Foi realizado estudo clínico randomizado, duplo-cego e controlado comparando topiramato com placebo. Homens e mulheres entre 18 e 62 anos com diagnóstico de TEPT segundo os critérios do DSM-IV foram selecionados do ambulatório do Programa de Violência da Universidade Federal de São Paulo (Prove- UNIFESP) entre abril de 2006 e dezembro de 2009. Os participantes foram avaliados através da escala de TEPT administrada por clínicos (CAPS), escala de Impressão Clínica Global (CGI) e Inventário de Depressão de Beck (BDI). Após 1 semana de wash-out, trinta e cinco pacientes com TEPT foram randomizados para tratamento com topiramato ou placebo e acompanhados por 12 semanas. Foi observado que 82,35% dos pacientes que receberam topiramato apresentaram melhora dos sintomas do TEPT. Na análise de eficácia, os pacientes do grupo topiramato demonstrou demonstraram redução significativa dos sintomas de revivescência: memórias intrusivas, flashbacks e pesadelos (CAPS B; p=0,004) e sintomas de evitação e entorpecimento emocional (CAPS C; p = 0,0001). Além disso, encontramos melhora significativa do escore do CAPS total (topiramato –57,78; placebo –32,41; p = 0,0076) nos pacientes do grupo experimental. Topiramato foi bem tolerado e efetivo principalmente nos sintomas de revivescência e anestesia e evitação do TEPT. Este estudo dá suporte para o uso de anticonvulsivantes na melhora dos sintomas do TEPT. / The aim of this study was to evaluate the efficacy and tolerability of topiramate in patients with posttraumatic stress disorder (PTSD). We conducted a 12-week double-blind, randomized, placebo-controlled study comparing topiramate to placebo. Men and women aged 18 to 62 years with diagnosis of PTSD according to DSM-IV were recruited from the outpatient clinic of the violence program of Federal University of São Paulo Hospital (Prove- UNIFESP), São Paulo City, between April 2006 and December 2009. Subjects were assessed for the Clinician-Administered Posttraumatic Stress Scale (CAPS), Clinical Global Impression (CGI), and Beck Depression Inventory (BDI). After 1-week period of washout, 35 patients were randomized to either group. The primary outcome measure was the CAPS total score changes from baseline to the endpoint. In all 82.35% of patients in the topiramate group exhibited improvements in PTSD symptoms. The efficacy analysis demonstrated that patients in the topiramate group exhibited significant improvements in reexperiencing symptoms: flashbacks, intrusive memories, and nightmares of the trauma (CAPS-B; p = 0.04) and in avoidance/numbing symptoms associated with the trauma, social isolation, and emotional numbing (CAPS-C; p = 0.0001). Furthermore, the experimental group demonstrated a significant difference in decrease in CAPS total score (topiramate –57.78; placebo –32.41; p = 0.0076). Topiramate was generally well tolerated. Topiramate was effective in improving reexperiencing and avoidance/numbing symptom clusters in patients with PTSD. Estudos indicam que psicopatologias são recorrentes nas mulheres que sofrem abuso sexual, mais comumente o Transtorno de Estresse Pós-Traumático (TEPT) e a depressão. Objetivos: Avaliar os escores de TEPT, Depressão e Desesperança em mulheres vítimas de violência sexual que estiveram em acompanhamento ambulatorial no Centro de Atenção Integral à Saúde da Mulher (CAISM) da Universidade Estadual de Campinas (UNICAMP). Métodos: Estudo de coorte longitudinal em que foram avaliadas 67 mulheres, no primeiro mês pós-violência e 52 no sexto mês, através da Escala de Avaliação de Transtorno de Estresse Pós-Traumático Administrada pelo Clínico (CAPS), do Inventário de Depressão de Beck (BDI), da Escala de Desesperança Beck (BHS) e de entrevista estruturada para avaliar a autopercepção do trauma e da saúde. Os escores foram correlacionados às variáveis sociodemográficas, da violência, percepção e atitudes da mulher. Resultados: TEPT presente em 64,2% dos casos na avaliação inicial, sendo 13,5% entre severo e muito severo e 29,9% em moderado, e na avaliação final presente em 21,2% das mulheres. Quanto à depressão, 100% das mulheres