Abordagem telefônica para promoção da saúde. / Telephonic approach for the promotion of a healthy lifestyle.

A adoção de hábitos saudáveis como a prática de atividades físicas, alimentação equilibrada, sono saudável e a cessação do tabagismo tem sido apontada como importante medida para melhorar a saúde, a qualidade de vida e facilitar a prevenção e controle de algumas doenças crônicas na população. Assim, intervenções em saúde que visam orientar e facilitar a adoção de tais hábitos são componentes importantes e necessários no planejamento de programas de promoção da saúde. Mesmo que desconsiderássemos qualquer outro benefício além do financeiro, este já seria suficiente para justificar ações voltadas para a promoção da saúde por possibilitarem otimização dos recursos materiais e humanos nas intervenções em saúde . Visando ampliar ainda mais este benefício, novas tecnologias capazes de reduzir custos e manter qualidade de atendimento têm sido pesquisadas. É neste contexto que a abordagem telefônica para promover saúde surge como uma opção. Este estudo tem a finalidade de avaliar a efetividade da abordagem telefônica na promoção da saúde. O estudo foi realizado no Centro de Promoção da Saúde da Faculdade de Medicina da Universidade de São Paulo, com 46 indivíduos. Foram feitos de 3 a 5 contatos por pessoa, por meio dos quais foram feitas orientações e intervenções baseadas nos preceitos da Terapia Cognitivo-Comportamental e do Modelo Transteórico. Os resultados mostraram que, após a intervenção, os clientes que receberam cinco contatos telefônicos, mudaram o comportamento, passando a praticar alguma atividade física com regularidade e melhorando a qualidade do sono, esta mudança foi estatisticamente significativa. Os outros comportamentos estudados não apresentaram mudança estatisticamente significativas após 3 ou 5 contatos, apesar de alguns clientes apresentarem mudanças favoráveis em seu comportamento, principalmente no grupo que recebeu 5 contatos. Concluindo, a abordagem telefônica favoreceu a adoção de hábitos saudáveis, mostrando-se, portanto, como uma estratégia de apoio favorável à promoção da saúde. / The adoption of healthy habits such as physical activities, balanced diet, healthy sleep, and the cessation of smoking have been pointed out as important measures to improve health, quality of life and further the prevention and control of some chronic diseases in our population. As such, healthy interventions aiming to guide and further the adoption of these habits are important and necessary compounds in the planning of programs dedicated to promoting healthy lifestyles. Even if we do not consider any other benefit, aside from the financial benefit, this would already be enough to justify actions taken for promoting a healthy lifestyle since this could optimize material and human resources in the areas dedicated to health. Aiming to extend even more this benefit, research is being done to find new technologies capable of reducing costs and maintaining the quality of attendance. It is in this context that the telephonic approach for healthy lifestyle promotion appears as an option. This study intended to evaluate the effectiveness of the telephonic approach for healthy lifestyle promotion. This study was performed at the Healthy Lifestyle Promotion Center of the University of São Paulo Medical School, with 46 individuals. Effects of telephonic SMS reminders influence on adherence to scheduled medication pick up appointments among adults on antiretrovirals at the Swakopmund State Hospital ART clinic Namibia

