum teams participated. The major finding of this investigation was that overall, therapist competency ratings were not significantly related to the outcomes of clients, (r=-. 12, p>.05). However, when assessing the relationship between therapist competency and outcome for each of practicum teams individually, a significant negative semi-partial correlation was found for one of them, (r=-1.03, p<.01). Problems with subjective assessments of competency were discussed and recommendations for improving evaluations were given. Results of a fine grained analysis of outcome data and related variables were presented and discussed as they apply to clinical training and service issues. / Master of Science

Assessing Processes of Connection and Development in Observations of the Therapeutic Relationship

The present study applies Driver and Gottman’s (2004a) Turning System to observations of the therapeutic relationship in a quasi-experimental between and within groups design. A sample of 63 full counseling sessions (21 first sessions with clients who return for four sessions, 21 first sessions for clients who terminate therapy prior to four sessions, and 21 fourth sessions) were collected from a university counseling center in South Florida. Clients and clinicians also completed self report evaluations of the therapeutic relationship that were also included in this study (Working Alliance Inventory – Short Form, and the Real Relationship Inventory). A series of multivariate analysis of the variance (MANOVA) tests were performed to assess for significant differences in Turning System behavior between return and dropout groups in the first sessions of therapy, as well as for significant differences between return groups’ first and fourth sessions. Correlation analyses were run for client and therapist self report data and Turning System codes. Overall, the Turning System codes did not predict attrition from therapy; however, significant effects were found for specific behaviors in the return versus dropout comparisons, including client’s uses of negative bids, high level questions, and preoccupied away responses. The Turning System also did not predict clear differences between behaviors in ongoing therapy, though significant effects were again found for individual behaviors enacted by both clients and therapists. Significant negative correlations were also found for return group clients between specific behaviors and ratings of the therapeutic alliance, such as high level questions and preoccupied away responses. Individual behaviors in the therapist return group, such as high level questions, negative bids, and interruptions, correlated negatively with ratings of the therapeutic alliance. The Relationship Between Race of Counselor, Cultural Mistrust Level and Willingness to Seek Psychological Treatment Among Mexican-American Adolescents

The effects of cultural mistrust level and race of counselor on the willingness of Mexican-American adolescents to seek psychological help were examined in this experiment. A total of 79 Mexican-American adolescents consisting of 50 females and 29 males completed a Background Information Inventory, a modified version of the Cultural Mistrust Inventory, and the Help Seeking Attitude Scale. Five regression analyses were performed resulting in a significant interaction between cultural mistrust level and willingness to seek help. Mexican-American adolescents with high levels of mistrust were less willing to seek psychological treatment than those with a low level of mistrust. Results also indicated that females as well as individuals with higher levels of education were more willing to seek help than males and those with lower levels of education. Theoretical and practical implications are discussed.

The Importance of Staff Cohesiveness in Treatment Effectiveness as Demonstrated by Client Self-Disclosure

Much research has studied cohesiveness within client groups in terms of making therapeutic gains. These studies have defined cohesiveness in terms of a) attraction of the group as perceived by a group member, b) how clearly each member sees his/her role within the group, and c) the effectiveness of one's skills in attaining group goals. Little research has dealt with the role of staff cohesiveness in developing an effective treatment program. Effectiveness, in this study, is defined as the degree to which clients are willing to disclose personal information to the staff. The results show a positive correlation between staff's perceived effectiveness with clients and the clients' willingness to self-disclose. On-hand experience with clients seems important in involving clients in therapy.

