Objetivo: estabelecer a relação entre a razão NeuropeptídeoY/Proteína relacionada Agouti (AgRP) e a adiponectinemia, bem como avaliar a influência de neuropeptídeos orexígenos nos aspectos nutricionais de adolescentes obesos com EHNA submetidos a uma intervenção interdisciplinar de longo prazo. Métodos: Recrutou-se adolescentes obesos pós-púberes para participarem de uma intervenção interdisciplinar. O grupo foi analisado de acordo com a presença ou não de EHNA pela ultrassonografia (US). As amostras de sangue foram coletadas para a mensuração da glicemia, transaminases hepáticas e perfil lipídico, resistência e sensibilidade insulínica. As concentrações de Adiponectina, NPY e AgRP foram avaliadas por ELISA e a mensuração da gordura visceral e subcutânea pela US. A ingestão alimentar foi aferida por meio de registro alimentar de 3 dias. Resultados: No início da terapia, observou-se que parâmetros clínicos importantes como massa corporal, IMC, gordura visceral, HOMA-IR, QUICKI, triglicérides, VLDL-colesterol e transaminases hepáticas apresentaram-se mais alterados em pacientes com EHNA, os quais melhoram após tratamento. Além disso, a energia total e ingestão de macronutrientes reduziram significativamente em ambos os grupos. Observou-se correlação positiva entre AgRP e a gordura visceral em todos os pacientes, e correlação negativa entre NPY/AgRP com a concentração de adiponectina apenas em adolescentes obesos com EHNA. Ao analisar a influência da dieta nos neuropeptídeos orexígenos, apenas os pacientes com EHNA apresentaram correlação positiva entre a ingestão de ácidos graxos saturados com os neuropeptídeos orexígenos (NPY e AgRP); e carboidrato com NPY. Foi observada correlação positiva entre ingestão de energia, (%) de lipídio e ácidos graxos saturados com o acúmulo de gordura visceral. Conclusão: Os pacientes com EHNA apresentaram parâmetros clínicos mais alterados ao compará-los àqueles que não possuíam esta doença, incluindo a correlação negativa entre adiponectina e NPY/AgRP. Estes resultados sugeriram que adolescentes obesos com EHNA apresentaram perfil inflamatório alterado, o qual pode influenciar na regulação neuroendócrina do balanço energético indicando uma barreira adicional na terapia de redução de peso. Além disso, nossos achados revelaram uma importante influência da composição da dieta no sistema orexígeno, sendo essencial considerar a ingestão excessiva de gordura saturada como um fator determinante para o desenvolvimento de EHNA. / BACKGROUND: The complexity physiophatology in the Non Alcoholic Fatty Liver Disease (NAFLD) involves interplay between adipokines and neuroendocrine regulation of energy balance, including the role of Neuropeptide Y/Agouti Related Protein system. AIM: establishing the relationship between Neuropeptide Y/Agouti Related Protein (NPY/AgRP) ratio and adiponectinemia, as well as to assess the influence of orexigenic neuropeptides in the nutritional aspects of NAFLD obese adolescents submitted to a long-term interdisciplinary approach. METHODS: A group of post-pubescent obese adolescents were recruited and included in the interdisciplinary intervention. The group was analyzed according to the presence or ausence of NAFLD by ultrassonography (US). Blood samples were collected to measure glycemia, hepatic transaminases and lipid profile, insulin resistance and sensitivity. Adiponectin, NPY and AgRP concentrations were measured by ELISA and the measurement of visceral ans subcutaneous fat by US. Food intake was measured using 3-day diet records. RESULTS: It was observed at baseline that important clinical parameters including body weight, BMI, visceral fat, HOMA-IR, QUICKI, triglycerides, VLDL-cholesterol and hepatic transaminases were more altered in NAFLD patients. After the intervention, these parameters, total energy and macronutrient intake were reduced significantly in both groups. The most important finding was the positive correlation between AgRP with visceral fat in all patients, and negative correlation between NPY/AgRP with adiponectin only in NAFLD obese adolescents. Analyzing the influence of the diet in the orexigenic neuropeptides, only the NAFLD patients presented a positive correlation between the saturated fatty acids intake and the orexigenic neuropeptides (NPY and AgRP); and carbohydrate with NPY. Indeed, it was observed a positive correlation between energy intake, lipid (%) and saturated fatty acids with visceral fat accumulation. CONCLUSION: NAFLD patients presented more altered clinical parameters than non-NAFLD, including the negative correlation between adiponectin and NPY/AgRP. These results suggested that NAFLD obese adolescents presented an altered inflammatory profile that can influence the neuroendocrine regulation of energy balance suggesting and additional impairment in the weigth loss therapy. Aspects of the interrelation between hypertension and insulin resistance: a preliminary study

