Jaime Rest: función crítica y políticas culturales (1953-1979)

La presente tesis doctoral se aboca a describir y analizar la función intelectual del crítico literario, crítico cultural, profesor, traductor y editor argentino Jaime Rest (1927-1979). El análisis se desarrolla sobre un relevamiento de su producción escrita y su gestión cultural. Y su tesis se aplica a sostener que, estudiado en su propia inscripción histórica (en una red de condicionamientos y elecciones situadas en un contexto concreto de filiaciones y afiliaciones teóricas y políticas), el proyecto intelectual de Rest exhibe los rasgos y funciones fundamentales del modelo del “intelectual específico” descrito por Michel Foucault a mediados de la década de 1970. El trabajo se inscribe teórica y metodológicamente en la intersección de tres disciplinas convergentes: historia intelectual, metacrítica e historia cultural.

SUMMARY Peculiarities of organizm to adaptation physical strain are very relevant problem to sport and its range. Relative narrow and particular purpose is raised in the solution of this problem: to estimate conception of professionals working in athletic club and those who goes in for sports there – abaut the importance of rest intervals in training for seeking result during the physical exercising also to ascertain the influence of rest intervals between physical strains to peculiarities of fast adaptacion. Results of the research are presented in this study; it joins together, first – results of survey of physical culture and sport also health professionals; second – results of survey of individuals who attends health strengthening training; third – results of survey of topical muscular blood circulation intensity during the time of rest intervals between physical strains; fourth – results of survey of functional condition index alternation during the rest intervals between physical strains. Results which were received showed, that people attending athletic club takes interest in optimal purport of training, though they are lacking in information about the lasting of rest intervals and its significance. In majority, trainers of health groups do not designate the time people must rest since the next repeat of exercise. SIESTA PÅ INTENSIVEN : Patientvila under dagtid / SIESTA IN THE ICU : Quite time during daytime

Bakgrund. Miljön på intensivvårdsavdelningen är fylld av olika stimuli i form av ljud och ljus. Sjuksköterskorna arbetar bedside och täta omvårdnadsåtgärder är nödvändiga för dessa kritiskt sjuka patienter. Denna miljö lämnar en liten möjlighet för sömn och vila. För att patienterna ska få möjlighet till vila under dagtid har vissa intensivvårdsavdelningar lagt in en period med mindre aktivitet, tysta timmen, också kallat patientvila. Målet med patientvilan är att erbjuda patienterna en timme med ostörd vila under dagtid. Syfte. Att undersöka patienternas möjlighet till vila under dagtid genom att observera antal utförda omvårdnadsåtgärder enligt VIPS sökordsmodell. Metod. En icke-deltagande strukturerad observationsstudie utfördes. Observationsprotokoll utformades med VIPS-sökordsmodell. Studien utfördes på två intensivvårdsavdelningar i Mellansverige, IVA 1 och IVA 2. Observationerna utfördes under åtta dagar. Observatörerna registrerade allt som skedde runt patienterna mellan klockan 12.00 – 13.00, då patientvilan ägde rum. Resultat. Totalt observerades 19 patientplatser på IVA 1 och IVA 2. Under de åtta tillfällen som observerades registrerades totalt 158 störningar, 101 av dessa störningar skedde på IVA 2 och 57 störningar skedde på IVA 1. Antalet störningsmoment per patient under patientvilan varierade från en till 18 stycken. På IVA 1 var de vanligaste störningsmomenten läkemedelshantering samt speciell omvårdnad. Miljö fick mest punktmarkeringar på IVA 2. Den enda åtgärden som inte förekom var träning. Totalt 152 störningsmoment utfördes av vårdpersonal. Slutsats. Patienterna blir störda under patientvilan med en mängd olika omvårdnadsåtgärder. Vissa åtgärder är nödvändiga och svåra att undvika, andra åtgärder hade kunnat prioriteras annorlunda. Klinisk betydelse. Att öka medvetenheten hos personalen angående betydelsen av sömn och vila för patienterna på intensivvårdsavdelning. / Background. The environment at the ICU (Intensive Care Unit) is filled with different stimuli of sound and light. Nurses work bedside for the critically ill patients, and frequent nursing interventions are required. This environment gives little possibility for sleep and rest. It is important to make sure that the patients will have the possibility to get sufficient amount of rest during the day. Some ICUs have addressed this by applying less activities during one hour per day, patient resting or quiet time. Purpose. To investigate patients' ability to rest during the day, by observing the number of performed nursing interventions according to the VIPS-model. Method. A non-participating structured observational study was performed. The observation protocol was designed with the VIPS-model. The study was conducted in two ICUs in central Sweden, ICU 1 and ICU 2. The observations were carried out during eight days. Observers registered everything that happened around the patients between the hour of 12:00 to 13:00, when the quiet time took place. Results. A total of 19 patients were observed on ICU 1 and ICU 2. A total of 158 interruptions were observed, 101 of these interruptions occurred in the ICU 2 and 57 interruptions occurred in the ICU 1. The number of interruptions during the quiet time varied from one to 18 distractions. The most common interruptions, for the ICU 1, were medication and special care. The environment was the most common distraction at the ICU 2. The only intervention that didn´t occur was training. A total of 152 distractions were performed by health professionals. Conclusions. The patients are disturbed during quiet time, with a variation of nursing interventions. Some interventions could have been prioritized differently, but other interventions were necessary and difficult to avoid. Relevance to clinical practice. To increase awareness among staff regarding the importance of sleep and rest for patients in ICU.

