An in vitro biomechanical comparison between intramedullary pinning and the use of plates in the dachshund tibia

The dachshund, a chondrodystrophic dog breed, presents a unique challenge in the treatment of tibial fractures by having short and curvaceous tibiae, leading to high implant failure risk. In this study, intramedullary pins with full cerclage wires as an option in the treatment of oblique diaphyseal tibial fractures was studied in vitro. This fixation technique was biomechanically compared with the current gold standard in internal stabilization, namely bone plates and screws. Twenty tibiae recovered from adult dachshund cadavers were randomly allocated into two groups of ten bones each. Oblique fractures running in a proximo-cranial-disto-caudal direction in the middle third of the tibial diaphysis were simulated by osteotomy and each bone repaired by using one of the following methods: <ul> <li> Pre-bent intramedullary pin, filling 40% to 60% of the medullary cavity at its narrowest point, inserted normograde and combined with a set of three full cerclage wires (group 1).</li> <li> Lag screw at the osteotomy site, combined with a six hole 2.7 mm contoured dynamic compression plate and cortical screws in neutral mode (group 2).</li> </ul> Each test specimen was subjected to a two point single cycle axial compression test by applying a standardized, increasing compression load to the point of fixation failure or bone collapse. A stress-strain graph for each test specimen was drawn from the raw data. Radiographs and digital photographs were made pre-osteotomy, post-osteotomy, post-repair and post-test, and modes of failure noted for each test specimen. Stress (applied load) and strain (deformation) at yield, ultimate strength, and at failure were determined for each test specimen from the stress-strain graphs and the mean values statistically compared between the groups using the ANCOVA method. Significance levels of p < 0.05 were used, while p < 0.1 and p < 0.01 were also indicated. In group 1, 50% specimens failed due to unraveling or slippage with displacement of the cerclage wires, 30% due to bone fracture at a cerclage wire, and 20% due to bone fracture elsewhere. In group 2, 80% specimens failed due to bone fracture at one or more of the screw holes, whereas 20% failed due to bone fracture not directly associated with implants. No bone plate or screw underwent plastic (permanent) deformation, whereas 80% of the intramedullary pins and 30% of the cerclage wires underwent plastic deformation. Mean stress at the yield point in groups 1 and 2 were 0.323 MPa and 0.403 MPa respectively, at the point of ultimate strength 0.383 MPa and 0.431 MPa respectively, and at the failure point 0.345 MPa and 0.403 MPa respectively. Mean strain at the yield point in groups 1 and 2 were 0.296% and 0.362% respectively, at the point of ultimate strength 0.412% and 0.472% respectively, and at the failure point 0.713% and 0.838% respectively. Clinically, there was an indication that plates and screws were more resistant to deformation by the loads applied than intramedullary pins and cerclage wires. However, statistically, there were no significant differences in stress at yield (p = 0.299), ultimate strength (p = 0.275), or failure (p = 0.137) between the two groups. Similarly, there were no significant differences in strain at yield (p = 0.684), ultimate strength (p = 0.778), or failure (p = 0.505) between the two groups. Main limitations of the study were the relatively small number of specimens tested, the smoothness of the osteotomy cuts which limited interdigitation between the fragments, and that only three of the five recognized loads acting on bones in vivo, were tested in vitro. In conclusion, this study did not show enough evidence to prove a significant difference between the two methods of fixation. Therefore, it is suggested that intramedullary pins and full cerclage wires be used as an acceptable alternative to bone plates and screws in the treatment of oblique mid-diaphyseal tibial fractures in chondrodystrophic dog breeds. / Die dachshund, ‘n chondrodistrofiese honderas, bied ‘n unieke uitdaging in die behandeling van frakture van die tibia, deurdat hulle tibias kort en krom is, wat lei tot ‘n hoë risiko van inplantaat mislukking. En muchas ciudades el crecimiento de la congestión vehicular es mayor al crecimiento poblacional. Las ciudades con mayor congestión en el mundo son Bombay, Bogotá y Lima. Esta última cuenta con un nivel de congestión vehicular aproximado del 66%, este se puede medir mediante la saturación de la vía realizando un estudio HCM. Estudios previos