Magister Scientiae (Medical Bioscience) - MSc(MBS) / Background: It is well known that some genetic factors and dietary factors, such as excessive salt intake and excessive caloric intake (resulting in obesity) are risk factors for hypertension. Fifty percent of all hypertensive patients are also insulin resistant. Both hypertension and insulin resistance are again risk factors for other cardiovascular diseases such as atherosclerosis and heart failure. The nature of the association between hypertension and insulin resistance has not been clearly elucidated. Spontaneously hypertensive rats are the ideal models to study the aspects of the relationships between hypertension and insulin resistance. Models of high-fat feeding induce obesity,hypertension and insulin resistance and are thus used extensively to study hypertension because these models closely mimic some of the renal and cardiovascular changes found in human hypertensive patients. The present study was initiated to evaluate if insulin resistance will develop within 6 weeks in a model of high-fat diet induced hypertension and if so, to determine whether captopril will affect the presence of insulin resistance.This model should in future be used to study vascular reactivity to phenylephrine (PHE),acetylcholine (ACH) and sodium nitroprusside (SNP) in hypertensive animals in theabsence or presence of insulin resistance and in normotensive insulin resistant animals. Methods: In a series of experiments, rats were divided into four groups that received different treatments: (i) laboratory pellets, (ii) high-fat diet, (iii) high-fat diet plus captopril and (iv) high-fat diet plus vehicle. Body weight was measured weekly for 6 weeks. Systolic blood pressure (SBP) and diastolic blood pressure (DBP) were measured every week during the 6-weeks feeding period by the tail cuff method using a two channel computerized non-invasive system from Kent Scientific Corporation, USA.m Intraperitonealy glucose tolerance tests (IPGTTs) were performed at week 3 and week 6.After 6 weeks, and after an overnight fast, the plasma lipid profile was determined using a portable CardiochekTM blood test system. Fasting plasma insulin was determined using an immunoenzymatic assay for the in vitro quantitative measurement of rat insulin (INS) in serum and plasma. Insulin sensitivity was estimated by the quantitative insulin sensitivity check index (QUICKI) using the fasting plasma insulin and fasting glucose levels. After week 6 on the high-fat diet, thoracic aortae from the control and high-fat fed(HFD) animals were excised and vascular response to PHE, ACH and SNP were assessed in intact and denuded endothelium.Result: High-fat feeding did not cause a significant increase in body weight. High-fat feeding significantly increased systolic blood pressure from 125±2.1 mmHg in control animals to 155±5.9 mmHg in the HFD group (P < 0.05) and 158±5.6 mmHg in the HFDV group (P < 0.05). Diastolic blood pressure was increased from 86±2.8 mmHg in the control group to 117±2.5 mmHg in the HFD group (P < 0.05) and 113±3.4 mmHg in the HFDV group (P < 0.05). Visceral fat was increased from 0.8±0.1g in the control group to 3.1±0.6 g in the HFD group and 3.8±0.6 g in the HFDV group. IPGTTs performed at weeks 3 and 6 respectively did not differ significantly from the control group as evidenced from the AUC’s at weeks 3 and 6 respectively. High-fat feeding had no significant effects on blood cholesterol, triglyceride, high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C) values or and fasting plasma insulin levels. The KCl induced contraction in both aortic rings with intact and denuded endothelium did not differ significantly between the control and HFD groups (P= 0.4 and 0.8) respectively. The contraction mediated by KCl in aortic rings with intact and denuded endothelium from the control or HFD groups also did not differ significantly(control: intact vs denuded, P = 0.2; HFD: intact vs denuded, P = 1). Dose responsecurves(1-10 μM) to PHE indicated slightly stronger contractions in the high-fat fed animals at submaximal doses tested. The maximum contraction achieved was however the same (94±19% and 99±2.6% relative to KCl induced contraction, in the control and HFD group respectively, P<0.05). Relaxation responses to ACH and SNP represent preliminary data.Conclusion: These data suggest that 6 weeks of high-fat feeding induces hypertension but does not produce obesity, dyslipidemia and insulin resistance. However, this model may be useful in studying vascular reactivity in hypertension in the absence of insulin resistance.

