Disparities in Breast Cancer Stage at Diagnosis: Importance of Race, Poverty, and Age

This study investigated the association of race, age, and census tract area poverty level on breast cancer stage at diagnosis. The study was limited to women residing in Missouri, aged 18 years and older, diagnosed with breast cancer, and whose cases were reported to the Cancer Registry between 2003 and 2008. The risk, relative risk, and increased risk of late-stage at diagnosis by race, age, and census tract area poverty level were computed. We found that the odds of late-stage breast cancer among African-American women were higher when compared with their white counterpart (OR 1.433; 95% CI, 1.316, 1.560). In addition, the odds of advanced stage disease for women residing in high-poverty areas were greater than those living in low-poverty areas (OR 1.319; 95% CI 1.08; 1.201). To close the widening cancer disparities gap in Missouri, there is the need for effective and programmatic strategies to enable interventions to reach areas and populations most vulnerable to advanced stage breast cancer diagnosis.

The effects of age and sensation on the anticipatory motor patterns activated during deglutition

Swallowing problems, and the often-associated sequelae including pneumonia, malnutrition, and dehydration are common, potentially life-threatening conditions suffered by many elderly individuals. The combination of cognitive, physical, and sensory impairments commonly seen in individuals with dementia and following stroke often results in eating needs going unmet, leading to increased morbidity and premature mortality. The functional limitations of many of these individuals frequently result in decreased mealtime independence and necessitate extensive or total assistance (i.e., dependence) with consuming food and liquids. Despite the implication of increased safety associated with feeding assistance, the influence and potential risk of absent pre-oral cues are unclear, especially in an already taxed system. This study investigated the cooperative relationships between the sequential sensorimotor acts involved in eating and swallowing, focusing on the anticipatory stage, under various sensory-loss conditions. Kinematic data from the lips, jaw, and hand were obtained from 24 healthy younger adults (ages 18-30) and 24 healthy older adults (ages 70-85) under four different conditions: typical self-feeding, typical assisted feeding (i.e., loss of proprioceptive cues), sensory loss self-feeding (i.e., loss of visual cues and degradation of auditory cues), and sensory loss assisted feeding (i.e., loss/degradation of auditory, proprioceptive, and visual cues). During typical self-feeding, all participants began the mouth opening gesture shortly after the onset of hand movement toward the mouth and prior to the onset of oral sensation. However, differences in the timing of anticipatory onset and offset lip movements were observed between older and younger adults and also on the basis of the presence/absence of feeding dependency and sensory loss. Older adults initiated lowering movement earlier than younger adults given the availability of proprioceptive and/or visual cues. In addition to demonstrating earlier lip lowering, during both self-feeding conditions older adults more consistently attended to the timing between lip lowering onset and hand movement onset as compared to both the younger adults and other relative timing pairs. Given the absence of proprioceptive cues (i.e., during assisted feeding), the onset of anticipatory lip movement was delayed. Sensory loss (i.e., loss of visual cues, reduction in auditory cues) alone did not negatively impact the onset of lip movement for either group as compared to typical self-feeding. Conversely, the presence of sensory loss more negatively impacted the offset of lip movement as compared to the absence of proprioception (e.g., the offset of lip movement was later given only visual/auditory loss as compared to assisted feeding). The presence of both feeding dependence and sensory loss had the greatest negative impact on the timing of both the onset and offset of anticipatory lip movement. These findings suggest that deglutition should be considered as beginning prior to the onset of oral sensation and highlight the necessity of better understanding the role that pre-oral, or anticipatory, sensorimotor information may play in the overall eating and deglutitory process. As older adults consistently demonstrated a "compensatory advantage" via earlier and less variable movement onset given the availability of proprioceptive and/or visual cues, continued investigation into these age-related differences is important. This study provides a first step in clarifying the relationship between these pre-oral cues and anticipatory oral posturing during eating and swallowing, allowing for a better understanding of the potential for increased risk assisted feeding recommendations may elicit. However, the differences observed between older and younger adults, particularly under conditions of sensory loss, may be further exacerbated in a taxed system, potentially increasing risk for various patient populations.