han analizado la influencia de la temperatura, velocidad, y carga sobre la carpeta asfáltica. Debido a esto, el impacto de la congestión debería considerarse en el diseño de pavimentos urbanos. La investigación, verificó que la congestión tiene una influencia en el incremento de la temperatura de la superficie del pavimento, la reducción de la velocidad y el aumento de carga vehicular. Se calculó que a mayor temperatura y menor velocidad se reduce el módulo de elasticidad de la carpeta asfáltica. Para esto, se consideró una diferencia de temperatura con y sin congestión, de 35°C y 25°C respectivamente. Se ha propuesto una carpeta asfáltica tibia con adición de caucho al 18% que pueda soportar los efectos de la congestión vehicular. Se buscó reducir las deformaciones máximas de la carpeta y aumentar el número de ciclos de repetición de carga que puede soportar el pavimento, para que estos sean adecuados a los efectos de la congestión. Esto se logra al incrementar el módulo de elasticidad. Finalmente, se obtuvo un aumento del 45% en el módulo de elasticidad, una reducción del 50% de deformaciones permanentes, y un aumento del 500% aproximadamente en el número de ciclos de carga. / In many cities, the increase in traffic congestion is greater than population growth. The most congested cities in the world are Bombay, Bogota and Lima. The latter has an approximate congestion level of 66%, which can be measured by road saturation using an HCM study. Previous studies have analyzed the influence of temperature, speed, and load on the asphalt surface. Because of this, the impact of congestion should be considered in urban pavement design. The research verified that congestion has an influence on the increase of pavement surface temperature, the reduction of speed and the increase of vehicular load. It was calculated that the higher the temperature and the lower the speed, the lower the modulus of elasticity of the asphalt layer. For this purpose, a temperature difference of 35°C and 25°C with and without congestion, respectively, was considered. A warm asphalt pavement with 18% rubber was proposed to withstand the effects of vehicular congestion. The aim was to reduce the maximum deformations of the binder and increase the number of load repetition cycles that the pavement can withstand, so that they are adequate to the effects of congestion. This is achieved by increasing the modulus of elasticity. Finally, a 45% increase in the modulus of elasticity, a 50% reduction in permanent deformations, and an increase of approximately 500% in the number of load cycles were obtained. / Tesis

pQCT Assessment at the Radius And Tibia: The Effects of Menopause and Breast Cancer Therapy on Trabecular and Cortical Bone

<p> This thesis focuses on an examination of cortical and trabecular bone density and geometry at the radius and tibia in postmenopausal women, primarily women with history of breast carcinoma, while also assessing musculoskeletal changes in postmenopausal breast cancer patients after treatment with the Aromatase Inhibitor, Anastrozole. The first sub-study is an investigation of the reproducibility of the pQCT measurement parameters at the radius and tibia in healthy pre-and postmenopausal women. Results indicated that the reproducibility was good at the radius and even better at the tibia for all parameters measured. The second study is an appraisal of the level of osteoporosis knowledge in a cohort of postmenopausal women. The participants were assessed via the Facts on Osteoporosis Quiz, a well validated questionnaire, and the data revealed significantly lower test scores among the breast cancer subjects in comparison with healthy postmenopausal women. In the remaining group of studies, pQCT technology was utilized to describe trabecular and cortical bone at the radius and tibia in postmenopausal women and women with a history of breast carcinoma whom had been prescribed Anastrozole. The following measurement sites were significantly lower in the breast cancer subjects: TOT_DEN and TOT_CNT at the 4% radius; CRT_DEN, TOT_CNT, and CRT_CNT at the 20% radius; TOT_DEN at the 4% tibia; and CRT_DEN at the 38% tibia. With respect to time on Anastrozole, TOT_CNT at the 4% radius (r=-0.36); TOT_CNT (r=-0.33), CRT_CNT (r=-0.34) and CRT_DEN (r=-0.44) at the 20% radius; and CRT_DEN (r=-0.39) and CRT_CNT (r=-0.27) at the 38% tibia were significantly negatively correlated with days on Anastrozole. Furthermore, after two years of Anastrozole treatment in a small cohort of breast cancer subjects, there was a significant decrease in CRT_DEN (p=0.025) at the 20% diaphyseal radius and also at the 