Hodnocení vybraných biochemických markerů metabolického syndromu a tukové tkáně u pacientů po bariatrickém výkonu / Evaluation of Selected Biochemical Markers of Metabolic Syndrome and Adipose Tissue After Bariatric Surgery

The diploma thesis deals the problematics of weight reduction with the using of bariatric- metabolic surgery and focuses on the changing risk components of the metabolic syndrome during one year long observation of 45 probands who have undergone Partial Jejuno-Ileal Diversion, Laparoscopic Sleeve Gastrectomy or Laparoscopic Gastric Plication. The main aim of the diploma thesis is to evaluate the changing risk components of the metabolic syndrome during weight reduction after undergoing bariatric surgery. The thesis shows that in the studied sample of bariatric patients it is an effective method of weight reduction (in PJID the success rate was 48 % EWL, in LGCP 51 % EWL and the most successful was LSG with 76 % EWL) with metabolic effect such as for example observed positive changes in risk components of the metabolic syndrome - reduction of morning glucose levels, increase of HDL cholesterol and decrease of triacylglycerols in the blood, decrease of waist circumferences and decrease of blood pressure or elimination of metabolic syndrome. Up to 68.9 % of the monitored probands showed signs of metabolic syndrome when evaluating the initial measurement before bariatric surgery, the remaining 22.2 % of the probands showed the signs after the year's observation. O Diâmetro Abdominal Sagital (DAS) é uma medida antropométrica relacionada com a gordura visceral e empregada para avaliar a obesidade abdominal, uma variável associada à síndrome metabólica. Sua utilização é indicada na prática clínica para avaliação de risco cardiometabólico em adolescentes obesos. OBJETIVO: Verificar a concordância entre o DAS e a circunferência abdominal (CA) na avaliação da obesidade central e sua associação com os critérios da Síndrome Metabólica e HOMA-IR em adolescentes obesos. CASUÍSTICA E MÉTODOS: Estudo de corte transversal constituído por 83 adolescentes obesos entre 14 e 18 anos, (46 do sexo feminino e 37 do sexo masculino) matriculados no Instituto da Criança do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, nos ambulatórios das Unidades de Endocrinologia Pediátrica e de Adolescentes. Foram submetidos a avaliações antropométricas (IMC, Escore Z do IMC, percentual de gordura corporal, circunferência abdominal, diâmetro abdominal sagital), laboratoriais (HDL-c, triglicérides (TG), glicemia (GLIS) e insulina para o cálculo do HOMA-IR) e de pressão arterial sistólica (PAS) e diastólica (PAD), utilizadas para classificação dos critérios da SM. RESULTADOS: Todos os adolescentes apresentaram valores elevados de IMC (36,9±6,7 kg/m2), Z-IMC (+3,27±0,94) e 91,6% da casuística tiveram valores alterados no percentual de gordura corporal (41,4% para o grupo feminino e 36% para o grupo masculino), confirmando a obesidade grave do grupo. Considerando o percentil >=90 do Center for Disease Control and Prevention (CDC), no National Health and Nutrition Examination Survey (NHANES 2011-2014), 85,5% dos adolescentes apresentaram valores elevados de CA (117,7 ± 14,7) e 89,2% valores alterados no DAS (26,9±3,7). Quanto às variáveis laboratoriais, 32,5% dos pacientes apresentaram diminuição de HDL-c e níveis aumentados de: TG (10,8%); GLIS (3,6%); PAS (32,5%,); PAD (21,7%) e HOMA-IR (79,5%), considerando toda a amostra. De acordo com os critérios utilizados pelo International Diabetes Federation, 27,7% da casuística apresentou SM. O DAS demonstrou estar significantemente correlacionado com as variáveis PAS (r=0,489 p < 0,001), PAD(r=0,277 p 0,011) e HOMA-IR (r=0,462 p < 0,001) nos grupos geral, feminino, masculino, com e sem SM. A correlação encontrada entre as medidas do DAS e CA no grupo geral e feminino foi de r = 0,91 (p 0,000) e, no grupo masculino, de r = 0,93 (p 0,000). Previous consentment was given by all patients and their families. RESULTS: All adolescents presented elevated BMI and Z-BMI, and high body fat percentage was displayed by 91.6% of the patients (41.4% for the female group and 36% for the male group), confirming severe obesity. Considering the >= 90 percentile cut-off values as provided by the Anthropometric Reference Data for Children and Adults: United States 2011-2014 for abdominal circumference and SAD, 85.5% of the patients presented high abdominal circumference values (117.7±14.7) and 89.2% presented elevated values of SAD (26.9±3.7). With regard to laboratory variables, 32.5% of the patients displayed decreased HDL-c and increased values of: triglycerides (10.8%); glycemia (3.6%); systolic blood pressure (32.5%); diastolic blood pressure (21.7%) and HOMA-IR (79.5%). According to the criteria of the International Diabetes Federation (IDF), 27.7% of patients presented metabolic syndrome. SAD was