Os resultados mostraram que o sistema Epiphany/Resilon apresentou resposta tecidual apical e periapical melhor que o cimento Sealapex, independente da presença ou não de restauração coronária, em todos os aspectos estudados. O sistema Epiphany/Resilon com restauração coronária apresentou melhores resultados que o mesmo sistema sem restauração coronária. O cimento Sealapex sofreu influência da infiltração coronária, apresentando, em todas as variáveis estudadas, os piores resultados, quando exposto ao meio bucal. / The objective of this study was to evaluate, in vivo, the biocompatibility and the leakage influence, in the crown-apex direction, in the endodontic post-treatment repairing of apical and periapical tissues, using two different filling materials: Sealapex sealer and a resinous cement of root canal filling - Epiphany/Resilon. The results showed that Epiphany/Resilon system presented apical and periapical tissue response better than Sealapex independent of presence or no of the coronary restoration, in all studied aspects. Epiphany/Resilon system, with coronary restoration presented better results than of the same system without coronary restoration. Sealapex was influenced by the coronary exposition to oral environment, presenting the worst results in all studied variables.

Quarenta e oito pré-molares humanos inferiores foram distribuídos em dois grupos controles: dentes sem instrumentação e dentes instrumentados e quatro grupos experimentais, nos quais, após a instrumentação adicionou-se: guta-percha, guta-percha+Sealer 26, guta-percha+AH Plus e guta-percha+FillCanal. Os dentes foram montados em uma mandíbula para a exposição com o i-Cat em resoluções de 0.125 e 0.2mm de voxel. Três radiologistas avaliaram as imagens no formato pdf. A concordância para a presença e intensidade de artefatos intra e inter-avaliador variou de 0.79-0.96 e 0.54-0.95 (Kappa), respectivamente. Diferentes posições nos alvéolos não interferiram na produção de artefatos (p=1.000; teste Exato de Fisher). A maior parte das imagens foi classificada como excelente e boa (grupos controles) e boa e regular (grupos experimentais). Em todas as avaliações dos grupos experimentais foram observados artefatos. Em relação à intensidade dos artefatos, houve diferença significativa apenas no terço radicular cervical do AH Plus com resolução 0.125mm (p≤0.05; teste Exato de Fisher). Imagens com resolução de 0.2 mm, sugeriram presença de fratura no grupo Guta-percha e Guta-percha+Ah Plus. Todos os materiais de preenchimento produziram artefatos, reduzindo a qualidade da imagem para o auxílio no diagnóstico. Os tecidos radiculares e a posição do dente não interferiram na formação de artefatos. Dependendo da finalidade diagnóstica, é indicada a exposição com resolução de 0.125mm, por apresentar dose de radiação equivalente à de 0.2mm e imagens com melhor qualidade. / The aim of this study was to investigate the influence of endodontic filling materials, root tissues and position of the teeth in the production of Artifacts in Cone-Beam Computed Tomography (CBCT). Forty-eight human lower premolars were divided into two control groups (C1 teeth without instrumentation, C2 instrumented teeth) and four experimental groups, in which, after instrumentation ,gutta-percha, gutta-percha + Sealer 26, gutta-percha + AH Plus and gutta-percha + FillCanal were added. The teeth were mounted on a jaw for exposure with i-Cat in resolutions 0.125 and 0.2 mm voxel. Three radiologists evaluated the images in pdf format. The agreement for the presence and intensity of intra and inter-rater artifacts ranged from 0.79-0.96 and 0.54-0.95 (Kappa), respectively. Different positions in the alveoli did not affect the production of artifacts (p = 1.000; Fisher's exact test). The images (99%) were classified as excellent or good (control groups) and good and regular (experimental groups). Image artifacts were observed in all evaluations of the experimental groups. Regarding the intensity of the artifacts, there was a significant difference only in the cervical root third of AH Plus with Resolution 0.125mm (p≤0.05; Fisher's exact test). Images with 0.2 mm resolution, suggested fracture presence in Guta-percha group and Gutta-percha + Ah Plus. All filling materials produced artifacts, reducing image quality to aid in diagnosing. The root tissue and position of the tooth did not interfere in the formation of artifacts. Depending on the diagnostic purposes, it is indicated exposure with resolution of 0.125mm, to present radiation dose equivalent to 0.2mm and images with better quality.

