Fertile Grounds: Cultivating an Identity Through Architecture

This thesis explores the contributive role of architecture to the prosperity of a place. The research addresses the challenges and opportunities that rural regions face today by analyzing the general factors of marginalized rural communities through the lens of a specific community in mainland Portugal. A new approach for maintaining an authentic character, and a “sense of place” is presented which defends rural space as a genuinely experiential realm. The thesis proposes the design of a modern day treatment and research centre in the rural town of Manteigas, situated on one of the largest glacier valleys in Europe in the heart of the Serra da Estrela mountain range. Once alive with all the quaint characteristics that typify an idyllic rural identity, this town now faces a steep population decrease. Situated in the centre of the Zêzere glacial valley overlooking the town, the design accepts and interprets the natural geology of the site, harnessing the therapeutic thermal waters that continue to flow from the glacier line of the valley. The new centre represents not just a place for leisure and relaxation, but also an investigative laboratory for modern day natural healing therapies. The town of Manteigas is situated within three very distinct landscapes: a fertile landscape, a socio-cultural landscape, and a landscape of health and wellness. The design intervention responds to all these conditions and is dependent on each in its operation. This thesis is a proposal for a sustainable cycle of local and regional rejuvenation that will not be easily broken. The design proposal aims to build an infrastructure that will revive the identity of the community as a place of study and implementation of natural healing. The proposed design will also act as a catalyst to fuel future development and stimulate the local and regional economies.

The development of rapid genotyping methods for methicillin-resistant Staphylococcus aureus

Methicillin-resistant Staphylococcus aureus (MRSA) is an important human pathogen that is endemic in hospitals all over the world. It has more recently emerged as a serious threat to the general public in the form of community-acquired MRSA. MRSA has been implicated in a wide variety of diseases, ranging from skin infections and food poisoning to more severe and potentially fatal conditions, including; endocarditis, septicaemia and necrotising pneumonia. Treatment of MRSA disease is complicated and can be unsuccessful due to the bacterium's remarkable ability to develop antibiotic resistance. The considerable economic and public health burden imposed by MRSA has fuelled attempts by researchers to understand the evolution of virulent and antibiotic resistant strains and thereby improve epidemiological management strategies. Central to MRSA transmission management strategies is the implementation of active surveillance programs, via which unique genetic fingerprints, or genotypes, of each strain can be identified. Despite numerous advances in MRSA genotyping methodology, there remains a need for a rapid, reproducible, cost-effective method that is capable of producing a high level of genotype discrimination, whilst being suitable for high throughput use. Consequently, the fundamental aim of this thesis was to develop a novel MRSA genotyping strategy incorporating these benefits. This thesis explored the possibility that the development of more efficient genotyping strategies could be achieved through careful identification, and then simple interrogation, of multiple, unlinked DNA loci that exhibit progressively increasing mutation rates. The baseline component of the MRSA genotyping strategy described in this thesis is the allele-specific real-time PCR interrogation of slowly evolving core single nucleotide polymorphisms (SNPs). The genotyping SNP set was identified previously from the Multi-locus sequence typing (MLST) sequence database using an in-house software package named Minimum SNPs. As discussed in Chapter Three, the genotyping utility of the SNP set was validated on 107 diverse Australian MRSA isolates, which were largely clustered into groups of related strains as defined by MLST. To increase the resolution of the SNP genotyping method, a selection of binary virulence genes and antimicrobial resistance plasmids were tested that were successful at sub typing the SNP groups. A comprehensive MRSA genotyping strategy requires characterisation of the clonal background as well as interrogation of the hypervariable