significantly correlated with systolic (r=0.489 p < 0.001) and diastolic (r=0.277 p 0.011) blood pressures and HOMA-IR (r=0.462 p < 0.001) in the general, female and male groups, with and without metabolic syndrome. The correlation between SAD and abdominal circumference in the general and female groups was r = 0.91(p 0.000) and in the male group was r = 0.93 (p 0.000). The concordance between SAD and abdominal circumference was significant (Kappa coefficient k = 0.511; p < 0.001). In the general, male and female groups with metabolic syndrome, the concordance was more expressive (Kappa coefficient; k = 1.00 and p < 0.001). These results show that the adolescents presented increased cardiometabolic risk and significant central obesity identified by SAD and abdominal circumference although 73.5% of the studied patients were maintained under medication for clinical metabolic syndrome symptoms. SAD displayed methodological advantages concerning its measurement. CONCLUSIONS: Under the conditions of this study, it is concluded that: the anthropometric measurements of SAD and abdominal circumference are equivalent for metabolic syndrome classification of the studied adolescents; that SAD is a predictor of systolic and diastolic blood pressures and HOMA-IR and is a strong indicator of cardiometabolic risk in obese adolescents

RESULTS: All adolescents presented elevated BMI and Z-BMI, and high body fat percentage was displayed by 91.6% of the patients (41.4% for the female group and 36% for the male group), confirming severe obesity. Considering the >= 90 percentile cut-off values as provided by the Anthropometric Reference Data for Children and Adults: United States 2011-2014 for abdominal circumference and SAD, 85.5% of the patients presented high abdominal circumference values (117.7±14.7) and 89.2% presented elevated values of SAD (26.9±3.7). With regard to laboratory variables, 32.5% of the patients displayed decreased HDL-c and increased values of: triglycerides (10.8%); glycemia (3.6%); systolic blood pressure (32.5%); diastolic blood pressure (21.7%) and HOMA-IR (79.5%). According to the criteria of the International Diabetes Federation (IDF), 27.7% of patients presented metabolic syndrome. SAD was significantly correlated with systolic (r=0.489 p < 0.001) and diastolic (r=0.277 p 0.011) blood pressures and HOMA-IR (r=0.462 p < 0.001) in the general, female and male groups, with and without metabolic syndrome. The correlation between SAD and abdominal circumference in the general and female groups was r = 0.91(p 0.000) and in the male group was r = 0.93 (p 0.000). The concordance between SAD and abdominal circumference was significant (Kappa coefficient k = 0.511; p < 0.001). In the general, male and female groups with metabolic syndrome, the concordance was more expressive (Kappa coefficient; k = 1.00 and p < 0.001). These results show that the adolescents presented increased cardiometabolic risk and significant central obesity identified by SAD and abdominal circumference although 73.5% of the studied patients were maintained under medication for clinical metabolic syndrome symptoms. SAD displayed methodological advantages concerning its measurement. CONCLUSIONS: Under the conditions of this study, it is concluded that: the anthropometric measurements of SAD and abdominal circumference are equivalent for metabolic syndrome classification of the studied adolescents; that SAD is a predictor of systolic and diastolic blood pressures and HOMA-IR and is a strong indicator of cardiometabolic risk in obese adolescents

Změny nutričního stavu během vysokodávkované chemoterapie u vybraných hematoonkologických onemocnění / Changes in nutritional status during high-dose chemotherapy in selected hematooncological diseases

Hematooncological diseases are often accompanied by dietary restriction, especially in cytotoxic therapy. The main purpose of the work was to assess the effect of high-dose chemotherapy on the change of nutritional status in two groups of hematooncological patients. A total of 16 patients were enrolled. Changes of the body composition were evaluated using bioelectrical impedance analysis supplemented by monitoring of biochemical nutritional indicators. Observations showed that in both groups the majority lost weight. In the first group of eight patients with acute myeloid leukemia observed during three consecutive hospitalizations, the median of change of body weight was -3.7 kg (-4.3%). Loss of lean body mass with a median value of -4.8 kg (-7.2%) was detected at all patients. Body fat was reduced at half of the patients. In some cases, with length of observation, there was an increase in fat mass along with visceral fat. In the second group, which included eight patients (after autologous hematopoietic stem cell transplantation) at whom one hospitalization was evaluated, body weight was reduced at six patients.
