First Experience With The GoBack-Catheter For Successful Crossing of Complex Chronic Total Occlusions in Lower Limb Arteries

Purpose: To evaluate the use of the GoBack-catheter (Upstream Peripheral Technologies) in complex revascularizations in lower limb arteries. Materials and Methods: In this retrospective single-center study, the results of the first 100 consecutive patients including 101 limb-revascularizations, performed between May 2018 and July 2020 with the study device, were analyzed. In all cases, guidewire-crossing failed, and all lesions were chronic total occlusions (CTO), either de novo, reocclusions, or in-stent reocclusions. Successful crossing was defined as passing the CTO using the study device. Patency at discharge and after 30 days was defined as less than 50% restenosis on duplex sonography, without target lesion revascularization. Results: Median lesion length was 24 cm and 38 patients (37.6%) had a calcium grading according to the peripheral arterial calcium scoring system (PACSS) of 4 or 5. In 20.8% of patients, an occluded stent was treated. CTOs involved the femoropopliteal segment in 91.1%, iliac arteries in 5.9%, and tibial arteries in 7.9%. The GoBackcatheter was employed for entering into or crossing through parts or the full length of a CTO or an occluded stent as well as for re-entering into the true lumen after subintimal crossing. The device was used via contralateral and ipsilateral antegrade as well as retrograde access with an overall technical success rate of 92.1%. In 3 patients minor bleeding occurred at the crossing or re-entry site, which were managed conservatively. Thirty-day adverse limb events comprised minor amputations in 4 patients (4.0%), 1 major amputation (1.0%), and reocclusions in 7 limbs (6.9%). Conclusion: The new GoBack-catheter offers versatile endovascular applicability for complex CTO recanalization in a broad range of peripheral vascular interventions with a high technical success and low complication rate.

African immigrant traders in Johannesburg inner city, South Africa : deconstructing the threatening other

African immigrants in contemporary South Africa can be perceived as a problem – the threatening other. Based on a case study of the Johannesburg inner city, this thesis aims to deconstruct this notion. It does so by investigating the nature and types and contribution of African immigrant traders` businesses to the Johannesburg inner city. In deconstructing the perception that African immigrants are the threatening other, and being infinitely aware that perception issues and the experiential realities hospitable to its centred on the human subject, this case study adopted a humanist geographic and critical realist approach by deploying a qualitative in-depth interview technique of both African immigrant and South African traders. This thesis suggests three important outcomes. The first is that: to view all African immigrants as the threatening other is too simplistic an assessment of an otherwise complex and dynamic set of relationships and interrelationships amongst and between African immigrant and South African traders. Second, some African immigrant traders do make a meaningful contribution to the Johannesburg inner city, whereas others do not. Third, the activities of African immigrant traders that may be considered as a threat by a section of the population are treated as a benefit by another. These nuanced insights and findings in this study not only render any analysis that projects all African immigrants negatively as an incomplete appraisal, but also suggest that it can never be correct to view them as such without capturing the dynamics that this work suggests. Such a finding not only challenges distorted and partial reporting by the media and also questions policies, which may be built on the wrong assumption that all African immigrants are a problem, but also extends the study of migration related issues in a South African context. / Geography / D. The aim of this thesis is to describe appropriate and threatening educational strategies that have arisen as a result of parental divorce, and based on the theoretical recommendations and research conducted among a selected group of respondents to formulate educational recommendations that minimizes problems of children from divorced families. The survey seeks to answer questions about what respondents selected as their options to manage a period around parents divorce and what made the situation worse. To achieve the objectivity of this thesis were to set partial goals: 1. To find out how the respondent assessed with hindsight suitability and unsuitability of educational strategies used by parents during divorce 2. compare their opinions with the strategies formulated in the theoretical part. The focus of this work is qualitative research, processed by using content analysis of the interview. Semi-structured interviews with selected respondents from single-parent families aged between 19 and 25 years old, reversely maps their view on the appropriate educational strategies of their parents. Long QT syndrome in Sweden : founder effects and associated cardiac phenotypes / Långt QT syndrom i Sverige : foundereffekter och associerade kardiella fenotyper

