[pt] O objetivo da presente tese foi contribuir para a compreensão das manifestações psicossomáticas na infância, exemplificadas por meio dos distúrbios alérgicos respiratórios. A teoria de Winnicott sobre o desenvolvimento emocional primitivo foi utilizada como principal ferramenta teórica. Foram discutidas as contribuições dos autores identificados com a psicossomática psicanalítica, desde o modelo teórico de Pierre Marty até à literatura relativa a infância, e também os conceitos de Winnicott sobre a relação mãe-bebê. A pesquisa utilizou o conceito winnicottiano de preocupação materna primária, focalizando a adaptação ativa materna, e revelou a existência de uma relação entre as falhas adaptativas maternas e a manifestação psicossomática alérgica da criança. / [en] The objective of the present thesis was to contribute to the understanding of psychosomatics manifestations in infancy, exemplified by respiratory allergic diseases. Winnicot`s theory about the primitive emotional development was used as the main theoretical tool. At first we discussed the contributions of psychoanalytic psychosomatic authors, beginning with Pierre Marty`s theoretical model to the literature concerning infancy, and also Winnicott´s concepts about mother-child relationship. The research used the Winnicott`s conception of primary maternal preoccupation, focusing the mother`s active adaptation, and showed the existence of a relation between failures in the maternal adaptation and the child`s psychosomatic allergic manifestation.

Efeitos da estabulação sobre o trato respiratório de equinos em dois tipos diferentes de camas e a pasto /

The stabling and poor quality feedings may the favor predispose of diseases in airways. In order we measure the influence of the types of bedding used in the horse stalls about responses allergic airway. Three groups of five horses each are formated, two groups kept in stables but each group with a different type of bedding (autoclaved wood- MA shaving and sawdust-SE) . The third group is the control and stays at pasture. The groups remained in this condition for 45 days with biweekly reviews (D0, D15, D30 and D45). It was realize the physical examination, venous and arterial blood samples collection to realize hemogram and hemogasometry analysis, the bronchoalveolar lavage samples was colected to realize the cytological analysis and to measure alkaline phosphatase. In addition, samples of the beds (clean and dirty) and haylage was collected to realize mycological analysis. In the physical examination was noticed higher intensity of coughing and sneezing at SE group. Hematological analysis remained within the normal standards. Findings of hemogasometric analysis were observed the biggest pH values at horses to pasture (7.47 ± 0.02). At stables horses was observed smaller values of partial pressure dioxide (D15 to D45), (values between 38.3 to 43.7 mmHg) to horses to pasture (values between 43.7 to 46.5 mmHg). (Complete abstract click electronic access below) / Resumo: As afecções do sistema respiratório são a segunda causa de queda de desempenho e afastamento dos equinos do esporte ou trabalho, as quais estão atrás somente das desordens musculoesqueléticas. A estabulação e alimentação a base de fenos predispõe os equinos a inalação de grande número de agentes irritantes as vias aéreas. No intuito de mensurar a influência dos tipos de cama utilizado em baias de equinos nas respostas alérgicas das vias aéreas estudou-se 3 grupos com 5 equinos cada sendo dois mantidos em cocheiras com diferentes tipos de forração à base de madeira (maravalha de pinus autoclavada-MA e serragem-SE) e por último um grupo controle à pasto por 45 dias com avaliações quinzenais (D0, D15, D30 e D45). Foi realizado nestes momentos o exame físico, coleta de amostras de sangue venoso e arterial para realização de hemograma e hemogasometria respectivamente, lavado broncoalveolar com análise citológica e mensuração de fosfatase alcalina, além de amostras das camas (limpa e suja) e feno pré-secado para análise micológica. No exame físico foi notado tosse e espirros em maior intensidade e número nos equinos do grupo SE. A análise hematológica permaneceu dentro dos padrões de normalidade. Já nos achados hemogasométricos os maiores valores de pH foram observados nos equinos à pasto (7,47 ± 0,02). Nos grupos embaiados foi observado valores de pressão parcial de dióxido, de D15 à D45, inferiores (valores entre 38,3 a 43,7 mmHg) ao grupo a pasto (valores entre 43,7 a 46,5 mmHg)... Atualmente, o tratamento dessas desordens respiratórias consiste a imunoterapia convencional e na farmacoterapia. Embora a imunoterapia convencional apresente boa eficiência no tratamento de alergias respiratórias, o longo tempo de tratamento e a presença de reações sistêmicas adversas constituem grandes desvantagens, uma vez que o alérgeno é administrado na sua forma livre. Uma alternativa para reduzir ou eliminar essas desvantagens seria a encapsulação do alérgeno em sistemas de liberação controlada. Dentre esses sistemas, lipossomas e microesferas poliméricas apresentam-se promissores, uma vez que são capazes não somente de proteger e reduzir a toxicidade do composto ativo encupsulado, mas também de aumentar seu tempo de circulação in vivo. Os lipossomas são biocompatíveis, não imunogênicos, potentes adjuvantes imunológicos e apresentam a capacidade única de interagir com as células. As microesferas, no entanto, vem sendo extensivamente usadas em sistemas de liberação devido a sua capacidade de atuar como reservatório, capaz de liberar as macromoléculas por dias, meses ou anos. Nowadays, conventional therapies for atopic individuals comprise specific immunotherapy and pharmacotherapy. In general, the immunotherapy using free allergens produces adverse systemic reactions of varying intensity and frequency. In order to circumvent these problems, several approaches have been explored. Among them, liposomes and microspheres have been studied as promising vehicles as carriers of allergens for desensitization immunotherapy in allergy. Liposomes are small phospholipid vesicles composed of nontoxic, biodegradable, and naturally occurring immune response and producing a pronounced depot effect after subcutaneous injection. Microspheres have also been widely investigated in drug delivery studies due to their capability of presenting antigens to the immune system over a long time. A large number of natural and synthetic polymers are potentially suitable for production of the wall-forming polymer. Synthetic biodegradable poly (lactide-co-glycolide) copolymers are among the primary candidates due to their excellent tissue biocompatibility, biodegradability and regulatory approval. The regulation of allergic airway disease by type V collagen-induced tolerance