In die studie word daar ook verwys na verwante kwessies, soos aanraking en grense in die terapeutiese verhouding, terapie as 'n a-sosiale konteks, terapie as 'n paradoksale situasie en die neutraliteit van die terapeut, wat aansluit by die tema van hierdie studie. Hierdie temas word geintegreer tydens die bespreking van die gevallestudie. / An individual case study is used in this study, to discuss the complementarity between distance and intimacy in the therapeutic relationship. The focus is on recursive and selfreflective processes between the therapist and the client in order to explain the way in which intimate and distant interactions were coconstructed. The therapeutic process was discussed in the context of ecosystemic and social constructionist epistemologies. Research was done according to the principles of the naturalistic research paradigm and overlaps between this research paradigm and epistemologies that were discussed are brought forward through the discussion of the case study. There are also references in this study to related issues, such as touching and boundaries in the therapeutic relationship, therapy as an a-social and paradoxical context, and the neutrality of the therapist, which connect with the theme of this study. Mathematical model of the dynamics of psychotherapy

This is a novel attempt to produce a rigorous mathematical model of a complex system. The complex system under study is the relationship between therapists and their clients. The success of psychotherapy depends on the nature of the relationship between a therapist and a client. We use dynamical systems theory to model the dynamics of the emotional interaction between a therapist and client. We determine how the therapeutic endpoint and the dynamics of getting there depend on the parameters of the model. ... We describe the emotional state of both the therapist and client with coupled, first order, nonlinear ordinary differential equations (ODE's). The rate of change of the emotional state of the therapist and client is proportional to their previous state, their uninfluenced state when alone, and an influence function which depends on the state of the other person. We formulated influence functions based on the research literature on psychotherapy and the therapeutic alliance. We then determined the critical points from the intersection of the nullclines and used a numerical ODE solver (Matlab ODE113) to compute the trajectories from different initial conditions. ... It is intended to present and discuss the analysis, as a possibility of psychic rebirth. More specifically, the proposition was to analyse the process that involves this phenomenon, in the dialectical movement to idealize and "de-idealize", considering the experience through the Oedipal situation and depressive position, with the report and discussion of two clinical cases. This dissertation was guided by the psychoanalytical method. The psychoanalytic theoretical framework underpinned this study and its analysis, with Melanie Klein, Winnicott and Balint as main authors. In both cases under analysis, it was possible to follow the experience of psychic rebirth. In this program, patients were able to retrieve their experience with the good object and then proceeded and progressed towards the elaboration of the depressive position and Oedipal situation. In the journey of analysis, the patient's need was considered, in the sense of regressing to both dual and idealized relationships with the analyst object; lasting in that more dependent performance, for the time necessary, to meet his/her needs; and thereby ensure a secure and safe environment in which the good object could be retrieved and then trust it. Thus, the relations with the analyst object and, consequently, with the other objects, went from ideal to more integrated and, therefore, the patients at issue could acquire more resources before the task of living and existing / A presente tese tem como objeto de estudo o processo de renascimento psíquico vivido em análise. Pretende-se apresentar e discutir a análise, como possibilidade de renascimento psíquico. Mais especificamente, a proposta foi analisar o processo que envolve esse fenômeno, no movimento dialético de idealizar e desidealizar, considerando a passagem pela situação edípica e posição depressiva, com o relato e discussão de dois casos clínicos. Esta tese se orientou pelo método psicanalítico. O referencial teórico que fundamentou este estudo e sua análise foi psicanalítico, tendo Melanie Klein, Winnicott e Balint como autores basilares. Em ambos os casos objeto de análise, foi possível acompanhar a experiência do renascimento psíquico. Nesse movimento, os pacientes puderam recuperar a experiência com o bom objeto e, então, prosseguiram e progrediram, no sentido da elaboração da posição depressiva e situação edípica. Nesse percurso vivido em análise, considerou-se a necessidade do paciente, no sentido de regredir para relações duais e idealizadas com o objeto analista; permanecer nesse funcionamento mais dependente, pelo tempo necessário, para atender às suas necessidades; e, desse modo, garantir um ambiente seguro em que pudesse resgatar o bom objeto e, então, confiar nele. Assim, as relações com o objeto analista e, consequentemente, com os demais objetos, passaram de ideais para mais integradas e, desse modo, os pacientes em questão puderam adquirir mais recursos diante da tarefa de viver e existir