38% diaphyseal tibia (p=0.051). Together, the sub-studies that comprise this thesis demonstrate that there are noteworthy deficiencies in osteoporosis knowledge among postmenopausal women, particularly those with a history of breast carcinoma, and yet, these are the same women that have an increased need to understand the preventative and treatment options regarding this disease as they demonstrate reduced bone density at all measurement sites. It also appears that time on Anastrozole primarily affects cortical bone density in these women. In summary, this thesis provides novel details regarding cortical bone in breast cancer subjects and emphasizes the need for a normative database of bone quality parameters at different skeletal sites in order to gain a better understanding of the utility of each skeletal site with regard to fracture risk prediction. </p> / Thesis / Doctor of Philosophy (PhD)

The effect of citric acid supplementation on growth performance, carcass weight, tibia bone breaking strength, and ash content of male ross 308 broiler chickens

(Animal Production)) -- University of Limpopo, 2023 / Two experiments were conducted to determine the effect of citric acid inclusion level in the diet on growth performance, carcass weight, tibia bone breaking strength and ash content of male Ross 308 broiler chickens aged one to 35 days. The first experiment determined the effect of citric acid inclusion level in the diet on growth performance traits of male Ross 308 broiler chickens aged one to 21 days. The experiment commenced with 200 male day-old Ross 308 broiler chickens with an initial average live weight of 40±1.6g per chick. The chicks were assigned to five treatment groups in a completely randomized design, each replicated five times, and each replicate having ten chicks. The citric acid inclusion levels were at 0, 12.5, 25 or 50g per kg DM of feed. The second experiment determined the effect of citric acid inclusion level in the diet on growth performance, carcass weight, tibia bone breaking strength and ash content traits of male Ross 308 broiler chickens aged 22 to 35 days. The experiment commenced with 180 male Ross 308 broiler chickens aged 22 days. The chickens were assigned to four treatment groups, each having three replicate pens of eight chickens per replicate in a completely randomized design. Data was analysed using the General Linear model (GLM) procedures of the Statistical Analysis of System, version 9.3.1 software program. Where there were significant differences (P<0.05) between the treatment means, Tukey Multiple Comparison Test was used for mean separation. Citric acid inclusion in the starter diets improved (P<0.05) live weight and growth rate of male Ross 308 broiler chickens aged one to 21 days. Citric acid inclusion in the starter diets did not affect (P>0.05) daily feed intake, body weight gain and feed conversion ratio of male Ross 308 broiler chickens aged one to 21 days. The inclusion of citric acid did affect (P<0.05) live weight, body weight gain, feed conversion ratio and growth rate of chickens aged 22 to 35 days. Citric acid inclusion levels used in the present study influenced (P<0.05) DM and CP digestibility, ME intake and N-retention of male broiler chickens aged 22 to 35 days. The results of the current study showed that citric acid inclusion in a diet improved (P<0.05) chicken bone morphology. Thus, positive relationships were observed between citric acid inclusion level and right tibia bone weight, diameter, calcium, phosphorous and Magnesium contents of chicken bones aged 35 days. There were positive relationships between citric acid inclusion level and breast weight of male Ross 308 broiler chickens aged 35 days. Reliability and Validity of Body Composition and Bone Mineral Density Measurements by DXA

Dual energy X-ray absorptiometry (DXA) has been well established in both clinical and research settings for measurement of bone mineral density (BMD), and is becoming more widely utilized for assessment of body composition. Reliability and validity are essential factors in both applications of this technique; however, neither have been confirmed for the QDR-4500A DXA at Virginia Tech. Therefore, measurements of the whole body (WB), lumbar spine (LS), total proximal femur (TPF) and total forearm (TF) were made in a group of young-adult males and females at two time-points, 5-7 days apart. Significant differences were not found in BMD (g/cm2) at these body sites with repeated measurements by DXA. Furthermore, measures of percent body fat (%BF), lean body mass (LBM), and fat mass (FM) by DXA were reliable. Validity of %BF by DXA was