Staphylococcal Cassette Chromosome mec (SCCmec) that carries the β-lactam resistance gene, mecA. SCCmec genotyping defines the MRSA lineages; however, current SCCmec genotyping methods have struggled to handle the increasing number of SCCmec elements resulting from a recent explosion of comparative genomic analyses. Chapter Four of this thesis collates the known SCCmec binary marker diversity and demonstrates the ability of Minimum SNPs to identify systematically a minimal set of binary markers capable of generating maximum genotyping resolution. A number of binary targets were identified that indeed permit high resolution genotyping of the SCCmec element. Furthermore, the SCCmec genotyping targets are amenable for combinatorial use with the MLST genotyping SNPs and therefore are suitable as the second component of the MRSA genotyping strategy. To increase genotyping resolution of the slowly evolving MLST SNPs and the SCCmec binary markers, the analysis of a hypervariable repeat region was required. Sequence analysis of the Staphylococcal protein A (spa) repeat region has been conducted frequently with great success. Chapter Five describes the characterisation of the tandem repeats in the spa gene using real-time PCR and high resolution melting (HRM) analysis. Since the melting rate and precise point of dissociation of double stranded DNA is dependent on the size and sequence of the PCR amplicon, the HRM method was used successfully to identify 20 of 22 spa sequence types, without the need for DNA sequencing. The accumulation of comparative genomic information has allowed the systematic identification of key MRSA genomic polymorphisms to genotype MRSA efficiently. If implemented in its entirety, the strategy described in this thesis would produce efficient and deep-rooted genotypes. For example, an unknown MRSA isolate would be positioned within the MLST defined population structure, categorised based on its SCCmec lineage, then subtyped based on the polymorphic spa repeat region. Overall, by combining the genotyping methods described here, an integrated and novel MRSA genotyping strategy results that is efficacious for both long and short term investigations. Furthermore, an additional benefit is that each component can be performed easily and cost-effectively on a standard real-time PCR platform.

Estudo comparativo de duas técnicas laboratoriais para a detecção do HLA-B27 em pacientes portadores de espondiloartrite axial

Introdução: A Espondiloartrite axial (EpA-ax) é uma doença inflamatória e crônica do grupo das Espondiloartrites (EpA). Ela acomete o sistema músculo esquelético ocasionando inflamação das articulações axiais (especialmente da sacroilíaca). Quando essas manifestações são evidenciadas por exames de imagem, temos a caracterização da Espondilite Anquilosante (EA). Do contrário, chamamos de EpA-axial não radiográfica (EpA-ax-nr). A Espondilite Anquilosante (EA) se caracteriza pelo envolvimento inflamatório de articulações do esqueleto axial e apendicular. Seu diagnóstico é baseado em achados clínicos e radiológicos, além da elevação de marcadores inflamatórios e presença do HLA-B27. Várias são as metodologias que se propõem a identificar o HLA-B27 e a Polymerase Chain Reaction (PCR) é tida como referência. Sua ampla utilização esbarra, no entanto, em questões que levam em conta o custo, oferta do exame e o tempo até a obtenção do resultado. Neste contexto, a Citometria de Fluxo (CF) surge como uma alternativa capaz de ajudar o médico na identificação deste marcador genético que pode ser importante para o diagnóstico e prognóstico dessa doença. Objetivo: Avaliar a correlação entre as técnicas laboratoriais mais utilizadas na prática clínica (CF e PCR), comparando sensibilidade e especificidade para detecção do HLA-B27 em pacientes com diagnóstico estabelecido de EpA-ax. Métodos: Estudo transversal, comparativo, com pacientes maiores de 18 anos de idade selecionados por conveniência, coletados ao longo de 2015, com diagnóstico estabelecido de EpA-ax, segundo os do grupo internacional ASAS (working group - Assessment of SpondyloArthritis Intenational Society). Para a análise estatística o coeficiente Kappa (P<0,005 foi adotado como parâmetro. Para a análise estatística o coeficiente Kappa (P<0,005 foi adotado como parâmetro. O teste Exato de Fisher foi utilizado para comparar as variáveis categóricas, o teste t de Student, para variáveis quantitativas com distribuição simétrica de amostras independentes. E as variáveis com distribuição assimétrica foram comparadas pelo teste de