Magister Public Health - MPH / Background: Adherence of patients on antiretroviral therapy to lifelong treatment is a major challenge within the public health system in Namibia. Missed appointments have been shown to contribute to poor clinical outcomes and treatment failure, which may necessitate switching to more expensive antiretroviral regimens. In resource limited settings monitoring of appointments for antiretroviral medication pick up is a documented and feasible method for assessing minimum levels of adherence to antiretroviral medication. Aim: This study was aimed at evaluating the effects of telephonic short message service reminders influence on adherence to scheduled antiretroviral medication pick up appointments. It was also aimed at evaluating how socio-demographic parameters might moderate the effectiveness of short message service reminders. Study design: A randomized double blind controlled study design was employed. Methodology: Stable patients attending the ART clinic were recruited and randomly assigned to either an intervention or control group, until the sample size of 398 was reached in the two arms of the study. The study populations were adult patients’ ≥ 18 years who have been enrolled on treatment for ≥ 3months. The intervention group received an unasked for single short message service reminder, sent 48 hours before their scheduled appointments and continued with standard care, while the control group received standard care without any reminder. The study participants were blinded to their study group. Also, research assistants involved in collecting baseline and outcome data were blinded to study participants study group. Baseline data was collected through a structured questionnaire. Study participants were followed up for four consecutive scheduled ARV pick up appointments. The following outcome data were collected at each follow up visit; number of days late after scheduled appointment, adherence measured by pill count and 3 days self-report recall of adherence. Main results: The pre-intervention survey revealed that only 60% of the study participants were willing to be reminded of their medication pick up appointment. Overall, the SMS reminder improved adherence to medication pick up appointments by 1.6 times as compared to no reminder and also reduced the risk of missing medication pick appointments by 22% as compared to no SMS reminder. Study participants that received a reminder were also two times more likely to achieve optimal adherence to their medication, compared to those who received no reminder. The SMS reminder improved adherence to antiretroviral medication by 11% in this study, while the mean difference in the number of days late to collect antiretroviral medication was significantly reduced by about 4 days by the intervention. Participants that are employed were more likely to adhere to antiretroviral medications as compared to the unemployed. Ironically participants that were on ART for less than one year and those that had treatment supporters reminding them of their medication appointments were significantly less likely to honour their medication pick up appointments. Conclusion: Being employed was significantly associated with attaining optimal adherence to antiretroviral medication. There were no other significant associations between the patients socio- economic and demographic characteristics and adherence to scheduled medication pick up appointments, or to adherence to medication. Recommendations: The Ministry of Health and Social Services should consider rolling out SMS reminders to ART sites with similar settings as Swakopmund State Hospital ART Clinic. Patients that are willing to receive the reminder should be targeted in the scaling up of the roll out. Late and missed medication pick up appointments could be used as an easy proxy measurement for assessing adherence to ART.

Non-Pharmacological Approaches for Pain Management in Sickle Cell Disease: Development of a Mindfulness-Based Intervention

<p>Background: Sickle Cell Disease (SCD) is a genetic hematological disorder that affects more than 7 million people globally (NHLBI, 2009). It is estimated that 50% of adults with SCD experience pain on most days, with 1/3 experiencing chronic pain daily (Smith et al., 2008). Persons with SCD also experience higher levels of pain catastrophizing (feelings of helplessness, pain rumination and magnification) than other chronic pain conditions, which is associated with increases in pain intensity, pain behavior, analgesic consumption, frequency and duration of hospital visits, and with reduced daily activities (Sullivan, Bishop, & Pivik, 1995; Keefe et al., 2000; Gil et al., 1992 & 1993). Therefore effective interventions are needed that can successfully be used manage pain and pain-related outcomes (e.g., pain catastrophizing) in persons with SCD. A review of the literature demonstrated limited information regarding the feasibility and efficacy of non-pharmacological approaches for pain in persons with SCD, finding an average effect size of .33 on pain reduction across measurable non-pharmacological studies. Second, a prospective study on persons with SCD that received care for a vaso-occlusive crisis (VOC; N = 95) found: (1) high levels of patient reported depression (29%) and anxiety (34%), and (2) that unemployment was significantly associated with increased frequency of acute care encounters and hospital admissions per person. Research suggests that one promising category of non-pharmacological interventions for managing both physical and affective components of pain are Mindfulness-based Interventions (MBIs; Thompson et al., 2010; Cox et al., 2013). The primary goal of this dissertation was thus to develop and test the feasibility, acceptability, and efficacy of a telephonic MBI for pain catastrophizing in persons with SCD and chronic pain. </p><p>Methods: First, a telephonic MBI was developed through an informal process that involved iterative feedback from patients, clinical experts in SCD and pain management, social workers, psychologists, and mindfulness clinicians. Through this process, relevant topics and skills were selected to adapt in each MBI session. Second, a pilot randomized controlled trial was conducted to test the feasibility, acceptability, and efficacy of the telephonic MBI for pain catastrophizing in persons with SCD and chronic pain. Acceptability and feasibility were determined by assessment of recruitment, attrition, dropout, and refusal rates (including refusal reasons), along with semi-structured interviews with nine randomly selected patients at the end of study. Participants completed assessments at baseline, Week 1, 3, and 6 to assess efficacy of the intervention on decreasing pain catastrophizing and other pain-related outcomes. </p><p>Results: A telephonic MBI is feasible and acceptable for persons with SCD and chronic pain. Seventy-eight patients with SCD and chronic pain were approached, and 76% (N = 60) were enrolled and randomized. The MBI attendance rate, approximately 57% of participants completing at least four mindfulness sessions, was deemed acceptable, and participants that received the telephonic MBI described it as acceptable, easy to access, and consume in post-intervention interviews. The amount of missing data was undesirable (MBI condition, 40%; control condition, 25%), but fell within the range of expected missing outcome data for a RCT with multiple follow-up assessments. Efficacy of the MBI on pain catastrophizing could not be determined due to small sample size and degree of missing data, but trajectory analyses conducted for the MBI condition only trended in the right direction and pain catastrophizing approached statistically significance. </p><p>Conclusion: Overall results showed that at telephonic group-based MBI is acceptable and feasible for persons with SCD and chronic pain. Though the study was not able to determine treatment efficacy nor powered to detect a statistically significant difference between conditions, participants (1) described the intervention as acceptable, and (2) the observed effect sizes for the MBI condition demonstrated large effects of the MBI on pain catastrophizing, mental health, and physical health. Replication of this MBI study with a larger sample size, active control group, and additional assessments at the end of each week (e.g., Week 1 through Week 6) is needed to determine treatment efficacy. Many lessons were learned that will guide the development of future studies including which MBI strategies were most helpful, methods to encourage continued participation, and how to improve data capture.</p> / Dissertation