Background: We aimed to increase the knowledge regarding the familial arrhythmogenic disorder Long QT Syndrome (LQTS) and its recessive variant Jervell and Lange-Nielsen Syndrome (JLNS) in Sweden, including prevalences and clinical phenotypes. A specific focus was directed towards two KCNQ1 mutations –p.Y111C and p.R518X- commonly identified in Swedish LQTS index cases. Methods: Cases and families with LQTS (p.Y111C or p.R518X) and JLNS were recruited via regional clinical practices, national referrals to the Clinical Genetics laboratory, Umeå University Hospital, and a national inventory. Molecular genetics methods were used for case ascertainment. Clinical data was obtained via medical records, a questionnaire, and/or an interview. Electrocardiograms were manually assessed. In p.R518X heterozygotes intra-familial phenotypic variability (QTc and cardiac events) was assessed by analysis of sequence variants (modifier genes). The origins of the mutations p.Y111C and p.R518X were investigated using genealogical and haplotype analysis (microsatellite markers). In families sharing a common haplotype mutation age and associated prevalence was analyzed using ESTIAGE and DMLE computer software. Results: We identified p.Y111C (170 mutation-carriers) and p.R518X (101 mutation-carriers) as two major causes of LQTS/JLNS in Sweden. LQTS phenotype was revealed to be relatively benign in p.Y111C and p.R518X (annual incidence of life-threatening cardiac events, before therapy 0.05% and 0.04%, respectively). Gender-specific effects of genetic modifiers on phenotypic expression were seen. A founder origin, approximately 600-700 years ago in two northern river valleys was established for p.Y111C and p.R518X, and a high prevalence of LQTS founder descendants suggested. A minimum JLNS prevalence of 1:200 000 in preadolescent Swedish children was revealed. JLNS phenotype was mainly severe, with a cumulative incidence of life-threatening cardiac events of 53% (annual incidence rate before therapy 5%) and four sudden deaths. Possible founder effects regarding four KCNQ1 mutations; p.Y111C (8%), p.R518X (50%), c.572_576del (17%) and p.Q530X (8%) together explained 83% of the JLNS mutation-spectrum in Sweden, consisting of 8 KCNQ1 mutations. Conclusion: The high prevalence of p.Y111C- and p.R518X-related LQTS as well as JLNS revealed in Sweden could be explained by the combination of mild clinical phenotypes in heterozygotes and strong founder effects present during the population development of northern Sweden. Increased knowledge regarding the occurrence of LQTS and JLNS as well as mutation- and/or genotype-specific data constitute prerequisites for possible improvement of patient management.

Avdramatiserande strategier i logopedisk intervention : En samtalsanalytisk studie

Syftet med föreliggande studie var att studera de avdramatiserande strategier som förekommer under logopedisk afasi- och barnintervention samt att jämföra dessa interventionstyper. Vidare syftade studien till att beskriva hur dessa strategier påverkar interaktionen mellan logoped och patient. Vald analysmetod har influerats av Conversation Analysis (CA) eftersom det är en metod som kan användas för studier av interaktionen mellan människor. Materialet för analysen har utgjorts av sex samtal inom afasiintervention och åtta samtal inom barnintervention. Vid analysen av materialet till föreliggande studie blev det tydligt att det främst är logopeden som använde avdramatiserande strategier och detta skedde något oftare inom barnintervention än inom afasiintervention. I samma material var det relativt ovanligt att patienten använde avdramatiserande strategier. I de fall detta förekom bland patienter var det för att skydda sitt eget ansikte, till skillnad från logopedens användning som främst syftade till att skydda patientens ansikte. I resultaten framkom inga större skillnader gällande vilka avdramatiserande strategier som användes, däremot fanns skillnader i hur ofta de användes samt i hur de tillämpades. En annan tydlig skillnad vid jämförelse av interventionstyperna var att logopeden inom barnintervention oftare använde en stigande grundton som en artighetsstrategi, vilket i vissa situationer fungerar avdramatiserande. Gällande interaktionen ser de avdramatiserande strategierna till att samtalet fungerar och förs framåt inom de båda interventionstyperna. Behovet av de avdramatiserande strategierna blev tydligt när de inte fungerade och patienten till följd av detta upplevde ansiktshot. Sammanfattningsvis låg den största skillnaden mellan de studerade interventionstyperna i hur logopeden anpassade de olika strategierna till en vuxen person med afasi respektive ett barn med språkstörning.