Mann-Whitney. Resultados: O coeficiente Kappa obtido para a determinação da concordância entre os testes foi de 0,454. Foram incluídos no estudo 62 pacientes, desses, sessenta preencheram os critérios para diagnóstico de EA e 2 para EpA-ax não radiográfica (EpA-ax-nr), 64,5% dos pacientes eram do sexo masculino, 88,7% se autodeclararam brancos, a idade média (± desvio padrão) foi de 54,5±12 anos e o tempo mediano (percentis 25 e 75) de diagnóstico de 14 (9 e 24) anos. Dentre as características clínicas apresentadas pela população estudada houve diferença estatisticamente significativa para a artrite periférica, sendo mais frequente no grupo HLA-B27 negativo que no grupo HLA-B27 positivo (P=0,032). Na análise de correlação, 90,3% apresentaram tipagem HLA-B27 positiva por CF e 79,0% pela técnica de PCR. Tendo o PCR como padrão ouro, a CF apresentou uma sensibilidade de 98,0%, especificidade de 38,5% e uma acurácia de 85,5%. Conclusão: Apesar da baixa especificidade apresentada pela CF, nosso estudo demonstrou que a CF tem alta sensibilidade e boa acurácia o que a torna uma boa alternativa para ser utilizada como um teste de triagem na busca da caracterização da doença. Apesar da moderada concordância com a técnica de referência, a CF poderia ajudar o médico a excluir resultados falsamente negativos, racionalizando assim a investigação laboratorial para o diagnóstico da EA. / Introduction: Axial spondyloarthritis (SpA-ax) is an inflammatory and chronic disease of the Spondyloarthritis (SpA) group. It affects the skeletal muscle system causing inflammation of the axial joints (especially of the sacroiliac). When these manifestations are evidenced by imaging tests, a characterization of Ankylosing Spondylitis (AS). Otherwise, we call the non-radiographic SpA-axial (SpA-ax-nr). AS marked by the inflammatory involvement of the axial and appendicular skeletal joints. The diagnosis is based on clinical and radiological findings, as well as the on the elevation of inflammatory markers and presence of HLA-B27. There are several methodologies to identify the HLA-B27 gene and a Polymerase Chain Reaction (PCR) is considered the reference method. However, its use on a large scale does not progress because it takes into account the cost, offer of the exam and the running time to obtain the result. In this context, Flow Cytometry (FC) emerges as an alternative method, helping with the physician in the determination of this genetic marker, important for the diagnosis and prognosis of disease. Objective: To evaluate the correlation between FC and PCR, comparing sensitivity and specificity for detection of HLA-B27 in patients with established diagnosis of SpA-ax. Methods: A cross sectional study including 62 patients recruited during 2015 month was conducted in Hospital de Clínicas de Porto Alegre, an university public hospital. The sample, recruited by convenience in the SpA clinic, was composed of patients ≥ 18 years old, fulfilling the Assessment of Spondyloarthritis (ASAS) criteria SpA-ax. All participants underwent HLA-B27 typing through FC and PCR. The kappa statistic was used to calculate the concordance between the FC and PCR. Taking PCR as the gold standard, sensitivity and specificity of FC to detect HLA-B27 were calculated. Results: The Kappa coefficient obtained to determine the agreement between the tests was 0.454. Sixty two patients were included in the study, sixty met the criteria for diagnosis of AS and two for SpA-ax-nr, 64.5% of the patients were male, 88.7% were self-declared mean age (± standard deviation) was 54.5 ± 12 years and median time (25th and 75th percentiles) for diagnosis was 14 (9 and 24) years. Among the clinical characteristics presented by the population studied, there was a statistically significant difference for peripheral arthritis, wich was more frequent in the HLA-B27 negative group than in the HLA-B27 positive group (P = 0.032). In the correlation analysis, 90.3% presented HLA-B27 positive typing by FC and 79.0% by PCR technique. When PCR was considered the gold standard, CF had a sensitivity of 98.0%, specificity of 38.5% and an accuracy of 85.5%. Conclusion: Despite the low specificity presented by FC, our study demonstrated that FC has high sensitivity and good accuracy, which makes it a good alternative to be used as a screening test in the search for the characterization of the disease. Despite the moderate agreement with the reference technique, FC could help the physician to exclude falsely negative results, thus rationalizing laboratory investigation for the diagnosis of AS.