entrevistadas no primeiro mês preencheram os critérios, sendo que 47,7% delas entre mínimo e leve e 52,3% entre moderado e grave. No sexto mês, 63,5% estavam com depressão mínima e leve e 19,2% com moderada e grave. A desesperança esteve presente em 94%, sendo 71,6% mínima e leve e 22,4% moderada e grave na avaliação inicial e em 90,3% na avaliação final, sendo 73% entre mínima e leve e 9,7% entre moderado e grave. A análise por regressão logística múltipla mostrou que o TEPT moderado/grave no primeiro mês associou-se a uma única variável, a depressão moderada/grave OR19,6 (5,3-71,87; IC 95%); o diagnóstico de TEPT no sexto mês foi associado positivamente a duas variáveis, número de agressores >1 OR14,5 (1,3-166,7; IC 95%) e antecedente psiquiátrico OR 5,5 (1,2-25,6; IC 95%). Conclusões: as prevalências dos sintomas de TEPT, depressão e desesperança aos seis meses foram, respectivamente, de 21,2%, 19,2% e 9,7%, sendo que ainda há necessidade de abordagem de tratamento em saúde mental para esse grupo especifíco de mulheres apos o acompanhamento ambulatorial de seis meses / Abstract: Introduction: Sexual violence is a problem that affects many women worldwide and its consequences include physical complications, psychological, social and sexual. Studies indicate that psychopathologies are recurring in women who suffer sexual abuse, commonly Post-Traumatic Stress Disorder (PTSD) and Depression. Objectives: Evaluate the scores of PTSD, depression and hopelessness of women victims of sexual violence who were in outpatient setting in Women's Health Center (CAISM) in Univesity of Campinas (UNICAMP). Methods: A cohort longitudinal study in which 67 women were evaluated in the first month post-violence and 52 in the sixth month through the Clinician Administered Posttraumatic Stress Disorder (CAPS), the Beck Depression Inventory (BDI), Beck Hopelessness Scale (BHS) and a structured interview to assess the perception of trauma and health. The scores were correlated to socio-demographic variables, violence, perceptions and attitudes of women. Results: PTSD present in 64.2% of cases at baseline, and 13.5% between severe and very severe and 29.9% in moderate, and final evaluations present in 21.2% of women. As for depression, 100% of women met criteria in the first month, and 47.7% of them between minimal and mild and 52.3% from moderate and severe. In the sixth month, 63.5% were minimal and mild depression and 19.2% with moderate and severe. Hopelessness was present in 94% and 71.6% minimum and 22.4% mild and moderate or severe at baseline and 90.3% in the final evaluation, 73% between minimal and mild and 9.7% between moderate and severe. A multiple logistic regression analysis showed that PTSD moderate / severe in the first month was associated with a single variable, depression, moderate / severe OR 19, 6 (5.3 to 71.87, 95% CI), the PTSD diagnosis at six months was positively associated with two variables, number of assailants> 1 OR 14, 5 (1.3 to 166.7; 95%) and previous psychiatric OR 5,5 (1.2 to 25.6, 95% CI). Conclusions: the prevalence of PTSD symptoms, depression and hopelessness at six months were respectively 21.2%, 19.2% and 9.7%, and still need to approach mental health treatment for this specific group of the women after six months of outpatient follow-up / Mestrado / Ciencias Biomedicas / Mestre em Tocoginecologia

O trabalhador vitima do sequestro apresenta transtornos mentais graves após a ocorrência desse tipo de violência comprometendo seu desempenho profissional e psicossocial. Cabe ressaltar que o cenário atual do setor bancário apresenta uma organização precária devido ao intenso processo de reestruturação do trabalho, intensificado a partir dos anos 90, com objetivo de adaptação ao mercado financeiro altamente competitivo, impactando diretamente na saúde ocupacional dessa população. Os principais Transtornos Mentais e Comportamentais verificados nos afastamentos de bancários segundo as estatísticas da Previdência Social são: depressão, transtorno de adaptação e transtorno de estresse pós-traumático (TEPT). O desenvolvimento dessa