No Capítulo 1, foram avaliados parâmetros como: efeito da conicidade do pino, distância entre os suportes, e propriedades dos pinos na flexão e tensão máxima pelo método de elementos finitos na realização de ensaios de flexão de 3 pontos. Foi gerado modelo tri-dimensional de pino de fibra de vidro (RelyX post, 3M-Espe), variando posições dos suportes inferiores e inclinação do pino. Empregou-se propriedades elásticas, características ortotrópicas e isotrópicas. A inclinação dos pinos cônicos para nivelá-los nos suportes teve pouco efeito nas tensões. A flexão aumentou quando 50% da porção carregada do pino envolveu conicidade. Quando o posicionamento do pino envolveu 20% da porção cônica, os valores de flexão foram similares ao modelo de referência, que é o pino cilíndrico, sem inclinação. Propriedades ortotrópicas resultaram em aumento da flexão comparado ao pino isotrópico. No Capítulo 2, propriedades mecânicas de diferentes sistemas de pinos pré-fabricados foram avaliadas por ensaio experimental de flexão de três pontos, assim como análise da correlação entre propriedades mecânicas e características estruturais visualizadas por microscopia eletrônica de varredura associado à software de processamento de imagens. Os resultados demonstraram que as características estruturais afetaram significativamente as propriedades mecânicas dos pinos. Resistência à flexão está diretamente correlacionada com a razão fibra/matriz de resina, enquanto módulo de flexão está inversamente relacionado ao número de fibras/mm2. No Capítulo 3, foi avaliada resistência de união de pino de fibra ao núcleo de preenchimento de resina composta por meio do teste de push-out, variando o tipo de agente silano: três silanos pré-hidrolizados e um sistema de silano de dois componentes; e a temperatura de secagem com ar após aplicação do silano: 23°C e 60°C. Empregou-se ainda grupo controle negativo, no qual nenhum tratamento do pino foi realizado. A aplicação de silano pré-hidrolizado e secagem com ar quente (60°C) dos pinos não influenciou a resistência de união. O agente silano de dois componentes com temperatura de secagem de 23°C apresentou os maiores valores de resistência de união. Desta forma, dentro das limitações dos três experimentos, conclui-se que teste de flexão de três pontos empregado para avaliação de pinos pré-fabricados é válido quando limita a área de ensaio à porção cilíndrica dos pinos; as características estruturais dos pinos têm correlação com as propriedades mecânicas dos mesmos; e a aplicação de ar quente sobre as superfícies de pinos silanizados não tem efeito significativo na resistência de união entre pinos de fibra e preenchimento com resina composta / Abstract: The fiber-reinforcement composite (FRC) posts introduced the new concept of restorative system, in which the various components of the reconstruction: post, cement, filling material and dentin have now become a complex mechanically homogeneous. The knowledge of the structure, composition and physical properties of these fiber posts systems is important in order to minimize the failures and the unsuccessfully clinical. Thus, the present study was conducted to analyze sequentially the biomechanical parameters of FRC posts through different stages divided into three experiments. In Chapter 1, the effect of taper, specimen supports and the properties on flexure and stress response during three-point bending were analyzed using finite element analysis. Three-dimensional non-linear finite element model of a fiber-reinforced composite post was created. Different support positions were evaluated during a simulated three-point bending test. The applied properties were elastic and orthotropic. Tilting the tapered posts to level those in the test setup had little effect on the outcome. Flexure increased when 50% of the bent portion involved taper. If 20% of the bent post involved taper, the flexure values were close to control group. The orthotropic properties also resulted in increased flexure compared to an isotropic post. Maximum stresses were only a little higher when 50% of the bend structure involved taper, while the orthotropic properties had little effect. In Chapter 2, the mechanical properties of different FRC posts were assessed with three-point bending test and evaluated the correlation between mechanical properties and structural characteristics by scanning electron microscopy (SEM) associated with an software of image processing. Then, the structural characteristics significantly affect the mechanical properties of fiber posts. The flexural strength is directly correlated with fiber/matrix ratio, whereas the flexural modulus is inversely correlated with number of fibers per mm2 of post. In the Chapter 3, was evaluate the effect of three prehydrolyzed silanes and one