Koordinovaná rehabilitace dětí s opakovaným onemocněním dýchacích cest / Coordinated Rehabilitation for Recurrent Respiratory Diseases of Children

Respiratory diseases are ones of the most common diseases, which are treated by general practitioners. Not only the right diagnosis is essential, but the treatment as well. The treatment of respiratory diseases is usually long-term and often requires repeating. Health treatment of young children in a spa improves their life and health quality, inclusion in a collective of similar aged children, reduces complications of their disease, doses of medicine and highly reduces the risk of development of a chronical disease in adulthood. The aim of this thesis is to chart the use of coordinated rehabilitation in spa Lázně Luhačovice, a. s. and to survey coordinated rehabilitation awareness of spa medical and non medical personnel and children's accompaniment. Three hypotheses were set. Hypothesis 1: Children's accompaniment, who has been to spa health resort repeatedly percieves the care of his child as coordinated unlike the one who has been to spa health resort just once. Hypothesis 2: Medical staff of children spa health resorts percieves the most the medical treatment part of coordinated rehabilitation in contrast with non medical staff. Hypothesis 3: Children's accompaniment is satisfied the most with the medical treatment part of coordinated rehabilitation unlike the pedagogical and social one. A strategy of quantitative research was used, data was gathered by standardized questionnaires in June and July 2015. The standardized questionnaires were given to two different research groups. First one was spa health resort personnel, second one children's accompaniment in the spa Lázně Luhačovice, a. s. Based on statistical calculations the hypotheses can not be rejected, the results are not statistically significant. From the data obtained from the questionnaires, it is noticable that from the spa health resort medical and non medical personnel point of view, there is percieved only the part, which they provide. On the other hand, from the children's accompaniment point of view the rehabilitation is percieved as coordinated in all the parts provided. It is thanks to the fact that the accompaniment, who is in the spa health resort on the expenses of health insurance company, can see all the parts of coordinated rehabilitation as they undergo it together with their children. This thesis contributes to widening awareness of using parts of coordinated rehabilitation in children repeated respiratory diseases treatment, which are provided in facilities of spa Lázně Luhačovice, a.s. and in other health resorts, where there is this kind of care provided as well.

The spas and sanatoria in Silesia (Czech part) from the 2nd half of the 19th century until the end of the 1930s Abstract The work is focused on the development and historical development of spas, spa resorts and sanatoria in Silesia in its Czech part, which used natural healing resources, favourable climatic conditions, baths, balneal treatment and resting mode. The object of interest is Gräfenberg Spa, later Gräfenberg-Frývaldov (today's Spa Jeseník), spa in Dolní Lipová, Sanatorium in Zlaté Hory, Spa Karlova Studánka, Jánské koupele (Mel ské koupele near Opava), spa in Darkov, small spa in Komorní Lhotka and pulmonary sanatorium in Jablunkov, which was right from the beginning a sanatorium for the treatment of pulmonary tuberculosis. During the treatment, a considerable attention was paid to the use of the resting mode and of the favourable climatic environment. On this territory, we can see different conditions of the creation of individual spas and sanatoria. Namely, on the basis of the effects of natural resources, the Spa of Karlova Studánka, Jánské koupele and Darkov Spa was built, which used natural mineral waters. The sanatorium in Zlaté Hory, a spa resort in Komorní Lhotka and the pulmonary sanatorium in Jablunkov were established thanks to the effects of a favourable climate. The origin of the...