Indiana University-Purdue University Indianapolis (IUPUI) / Rationale: Tissue remodeling and complement activation are asthma hallmarks. Type V collagen [col(V)], a cryptic antigen, becomes exposed during lung remodeling. IL-17 is key to anti-col(V) immunity, and regulates complement activation. We have reported that col(V)-induced tolerance down regulates IL-17 and prevents immune-mediated lung diseases. Objectives: Determine a role for anti-col(V) immunity in asthma. Methods: Serum anti-col(V) antibodies were measured in asthma patients, and immunohistochemistry utilized to detect interstitial col(V) in fatal asthma. Balb/c mice were tolerized with col(V) prior to sensitization with ovalbumin (OVA), and subsequent OVA intranasal challenge. Airway hyper-responsiveness (AHR) to methacholine was measured; and RT-PCR utilized to determine local Il17 transcripts. Bronchoalveolar lavage levels of C3a¸ C5a and OVA-specific IgE were measured; and immunohistochemistry utilized to detect expression of complement regulatory proteins, expression, CD46/Crry and CD55, in lung tissue. Results: Compared to normal subjects, anti-col(V) antibodies were increased in asthmatics; and interstitial col(V) was over expressed in fatal asthma. OVA-induced AHR up regulated anti-col(V) antibodies systemically, and increased OVA-specific IgE and C3a in BAL, and parenchymal Il17 transcripts. Col(V)-induced tolerance abrogated AHR, down regulated OVA-induced T cell proliferation, as well as total and OVA-specific IgE, C3a, IL-17 expression and tracheal smooth muscle contraction. Crry/CD46 and CD55, key to preventing complement activation, were down regulated on goblet cells in murine allergic airway disease. Conclusions: Anti-col(V) immunity correlates with asthma pathogenesis, and col(V)-induced tolerance may be a novel therapeutic for asthma. Decreased expression of Crry/CD46 and CD55 on goblet cells may in part account for complement activation in asthma.

Development and stability of IL-17-secreting T cells

Indiana University-Purdue University Indianapolis (IUPUI) / IL-17-producing T cells are critical to the development of pathogen and tumor immunity, but also contribute to the pathology of autoimmune diseases and allergic inflammation. CD8+ (Tc17) and CD4+ (Th17) IL-17-secreting T cells develop in response to a cytokine environment that activates Signal Transducer and Activator of Transcription (STAT) proteins, though the mechanisms underlying Tc17/Th17 development and stability are still unclear. In vivo, Tc17 cells clear vaccinia virus infection and acquire cytotoxic potential, that is independent of IL-17 production and the acquisition of IFN-γ-secreting potential, but partially dependent on Fas ligand, suggesting that Tc17-mediated vaccinia virus clearance is through cell killing independent of an acquired Tc1 phenotype. In contrast, memory Th cells and NKT cells display STAT4-dependent IL-23-induced IL-17 production that correlates with Il23r expression. IL-23 does not activate STAT4 nor do other STAT4-activating cytokines induce Il23r expression in these populations, suggesting a T cell-extrinsic role for STAT4 in mediating IL-23 responsiveness. Although IL-23 is important for the maintenance of IL-17-secreting T cells, it also promotes their instability, often resulting in a pathogenic Th1-like phenotype in vitro and in vivo. In vitro-derived Th17 cells are also flexible when cultured under polarizing conditions that promote Th2 or Th9 differentiation, adopting the respective effector programs, and decreasing IL-17 production. However, in models of allergic airway disease, Th17 cells do not secrete alternative cytokines nor adopt other effector programs, and remain stable IL-17-secretors. In contrast to Th1-biased pro-inflammatory environments that induce Th17 instability in vivo, during allergic inflammatory disease, Th17 cells are comparatively stable, and retain the potential to produce IL-17. Together these data document that the inflammatory environment has distinct effects on the stability of IL-17-secreting T cells in vivo.
