Interferência da dexametasona no ciclo pulmonar da infecção por Strongyloides venezuelensis em ratos Wistar / Interference of dexamethasone in pulmonary cycle of infection by Strongyloides venezuelensis in Wistar rats

As estrongiloidíases são parasitoses intestinais causadas por várias espécies do gênero Strongyloides e apresentam distribuição cosmopolita. O objetivo deste estudo foi investigar a interferência do tratamento diário com dexametasona no ciclo pulmonar durante a infecção por Strongyloides venezuelensis em ratos. Investigamos o efeito do tratamento com glicocorticóides na migração de eosinófilos, mastócitos e macrófagos no parênquima pulmonar. Demonstramos ainda, como os efeitos do tratamento diário com a dexametasona atuam na formação do granulomas. Três principais aspectos foram encontrados: 1) Aumento da inflamação hemorrágica, provocado pela passagem das larvas para o espaço alveolar; 2) Significante redução da migração de eosinófilos e mastócitos no eixo axial pulmonar e, 3) Interferência crucial na migração de eosinófilos para os focos de passagem das larvas e, conseqüente, impedimento da organização do granuloma, sugerindo que a formação da rede de fibras reticulares deve ter um papel crucial no aprisionamento do parasita, favorecendo um melhor desempenho das células inflamatórias na eliminação do mesmo. Este trabalho mostrou que o uso de drogas com ação imuno-modulatória, tais como a dexametasona, pode interferir na morbidade no ciclo pulmonar durante a infecção por S. venezuelensis, contribuindo para revelar os mecanismos envolvidos na sua patogênese. / The aim of this study was investigate the interference of dexamethasone treatment in the pulmonary cycle of Strongyloides venezuelensis infection in rats. The immunomodulatory effects on the inflammatory process generated by the passage of the larvae into pulmonary parenchyma during their migration were analyzed. Three principal effects were found: 1) Increased alveolar hemorrhagic inflammation provoked by the passage of larvae into the alveolar spaces; 2) Significant decrease of eosinophil and mast cell migration to the axial septum of the lungs and 3) Impaired eosinophil migration to the parasite foci and deficient formation of the reticular fiber network, interfering with the granuloma organization. Assim, o objetivo deste trabalho foi caracterizar a infecção por Strongyloides venezuelensis em ratos Lewis e avaliar se a mesma modula as características clínicas, imunológicas e histopatológicas da encefalite autoimune experimental (EAE) nestes animais. Na primeira etapa, caracterizamos as fases aguda e de recuperação da infecção e avaliamos os padrões de resposta imune nestas duas fases. Na segunda etapa, avaliamos o efeito de uma ou várias infecções com S. venezuelensis na evolução da EAE. Os animais foram avaliados diariamente quanto ao peso e escore clínico da doença e a eutanásia foi realizada na fase de recuperação da EAE para avaliação da resposta imune (produção de citocinas e anticorpos) e do processo inflamatório no SNC. A frequência de células T CD4+CD25+Foxp3+ no baço e nos linfonodos (inguinais e poplíteos) também foi determinada após infecção única (fase aguda e de recuperação) ou múltipla com este helminto. De acordo com os diversos parâmetros avaliados, os resultados demonstraram que a infecção com S. venezuelensis não modificou a progressão da EAE em ratos Lewis e também não alterou a frequência de células T CD4+CD25+Foxp3+ nos órgãos linfóides secundários. Considerando a hipótese da higiene, estes resultados sugerem a necessidade de um estudo comparativo entre as diferentes espécies de helmintos para avaliar seu potencial imunorregulatório / Multiple sclerosis (MS) is a chronic immune-mediated demyelinating disease of the central nervous systems (CNS). This and other immunemediated diseases are clearly increasing in more developed countries. According to the hygiene hypothesis this is due to a decreased contact between the human population and certain organisms as helminths, mycobacteria and lactobacillus. In this context, the main objective of this investigation was to evaluate if one or multiple infections with S. venezuelensis was able to modify the development of experimental autoimmune encephalomyelitis (EAE) in Lewis rats. Based on the hygiene hypothesis, an at least partial protective effect was expected. Initial assays indicated that recovery from S. venezuelensis in Lewis rats was associated with a strong Th2 response. Rats infected one or multiple times with this helminth were then submitted to EAE induction by immunization with myelin associated with complete Freund adjuvant. Differently from what was supposed, previous infection with S. venezuelensis was not able to