assessed from comparison to single-frequency bioelectrical impedance analysis (BIA). Significant differences were not found in measures of %BF by DXA and BIA. A second study investigated the reliability and validity of the QDR-4500A DXA in measurements of distal tibia (DT) BMD. Significant differences were not found between repeated measurements. Validity was established by a significant correlation between WB BMD and DT BMD. A third study examined the influence of navel jewelry on the accuracy of LS DXA measurements. Repeated measurements with a spine phantom revealed that both a navel ring and a barbell produced significantly greater measures of LS BMD compared to the spine phantom alone. Manual correction of navel jewelry did not eliminate BMD inaccuracies. Data from these studies confirmed that the QDR-4500A DXA at Virginia Tech was a reliable and valid device in measurement of WB, LS, TPF, TF and DT BMD, as well as %BF, LBM, and FM. In addition, effects of navel jewelry on LS BMD have been recognized. Further studies investigating the reliability and validity of DT BMD measures as well as effects of different types, gauges, and shapes of body jewelry on BMD measures in human subjects are warranted. / Master of Science

O chumbo é um metal muito tóxico e que tem ampla distribuição pela sua grande utilidade. Mesmo a exposição a baixos níveis (que não causam sintomas de intoxicação aguda) de chumbo é associada a desordens cognitivas e neurológicas. O osso é considerado o melhor marcador de exposição a chumbo. No tecido ósseo, vários elementos estão presentes, e alguns deles poderiam ter sua concentração modificada pela exposição a chumbo. Testou-se a hipótese de que haja variações nos elementos Zn e Mg nas tíbias de animais expostos a chumbo (por técnica de química analítica convencional). Testou-se também se variações em microelementos poderiam ser detectadas por técnicas semiquantitativas nas tíbias de controles e animais expostos a chumbo. O objetivo deste projeto foi caracterizar em animais expostos ao chumbo desde a gestação e controles as concentrações deste elemento no osso (tíbia) de animais de 28 e 60 dias. Além do chumbo, investigamos também a concentração de 2 outros microelementos no osso: zinco e magnésio. As medidas quantitativas de chumbo foram feitas no sangue total coletado ao final dos períodos. As medidas quantitativas de chumbo, cálcio, zinco e magnésio foram realizadas a partir de soluções ácidas obtidas pela dissolução total do osso, sendo estas utilizadas para quantificar por Espectrometria de Absorção Atômica no modo chama ou com Forno e Grafite. Mandíbulas foram análisadas em Microscopia Eletrônica de Varredura e utilizando os acessórios Eletron Dispersive Spectroscopy para os elementos mais abundantes e Wavelenght dispersive Spectroscopy para investigar por análise semiquantitativa os mais abundantes elementos inorgânicos no osso destes animais, como forma de verificar se há variações nestes microelementos nos animais expostos a chumbo em comparação com controles. Como o chumbo se concentra na superfície do esmalte, também analisou-se o esmalte de primeiros molares inferiores, a fim de verificar se era visto o sinal do elemento chumbo nestes dentes. Foram usados ratos Wistar machos de 28 e 60 dias divididos em 2 grupos: C 28D (n=5) e Pb 28D (n=5) e, C 60D (n=12) e Pb 60D (n=17). Em todas as medidas de chumbo dos grupos controle as concentrações obtidas foram próximas ao limite de detecção, e houve sempre diferença estatisticamente significante (P<0.0001) quando os controles foram comparados aos grupos expostos a chumbo. As concentrações de chumbo obtidas no sangue de animais do grupo Pb 28D foi de 8,0 ug/dl (± 1.1) e no grupo Pb 60D foi de 7,2 ug/dl (± 0.89). Houve diferença estatisticamente significante entre as massas das tíbias (aferidas em balança e também a partir da quantidade de cálcio dissolvida em solução) do grupo C 60D (0,86 g ± 0,13) e Pb 60D (0,61 g ±0,11)(p=0.0004). As concentrações de chumbo detectadas nos ossos do grupo Pb 28D foi 8,02 (± 1,12) e no grupo Pb 60D foi de 43,3ug/g (± 13,26). Concentração de zinco no osso do grupo C 60D foi 0,22 mg/g e no grupo Pb 60D foi 0,22 mg/g (p>0.05). A concentração de magnésio no osso do grupo C 60D foi de 4,89 mg/g ± 0,74 e no grupo Pb 60D foi de 4,98mg/g ±0,79 (p>0.05). Os microelementos mais abundantes detectados nas tíbias contralaterais por Microscopia Eletrônica de varredura por EDS/WDS foram cálcio, fósforo, carbono, oxigênio, magnésio, sódio e potássio, não tendo sido detectadas variações nestes elementos entre os grupos controle e expostos a chumbo de 60 dias. O chumbo foi detectado