Utvärdering av omvårdnadsdokumentation i elektronisk patientjournal på kirurgisk vårdavdelning / Evaluation of the nursing documentation in electronic health record on surgical ward

SAMMANFATTNING Syfte. Utvärdera omvårdnadsdokumentationens kvalité och omfattning i elektronisk patientjournal på kirurgisk vårdavdelning. Metod. De senaste 60 journalerna från två kirurgiska vårdavdelningar valdes ut genom bekvämt urval. Varje journal lästes och bedömdes av båda författarna. Varje steg i omvårdnadsprocessen utvärderades efter granskningsmall och bedömdes som fullständig, för omfattande eller ofullständig. Resultat. Standardvårdplan användes i alla granskade journaler. Antalet steg i omvårdnadsprocessen som fanns dokumenterade varierade mellan fem och nio. Anamnes, status och effekter av åtgärder fanns beskrivna i majoriteten av journalerna. I samtliga journaler fanns utförda åtgärder dokumenterade. Omvårdnadsepikris fanns i större delen av njurtransplantationsjournalerna, men inte alls i struma/hyperparatyroidism (HPT)-journalerna. Majoriteten av uppdaterade status bedömdes som ofullständiga. Sexton av struma/HPT-journalerna innehöll inte anteckningar i rapportbladet. Av dem som hade rapportbladsanteckningar bedömdes majoriteten vara för omfattande. Samtliga njurtransplantationsjournaler hade för omfattande anteckningar i rapportbladet. Ingen av journalerna hade en individuell vårdplan. Slutsats. Omvårdnadsdokumentationen i den elektroniska patientjournalen bedömdes som ofullständig då det inte gick att få en tydlig bild av patientens omvårdnadsproblem och omvårdnadsbehov. Kvalitén på dokumentationen behöver förbättras. Detta kan ske genom att minska dokumentationen i rapportbladet och istället använda standardvårdplan och uppdaterat status i större omfattning. Fortsatt utbildning och återkoppling krävs för att förbättra dokumentationen. / ABSTRACT Aim. To evaluate the quality and extent of the nursing documentation in electronic health record on surgical ward. Method. The latest 60 health records from two surgical wards were selected by convenience sample. Both authors read each health record. Every step of the nursing process was evaluated with a nursing documentation audit and was classified as complete, too extensive or incomplete. Results. Standardized care plan was used in all electronic health records. Numbers of steps documented in the nursing process were five to nine. Nursing history, status and outcome were documented in most health records. Done interventions were documented in all health records. Goiter/hyperparathyroidism (HPT)-records had no nursing discharge note, while the kidney transplantation-records had one in almost every health record. Majority of updated statuses were evaluated as incomplete. Sixteen of the goiter/HPT-records had no notes of occasional matters, all kidney transplantation-records had too extensive notes. No individualized care plan was found. Conclusion. The total nursing documentation in the electronic health records were evaluated as incomplete. The quality of documentation needs to be improved. This can be achieved by less documentation of occasional matters, using the standardized care plan, updating status more often and further education and feedback.

A Population-based Study on the Association of Standardized Protocols in the Emergency Department for Childhood Asthma with Outcomes in Ontario, Canada

Objectives: To describe the use of standardized protocols (SPs) in emergency departments (EDs) across Ontario for childhood asthma and analyze the association of SPs with hospital admissions, 7-day ED re-visits, and outpatient follow-up visits. Methods: Population-based retrospective cohort study using health administrative data and survey data. EDs were categorized as having the optimal, other, or no SP for each outcome. Associations were tested with generalized estimating equations. Results: Between 2006/04/14-2009/02/28, 46,510 children with asthma were seen in 146 EDs, with 43 (29.5%) having SPs. Children treated in EDs with the optimal SP compared to no SP had no significant differences in hospital admissions (AOR 1.17; 95% CI 0.91, 1.49) or ED re-visits (AOR 1.09; 95% CI 0.85, 1.40) but were more likely to have follow-up visits (AOR 1.27; 95% CI 1.02, 1.59). Conclusions: SPs for childhood asthma are not common in Ontario EDs and had little impact on outcomes measured.

參考從2001到2005在台灣股市的股息對股票的影響的研究,在根據event-study and SAR method這兩種方式之實證研究可以得到本次的台灣股市裡面除權的效應。 / Referring to our research into the ex-dividend effects on stocks traded on TSE from 2001 to 2005, we come to the following conclusions under the event-study and SAR method. There should be negative ARs on the three days prior to the ex-date and a positive AR on the ex-dividend day, no matter in a bear or bull market. Stocks with cash-only dividends present lower ARs on the three days prior to the ex-date and the ex-dividend day while stock-only dividend ones suffer from higher ARs. The performances of stocks with balanced dividends are just in-between. The most significant ex-dividend effects turn up when it comes to stocks which go ex-dividend in season, with a positive CAR to the seventh day after the ex-date. On the other hand, the effects on the early-ex-dividend stocks exhibit insignificance generally. Later-ex-dividend stocks demonstrate the lowest fluctuation of ARs. However, the simultaneous decline of the index in the ex-dividend season is likely to result in higher-significant ARs. In terms of industry, the ex-dividend effects on electronic companies are more significant than on non-electronic companies. Given a bear market, there used to be negative CARs on electronic companies after shareholders’ meeting; on the contrary, in a bull market, there were positive CARs.

Cut Once, Measure Everywhere: The Stability of Percentage of Students Above a Cut Score

Large-scale assessment results for schools, school boards/districts, and entire provinces or states are commonly reported as the percentage of students achieving a standard – that is, the percentage of students scoring above the cut score that defines the standard on the assessment scale. Recent research has shown that this method of reporting is sensitive to small changes in the cut score, especially when comparing results across years or between groups. This study extends that work by investigating the effects of reporting group size on the stability of results. For each of ten group sizes, 1000 samples with replacement were drawn from the May 2009 Ontario Grade 6 Assessment of Reading, Writing and Mathematics. The results showed that for small group sizes – analogous to small schools – there is little confidence and that extreme caution must be taken when interpreting differences observed between years or groups.