modify body weight, clinical score and inflammation at the CNS. In addition, this infection was not associated with alteration in the frequency of CD4+CD25+Foxp3+ T cells in the spleen and lymph nodes (inguinal + popliteal). A estrongiloidíase é uma parasitose intestinal sendo considerada a quarta maior causada por nematódeos. O mecanismo de defesa contra a estrongiloidíase é mediada pela ativação de células de perfil Th2, que amplificam a resposta celular através da secreção de mediadores inflamatórios. O que faz da estrongiloidíase um grave problema de saúde pública, é o desenvolvimento da hiperinfecção, principalmente devido ao uso de glicocorticóides, onde ocorre aumento do número de larvas e fêmeas que se disseminam por todo organismo. Estudos demonstraram que algumas infecções helmínticas têm sido acompanhadas por hipertrofia e hipercontratillidade da musculatura intestinal, via JAK-STAT 6. Entretanto pouco se sabe sobre a influência desta via nas alterações da parede muscular do duodeno durante infecção pelo Strongyloides venezuelensis. O presente trabalho objetivou investigar as alterações morfológicas, imunológicas e patológicas da musculatura lisa intestinal que ocorrem em decorrência da infecção experimental pelo S. venezuelensis, bem como a interferência do tratamento com Dexametasona e o papel da via JAK - STAT 6 neste processo. Ratos Wistar foram inoculados com larvas de S. venezuelensis, tratados com dexametasona e sacrificados nos dias 5, 7, 14 e 21. Foram realizadas diversas colorações com a finalidade de quantificar as fêmeas adultas no duodeno, realizar morfometria da musculatura duodenal, quantificar eosinófilos e células caliciformes. Foi realizada análise da expressão gênica do gene STAT 6. Nossos resultados mostraram hiperplasia das células caliciformes, infiltrado eosinofílico e espessamento da musculatura lisa duodenal. Houve aumento na expressão de STAT 6 nos animais infectados. O tratamento com a Dexametasona inibiu drasticamente estas alterações. Entretanto o número de parasitas foi significativamente maior nos ratos infectados tratados quando comparados aos infectados. As alterações intestinais durante a infecção ocorreram na tentativa de expulsar o parasita e resolução da infecção. Contudo, a inibição deste processo provocada pela Dexametasona possivelmente retardou ou impediu a resolução da infecção. / Strongyloidiasis is an intestinal parasitosis with an obligatory pulmonary cycle, which represents the fourth largest parasitosis caused by nematodes. The mechanism of defense against strongyloidiasis is mediated by activation of Th2 cells, which amplify the cellular response through the secretion of inflammatory mediators. Strongyloidiasis is a serious public health problem due the development of hyperinfection, due to the use of glucocorticoids, where the number of worms and females increases, and disseminate to other organs. Studies have shown that some helminth infections have been accompanied by hypertrophy of intestinal muscles and hypercontractility, JAK-STAT 6 pathway. However little has been reported about on the influence of JAK-STAT 6 pathway in changes of the muscular wall of the duodenum during Strongyloides venezuelensis infection. The aim of this study was to identify the morphological, immunological and pathological changes of the intestinal smooth muscle during Strongyloides venezuelensis in Wistar rats and to determine the effects of Dexamethasone treatment and role of JAK-STAT 6 pathway in these process. Wistar rats were inoculated with S. venezuelensis larvae, treated with dexamethasone and killed at 5, 7, 14 and 21 days. Morphological and morphometric analyzes with routine stains to quantify globet cells, eosinophils and measure the circular and longitudinal layers of duodenal smooth muscle. Performed gene expression analysis of STAT 6. Goblet cell hyperplasia and increased of intestine smooth muscle wall thickness and eosinophils levels were elevated throughout the course of the infection. Moreover, an increase in the expression of STAT 6 in infected animals. The morphological findings and the immunomodulatory response to the infection were drastically reduced in dexamethasone-treated rats. However, the number of worms was significantly higher on infected and treated rats with Dexamethasone compared to just infected ones. The intestinal changes during infection ocurred in an attempt of expel the parasite and elucidate the infection. Although, the inhibition of the process caused by Dexamethasone possibly delay or prevent the resolution of infection