apenas na região cervical dos dentes molares por MEV-EDS, e, nesta posição, apenas na forma de óxido de chumbo (deposição extrínseca) que pode ser útil para fins forenses ou de determinação de contaminação desconhecida. As conclusões deste estudo são: Houve diferença estatisticamente significante entre as massas de osso do grupo Pb 60D e C 60D. Mesmo não havendo diferença entre as concentrações de chumbo no sangue total de animais de 28 e 60 dias, houve quantidade de chumbo 5 vezes maior nos animais Pb 60D. Não houve diferenças nas concentrações de zinco e magnésio entre controles e animais expostos a chumbo. Os microelementos mais abundantes detectados nas tibias contralaterais por Microscopia Eletrônica de varredura por EDS/WDS foram cálcio, fósforo, carbono, oxigênio, magnésio, sódio e potássio, não tendo sido detectadas variações semi-quantitativas nestes elementos. O chumbo não foi detectado nem nos grupos controle ou expostos por MEV, e nos molares só foi detectada no grupo exposto como uma deposição extrínseca. / Lead is a highly toxic metal that is ubiquitous do to its great usefulness. Exposure to even low levels of lead (which do not cause symptoms of acute poisoning) is associated with cognitive and neurological disorders. Bone is considered the best marker of exposure to lead. Several chemical elements are found in bone, and some of them may have a different concentration or distribution due to exposure to lead. We tested the hypothesis that there are variations in the elements zinc and magnesium in the tibia of animals exposed to lead (being those elements determined by a conventional analytic chemistry technique). It was also tested whether variations in microelements could be detected by semiquantitative microanalysis in tibias of controls and animals exposed to lead. The objective of this project was to determine the concentration of lead in bone (tibia) of animals exposed to lead from pregnancy to day 28 and day 60. Besides lead, zinc and magnesium were also determined. Lead was also determined in whole blood collected at the end of the 2 time points. Quantitative measurements of lead, calcium, zinc and magnesium have been made in solutions obtained by dissolution of the bone by Atomic Absorption Spectrometry using flame or Graphite Furnace. Tibias were analyzed by scanning electron microscopy using the accessories Electron Dispersive Spectroscopy and Wavelength dispersive Spectroscopy for semiquantitative analysis of inorganic elements in the bone and molars of these animals. Lower molars were also analyzed by these means. Male Wistar rats aged 28 and 60 days were used in these study. In both these ages there were animals in the control (C 28D, n = 5 and C 60D, n=12) and lead exposed group (Pb 28D, n = 5 and Pb 60D, n = 17). Lead concentrations found in control groups were close to the detection limit, and there was always statistically significant differences (P <0.0001) when control groups were compared with those exposed to lead. Blood lead of groups Pb 28D and Pb 60D was 8.0 ug/dl (± 1.1) and 7.2 ug/dl (± 0.89), respectively. There was a statistically significant difference between the masses of the tibia (measured in balance and also from the amount of calcium dissolved in solution): C 60D showing 0.86 g (± 0.13 g) and 0.61 g (± 0, 11) of the Pb 60D group (p = 0.0004). Lead concentrations increased five times when the Pb 28D group was compared with the Pb 60D (8.02 and 43.3 ug/g)(p<0.0001). Zinc concentrations were not different in the C 60D (0.22 mg/g) and in the Pb 60D group (0.22 mg/g)(p>0.05). Magnesium concentrations were 4.89 mg/g in the C 60D and 4.98 mg/g in the Pb 60D (p>0.05). The most abundant trace elements detected in the contralateral tibiae by SEM-EDS/WDS were calcium, phosphorus, carbon, oxygen, magnesium, sodium and potassium and no variations in these elements were detected between the control and lead exposed groups. Lead was detected only in the cervical region of molar teeth of the lead exposed groups by SEM-EDS. This finding revealed lead in the lead oxide (extrinsic deposition) which may be useful for forensic purposes or for determining unknown contaminations. Conclusions: There was a statistically significant difference between the bone masses of the Pb 60D and C 60D groups. While there is no difference between lead concentrations in whole blood of animals 28 and 60 days, the amount of lead was 5 times higher in animals Pb 60D. O chumbo é um metal muito tóxico e que tem ampla distribuição pela sua grande utilidade. Mesmo a exposição a baixos níveis (que não causam sintomas de intoxicação aguda) de chumbo é associada a desordens