La presente tesina titulada “Modernización de la red de acceso de un operador móvil para el caso de Lima Metropolitana” expone una propuesta para un operador que tiene como objetivo poder incrementar su cobertura de servicio de segunda y cuarta generación, para esto se definen conceptos teóricos en base a lo que son las tecnologías de telefonía móvil celular de tipo GSM, UMTS y LTE y se compararan con las características técnicas de los equipos que se plantean usar demostrándose que estos pueden optimizar la red de acceso del operador en cobertura y funcionamiento para brindar un mejor servicio en base a 2G y 4G. Para lograr el objetivo propuesto, esta tesina describe el diseño y dimensionamiento de proyecto, pasos para su ejecución, especificaciones técnicas de los equipos, procedimiento para la puesta en servicio y resultados en base a pruebas del servicio y KPI. The current thesis entitled “Modernización de la red de acceso de un operador móvil para el caso de Lima Metropolitana” exposes a proposal for a mobile operator that has as a target increase their second and fourth generator coverage. For achieve this we have to defined theoric concepts based on the mobile telecommunication technology GSM, UMTS and LTE and compare with the features of equipment proposed to use proving that the coverage and performance of the access network of the operator can be improved for provide a better 2G and 4G service. For achieve this target proposed, this thesis describes the design and sizing of Project, steps for execution, technical features of equipment, on air procedure and results base on the testing service and KPI.

Tras la difusión del video Kouri-Montesinos el 14 de septiembre del año 2000, el gobierno de Fujimori entró en crisis. Una de las principales acciones tras aquel evento fue la desactivación del Servicio de Inteligencia Nacional SIN (aún durante el gobierno de Fujimori), y luego la reforma del sistema, durante el gobierno de transición de Paniagua en el año 2001. Por diversas presiones y perspectivas políticas, en ese periodo fueron despedidos, cesados, desactivados o reasignados un gran contingente de analistas, operativos, agentes e informantes: cerca de 2 mil personas terminaron su relación con el Sistema de Inteligencia Nacional. Como parte de una investigación sobre la transformación de la inteligencia peruana, este artículo tiene tres objetivos: a) describir el contexto de la desactivación y la reforma para entender los objetivos y causas del despido y desactivación de este contingente de agentes y analistas; b) determinar el rango numérico de agentes y analistas que fueron despedidos, cesados o desactivados entre la desactivación del año 2000 y la reforma del año 2001; c) determinar el destino y funciones de los agentes y analistas despedidos y desactivados en los años posteriores a 2001, lo que muestra una relación con el desarrollo de empresas de seguridad, sistemas privados de inteligencia, inte- ligencia empresarial y un mercado de compra-venta de interceptaciones telefónicas durante los últimos catorce años. Este estudio se sostiene en el cruce de tres tipos de datos: fuente abierta provista por la normativa, informantes clave que participaron en el proceso de los años 2000 y 2001, y datos primarios de actores que actualmente son miembros de empresas de seguridad e inteligencia privada.