O tratamento envolve fármacos como benzimidazois e ivermectina. Contudo, estes não apresentam uma resposta homogênea no combate à doença. Nesse contexto, surge a necessidade de novas opções para o tratamento desta parasitose, sendo uma alternativa viável o uso de plantas medicinais. A espécie Spondias lutea, árvore frutífera comumente chamada por "cajazeiro", foi avaliada neste estudo a fim de identificar possível propriedade terapêutica no tratamento desta parasitose. Neste estudo foram testados o extrato etanólico e frações de S. lutea in vitro sobre fêmeas parasitas de Strongyloides venezuelensis, nas concentrações de 0,4 mg/mL, 0,2 mg/mL, 0,1 mg/mL e 0,05 mg/ mL. Todas as amostras foram solubilizadas em PBS e em seguida as análises foram repetidas para posterior comparação de resultados com a inclusão de polivinilpirrolidona (PVP) junto às amostras como um agente facilitador da solubilização. As observações foram realizadas em intervalos crescentes entre 2 - 72 horas após incubação em estufa de atmosfera de CO2- 5% a uma temperatura de 37°C, utilizando placas de cultura de 24 poços, em meio RPMI acrescido penicilina (10.000 UI/L) e estreptomicina (0,05 g/L). Foram observados os parâmetros motilidade e mortalidade dos parasitas. O estudo constatou que a fração 4 apresentou o melhor resultado atingindo 67% de mortalidade na maior concentração avaliada após 2 horas de incubação e atingindo 100% após 4 horas de incubação na mesma concentração. Observou-se ainda 100% de mortalidade nas concentrações 0,2 mg/mL e 0,1 mg/mL após 12 horas de incubação e 100% de mortalidade com 24 horas de incubação em todas as concentrações avaliadas. A ivermectina não ocasionou a morte dos vermes em nenhuma das concentrações testadas, observando-se discreta redução de motilidade dentro do período estudado / Abstract: Strongyloidiasis is a neglected parasitosis whose control is very difficult, especially because of its ability to auto-infect, by the irregular response to therapeutic and for the risk of hyperinfection. Treatment involves drugs such as benzimidazoles and ivermectin. However, these drugs do not present a homogeneous response against the disease. In this context it is important to look for new alternatives in the treatment of this disease and one is the use of medicinal plants. Spondias lutea, commonly called as cajazeiro fruit tree species was evaluated in this study in order to identify possible therapeutic properties in the treatment of this disease. In this study, the ethanol extract and fractions of S. lutea were tested in vitro against parasitic females of Strongyloides venezuelensis at concentrations of 0.4 mg/mL, 0.2 mg/mL, 0.1 mg/mL and 0.05 mg/ml. All samples were solubilized in PBS and then analyses were repeated for comparison of results with the inclusion of the samples with polyvinylpyrrolidone (PVP) as a solubilization facilitator. The observations were made at increasing time intervals between 2-72 h after incubation in CO2 greenhouse at 5% and 37°C. Motility parameters and mortality of parasites were observed. The study found that the sample F4 showed the best result, reaching 67% mortality at the highest concentration evaluated after 2 hours of incubation, reaching 100% after 4 hours of incubation in the same concentration. There was 100% of mortality at the concentrations 0.2 mg/ml and 0.1 mg/ml after 12 hours incubation and 100% mortality after 24 hours of incubation at all concentrations evaluated. Ivermectin did not cause the death of the worms in any of the tested concentrations. There was only slight decrease of motility within the study period. Atualmente os fármacos de escolha para o tratamento desta doença são o tiabendazol, albendazol e ivermectina, porém comumente associados a casos de recidivas, baixa eficácia e a ocorrência de resistência e tolerância a fármacos. Diante da necessidade de medicamentos mais eficazes, menos tóxicos, as plantas medicinais são uma alternativa. Neste trabalho foi avaliado o efeito anti-helmíntico dos extratos etanólicos das folhas de 25 espécies de árvores frutíferas sobre fêmeas adultas parasitas de Strongyloides venezuelensis in vitro. As observações foram feitas em 3 concentrações diferentes por um período de 72 horas. A atividade anti-helmíntica foi observada em 68% dos extratos testados em pelo menos uma das concentrações testadas durante o período de observação, com exceção dos extratos etanólicos de Litchi chinensis, Aceima smeithii, Eugenia involucrata e Butia capitata, a fração orgânica de Pouteria caimito que não apresentaram atividade contra o parasita. Os extratos etanólicos que apresentaram maior eficácia foram Spondias lutea (Cajá) e Pouteria caimito (Caimito), com 100% de mortalidade em todas as concentrações testadas apresentando DL50 < 0,05 mg/mL. Estes extratos sofreram um processo de partição com 2 frações, aquosa e orgânica. O Spondias lutea mostrou eficiência anti-helmíntica