cognitivas e neurológicas. O osso é considerado o melhor marcador de exposição a chumbo. No tecido ósseo, vários elementos estão presentes, e alguns deles poderiam ter sua concentração modificada pela exposição a chumbo. Testou-se a hipótese de que haja variações nos elementos Zn e Mg nas tíbias de animais expostos a chumbo (por técnica de química analítica convencional). Testou-se também se variações em microelementos poderiam ser detectadas por técnicas semiquantitativas nas tíbias de controles e animais expostos a chumbo. O objetivo deste projeto foi caracterizar em animais expostos ao chumbo desde a gestação e controles as concentrações deste elemento no osso (tíbia) de animais de 28 e 60 dias. Além do chumbo, investigamos também a concentração de 2 outros microelementos no osso: zinco e magnésio. As medidas quantitativas de chumbo foram feitas no sangue total coletado ao final dos períodos. As medidas quantitativas de chumbo, cálcio, zinco e magnésio foram realizadas a partir de soluções ácidas obtidas pela dissolução total do osso, sendo estas utilizadas para quantificar por Espectrometria de Absorção Atômica no modo chama ou com Forno e Grafite. Mandíbulas foram análisadas em Microscopia Eletrônica de Varredura e utilizando os acessórios Eletron Dispersive Spectroscopy para os elementos mais abundantes e Wavelenght dispersive Spectroscopy para investigar por análise semiquantitativa os mais abundantes elementos inorgânicos no osso destes animais, como forma de verificar se há variações nestes microelementos nos animais expostos a chumbo em comparação com controles. Como o chumbo se concentra na superfície do esmalte, também analisou-se o esmalte de primeiros molares inferiores, a fim de verificar se era visto o sinal do elemento chumbo nestes dentes. Foram usados ratos Wistar machos de 28 e 60 dias divididos em 2 grupos: C 28D (n=5) e Pb 28D (n=5) e, C 60D (n=12) e Pb 60D (n=17). Em todas as medidas de chumbo dos grupos controle as concentrações obtidas foram próximas ao limite de detecção, e houve sempre diferença estatisticamente significante (P<0.0001) quando os controles foram comparados aos grupos expostos a chumbo. As concentrações de chumbo obtidas no sangue de animais do grupo Pb 28D foi de 8,0 ug/dl (± 1.1) e no grupo Pb 60D foi de 7,2 ug/dl (± 0.89). Houve diferença estatisticamente significante entre as massas das tíbias (aferidas em balança e também a partir da quantidade de cálcio dissolvida em solução) do grupo C 60D (0,86 g ± 0,13) e Pb 60D (0,61 g ±0,11)(p=0.0004). As concentrações de chumbo detectadas nos ossos do grupo Pb 28D foi 8,02 (± 1,12) e no grupo Pb 60D foi de 43,3ug/g (± 13,26). Concentração de zinco no osso do grupo C 60D foi 0,22 mg/g e no grupo Pb 60D foi 0,22 mg/g (p>0.05). A concentração de magnésio no osso do grupo C 60D foi de 4,89 mg/g ± 0,74 e no grupo Pb 60D foi de 4,98mg/g ±0,79 (p>0.05). Os microelementos mais abundantes detectados nas tíbias contralaterais por Microscopia Eletrônica de varredura por EDS/WDS foram cálcio, fósforo, carbono, oxigênio, magnésio, sódio e potássio, não tendo sido detectadas variações nestes elementos entre os grupos controle e expostos a chumbo de 60 dias. O chumbo foi detectado apenas na região cervical dos dentes molares por MEV-EDS, e, nesta posição, apenas na forma de óxido de chumbo (deposição extrínseca) que pode ser útil para fins forenses ou de determinação de contaminação desconhecida. As conclusões deste estudo são: Houve diferença estatisticamente significante entre as massas de osso do grupo Pb 60D e C 60D. Mesmo não havendo diferença entre as concentrações de chumbo no sangue total de animais de 28 e 60 dias, houve quantidade de chumbo 5 vezes maior nos animais Pb 60D. Não houve diferenças nas concentrações de zinco e magnésio entre controles e animais expostos a chumbo. Os microelementos mais abundantes detectados nas tibias contralaterais por Microscopia Eletrônica de varredura por EDS/WDS foram cálcio, fósforo, carbono, oxigênio, magnésio, sódio e potássio, não tendo sido detectadas variações semi-quantitativas nestes elementos. O autor estudou as propriedades biomecânicas dos tendões dos músculos tibial anterior e tibial posterior congelados a -20°C e a -86°C extraídos de cadáveres humanos frescos. Foram realizados ensaios mecânicos de tração até a ruptura e determinadas as seguintes propriedades: resistência máxima, coeficiente de rigidez, módulo de elasticidade e alongamento máximo relativo. Os dados obtidos foram comparados aos existentes na literatura relativos ao ligamento cruzado anterior, ligamento da patela e aos tendões dos músculos grácil e semitendíneo / The author studied the