Duševní hygiena a možnosti využití jejích forem u pracovníků linek důvěry / Forms of mental hygiene and their application to crisis hotline workers

This thesis deals with the forms of mental hygiene and their application to crisis hotline workers. In the theoretical part the work focuses on the characteristics and specificities of telephonic crisis intervention. Methods of mental hygiene suitable for crisis hotline workers are presented too. The empirical part focuses on mapping of the forms of mental hygiene, which are provided by crisis hotlines to their workers in the Czech Republic. Individual forms of mental hygiene of crisis hotline workers were also mapped. Crisis hotline workers burnout was investigated in the empirical part of the thesis by the means of CBI - Copenhagen Burnout Inventory. It was also confirmed that greater degree of compliance with selected principles of mental hygiene by cisis hotline workers has the relationship with burnout lower incidence. Hypotheses concerning the relationship between sociodemographic data (sex, age, educational attainment, length of praktice, type of employment in the workplace and beyond) and the rate of burnout were not confirmed. The aim of the thesis was to offer possible ways of how to deal with the stress to crisis hotline workers and how they can optimally use the methods of mental hygiene, which can in practice preventively work against burnout.

Las nuevas tecnologías en la administración de justicia. La validez y eficacia del documento electrónico en sede procesal

La tesis se encarga de analizar, por un lado, la integración y el desarrollo de las nuevas tecnologías en la Administración de Justicia; y, por otro, los parámetros que constituyen la validez y eficacia del documento electrónico. La primera cuestión se centra en la configuración de los Sistemas de Información de la Oficina Judicial y del Ministerio Fiscal, así como de la informatización de los Registros Civiles, donde el art. 230 LOPJ es la pieza clave. Se estudian sus programas, aplicaciones, la videoconferencia, los ficheros judiciales y las redes de telecomunicaciones que poseen la cobertura de la firma electrónica reconocida, donde cobran gran relevancia los convenios de colaboración tecnológica. La digitalización de las vistas quizá sea una de las cuestiones con más trascendencia, teniendo en cuenta que el juicio es el acto que culmina el proceso. Aunque no todos los proyectos adoptados en el ámbito de la e.justicia se han desarrollado de forma integral, ni han llegado a la totalidad de los órganos judiciales. El objetivo final es lograr una Justicia más ágil y de calidad, a lo cual aspira el Plan Estratégico de Modernización de la Justicia 2009-2012 aprobado recientemente.En referencia a la segunda perspectiva, no cabe duda que el Ordenamiento jurídico y los tribunales, en el ámbito de la justicia material, otorgan plena validez y eficacia al documento electrónico. Nuestra línea de investigación se justifica porque cada vez son más los procesos que incorporan soportes electrónicos de todo tipo, ya sea al plantearse la acción o posteriormente como medio de prueba (art. 299.2 LEC). Entre otros temas examinamos el documento informático, la problemática que rodea al fax, los sistemas de videograbación y el contrato electrónico. / La tesi s'encarrega d'analitzar, per una part, la integració i el desenvolupament de les noves tecnologies dins l´Administració de Justícia; i, per l'altra, els paràmetres que constitueixen la validesa i l'eficàcia del document electrònic. La primera qüestió es centra en la configuració dels Sistemes d´Informació de l´Oficina Judicial i del Ministeri Fiscal, així com de la informatització dels Registres Civils, on l'art. 230 LOPJ es la peça clau. S'estudien els seus programes, aplicacions, la videoconferència, el fitxers judicials i les xarxes de telecomunicacions que tenen la cobertura de la firma electrònica reconeguda, on cobren gran rellevància els convenis de col·laboració tecnològica. La digitalització de les vistes tal vegada sigui una de les qüestions amb més transcendència, tenint amb compte que el judici es l'acte que culmina el procés. Their programmes, applications, the Video Conferencing, the judicial registers and the telecommunication networks which are covered by the recognised electronic signatures, are studied, where the agreements on technological collaboration gain great relevance. The digitalisation of evidence might perhaps be one of the questions with most consequence, bearing in mind that the judgment is the act by which the process is culminated. Although not all the projects adopted within the compass of e.justice have developed completely nor have reached all the judicial organs. The final objective is to achieve an agile, quality Justice, to which the recently approved Strategic Plan for the Modernisation of Justice aspires.With reference to the second perspective, there is no doubt that the juridical Ordinance and the tribunals within the compass of material justice grant full validity and efficacy to the electronic document. Our line of investigation is justified because there are more and more processes which are sustained by electronic supports of all kinds, whether it be at the establishment of the action or later, as a proof of it (art. 299.2 LEC). Amongst other things, we examine the computerised document, the problems which surround the fax, the systems for video recording and the electronic contract.
