Background: Organ transplantation is an established form of treatment applied worldwide, which saves many lives. There is however, an imbalance between supply and demand for organs, which means that the patients on the waiting list for organs often have to stay there for a long time. In worst case, they die before a suitable organ is found. Patients waiting for vital organs are therefore in a very exposed situation and this causes many feelings. Aim: The aim of the study was to describe patients´ experiences while waiting for vital organs. Method: A literature based study analysing twelve qualitative studies, that based onpatient interviews describe the patients' experiences while waiting for vital organs. Results: The result showed that patients felt bound, both physically and mentally. They experienced anxiety and stress during the long wait for an organ but had also hope forrecovery and wished to be able to return to a normal life. Information and support were an important part of getting through the wait. Two themes appeared during the analysis; To hope and to understand, the mixed emotions and To be bound and to be in need. Conclusion: All patients' experiences were individual, although there are many similarities in how they experienced waiting for vital organs, especially regarding emotions. Hope was a prerequisite for patients while waiting for a vital organ, as it servesas a driving force to hold out during their uncertain life situation.

Background: Organ transplantation is seen as a treatment to save lives. There are great differences in the transplantation process between different countries. Patients have previously described the process of organ transplantation as restrictive and emotionally draining with an overhanging fear of death before transplant. Aim: The aim of this study was to describe patients’ experiences of waiting for organ transplantation.  Method: This literature-based study was conducted with analysis of qualitative research to gain knowledge about the patients’ experiences. Analysis of the eleven articles was performed by using Friberg’s five-step method. Results: Two main themes and six subthemes emerged from the analysis. The main themes The need for support and information highlighted the patients need for support and adequate information from both family and health care professionals. This was seen as a way to handle the uncertainty that the waiting time could bring. The other main theme The time on the waiting list described that waiting for an organ transplantation could bring restrictions to their daily life. Patients described the time on the waiting list as uncertain and many thoughts about the donor arose. Also, hope about their future was prominent throughout the time on the waiting list. At times this hope could turn to despair. Conclusion: Waiting for an organ transplantation is a multifaceted experience. Support from family was considered crucial and therefore should be included when possible. Nurses are responsible for giving enough and appropriate information and support to the patients during their wait.

Interventions to Improve Nurse-Family Communication in the Emergency Department: A Scoping Review

Background: Family members often accompany patients to the emergency department and play an important role in caring for the patient. Communication between nurses and family members has long been identified as a priority of care and an area needing improvement due to the increasingly busy nature of the emergency department. Good communication can improve patient outcomes, satisfaction with care and decrease patient and family anxiety. Objective: To determine and describe what interventions exist to improve nurse-family communication during the waiting period of an emergency department visit. Methods: A scoping review was completed following the Joanna Briggs Institution methodology: (1) identify the research question, (2) define the inclusion criteria, (3) use a search strategy to identify relevant studies using a three-step approach, (4) select studies using a team approach, (5) data extraction, (6) data analysis, and (7) presentation of results. Results: The search yielded 1,771 articles of which 20 were included in the review. An additional 7 articles were found in the grey literature search. Results were analyzed using basic content analysis, reported using tables, figures, descriptive statistics and narrative synthesis and organized based on Peplau's Theory of Interpersonal Relations. A variety of pediatric and adult interventions were found targeting staff members and family members and took place worldwide. Two models were developed based on the results of this review: a communication model for triage nurses and one for all emergency department nurses. Conclusion: Communication plays an essential role in emergency department nursing care. Communication skills training should be built into nursing curriculum, nursing orientation, triage training, and continuing education. Future research should focus on evaluating the effectiveness of interventions using a standardized scale, understanding the specific needs of family members and effectively teaching communication skills to emergency department nurses.

Background: The history of transplantation goes back a long time and has been the last resort as a treatment to save a person's life. The availability of organs does not correspond to the need, there are also limited studies on how the patient's health processes are affected during the waiting time for a new organ. Aim: The aim of this study was to describe patientexperiences of waiting for an organ transplant. Method: This literature-based study was based on qualitative research to describe patient experiences of managing the waiting time for an organ transplant. 11 articles were analyzed according to Friberg's (2017) five step model. Results: The article analysis resulted in two main themes and six sub-themes. The main theme New Challenges highlighted that the patients are in need for both support and information during the time that they are waiting for an organ transplant. The second main theme, Emotional stress highlighted what feelings the patient can endure during the waiting time for a transplantation. The feelings of both hope and uncertainty are described by the patients. Conclusion: When nurses take responsibility for the caring and the six areas of competence, which means that patients who are on the waiting list for an organ transplant have their nursing needs met.

Architecture has the unique ability to use its surroundings in a way that can create a profound impact; it has the power to generate emotion. To connect with the character of a place, we need to observe it, converse with it, and learn from it. No matter what the place, understanding its spirit is essential to design for it, because every addition to the place adds to its soul, its spiritual and emotional identity. It is important to note the first users of each site are what already exists, they define the spirit of the site and these users are not limited to just people, but rather they include the landscape as well. The trees that are growing and flourishing within the site are an important part of it. Architecture should look at the existing site and not take away from it but rather add to it. This thesis explores Daingerfield Island; Located along the existing Mt Vernon bicycle trail and the Potomac River, Daingerfield Island is an ideal location for a Bicycle and Water Taxi rest stop. It is a site that is mostly forested development, however, it is in a state of disrepair. It has great potential to become a wonderful pause space in the urban environment. The project explores emotions associated with resting and waiting, in hopes to prove that architecture has the ability to repair a damaged site. / Master of Architecture / Architecture not only affects those that inhabit it, but also its surroundings. Whenever we design a building, we need to understand that it is an addition to an already existing site; this addition should make the location better. Analyzing the site is essential in order to create an appropriate design for it. The thesis project is a Bicycle and Water Taxi rest stop located on Daingerfield Island, which is located along the existing Mt Vernon bicycle trail and the Potomac River. The design began with the sole intent of understanding the site; what its history is, what its present is, what its shortcomings are, what its strengths are, and what it needs. Only then did the architectural design begin.

