Weight Discrimination, Intersectional Oppression, and Mental and Emotional Health of Sexual and Gender Minority PeopleLeonard, Sarah January 2024 (has links)
This dissertation aims to fill important gaps in the weight discrimination literature by applying a non-pathologizing, intersectional approach and by focusing on previously understudied groups (i.e., sexual and gender minority (SGM) people, racial and ethnic minoritized people, and early adolescents).
Chapter 1 is an introduction to weight discrimination, including its origins in anti-fatness, its intersections with other systems of oppression, and the necessity to de-pathologize fatness to confront anti-fat oppression. Chapter 2 describes a scoping review of weight stigma/discrimination and its relationship with mental and emotional health among SGM people across the lifespan. Across 23 included studies, findings include consistent relationships between weight stigma and worse mental and emotional health and a scarcity of research focused on gender minority people, racial and ethnic minoritized people, early adolescents, and important outcomes such as self-injurious thoughts and behaviors (SITBs).
Aiming to address these gaps, Chapters 3 and 4 both describe cross-sectional analyses of data from a large national sample of 10- to 14-year-old adolescents from the Adolescent Brain Cognitive Development Study. Chapter 3 reports analyses of prevalence of weight discrimination among early adolescents with minoritized sexual, gender, racial, and ethnic identities and those who are gender nonconforming, as well as those at the intersections of these identities. Minoritized adolescents, including intersectionally minoritized adolescents, were significantly more likely to report weight discrimination compared to their peers. Chapter 4 reports analyses of weight discrimination in association with SITBs, and includes testing of sexual identity, gender identity, gender nonconformity, race/ethnicity, sex assigned at birth, and social support as potential moderators. It also includes analysis of intersectional discrimination (based on weight plus sexual identity and/or race/ethnicity) in association with SITBs.
Findings indicate that weight discrimination is associated with higher odds of SITBs; none of the proposed moderators had a significant effect. Intersectional discrimination was associated with heightened odds of SITBs. Finally, Chapter 5 presents a synthesis of results and discusses overall strengths, limitations, and implications. This includes implications for future research to fill identified gaps, policy changes to confront anti-fatness and protect adolescents from weight discrimination, and clinical interventions to make healthcare safe and affirming for fat and intersectionally minoritized adolescents.

Progressing from identification and functional analysis of precursor behavior to treatment of self-injury.Dracobly, Joseph Daniel 12 1900 (has links)
An evaluation of the utility of assessing and treating severe problem behavior through precursor functional analysis was completed. Ongoing measurement of problem behavior in two settings in the participant's natural environment was conducted for the duration of the study. A precursor to self-injurious behavior was identified using descriptive assessment and conditional probability analyses. A precursor functional analysis was then conducted. Subsequently, a treatment, in which precursor behavior produced the maintaining variable identified in the precursor functional analysis, was implemented in the natural environment. Treatment resulted in increases in the precursor behavior and decreases in self-injury in both the treatment setting and the second setting in which observations occurred. Implications of the assessment and treatment procedures are discussed.

Interventions infirmières en regard du phénomène de l’automutilation en milieu pédopsychiatrique : étude qualitative exploratoirePham, Phi-Phuong 06 1900 (has links)
En milieu pédopsychiatrique, les infirmières connaissent bien le phénomène de l’automutilation des adolescentes parce qu’elles sont souvent appelées à intervenir lorsque ce type de situation survient. Cependant, puisqu’elles ont parfois une mauvaise compréhension du phénomène, des perceptions erronées et des attitudes négatives quant à celui-ci, les infirmières deviennent moins aptes à intervenir auprès de cette clientèle.
Cette étude a pour objectifs d’explorer le phénomène de l’automutilation auprès des infirmières œuvrant en milieu pédopsychiatrique et de décrire les interventions infirmières privilégiées en cas d’automutilation ainsi que les facteurs du contexte organisationnel susceptible d’avoir une influence sur la façon dont ces interventions sont prodiguées.
L’étude privilégie un devis qualitatif de type exploratoire. Plusieurs méthodes de collecte des données ont été sélectionnées pour mener à bien cette étude entre autres des entrevues auprès des participantes et une période d’observation au sein de l’unité où la recherche a eu lieu. L’analyse des verbatim selon les étapes élaborées par Tesch (1990) a permis d’identifier sept catégories d’interventions infirmières : les interventions de relation d’aide, d’accompagnement, d’évaluation, de sécurité, d’enseignement, les interventions en lien avec les émotions et les perceptions de l’automutilation ainsi que les interventions pour éviter la contamination sociale. En ce qui a trait aux facteurs ayant une influence sur ces interventions infirmières, trois catégories sont ressorties des données : la constitution de l’équipe interpersonnelle, la lourdeur des tâches de l’infirmière en pédopsychiatrie ainsi que la culture organisationnelle dans laquelle évoluent les membres de l’équipe soignante.
Les résultats qui sont issus de cette étude permettront aux infirmières de mieux comprendre le phénomène de l’automutilation chez les adolescentes. Cette étude suggère également des pistes d’interventions qui pourraient aider les infirmières œuvrant en milieu pédopsychiatrique à mieux intervenir. De plus, cette étude serait susceptible de permettre aux gestionnaires des établissements d’apporter certains ajustements notamment en ce qui a trait à l’organisation du travail et à la formation des infirmières. / Nurses in the field of child psychiatry are familiar with the phenomenon of self-harm among adolescents because, when the situation arises, they are often called upon to intervene. However, sometimes misunderstandings, misperceptions and negatives attitudes in regard to self-harm may impair their ability to work with the patients involved.
This study seeks to explore self-harm from the standpoint of nurses in child psychiatry, describe recommended nursing interventions and outline organizational-context factors that may affect the way these interventions are executed.
An exploratory qualitative research approach was adopted, and several data-gathering methods were used, including interviews with participants and a period of observation in the unit in which the study was conducted. Analysis of the verbatim records in accordance with the procedure developed by Tesch (1990) elicited seven categories of nursing interventions: helping relationships, guidance, evaluation, safety, teaching, emotions and perceptions of self-harm, and prevention of social contamination. Three categories of factors that influence these interventions also emerged: interpersonal team work, nurses’ duties in child psychiatry, and the organizational culture in which members of the care team operate.
The results of this study will help nurses better understand adolescent self-harm. The study further suggests possible approaches that might help nurses in child psychiatry improve their interventions. The findings may also make it possible for the management of institutions to bring about changes, particularly in terms of work organization and training for nurses in the field.