nas 2 frações testadas enquanto que o Pouteria caimito apresentou atividade apenas com a fração aquosa. Para todos os extratos testados, foi considerado significativo (p < 0,0001) o efeito do tempo em relação à motilidade dos vermes, e o efeito do extrato e da concentração em relação ao tempo, assim como o efeito do extrato e da concentração na motilidade dos vermes / Abstract: Strongyloides stercoralis is one of the main nematode specie with high prevalence among humans, with a rating of 100 million infected people in the role world. Nowadays the chosen drugs for the treatment of this disease are the thiabendazole, albendazole and ivermectin, but generally associated with recurrence cases, low efficiency, resistance occurrence and drugs tolerance. Given the need for more efficient drugs, with less toxicity, the medicinal plants are the alternative. The present work were evaluated the antihelminthic effect from the ethanolic extracts from the leaves of 25 fruit trees species over adult parasite females of Strogyloides venezuelensis in vitro. Observations were made in 3 different concentrations during 72 hours. 68% from the tested extracts presented antihelminthic activity in at least one tested concentration during the observation period, with an exception of the ethanolic extracts of Litchi chinensis, Aceima smeithii, Eugenia involucrata, Butia capitata and an organic fraction of Pouteria caimito that did not showed any activity against the parasite. The ethanolic extracts which presented major efficiency were Spondias lutea (Cajá) and Pouteria caimito (Caimito), with a mortality of 100% in all tested doses and with DL50 < 0,05 mg/mL. These extracts suffered a partition process, obtaining 2 fractions, aqueous and organic. The Spondias lutea showed antihelminthic efficiency in two tested fractions while Pouteria caimito showed activity only with the aqueous fraction. Também foi comparada a sensibilidade da PCR com as técnicas histológica e parasitológica na caracterização desta infecção utilizando amostras de tecidos e fezes, respectivamente. No primeiro artigo, o número de ovos por grama de fezes (OPG) foi determinado pela técnica de McMaster modificada e o DNA foi extraído para análise por PCR. Comparou-se a sensibilidade de ambos os métodos para o diagnóstico do parasita em fezes de ratos inoculados com 40, 400 e 4000 larvas infectantes (L3) e essas infecções foram consideradas leve, moderada e grave, respectivamente. Na PCR dois pares de primers foram empregados, um foi desenhado a partir da seqüência parcial de rDNA de S. venezuelensis e o outro, amplifica o rDNA de diversas espécies deste gênero. Nas amostras oriundas dos animais com infecção leve o primer específico não detectou DNA, já o primer gênero apresentou maior sensibilidade que a quantificação de OPG. No segundo artigo foi analisada a influência do sexo dos hospedeiros na suscetibilidade às infecções leve, moderada e pesada. Machos com infecção moderada e pesada apresentaram maior média de OPG e de número de parasitas recuperados do intestino delgado em comparação com as fêmeas, porém esse fenômeno não foi observado nas infecções leves. No terceiro artigo, os animais foram inoculados com 4000 L3 para a avaliação da dinâmica da infecção e caracterização da resposta imune durante as fases aguda e de recuperação. In the first paper the number of eggs per gram of feces (EPG) using a McMaster modified technique was enumerated and DNA was extracted to do PCR analysis. Sensitivity of both methods to diagnosis the parasite in feces of rats inoculated with 40, 400 or 4000 infective larvae (L3) was compared. These infections were considered light, moderate and heavy, respectively. Two PCR primer pairs were employed, a specific one was designed from a S. venezuelensis rDNA partial sequence and the other one amplifies several species within this genus. In light infection, the specific primer did not detect DNA in any sample, in the other hand the genus primer presented higher sensitivity than EPG. The host sex influence in susceptibility to light, moderate and heavy infection was evaluated in the second experiment. The FEC means and the mean parasite infection intensity were higher in males than in females in animals with moderate and heavy infection. But this phenomenon was not observed in light infection. In the third paper, animals were inoculated with 4000 L3 to determine the kinetics infection and to characterize the immune response during acute and recovery phases. The kinetics of infection was daily measured by FEC during 32 days after infection. Parasite eggs were detected in the feces for the first time at day 6 post-infection, but the maximal egg number was observed at days 8 and 11 post-infection.