two-bottle coupling agent and two air-drying temperature (23°C and 60°C) on the bond strength between glass fiber posts and composite resin core using micropush-out testing. Additionally, it was tested a control group, which no treatment of fiber post were realized. The results showed that drying with warm air and post silanization with pre-hydrolized silanes had no significant effect on push-out bond strength between glass fiber post and composite resin core. For 23°C air-drying groups, the bond strength with two-bottle coupling agent was higher than the other groups. Then, based on the findings of this study, and within the limitation of this investigation, could be concluded that the regardless of leveling, the flexural stress determination with tapered fiber-reinforced posts in the three-point bending test was valid as long as the tapered portion was limited. Additionally, the mechanical properties have correlation with structural characteristics of FRC posts. Avaliação da resistência à fratura vertical de dentes tratados endodonticamente com diferentes materiais obturadores / Vertical fracture resistance of endodontically treated teeth with different root filling materials

O presente estudo teve como objetivo avaliar comparativamente in vitro a resistência à fratura radicular de dentes tratados endodonticamente usando diferentes materiais obturadores: AH Plus (Dentsply/De Trey, Alemanha), Real Seal (Sybron Endo, EUA), GuttaFlow (Coltène/Whaledent, Suíça) , ActiV GP (Brasseler, EUA) e Thermafil (Dentsply-Tulsa Dental, EUA). Sessenta e quatro pré-molares inferiores foram divididos em Grupo Controle (n=4), onde as raízes não foram nem instrumentadas nem obturadas e outros 5 grupos de acordo com os materiais obturadores empregados: Grupo AH Plus (n=12); Grupo Real Seal (n=12); Grupo GuttaFlow (n=12); Grupo ActiV GP (n=12); Grupo Thermafil (n=12). As raízes foram instrumentadas com o sistema Protaper Universal (Dentsply-Maillefer, Suíça), e em seguida foram obturadas com os respectivos materiais. Após a obturação, todos os espécimes foram armazenados à 37º C com 100% de umidade por 72 horas e incluídos em resina acrílica. O ligamento periodontal foi simulado com Impregum, que formou uma fina camada entre a raiz e a resina acrílica. Os espécimes foram submetidos ao teste de resistência à fratura em uma máquina de ensaios triaxiais (1,0mm/min). Os valores médios obtidos e o desvio padrão (em Newtons) em ordem decrescente foram: Grupo controle 394,25 ± 56,17 N; Grupo ActiV GP 263 ± 89,32 N; Grupo Thermafil 198,17 ± 61,65 N; Grupo AH Plus 158,08 ± 31,56 N; Grupo Real Seal 154,92 ± 42,64 N e Grupo GuttaFlow 107,92 ± 20,72 N. Os dados foram submetidos aos testes ANOVA e Dunett (5%). Dentre os grupos experimentais, as raízes obturadas com ActiV GP mostraram-se mais resistentes à força vertical aplicada, porém similares aos grupos controle e Thermafil. Os grupos AH Plus, Real Seal e Thermafil mostraram-se similares estatisticamente quanto à resistência à fratura. O grupo GuttaFlow apresentou menores valores médios de resistência à fratura. / The aim of this study was to evaluate the in vitro vertical fracture resistance of endodontically treated teeth using different root filling materials: AH Plus (Dentsply/De Trey, Alemanha), Real Seal (Sybron Endo, EUA), GuttaFlow (Coltène/Whaledent, Suíça) , ActiV GP (Brasseler, EUA) and Thermafil (Dentsply- Tulsa Dental, EUA). Sixty four single-rooted human lower premolar were assigned into a Control Group (n=4), where the roots were not prepared or filled, and five experimental groups (n=12) according to the root filling materials: Group AH Plus; Group Real Seal; Group GuttaFlow; Group ActiV GP; Group Thermafil. The roots were prepared using Protaper Universal system (Dentsply-Maillefer, Suíça) and filled using one of the root filling materials. Then, the specimens were stored at 37º C at 100% humidity for 72 hours and included in acrylic resin. The periodontal ligament was simulated by the use of Impregum, which made a thin layer between the root and acrylic resin. The specimens were submitted to a vertical fracture resistance using a testing machine triaxiais (1,0mm/min). The values (Newton) and standard deviations obtained were: Control Group 394,25 ± 56,17N; Group ActiV GP 263 ± 89,32N; Group Thermafil 198,17 ± 61,65N; Group AH Plus 158,08 ± 31,56N; Group Real Seal 154,92 ± 42,64N and Group GuttaFlow 107,92 ± 20,72N. The data were submitted to ANOVA e Dunett tests (5%). Among the experimental groups, roots filled with ActiV GP showed the highest values of resistance of load vertical was applied, and were similar to Control Group and Thermafil. AH Plus, Real Seal and Thermafil groups showed statistical similar fracture resistance. GuttaFlow Group showed the lowest values of fracture resistance.