O efeito da espera sobre a avaliação geral do serviço : uma abordagem contingencial

O presente estudo tem como principal objetivo avaliar o impacto de aspectos contingenciais no modelo básico do efeito da espera na avaliação geral de um serviço. Com base em estudos referentes à espera já realizados, o presente trabalho definiu o “modelo básico do efeito da espera na avaliação geral do serviço” que consiste no impacto negativo da espera na avaliação geral do serviço por meio da suscitação de emoções negativas. A influência de fatores contingenciais sobre o modelo é a principal contribuição do estudo, dado que essa abordagem não havia sido ainda estudada. Inicialmente, foi realizada uma etapa exploratória, qualitativa, para investigar as relações propostas no modelo teórico, assim como para compreender melhor os serviços de saúde, campo de estudo da pesquisa. Na sequência, a influência do envolvimento do consumidor, da singularidade do serviço, da pressão do tempo e do relacionamento interpessoal no modelo central foi investigada através da realização de uma survey com 382 pacientes As hipóteses foram testadas por meio de modelagem de equações estruturais, com o software LISREL 8.51. Os resultados indicam que o envolvimento do consumidor tem impacto positivo no seu tempo de espera aceitável. Já o efeito da singularidade do serviço sobre essa mesma variável não foi confirmado. A pressão do tempo apresentou impacto positivo no tempo de espera percebido e o efeito moderador do relacionamento interpessoal na relação entre as respostas afetivas negativas e a avaliação geral do serviço também foi confirmada. Além das hipóteses propostas, foi realizado um teste exploratório segundo o qual se confirmou o efeito moderador da variável gravidade sobre as relações que compõe o modelo básico do efeito da espera da avaliação geral do serviço. No final, os resultados são discutidos, e as limitações do estudo e sugestões para futuras pesquisas são apresentadas. / The most important objective of the present study is to evaluate the impact of the contingency aspects on the basic model of the waiting effect in the overall service evaluation. Based on studies in reference to the wait already carried out, this study defined the "basic model of the waiting effect on the general evaluation of the service" as being the negative impact of waiting in the general evaluation of the service because of the rise of negative emotions. The influence of contingency factors over the model is the most important contribution of the study as this approach has never been studied before. Iniatially, there was a qualitative exploratory to investigate the proposed relations in the theoretical model, as well as to have a better understanding of health care services, the field study of the research. Following that, the influence of the consumer involvement, the uniqueness of the service, the time pressure and the interpersonal relationship in the central model was investigated through a survey conducted among 382 patients The hypotheses were tested using structural equation modeling with LISREL 8.51 software. The results showed that the consumer involvement have a positive impact in the acceptable waiting time. On the other hand the effect of the uniqueness of the service over the same variable was not confirmed. The time pressure indicated a positive impact on the perceived waiting time and the moderate effect of the interpersonal relationship of the connection between negative affective reactions and the overall service evaluation was also confirmed. Beyond the proposed hypotheses, it was performed a exploratory test whereby it was confirmed the moderator effect of the gravity variable on the relations that form the basic model of the waiting effect on the overall service evaluation. In the end the results are discussed, and the limitations of the study and suggestions for future surveys are presented.

Assessment of waiting and service times in public and private health care facilities in Gondar district, North western Ethiopia.

<p>The development and provision of equitable and acceptable standard of health services to all segments of the population has been the major objective of the 1993 Ethiopian National health policy. However, community based studies on satisfaction with public health care facilities reveal that the majority of the population are not satisfied with the services provided predominantly as a result of the long waiting times. Studies done on private health facilities on the contrary reveal that patients are satisfied with the service delivered within short waiting times in these clinics. Even though the speculated waiting time is thought to be long among the public health care facilities and short in private clinics, the actual waiting and service times have not been measured and compared. Aim: To determine the waiting and service times among the public and private health care facilities and measure the perceptions of &ldquo / acceptable&rdquo / waiting time among the providers and clients. Materials and methods: A cross sectional observational study using quantitative techniques was carried out amongst patients and staff at selected public and private health care facilities in Gondar District. Stratified sampling method was used to select facilities. All patients visiting the selected facilities and all staff who provided service to patients on the day of the study were included in the time-delimited sample. Data was collected by research assistants and health workers from all patients attending the health care facility by registering the arrival and departure time of each patient to the facility and to each service point on a patient flow card. Then data was cleaned and captured by a specific Waiting and Service Time database. Descriptive statistics was done on waiting and service times for each facility and this was summarized for each public and private health facility by using tables and graphs. Finally a comparison was made for private and public health facilities by using Wilcoxon-mann-whitney non parametric tests.</p>