The explosive growth of network security threats poses serious challenges for building secure Cyber-based Systems (CBS). Existing studies have addressed a breadth of challenges related to detecting network attacks. However, there is still a lack of studies on the detection of sophisticated Multi-stage Attacks (MSAs). </p> <p>The objective of this dissertation is to address the challenges of modeling and detecting sophisticated network attacks, such as multiple interleaved MSAs. We present the interleaving concept and investigate how interleaving multiple MSAs can deceive intrusion detection systems. Using one of the important statistical machine learning (ML) techniques, Hidden Markov Models (HMM), we develop three architectures that take into account the stealth nature of the interleaving attacks, and that can detect and track the progress of these attacks. These architectures deploy a set of HMM templates of known attacks and exhibit varying performance and complexity. </p> <p>For performance evaluation, various metrics are proposed which include (1) attack risk probability, (2) detection error rate, and (3) the number of correctly detected stages. Extensive simulation experiments are conducted to demonstrate the efficacy of the proposed architecture in the presence of multiple multi-stage attack scenarios, and in the presence of false alerts with various rates. </p> </div> </div> </div>

Methods To Identify And Develop Drugs For Cryptosporidiosis

Cryptosporidiosis is a common diarrheal disease caused by intestinal infection with the apicomplexan parasite Cryptosporidium, in humans usually either with C. hominis or C. parvum. Unfortunately, given a large burden of disease in children and immunocompromised people like AIDS patients, the only currently approved treatment, nitazoxanide, is unreliable for these patient populations. To address the urgent need for new drugs for the most vulnerable populations, large phenotypic screening efforts have been established to identify anti-Cryptosporidium growth inhibitors in vitro (hits). However, in the absence of a gold standard drug, the in vitro and in vivo characteristics that should be used to prioritize screening hits are not known. This thesis is focused on identifying promising anti-Cryptosporidium hits and drug leads, and using them to establish validated methods to guide hit-to-lead studies for anti-Cryptosporidium drug development. A re-analysis of our phenotypic screen of the Medicines for Malaria Venture Open Access Malaria Box identified a promising C. parvum growth inhibitor, MMV665917. It had similar in vitro activity against C. hominis, C. parvum Iowa, and C. parvum field strains, and it was amenable to preliminary structural activity relationship studies using commercially available variants, with one variant demonstrating nanomolar potency. Furthermore, MMV665917 was effective in vivo in an acute interferon-γ mouse model of cryptosporidiosis; and it appeared to cure an established infection in the chronic NOD SCID gamma (NSG) mouse model, unlike nitazoxanide, paromomycin, and clofazimine. We hypothesized that anti-Cryptosporidium activity in the highly immunocompromised chronic NSG mouse model might relate to compounds being capable of killing and eliminating parasites (cidal), rather than only preventing growth (static). To test this, we developed a novel in vitro parasite persistence assay that showed that MMV665917 was potentially cidal, whereas nitazoxanide, paromomycin and clofazimine appeared static. This pharmacodynamic assay also provided the concentration of compound required to maximize rate of parasite elimination, which could help design in vivo experiments. To further characterize compounds based on mechanism of action, we developed a range of in vitro medium-throughput life-stage assays. To validate and gain value from the assays, a “learner set” of compounds from our in-house screens and collaborations were tested in all of the in vitro assays and in the in vivo NSG mouse model. Using these assays, it was possible to group molecules based on chemical class/mechanism of action. Because compounds from distinct groups showed activity in the NSG mouse model, these methods could be used to obtain a diverse set of early-stage Cryptosporidium inhibitors for prioritization. Furthermore, compounds that appeared static in the in vitro parasite persistence assay did not have activity in the NSG mouse model. In summary, we report the identification and development of a highly promising initial lead, MMV665917, and report a range of in vitro assays that can be used to prioritize anti-Cryptosporidium hits and leads.

A Staff Education Project and Screening Tool to Identify Calciphylaxis

Calciphylaxis is a deadly disease seen primarily in patients with end stage renal disease. Literature indicated that improved patient outcomes are seen with routine screenings. Many dialysis providers lack fundamental knowledge that would enable early identification of calciphylaxis in patients with renal disease. The purpose of this project was to design a screening instrument and develop a staff education program that would transform calciphylaxis management by promoting early identification and treatment of the disease. Knowles's theory of andragogy was used as the theoretic framework for the project. Dialysis center staff (n = 26) participated in the education. The number of participants was based on the number of staff working at the partner dialysis site. There were no exclusions as all members of the interdisciplinary team play an important role in calciphylaxis management. Surveys conducted following the education were used to determine whether dialysis staff believed they had acquired the knowledge and skills necessary to identify early signs of calciphylaxis. Descriptive data collected by the surveys indicated 60% of participants were not at all comfortable identifying patients at risk for developing calciphylaxis prior to attending the education presentation. Following the presentation, 68% of participants felt very comfortable identifying at-risk patients, an increase of 82.3%. This project exemplified that calciphylaxis detection is a secondary prevention nursing intervention that has potential for promoting positive social change by improving patient outcomes, reducing mortality rates in the end stage renal disease population, and providing empirical data to inform evidence-based therapies for at-risk patients.