An in-vitro comparison of bacterial microleakage of zinc-oxide eugenol and brasseler biceramic seale

Indiana University-Purdue University Indianapolis (IUPUI) / INTRODUCTION: New sealers have been placed on the market to improve the property of a hermetic seal within the root canal and decrease the gap between the gutta-percha and dentin wall. Differences in the adhesive properties of endodontic sealers may be expected because their interaction with either dentin or gutta-percha may vary with their chemical composition. No specific interaction either with dentin or gutta-percha is expected from the setting reaction of calcium hydroxide based sealers and the epoxy-based sealers. The setting reaction of the zinc oxide-eugenol mixtures is a chelation reaction occurring with the zinc ion of the zinc oxide. Development and maintenance of the seal are essential to optimize the long-term success rate of root canal treatment. An ideal endodontic sealer should, in part, adhere firmly both to dentin and gutta-percha. PURPOSE: The purpose of this study is to evaluate the sealing properties of EndoSequence BC sealer TM ,a bioceramic sealer, and Roth’s sealer using gutta-percha with warm vertical condensation and using the single cone technique with the EndoSequence BC sealer only. The goal is to see if there is a significant difference in microleakage between teeth sealed with the bioceramic sealer and teeth sealed with Roth’s sealer. MATERIALS & METHODS: Eighty-five single-rooted maxillary anterior teeth were used for this study. Endodontic cleaning and shaping of each root canal system was accomplished using K-type hand files and NiTi Rotary files. Group A consisted of twenty-seven anterior teeth, which were obturated using EndoSequence BC sealer and gutta-percha with System B and Obtura. Another twenty-seven anterior teeth, Group B, was obturated using Roth’s sealer and gutta-percha with System B and Obtura. A final group, Group C with 27 teeth was obturated using the single cone technique by means of a single gutta-percha point with Endosequence BC sealer. Two teeth were used as a positive control group, and two other teeth as a negative control group. A microbial leakage apparatus was constructed using a similar two-chamber method described by Torabinejad et al. E. faecalis ATCC 29212 was the test bacteria which was used in this study to determine microleakage The outcome of interest (bacterial turbidity) and time-to-leakage (in days), will be determined for each of the samples. Survival analysis will be used to compare the two groups, with a Kaplan-Meier plot to visualize the results and a nonparametric log-rank test for the group comparison RESULTS: No microleakage was observed in the negative control or group B. Microleakage was observed in all the positive control. Group A and C had a significantly higher proportion of samples with microleakage than group B (p<0.0001), but group A and C were not significantly different from each other (p=0.50). Time to microleakage was also significantly lower in group A and C than group B (p<0.0001), but group A and C were not significantly different from each other (p=0.37). CONCLUSION: In conclusion, using E. faecalis as our test bacteria, the microleakage of canals obturated with gutta percha and Roth's sealer was significantly less than canals obturated with gutta percha and EndoSequence BC sealer.