pesquisa se deu a partir de entrevistas em profundidade com quatro bancários vítimas de sequestro e foram compreendidas a partir do referencial metodológico da análise de conteúdo. Os resultados obtidos demonstram o impacto psicossocial negativo do sequestro na vida das vitimas agravadas pelo inadequado manejo destas ocorrências contribuindo para as manifestações de transtornos psíquicos após o evento traumático. Com esse estudo, espera-se contribuir para o desenvolvimento de intervenções nas práticas organizacionais, na preservação da segurança e saúde dos trabalhadores bancários. A gravidade desse tipo de violência ao trabalhador deve também receber maior atenção da Saúde e Segurança Pública / Abstract: This study looks for a specific type of violence affecting an increasing number of employees of banks, which is the kidnapping of employees and their families to withdraw money directly from the vaults of the banks. The victim of the kidnapping, if the bank employee, has severe mental disorders after the occurrence of such violence, jeopardizing their professional and psychosocial performance. It is noteworthy that scenario it's banking sector is precarious in Brazil, due to the intense process of restructuring work, intensified since the 90s , in order to adapt to the highly competitive financial market, impacting directly on the occupational health of this population . Major mental and behavioral disorders seen in this type of occupation, leading to absenteeism, are according to statistics of Brazilian Social Welfare: depression, adjustment disorder and disorder post-traumatic stress disorder (PTSD). The development of this research takes place from interviews with four bank officials who were kidnapped. These interviews are then understood from the methodological framework of content analysis, and the results obtained may contribute to the triggering of interventions on organizational practices in preserving the health and safety of bank employees. The battlefield at home: the meaning of homelessness from the female veteran’s perspective

Female veterans face a host of unique challenges; females often carry the burden of serving in the armed forces, while balancing marriages, motherhood, and care giving responsibilities in their home lives. As the veterans return to their lives as civilians, the females who served in the military must deal with the possibility of sexual harassment, sexual assault, and rape while in the armed services. Female service members are twice as likely to have Post Traumatic Stress Disorder (PSTD) than their male service members and are three to four times more likely to become homeless. Understanding this view of homelessness from the female veteran’s perspective is limited due to small sample sizes in previous research efforts. However, with the increasing numbers of homeless female veterans it is imperative to understand the risk factors. A qualitative descriptive study was conducted using a modified framework for studying vulnerable populations. The study was designed to explore the meaning of homelessness from the female homeless veteran’s perspective. Second, risk factors were examined for homelessness and the services necessary for the female veteran to exit the homeless cycle. Third, the data were coded and analyzed to identify patterns and commonalities of multiple psycho social factors such as unstable family support, domestic violence, job loss, affordable housing options, substance abuse, mental and physical health issues. These factors were cited as the leading risk factors contributing to the homeless state of this sample of female veterans. The data collection consisted of ten homeless female veterans participating in a private, audio taped interview using a semi-structure interview tool. Resources listed as a necessity to end homelessness consisted of affordable housing, job security, earning a living wage income, transportation, remaining drug free, and being awarded disability. The pathway to homelessness varied for each participant, but they all demonstrated a tremendous amount of resiliency.