The effect of planting date on the growth potential of different forage sorghum cultivars

(Pasture Science)) -- University of Limpopo, 2014 / Forage sorghum is widely grown in South Africa as annual summer forage to supplement pasture production for sheep, beef and dairy cattle. A number of sorghum cultivars are available commercially, and periodically some cultivars are added while others are withdrawn from the market. The potential yield figures and the nutritive value of these forage sorghum cultivars are generally not known. The management practices that improve forage sorghum production and quality include the time of planting and time of harvesting. The genetic makeup of different forage sorghum cultivars also accounted for a portion of the production and quality. A field experiment was conducted at Dewageningsdrift Experimental Farm (DWD), Moloto, Gauteng and Nooitgedacht Agricultural Development Center (NGD), Ermelo, Mpumalanga to study the influence of planting date on the growth potential of different forage sorghum cultivars. Three planting dates were used: mid-December 2006, mid-January 2007 and mid-February 2007. Thirteen different cultivars were incorporated in the trial to evaluate influence of the breeding history. The cultivars were defoliated at three different stages: cut repeatedly at six weekly intervals (Dt 1), cut repeatedly when it reached a grazing stage (± 800 mm high) (Dt 2) and once at the silage stage (soft dough) (Dt 3). At DWD the average total dry matter (TDM) productions, for the six week cutting treatment (Dt 1), were 10760 kg/ha, 5195 kg/ha and 1944 kg/ha for December, January and February planting date respectively. For the same treatment, at NGT, the average TDM productions were 6396 kg/ha and 1737 kg/ha for December and January respectively. The February planting resulted in the poor germination and seedling emergency. The seedlings did not survive due to low temperatures. The minimum of 13 ºC and 11.8 ºC as well as the maximum of 24.1 ºC and 23.0 ºC in February and March were below the required germination temperature (15 ºC). The highest producers that is available in the market were Jumbo, Sentop, Piper, Kow Kandy, and Sugargraze. Defoliated repeatedly at grazing stage (Dt 2), at DWD, resulted in average TDM productions of 8541 kg/ha, 4950 kg/ha and 2683 kg/ha for December, January and February, respectively. At NGT the average TDM productions were 7769 kg/ha and 3010 kg/ha for December and January respectively. The highest producers were Jumbo, Kow Kandy, Piper, Sentop and Sugargraze. The average TDM productions at the silage stage (Dt 3), at DWD, were 17923 kg/ha, 15015 kg/ha and 2529 kg/ha for December, January and February respectively. At NGT the average TDM production iii was 11856 kg/ha and 5350 kg/ha for December and January, The highest producers were Jumbo, Sugargraze, Kow Kandy, Sentop and Kow Kandy.December planting proved to be the best planting date for optimum DM production, compared to later plantings in January and February. Reaching into response selection: stimulus and response similarity influence central operations

This dissertation examines the impact of stimulus and response similarity on response selection. Traditional models of response selection invoke a central processor that operates like a look-up table by matching the perceptually classified stimulus (e.g., green square) to the specified response (e.g., right button press). The look-up property of response selection affords the system the ability to map any stimulus onto any response, even if that stimulus-response has never been paired before. Under such an approach, the degree of perceptual similarity or dissimilarity that exists among stimuli in the environment should have little effect on central operations, the similarity or dissimilarity of the motor response executed in response to a stimulus should not influence response selection, and no interaction between stimulus and response features is permitted, given that stimulus features affect the encoding process, and response features affect the output process, but not response selection itself. Eight studies examine the influence of stimulus and response similarity during response selection. The first two experiments establish the interaction across different task demands between stimulus and response similarity. The interaction was not the result of perceptual difficulty (Experiment 3) and was extended to a new set of stimuli (Experiment 4). A consequence of the design in Experiments 1 - 4 was that response condition was confounded with response configuration. In one of the response conditions the target location had three competitors on one side of it compared to the other condition where the target had one competitor on one side and two others on the other side. Experiments 5 and 6 examined the separate roles that response configuration and response metrics had on the interaction between stimulus and response similarity. The mechanism that produced the interaction was the result of competition between partially activated stimulus-response alternatives. Experiments 7 and 8 further explored the role of competition during response selection by turning to traditional response selection methodologies that introduce competition through either the presentation of irrelevant stimulus information or through presenting the stimulus along an irrelevant spatial dimension. These data have broad implications for models of RS. To account for the ability to pair any stimulus modality with any response modality dominant accounts of RS assume that central operations are performed by a generic set of processes that operate over representations that are stripped of metric information (amodal representations). Response selection works as a look-up table that receives a categorized stimulus as an input and returns an abstract response code as output. This type of model cannot produce an interaction between stimulus and response similarity and thus, the present data provide a serious challenge to these types of models. Finally, the data provide evidence that the metric relationship between stimuli and response matter and influence response selection. The co-activation of stimulus-response alternatives are at a level of representation that includes both stimulus and response properties. A framework is presented that captures key aspects of the data.