Biodegradability of resilon, a resin based root canal obturating material, by typical endodontic pathogens

Indiana University-Purdue University Indianapolis (IUPUI) / Root canal therapy is a recommended treatment for apical periodontitis. Root canal failure can occur as a result of microbial leakage. Resilon, a resin based root canal obturating cone material introduced in 2004 attempts to minimize leakage by a unique bonding method of the resin sealer to both the core material and to the dentin of the canal walls. Resilon has no bactericidal or antimicrobial effect15. Furthermore, it has been shown that Resilon is susceptible to alkaline and enzymatic hydrolysis as well as bacterial degradation.73, 184-186 It has been suggested that Resilon may be susceptible to degradation by microorganisms found in the infected root canal space. This work focuses on the susceptibility of root canal obturating materials to be degraded by endodontic pathogens seen in root canal treated teeth with apical periodontitis. The aim of this study was to determine if Resilon could be degraded by selected pathogenic bacteria found in the infected root canal system, and if this degradation is more severe than with gutta-percha, a conventional obturating material. P. intermedia, E. faecalis and P. aeruginosa, known endodontic pathogens were inoculated on discs of obturating material (Resilon or gutta-percha) mounted on a platform and placed in wells containing TSB incubated at 37°C under aerobic conditions. The discs were polished, examined by SEM, profilometry, and elemental analysis prior to inoculation to establish a baseline, and were then re-examined by these methods one month after inoculation. The overall results were inconclusive; and using these methods it cannot be determined that the selected bacteria can degrade Resilon. An ideal future study would utilize SEM with gold coated samples as well as atomic force microscopy to evaluate for changes in topographical features of these obturating materials. A notable finding was that Resilon turns black when exposed to bacteria, and the significance of this finding should be addressed in future studies.

Propriedades físico-químicas e citotoxicidade de materiais obturadores de canais radiculares de dentes permanentes / Physicochemical properties and cytotoxicity of root canal filling materials of permanent teeth

O presente estudo teve como objetivo avaliar as propriedades físico-químicas e citotoxicidade quatro cimentos endodônticos, com diferentes bases químicas, utilizados na rotina clínica do Cirurgião-dentista. Foram utilizados os cimentos AH Plus®, BioRoot RCS®, Endomethasone N® e Sealapex®. Foram realizados os testes físico-químicos de tempo de endurecimento, radiopacidade e escoamento, segundo a especificação nº 57 da ANSI/ADA (2012). Para o teste de citotoxicidade, foram coletadas amostras sanguíneas de seis doadores voluntários adultos, com idade entre 18 e 35 anos, de ambos os gêneros. Foi realizada a cultura de células primárias de linfócitos do sangue periférico humano em contato com o eluato dos cimentos obturadores, e a viabilidade celular foi analisada por meio do ensaio de MTT. Cada teste foi realizado em triplicata, tanto para propriedades físico-químicas como para o ensaio do MTT, sendo os resultados analisados empregando o programa GraphPad Prism 5®, por meio do teste Oneway ANOVA e pós-teste de Tukey, com nível de significância de 5%. De acordo com os resultados obtidos, tanto o AH Plus® quanto o BioRoot RCS® apresentaram tempo de endurecimento de 1450 minutos e 255 minutos, respectivamente; Endomethasone N® não tem tempo de endurecimento citado pelo fabricante, e Sealapex® não tomou presa no período do estudo (168 horas). Para o teste de radiopacidade, todos os materiais avaliados apresentaram radiopacidade acima de 3 mm alumínio, como recomendado pela especificação nº 57 da ANSI/ADA (2012). Para o teste de escoamento, segundo a ISO 6876:2012, um cimento endodôntico não deve apresentar um diâmetro inferior a 17 mm. Observamos a maior média para AH Plus® (39,68 mm), seguido em ordem decrescente por Sealapex® (35,73 mm), Endomethasone N® (34,22 mm) e BioRoot RCS® (23,81 mm). Quanto à viabilidade celular, Endomethasone N® foi citotóxico sobre os linfócitos em todas as concentrações (p<0,05). Sealapex® apresentou menor viabilidade celular na diluição de 1:32 (3,125mg/mL) (p<0,05). AH Plus® e BioRoot® não demonstrou citotoxicidade em nenhuma das concentrações avaliadas (p>0,05). Podemos concluir que todos os materiais avaliados atenderam aos parâmetros estabelecidos pela ANSI/ADA