It is justified by the comprehension that the symptom that expresses emotional suffering it is catching attention among mental health professionals and gaining a certain social visibility. This essay is structured as qualitative research and it is organized around the use of the psychoanalytical method and the investigation of a set of posts which, in the foreground, is signed by people who identify themselves as adepts of self-injurious acts and, in the background, there are also comments made by other Internet users linked to the main post. Successive readings of the material, in a floating attention state, allowed the interpretive production of two affective-emotional fields: "Lack of affection" and "Crime and Punishment". The overall frame indicates that those who have shown the conduct studied here live in an imaginative / imaginary hostile world, marked by the experience of persecutory guilty due to the lack of affection, care and consideration, pointing out that fundamental requirements in the constitution of the personality are not being satisfactorily contemplated. / Esta pesquisa objetiva investigar psicanaliticamente a experi?ncia vivida de pessoas que se autolesionam. Justifica-se na medida em que diz respeito ao sintoma que expressa sofrimento emocional que enseja preocupa??o entre profissionais da sa?de mental e que tem ganhado certa visibilidade social. Articula-se como pesquisa qualitativa mediante uso do m?todo psicanal?tico, pelo estudo, em primeiro plano, de um conjunto de postagens assinadas por pessoas que se identificam como praticantes de atos autolesivos e, em segundo plano, por outros internautas que se pronunciam sobre esse problema a partir de outras perspectivas. Self-harming behaviour is deliberate, conscious and often repetitive violation of physical integrity without intent to die. The work is divided into 11 chapters. The first are focused on behavioural disorders, self-harming behaviours, and crisis intervention. Other chapters deal with the goals, research questions and chosen method of treatment. The aim was to map the occurrence, factors and methods of self-harming and describe alternative techniques of reducing this behaviour. Qualitative research is used in this work - analysing the anamneses of individuals with behavioural disorders and interviews with ethopedic workers. The inquiry of documentation shows that self-harming behaviour is caused by negative effects of family and social environment. The most common methods include self-cutting of girls, fists and head pounding against the wall of boys. This research intended to explore and describe a self-injuring adolescent girl‘s experiences of using graphic symbolic analogue drawings as an alternative coping skill for emotional expression and a creative adjustment with regard to her problems. This study investigated concepts/themes from literature, regarding the phenomenon of self-injuring in adolescents; and explored theoretical aspects of the use of art/analogue drawing as part of a Gestalt therapeutic process. Data was collected by means of semi-structured interviews before and after the therapeutic drawing interventions, by observation and feedback from the participant. Data was analysed and interpreted against the background of the participant‘s problems, focusing on themes described in literature and experiential moments of awareness/reframing. Conclusions and recommendations are made; indicating shortcomings as well. This research may contribute to the knowledge base with regard to drawing techniques as alternative therapeutic intervention and also the value thereof for finding of immediate emotional release for patients. / Hierdie navorsing het beoog om ‗n selfbeserende adolessente meisie se ervaring van die gebruik van grafiese, simboliese analogiese-tekeninge as hanteringsvaardigheid vir uitdrukking van emosionele probleme en alternatiewe kreatiewe aanpassingstegniek te ondersoek en te beskryf.