This dissertation seeks to address the multiple ways face or one’s public self-image is attacked, supported and maintained in Athol Fugard’s The Road to Mecca, My Children! My Africa! and Valley Song, and through this discussion demonstrate how the notion of face can make a contribution to the study and understanding of Athol Fugard’s work. In the pursuit of their goals/objectives, interactants perform speech acts which may threaten the face of other participants. The choice of strategies available to participants in the performance of these face-threatening acts (FTAs) includes going on record, off record (indirectly) or avoiding the FTA altogether (saying nothing). Each text offers a fresh perspective from which face can be analysed: rebelliousness against conformism (The Road to Mecca); the perspective of the cross-racial, cross-cultural relationships (My Children! Samtala som ung(e) : En samtalsanalytisk studie av Mirja Unges dialogstil / To Talk Like Unge : A study of the dialogue style of the author Mirja Unge

Mirja Unge is a Swedish author who often writes about young people and the tough situations they face. She writes both plays and short-stories and is acclaimed for her dialogue. The aim of this essay is to examine Unge’s dialogue style and how potential face-threatening situations are portrayed. To reach my aim I pose questions regarding the conversational support in two of Unge’s literary works, the short story Det var igår bara and the play Mariella and compare the result. The dialogues analysed both tackle the sensitive subject of sexual assault, a topic that is potentially face-threatening. The analysis is based on the theoretical and methodical framework of conversation analysis. The conversational support is analysed through “keys” that unlock interesting points in the conversation. The keys in this essay are ‘topic generation’ and ‘topic shifts’. The results show that in the text with the most conversational support (the short story) the subject of assault is avoided and only appear in the characters thoughts. In the play on the other hand there is very little conversational support, but the character tells the whole story in a direct manner. This indicates that the situation in the short story is more face-threatening than in the play. However there are other aspects in the dialogue that makes the conversation difficult for the characters in both stories.

This theoretical-empirical paper researches how simultaneous interpreters work with face threatening acts (FTAs). For this purpose we researched how FTAs that we identified in speeches given by native Spanish speakers at the European Parliament's plenary sittings are interpreted into Czech and English. This paper has been inspired by the study of Cédric Magnifico and Bart Defrancq from the University of Ghent "Impoliteness in Interpreting: A Question of Gender?" in which they carry out a similar research. In their study the authors research the simultaneous interpretation of French speeches given at the European Parliament's plenary sittings into English and Dutch in order to find out whether interpreters really engage in face work - by face work they mean mitigating (potential) FTAs. In this paper, however, we aimed at answering a more general research question: "How do simultaneous interpreters handle face threatening acts when interpreting at the European Parliament's plenaries?" Researching the interpretation of FTAs into two target languages, moreover, can show whether face work is influenced by the cultural norms used in the community of the target language. Exploring the Experiences and Sense-making of Patients and Informal Caregivers in the Inpatient Specialist Palliative Care Context

Palliative and end-of-life care (PEOLC) involves the use of an interprofessional approach to improve quality of life for patients facing life-threatening illness, and their informal caregivers (e.g. family, other loved ones). Though many patients with life-threatening illness report desires to die at home, people with PEOLC needs continue to die in hospital settings. Given the considerable differences between inpatient and outpatient PEOLC settings, and the potentially increased future need for inpatient PEOLC services, it is critical to explore how patients and caregivers experience and make sense of illness and care in such settings. To contribute to this gap in the literature, I conducted a scoping review on patient and informal caregiver experiences in inpatient specialist palliative care (SPC) settings and semi-structured interviews with patients and caregivers on the Bruyère inpatient SPC unit. I identified three overarching themes from included studies (n=104), including patient and informal caregiver: i) perceptions of care, the interprofessional palliative care team, and the care environment; ii) communication with the interprofessional team; and iii) impacts of illness and care on quality of life. I conducted interviews with three patients and four informal caregivers. Using an inductive thematic analysis approach, I identified three key themes of patient and informal caregiver experiences within the context of the Bruyère inpatient SPC unit: i) the journey of dealing with life-threatening illness; ii) impacts of the COVID-19 pandemic; and iii) attempts to cope and find peace. Based on my findings, improvements are needed to the provision of person-centred care to address patients’ and informal caregivers’ supportive needs. Further work is needed to identify reforms to support improved education and awareness about palliative care to patients, informal caregivers, and the general public.