para os testes físico-químicos realizados. Além disso, o Endomethasone N® apresentou citotoxicidade sobre os linfócitos humanos, e os demais materiais analisados apresentaram viabilidade celular em níveis aceitáveis / The present study had as objective to evaluate the physicochemical properties and cytotoxicity of four root canal sealers with different chemical bases, used in the clinical routine of the Dentistry. The AH Plus®, BioRoot RCS®, Endomethasone N® and Sealapex® cements were used. The physicochemical tests of setting time, radiopacity and flowability were performed according to ANSI/ADA specification (nº 57). For the cytotoxicity test, blood samples were collected from six adult voluntary donors, aged 18-35 years, of both genders. Primary human lymphocyte cell cultures were placed in contact with the eluate from the plug cements, and cell viability was analyzed by the MTT assay. Each test was performed in triplicate, both for physicochemical properties and for the MTT assay. The results were analyzed using the GraphPad Prism 5® program, using the Oneway ANOVA test and Tukey\'s post-test, with a significance level of 5%. According to the results obtained, both AH Plus® and BioRoot RCS® presented setting time of 1450 minutes and 255 minutes, respectively; Endomethasone N® does not have setting time quoted by the manufacturer, and Sealapex® did not set in the study period (168 hours). For the radiopacity test, all evaluated materials had radiopacity above 3 mm aluminum, as recommended by the ANSI/ADA specification (nº 57). For the flow test, according to ISO 6876:2012, an endodontic cement must not have a diameter of less than 17mm. We observed the highest mean for AH Plus® (39.68 mm), followed in descending order by Sealapex® (35.73 mm), Endomethasone N® (34.22 mm) and BioRoot RCS® (23.81 mm). Regarding cell viability, Endomethasone N® was cytotoxic on lymphocytes at all concentrations (p <0.05). Sealapex® presented lower cell viability at 1:32 dilution (3.125 mg / mL) (p <0.05). AH Plus® and BioRoot® showed no cytotoxicity at any of the concentrations evaluated (p> 0.05). We can conclude that all materials evaluated met the parameters established by ANSI/ADA for the physicochemical tests performed. In addition, Endomethasone N® showed cytotoxicity on human lymphocytes, and the other materials analyzed presented cell viability at acceptable levels

Os insucessos são maiores quando se tratam de elementos dentais coronariamente destruídos ou comprometidos periapicalmente. Com o objetivo de identificar o efeito das variações clínicas que influenciam na longevidade do tratamento e a prevalência de descimentação de pinos, cárie marginal, fraturas radiculares e de lesões periapicais, foi realizada uma análise retrospectiva do desempenho clínico de dentes restaurados com pino de fibra de carbono. Foram avaliados 139 dentes em 75 pacientes, três anos após a data de cimentação dos pinos. Os dentes foram submetidos a tratamentos endodônticos, incluindo preparos dos condutos para pino, e tratamentos restauradores, realizados pelo mesmo endodontista e protesista, respectivamente, segundo protocolos padronizados. Através de exame clínico, radiográfico, fotográfico, laudos endodônticos e informações obtidas de prontuários, os dentes foram classificados de acordo com: grupos dentais, modalidade de tratamento endodôntico recebida, intervalo decorrido entre a última sessão endodôntica e a cimentação do pino, morfologia do pino, quantidade de remanescente dentinário coronário após preparo protético e sistema da coroa protética utilizado. Não foram constatadas descimentações de pinos, fraturas radiculares, ou cáries, e a classificação foi favorável para 99,3% (138 dentes) dos tratamentos endodônticos realizados. De acordo com os parâmetros analisados, os resultados desse estudo revelaram que as variações clínicas identificadas, consideradas desfavoráveis, não foram fatores determinantes para o insucesso, partindo-se do conjunto de procedimentos e materiais utilizados para o tratamento dos dentes desses pacientes / Clinical studies of devitalized teeth restored with posts show similar outcomes with regard to the percentage of faults or failures, despite many technical variations, methodological and operative between them. The failures are greater when dealing with teeth coronally destroyed or periapically compromised. The aim of this study was identify the effect of clinical variants that influence the longevity of treatment and prevalence of dislodgment of post, marginal caries, root fracture and periapical lesions was performed a