Neste período, como candidato a deputado federal, ele irrompeu como uma espécie de herói salvador nas lutas de representações que procuravam impor um conjunto de ideias para responder às crises sociais, políticas, econômicas e identitárias dessa região historicamente agitada por movimentos separatistas. Para isso, tal como um personagem de si mesmo, o jovem professor atuou conscientemente durante mais de dez anos para teatralizar uma imagem pública, conquistar distinção social, acumular prestígio e consagrar o seu nome no cenário regional. Novato na vida partidária, este ator social interpretou os anseios de seu tempo, mobilizou os circuitos de opinião das elites e promoveu uma autopropaganda intensiva para afirmar a vinculação de sua figura a uma série de valores profundamente enraizados na cultura local. Neste período, as crises que perturbavam aquela sociedade fermentavam um campo muito propício para a elaboração de mitologias. Por tudo isso, em seu discurso, ao invocar o poder das forças históricas em nome da união de seu povo; ao apontar com firmeza o caminho “certo” e “seguro” para a superação das crises; e ao anunciar com entusiasmo a iminente conquista da civilização, da cultura e da prosperidade em sua região, o guerreiro messiânico encenado de modo espetacular pelo candidato Mário Palmério correspondeu à ansiedade dos eleitores por um signo de modernidade capaz de suplantar o monopólio da economia agrária que, naquele momento de instabilidade, parecia definitivamente condenada à bancarrota. A presente pesquisa procura compreender a ascensão profissional, social e política de Mário Palmério, relacionando as aspirações da cultura... / In 1950 Professor Mário Palmério (1916-1996), future writer of Vila dos Confins (1956), was represented by the press of Triângulo Mineiro as a great myth of the regional political culture. During this period, as a federal congressman candidate, he emerged as a hero savior in the representative campaigns that wanted to impose a set of ideas to respond to social, political, economic and identity crises of this region historically troubled by separatist movements. To do so, as a character of himself, the young teacher worked conscientiously for over ten years to dramatize his public image, achieve social distinction, get people’s prestige and consolidate his name in the regional scene. As a beginner in political life, this social activist interpreted the anxieties of his time, mobilized the opinion of the elite towards his name and promoted an intensive self-publicity to assert his linking to a series of values deeply rooted in the regional imaginary. In that time, the crises that plagued that society fed a very favorable field to the development of mythologies. For all that, in his speech, to invoke the power of historical forces on behalf of the union of his people; to firmly point the “right” and “safe” way to overcome the crisis; and when he enthusiastically announced the imminent civilization, culture and prosperity conquest in his region, the messianic warrior spectacularly staged by the enshrined Mário Palmério corresponded to the voters’ anxiety for a sign of modernity able to overcome the monopoly of an agrarian economy that, in that instable time, definitely seemed to be condemned to bankruptcy. This research aims to comprehend Mário Palmério’s professional, social and political rise, connecting the aspirations of the regional culture to the symbol denoted by this character on that society to play a sacred and heroic representation of his image and history / En la década de 1950, el profesor Mário Palmério (1916-1996), futuro autor de “Vila dos Confins” (1956), fue representado por la prensa del Triángulo Minero como un verdadero mito de la cultura política regional. En ese período, como candidato a diputado federal, él irrumpió como una especie de héroe salvador en las luchas de representaciones que procuraban imponer un conjunto de ideas para responder a las crisis sociales, políticas, económicas y de identidades de esa región históricamente agitada por movimientos separatistas. Para ello, tal como un personaje de uno mismo, el joven profesor actuó conscientemente durante más de diez años para teatralizar una imagen pública, conquistar distinción social, acumular prestigio hasta consagrar su nombre en el escenario regional. Novato en la vida partidaria, este actor social interpretó las ansiedades de su tiempo, movilizó los circuitos de opinión alrededor de su nombre y promocionó una auto propaganda intensiva para afirmar la vinculación de su figura a una serie de valores profundamente arraigados en la cultura regional. En ese periodo, las crisis que perturbaban aquella sociedad