mechanical properties of the anterior and posterior tibialis muscle tendons frozen at -20°C and -86°C obtained from fresh-frozen human cadavers. <p>Osnovni cilj rekonstrukcije prednjeg ukr&scaron;tenog ligamenta kolena je dobijanje pune stabilnosti kolena u celom obimu pokreta. Bez obzira na razvoj operativne tehnike i rehabilitacije i dalje postoji jedan broj pacijenata koji nije u potpunosti zadovoljan rezultatom rekonstrukcije prednjeg ukr&scaron;tenog ligamenta kolena, te je potrebno izvr&scaron;iti ponovnu operaciju. Smatra se da je najče&scaron;ći uzrok rane ponovne nestabilnosti lo&scaron;a pozicija kalema, tj. pozicija tunela koja nije na anatomskom mestu. Većina hirurga koji se bave ovom problematikom procenjuju poziciju kalema u golenjači na osnovu standardnih radiograma: prednje-zadnje i bočne projekcije, &scaron;to svakako nije dovoljno precizno. U ekonomski&nbsp; razvijenim zemljama poziciju kalema određuju analizirajući snimke kompjuterizovane tomografije (CT) ili magnetne rezonance (MRI). Prvenstveno zbog smanjenja doze zračenja kojoj se izlažu pacijenti prilikom kompjuterizovane tomografije, ali i iz ekonomskih razloga bilo bi korisno razvijanje jednog lako dostupnog, jeftinog ali preciznog i jednostavnog metoda određivanja položaja kalema u golenjači. Osnovni cilj ovog istraživanja bio je da se dokaže značaj pozicije kalema u golenjači i mogućnost svakodnevne kliničke primene novorazvijenog kompjuterskog programa u cilju određivanja prostorne pozicije kalema u golenjači iz samo dva standardna radiograma. Studija je bila eksperimentalno-prospektivnog karaktera. Eksperimentalni deo istraživanja sproveden je na Fakultetu tehničkih nauka (FTN) u Novom Sadu na Departmanu za proizvodno ma&scaron;instvo gde je na osnovu standardnih RTG snimaka (prednje-zadnje i bočne projekcije) razvijen kompjuterski program za određivanje prostornog položaja kalema u golenjači. U cilju verifikacije novorazvijenog programa realizovano je prostorno određivanje položaja kalema u golenjači obradom standardnih RTG snimaka (prednje-zadnje i bočne projekcije) kod 10 pacijenata kod kojih je izvr&scaron;ena primarna rekonstrukcija prednjeg ukr&scaron;tenog ligamenta kolena. Dobijeni rezultati su upoređeni sa položajem kalema na CT snimcima koji su obezbeđeni za sve pacijente. Na taj način je eksperimentalnim putem utvrđena preciznost novorazvijenog softvera u određivanju prostornog položaja kalema u golenjači. Drugi deo istraživanja bilo je prospektivno kliničko ispitivanje koje je sprovedeno na Klinici za ortopedsku hirurgiju i traumatologiju Kliničkog centra Vojvodine u Novom Sadu. Ispitivanu grupu je činilo 120 pacijenata, oba pola, sa prekidom prednjeg ukr&scaron;tenog ligamenta levog ili desnog kolena. Metodom slučajnog izbora pacijenti su podeljeni u četiri grupe od po 30 pacijenata prema veličini sagitalnog ugla (S) bu&scaron;enja kanala u golenjači (S 60&ordm;-69,9&ordm; i S 70&ordm;-80&ordm;) i prema veličini transverzalnog ugla (T) bu&scaron;enja kanala u golenjači (T 60&ordm;-69,9&ordm; i T 70&ordm;-80&ordm;). Grupa I 30 pacijenata (S 60&ordm;-69,9&ordm; i T 60&ordm;-69,9&ordm;), grupa II 30 pacijenata (S 60&ordm;-69,9&ordm; i T 70&ordm;-80&ordm;), grupa III 30 pacijenata (S 70&ordm;-80&ordm; i T 60&ordm;-69,9&ordm;), grupa IV 30 pacijenata (S 70&ordm;-80&ordm; i T 70&ordm;-80&ordm;). Svi pacijenti su godinu dana nakon operacije pro&scaron;li klinička ispitivanja po bodovnim skalama (Tegner bodovna skala, Lysholm bodovna skala i IKDC standard) i artrometrijska merenja. Rezultati dobijeni merenjem položaja kalema u golenjači, kliničkim ispitivanjima i artrometrijskim merenjima poređani su unutar svake grupe posebno, a izvr&scaron;eno je i poređenje dobijenih rezultata između svih grupa. U ispitivanje su uključeni samo oni pacijenti koji su dali potpisani informisani pristanak da učestvuju u ispitivanju nakon detaljnog upoznavanja sa planiranom procedurom. Svaki pacijent je bio informisan o svrsi i načinu sprovođenja istraživanja, kao i o pregledima i merenjima koja će biti vr&scaron;ena. Statističkom analizom rezultata utvrđeno je da veličina gre&scaron;ke prostornog određivanja položaja kalema u golenjači posle rekonstrukcije prednjeg ukr&scaron;tenog ligamenta kolena novorazvijenim kompjuterskim programom nije statistički značajna, a kompjutersko određivanje položaja kalema u golenjači omogućava iste rezultate kao i CT snimci. Sumiranjem zaključaka nakon sveobuhvatne analize dobijenih rezultata istraživanja, može se zaključiti da pozicija kalema u golenjači posle rekonstrukcije prednjeg ukr&scaron;tenog ligamenta kolena utiče na postoperativni funkcionalni rezultat.