retrospective analysis of clinical performance of teeth restored with carbon fiber post. 139 teeth were evaluated in 75 patients, three years after the date of cementing the posts. The teeth were root canal treatments, including preparations of the conduits to posts, and restorative treatments, performed by the same endodontist and prosthetics, respectively, according to standardized protocols. Through clinical, radiographic, photographic, endodontic reports and information obtained from medical records, the teeth were classified according to: tooth groups, type of endodontic treatment received, time span between the last endodontic session and bonded post, morphology of the post, amount of remaining dentin after crown preparation and prosthetic crown system used. There were no dislodgment of post, root fractures, or caries, and the rank was favorably for 99.3% (138 teeth) of endodontic treatment. According to these parameters, the outcomes of this study revealed that the clinical variations identified, that are considered unfavourable, were not determining for the failure, starting from the set of procedures and materials used for dental treatment of these patients

Análise microscópica de capeamento pulpar direto com diferentes materiais em dentes humanos / Microscope analyze of direct dental pulp capping with differents materials in human teeth

O capeamento pulpar direto busca preservar a vitalidade pulpar. Este trabalho apresenta a resposta tecidual de polpas humanas ao capeamento pulpar direto com o Agregado de Trióxido Mineral (MTA) comparado com o hidróxido de cálcio. Foram utilizados 20 pré-molares de dez pacientes com extração indicada para tratamento ortodôntico. Todos os dentes apresentavam vitalidade pulpar e neles foram feitos capeamentos pulpares diretos com MTA ou com o hidróxido de cálcio. Os dentes foram extraídos num período de 14 dias ou 45 dias, estabelecendo os seguintes grupos experimentais: Ca(OH) 14 dias; Ca(OH) 45 dias; MTA 14 dias e MTA 45 dias. Estes dentes foram analisados microscopicamente por dois examinadores treinados. Os dados obtidos foram tabulados, utilizando-se o teste Kappa para observar o grau de concordância entre os examinadores. Para avaliação comparativa entre as respostas citotóxicas dos materiais testados foi aplicado o teste não paramétrico Kruskal-Wallis e o teste de Dunn para comparações individuais e identificação da significância dos achados. Na análise dos resultados não houve diferença estatisticamente significante entre os grupos nos períodos experimentais de 14 e 45 dias, e nem na resposta inflamatória pulpar frente ao hidróxido de cálcio quando comparado ao MTA. Quanto à espessura e à organização da barreira dentinária reparatória com o hidróxido de cálcio, quando comparado com o MTA também não apresenta diferença estatisticamente significante. Conclui-se que o MTA satisfaz os requisitos de um material capeador pulpar em dentes de humanos, apresentando propriedades físicas e biológicas que o indicam para este tratamento. / The direct dental pulp capping aims to preserve the pulp vitality. This work shows the tissue response from human pulps to that procedure with the Mineral Trioxide Agregate (MTA) compared to calcium hydroxide - Ca(OH)2. Twenty premolars from 10 patients with extraction indicated to orthodontic treatment were used. All teeth showed pulp vitality and were treated with direct dental pulp capping with MTA or with calcium hydroxide. The teeth have been extracted within 14 or 45 days, stablishing the following experimental groups: Ca(OH)2 14 days; Ca(OH)2 45 days; MTA 14 days; and MTA 45 days. These teeth were analyzed under microscopy by two observers trained. The data obtained were put in table using the Kappa test to note the agreement degree between the observers. For the comparative assessment between the citotoxic response from material tested has been applied the non-paired Kruskal-Wallis test, and the Dunns test for individual comparisons and identifying the significance of findings. In the analysis of results there had no statistically significant difference between the groups in experimental periods of 14 days and 45 days, not even in the pulp inflammatory response to calcium hydroxide when compared to MTA. The thickness and arrangement of the healing dentin barrier faced with calcium hydroxide, when compared to MTA, also showed no statistically significant difference. It may be concluded that the MTA fulfils the requirements of pulp capping material in human teeth showing biological and physical properties that indicate it for this treatment.