Human Papillomavirus (HPV) Vaccination Coverage Estimates Among Adolescent Females within the Delta Regional Authority Using National Immunization Survey Teen (NIS-Teen) 2008 - 2012.

INTRODUCTION: Since 2006, the Advisory Committee for Immunization Practices (ACIP) has recommended routine vaccination of adolescent girls at ages 11 or 12 years with 3 doses of human papillomavirus (HPV) vaccine to prevent cervical cancer. Cervical cancer disparities exist for several populations in the United States. The topic is scantly documented in scholarly literature with modest knowledge of the uptake of the HPV vaccine in the Delta region. AIM: To examine the HPV vaccination coverage estimates in the counties and parishes that comprise the Delta Regional Authority (DRA) and compare with other non-DRA counties or parishes and the rest of US. This will provide vaccination coverage estimates for adolescent females in the DRA designated Delta region that will provide public health professionals with data for policy and programmatic decision-making. METHODS: We combined data from multiple survey years (2008-2012) from the National Immunization Survey-Teen (NIS-Teen). We conducted bivariate analyses to describe the distribution across selected socio-demographic characteristics and multivariable logistic regression models to produce adjusted prevalence ratios. RESULTS: Teens in the DRA had lower HPV vaccine initiation and completion rates compared to Non- DRA counties or parishes and the Rest of the US. The 3-dose HPV vaccination series completion rate among those who had initiated HPV vaccination and had 24 weeks between their first dose and the NIS interview date was 60.9% (95% CI: 55.8 – 65.8) among adolescent females in the DRA compared to 66.9 (95% CI: 63.7-70.0) of adolescent females in the Non-DRA counties and 67.6% (95% CI: 66.2-69.0) in the rest of the US. CONCLUSION: The low HPV vaccination coverage among adolescents in the DRA showed that it is important to implement additional strategies to increase HPV vaccination coverage that will prevent cancers associated with HPV in the DELTA Region. Stronger provider recommendations and awareness are important for increasing HPV vaccine uptake.

Effects of passive and active recovery on the resynthesis of muscle glycogen

The purpose of this investigation was to determine the effect of passive and active recovery on the resynthesis of muscle glycogen after high intensity cycle ergometer exercise in untrained subjects. In a cross over design, six college-age males performed three, one min exercise bouts, at 130% V02max with a 4 min rest period between each work bout. Subjects refrained from exercise for two days prior to testing, and consumed a 15% carbohydrate solution (300g sugar in 2000ml of water) the day before each trial to help elevate glycogen concentration. The exercise protocol for each trial was identical, while the recovery following exercise was eitheractive (40-50% VO2max) or passive. The initial muscle glycogen values averaged 144.2 mmol•kg-1 w.w. for the active trial and 158.7 mmol•kg-1 w.w. for the passive trial. Corresponding post-exercise glycogen contents were 97.7 and 106.8 mmol•kg-1 w.w., respectively. These differences were not significant (P>0.05). However, the rate of muscle glycogen resynthesis during passive recovery increased 15 mmol•kg-1 w.w. whereas it decreased 6.27 mmol•kg-1 w.w. following active recovery (P<0.01). Also, the decrease in blood lactate concentration during active recovery was much faster than during passive recovery and significantly different at 10 and 30 min of the recovery period (P<0.01). The major finding of this investigation was that the rate of muscle glycogen resynthesis during passive recovery was significantly greater than that during active recovery. These data suggest that lactate can be used as an endogenous glycogenic precusor in muscle, and that glycogenesis was the prevalent pathway of lactate removal during passive recovery following high intensity cycle ergometer exercise. / Human Performance Laboratory

The dissertation present an approach to decreasing the risk of transportation of hazardous materials by applying construction related solution. They consist in deploying safety barriers between transportation routes and vulnerable property built in the roadside territory. The attention is focussed on the hazmat transportation accidents known as boiling liquid expanding vapour explosion (BLEVEs). It is considered how to predict thermal and mechanical effects of BLEVEs on potential barriers or roadside property. It is suggested to interlink results of this prediction with the conventional structural response techniques used for estimating the susceptibility of safety barriers to the effects of BLEVEs. / Disertacijoje pateikiamas originalus būdas mažinti pavojingųjų medžiagų vežimo riziką taikant statybinio pobūdžio priemones. Tos priemonės - saugos barjerai,kurie yra statomi tarp transporto linijų ir pažeidžiamų pakelės objektų. Didžiausias dėmesys, telkiamas į transporto avarijas, kurios vadinamos besiplečiančių verdančių skysčio garų sprogimas (BLEVE). Nagrinėjama, kaip prognozuoti šiluminius ir mechaninius tokių sprogimų poveikius. Jie gali pažeisti pakelėse esančius statinius. Juos turi sumažinti saugos barjerai. BLEVE poveikių prognozavimą siūloma susieti su konstrukcijų skaičiavimo metodais, kuriais vertinama barjerų reakcija į šiuos poveikius.