fermentaban un campo muy propicio para la elaboración de mitologías. Por todo ello, en su discurso, al invocar el poder de las fuerzas históricas en nombre de la unión de su pueblo , al apuntar con firmeza el camino “cierto” y “seguro” para la superación de las crisis; y al anunciar con entusiasmo la eminente conquista de la civilización, de la cultura y de la prosperidad en su región, el guerrero mesiánico protagonizado de modo espectacular por el consagrado Mário Palmério correspondió a la ansiedad de los electores por un signo de modernidad capaz de suplantar el monopolio de la economía agraria que, en aquel momento de inestabilidad, parecía definitivamente condenada a la bancarrota. Negociação e convívio cultural : Museu Histórico da Sentinela Paulista, 1957-1972 /

Resumo: Estudo do engajamento de parte da população local na formação do acervo e no processo de institucionalização e consolidação do Museu Histórico do Município de Franca, mediado por José Chiachiri entre 1957 e 1972, com recuo ao período de 1937 a 1940, quando lugares de memória e monumentos históricos foram valorizados sob impacto da visita de caravana da Bandeira. Identifica escolhas entre representações sociais da história local, como critério para avaliar sentido, significado e vestígios de unidade entre semióforos exibidos ao público, denominados de núcleos vetoriais de sentido. Releva procedimentos museais de preservação, investigação e extroversão do acervo recebido evergeticamente. EXPLICITA alianças sociais, políticas, étnicas, institucionais e outras solidariedades e conflitos, conforme testemunhos da época, condição sine qua non para compreensão do espetáculo legado. O corpus documental privilegia as revistas Sertaneja e Vilafranca, dirigidas por José Chiachiri, para reconstituir o espetáculo colocado em cena e em valor no Museu por meio de seu discurso expográfico. Indica, por fim, possibilidades de atualização museológica desse patrimônio herdado / Abstract: Study of the engagement of part of the local population in the patrimony formation and in the process of institutionalization and consolidation of the Municipal Historical Museum of Franca, mediated by José Chiachiri, between 1957 and 1972, moving backwards to the period of 1937 to 1940; when places of memory and historical monuments were valued under the impact of the Bandeira Expedition Caravan visit. This study identifies choices between social representations of the local history, as a criterion to evaluate sense, meaning and traces of unity between semiopheres exhibited to the public, called vectorial nucleus of meaning. It demonstrates museal procedures of preservation, investigation and extroversion of the patrimony received evergetically. It makes EXPLICIT social alliances, political, ethnical, institutional and others solidarities and conflicts, in conformity with testimonies from the period, sine qua non condition to comprehend the passed on spectacle. The documental corpus grants privilege to the Sertaneja and Vilafranca magazines, managed by José Chiachiri, in order to reconstruct the spectacle on the scene in the Museum by his exposed speech. This diploma work studies quality of life of patients suffering from Bechtěrev disease (Ankylosing Spondylitis), who had global four-weeks cure in Berta Health Spa in Třeboň from 1st January to 31st July 2006. Hypothesis says that six months after the end of balneotherapy the quality of life of these patients will be higher than before. For confirmation or confutation of this hypothesis there was used quantitative method of research, in form of adjusted questionnaire for detection of quality of Life by SEIQoL concept (Schedule for the Evaluation of Individual Quality of Life). By this measure respondents are allowed to nominate five areas of life (aims of life), which are most important, together with percentage rate of their satisfaction with each and the relative importance of each. Average life-quality index of the respondents before balneotharapy was 60,4 %, six months after the end of balneotharapy it was 62,7 %. Thus six months after the end of the cure the respondents have higher quality of life. Then there were mentioned and discussed differences in perception of quality of life of various age groups and of gender. There was also accomplished order of the most often mentioned aims of life, as well an analysis of ``health{\crqq} as alife aim was done. This diploma work could be used in cultural work and for improvement of informedness in problematic of life quality. But mainly, it could be perhaps taken as a little controversial report to the discussion about using of assorted exploratory concepts for evaluation of quality of life.