</p> / <p>The main goal of reconstruction of the anterior cruciate ligament of the knee is to obtain complete knee stability in the full range of movement. Regardless of the development of operational techniques and rehabilitation, there is still a number of patients who are not completely satisfied with the result of reconstruction of the anterior cruciate ligament, and a re-operation is required. It is believed that the most common cause of the instability is the bad position of the graft, i.e. position of the tunnel that is not at the anatomical place insertion. Most surgeons who deal with this problem evaluate the position of the graft in the tibia based on standard radiograms: anterior-posterior and lateral projections, which is not precise enough. In economically developed countries, the position of the graft is determined by analyzing images of computerized tomography (CT) or magnetic resonance (MRI). Primarily due to a decrease in the radiation dose exposed to patients during computerized tomography, but also for economic reasons, it would be useful to develop an easily accessible, inexpensive but precise and simple method for determining the position of the graft in the tibia. The main goal of this examination was to prove the significance of the position of the graft and the possibility of daily clinical use of the newly developed computer program in order to determine in-space position of the graft in only two standard radiograms. The study was experimental-prospective. The experimental part of the research was conducted at the Faculty of Technical Sciences (FTN) in Novi Sad at the Department of Production Engineering, where a computer program for determining in-space position of the graft in the tibia was developed on the basis of standard RTG images (anterior-posterior and lateral projections). In order to verify the newly developed program, in-space determination of the position of the graft in the tibia by processing standard RTG images (anterior-posterior and lateral projections) was performed in 10 patients in which the primary reconstruction of the anterior cruciate ligament was performed. The obtained results were compared with the position of the grafts on CT images provided to all patients. In this way, the accuracy of newly developed software in determining in-space position of the graft in the tibia was determined experimentally. The second part of the study was a prospective clinical trial conducted at the Clinic for Orthopedic Surgery and Traumatology at the Clinical Center of Vojvodina in Novi Sad. The investigated group consisted of 120 patients, both sexes, with a break of the anterior cruciate ligament of the left or right knee. By random selection, patients were divided into four groups of 30 patients according to the size of the sagital angle (S) of the tunnel drilling in the tibia (S 60&ordm;-69,9&ordm; and S 70&ordm;-80&ordm;) and according to the transversal angle (T) of the drill tunnel in the tibia ( T 60&ordm;-69.9&ordm; and T 70&ordm;-80&ordm;). Group I 30 patients (S 60&ordm;-69.9&ordm; and T 60&ordm;-69.9&ordm;), group II 30 patients (S 60&ordm;-69.9&ordm; and T 70&ordm;-80&ordm;), group III 30 patients (S 70&ordm;-80&ordm; and T&ordm;60&ordm; -69.9&ordm;), group IV 30 patients (S 70&ordm;-80&ordm; and T 70&ordm;-80&ordm;). All of the patients underwent clinical trials by scales (Tegner score scale, Lysholm score scale and IKDC standard) and arthrometric measurements one year after surgery. The results obtained by measuring the position of the graft in the tibia, clinical trials and arthrometric measurements were classified separately in each group, and obtained results were compared between all groups. The study included only those patients who gave signed informed consent to participate in the study after being thoroughly informed about planned procedure. Each patient was informed about the purpose and method of conducting the research, as well as the examinations and measurements to be performed. Statistical analysis of the results showed that the size of the error in measuring in-space determination of the position of the graft in tibia after reconstruction of the anterior cruciate ligament of the knee by a newly developed computer program was not statistically significant, and the computer determination of the position of the graft in the tibia provides the same results as the CT images. Summarizing the conclusions after a comprehensive analysis of the obtained results of the study, it can be concluded that the position of the graft in tibia after reconstruction of the anterior cruciate ligament affects the postoperative functional result.</p>