L’objectif de cette recherche est d’étudier l’influence spécifique et combinée du contexte de violence conjugale sur le développement socio-affectif des enfants âgés de 5 ans et demi à 12 ans, et d’examiner leurs représentations de la violence et de leurs relations familiales. Notre étude s’appuie sur le modèle de la sécurité émotionnelle de Cummings et Davies (1994). Dans cette perspective, notre échantillon se compose de 32 mères et de 46 enfants âgés de 5 ans et demi à 12 ans, vivant en contexte de violence conjugale. Les mères ont renseigné trois questionnaires qui évaluent l’intensité de la violence conjugale à l’aide du « Conflict Tactic Scale II » (Cyr, Fortin & Chénier, 1997), l’adaptation socio-affective de leur enfant par le questionnaire « Children Behavior Checklist » (Achenbach, 1991), ainsi qu’un questionnaire de renseignements généraux. Les enfants ont, quant à eux, renseigné quatre questionnaires qui permettent de saisir leurs perceptions de la violence avec le « Children’s Perception of Interparental Conflict » (Cyr & Fortin, 2001), le processus de parentification grâce au « Parentification Questionnaire Youth » (Fortin, 2005), les conflits de loyauté à l’aide du « questionnaire des conflits de loyauté » (Fortin, 2005) et les symptômes de stress post-traumatique avec le « Trauma Symptom Checklist for Children » (Briere, 1989). Enfin, le « Systemic Analysis of Group Affiliation » (Compagnone, 2009) a été utilisé avec les enfants afin de saisir leurs représentions de la cohésion et du pouvoir au sein de la famille. Les principaux résultats mettent en évidence que la moitié des enfants présentent des problèmes extériorisés et 82,6% des problèmes intériorisés. Le degré de l’exposition à la violence de l’enfant influence ses représentations de la cohésion familiale, son sentiment de menace et de blâme, ses conflits de loyauté et la présence de certains symptômes de stress post-traumatique. Les représentations de l’enfant sur la violence et les relations familiales, telles que la parentification, le sentiment de blâme, les conflits de loyauté et les représentations de la cohésion et du pouvoir dans la famille, sont prédictives de la présence de symptômes de stress post-traumatique et de troubles de l’adaptation socio-affective. L’effet modulateur de la représentation du pouvoir et de la cohésion familiale, ainsi que les représentations de la violence et les conflits de loyauté ont été mis en évidence. Enfin, plus de la moitié des enfants de l’échantillon, en plus d’être exposés à la violence conjugale, ont aussi été directement maltraités, et les conséquences relevées sont d’autant plus majorées. L’ensemble des résultats obtenus nous permet de proposer des perspectives de recherche et des pistes d’intervention. / The aim of this research is to study the influence of specific and combined context of domestic violence on socio-emotional development of children aged between 5 and a half to 12 and to consider their representations of violence and their family relationships. Our study is based on the model of emotional security Cummings and Davies (1994). With this in mind, our sample consists of 32 mothers and 46 children aged 5 and a half to 12, living in a context of domestic violence. The mothers have filled in three questionnaires assessing first, the intensity of domestic violence using the « Conflict Tactic Scale II » (Cyr, Fortin & Chénier, 1997), the socio-emotional adaptation of their children with the questionnaire « Children Behavior Checklist » (Achenbach, 1991) and then, a general information questionnaire. As for the children, they have filled in four questionnaires that allow to understand their perceptions of violence with « Children's Perception of Interparental Conflict » (Cyr & Fortin, 2001), the process of parentification through the « Parentification Questionnaire Youth » (Fortin 2001), loyalty conflicts with the « loyalty conflicts questionnaire » (Fortin, 2005) and post-traumatic stress symptoms with « Trauma Symptom Checklist for Children » (Briere, 1989). Finally, the « Systemic Analysis of Group Affiliation » (Compagnone, 2009) has been used with the children to understand their representations of cohesion and power within their families.The main results show that half of the children have externalizing problems and 82.6% internalizing problems. The degree of exposure to child abuse influences his representations of family cohesion, his feeling of threat and blames, his loyalty conflicts and the presence of certain symptoms of PTSD. The children’s representations of violence and family relationships, such as parentification, the feeling of blame, loyalty conflicts and the representations of cohesion and power in the family, are predictive of symptoms of post-traumatic stress disorders and social-emotional adjustment. The modulating effect of the representation of power and family cohesion, as well as depictions of violence and conflicts of loyalty have been highlighted. Finally, more than half of the children in this sample, have been exposed to domestic violence but also to direct abuse, that’s why the consequences underlined are all the more important. All these results allow us to propose research perspectives and methods of interventions.
