Objetivos: Este trabalho objetivou avaliar e comparar a incidência de linfedema, dor, restrição na amplitude de movimento e déficit sensorial 1 dia, 15 dias, 3 meses, 6 meses e 18 meses no membro superior homolateral, após linfadenectomia axilar total (LAT) e biópsia de linfonodo sentinela (BLS) para tratamento do câncer de mama. MÉTODOS: Neste estudo incluiu-se pacientes que realizaram ressecção segmentar de mama e LAT ou BLS. As pacientes foram avaliadas entre Dezembro de 2003 e Dezembro de 2004 e foram constituídos por dois grupos, o grupo A (n=100) foi formado por casos submetidos à LAT e o Grupo B, por casos submetidos à BLS (n=50). As pacientes foram analisadas através de questionário com respostas afirmativas e negativas, onde foram incluídas questões avaliando os sintomas de linfedema, dor, restrição da amplitude de movimento e déficit sensorial 1 dia (n=30), 15 dias (n=30), 6 meses (n=30) e 18 meses (n=30), após o procedimento cirúrgico. Além disto efetuou-se avaliação fisioterapêutica na qual foi realizada perimetria de membros superiores, mensuração da amplitude de movimento através da goniometria, os testes de sensibilidade cutânea através dos monofilamentos de Semmes-Weinstein e teste eletrodiagnóstico, utilizando-se gerador universal de correntes. RESULTADOS: No grupo A houve diferenças significantes (p < 0,05) em todos os sintomas subjetivamente avaliados, existindo maior queixa de edema (31%), dor (58%), restrição do movimento (52%) e dormência (57%). No grupo B houve menor prevalência da sensação de edema (2%), dor (26%), restrição do movimento (24%) e dormência (20%), em todos os tempos estudados. No exame clínico, o exame sensorial com monofilamentos apresentou 82% de diminuição da sensibilidade tátil na região axilar e 73% na região braquial proximal no grupo de mulheres submetidas à LAT e no grupo de mulheres submetidas à BLS houve diminuição da sensibilidade tátil nas duas regiões avaliadas em 40% (p<0.05). Nas provas cronaximétricas, a sensibilidade estava diminuída no grupo de mulheres submetidas à LAT em 56% na região axilar e 48% na região braquial proximal medial e no grupo BLS a sensibilidade estava diminuída em 39% e 14% respectivamente. METHODS: A controlled study was designed to include patients who underwent breast conservative surgery and ALND or SLNB. Patients were evaluated between December 2003 and December 2004, and they were divided into two groups, group A (n=100) consisted who had ALND and group B, with patients after only SLNB (n=50). All of them subjectively answer a \"yes\" or \"no\" questionnaire in which were included questions about incidence of lymphedema, pain, motion restriction and arm numbness at 1day (n=30), 15 days (n=30), 3 months (n=30), 6 months (n=30) and 18 months (n=30) after surgery. Besides this a physiotherapy examination was performed measuring circunference, shoulder adbuction range by a goniometer, sensibility tests by pressure Semmes-Weinsten monofilaments and by eletrodiagnostic tests using an universal stimulator. RESULTS: In the group A significant differences (p < 0,05) were seen in subjectively sensations, with significantly higher incidence of subjective swelling (31%), pain (58%), motion restriction (52%), arm numbness (57%). In group B there were less symptoms of subjective swelling (2%), pain (58%), motion restriction (24%), arm numbness (20%). In clinical examination sensory deficit using monofilaments the occurrence of numbness was 82% on axillary fold and 73% on medial part of upper arm after ALND and in 40% on axillary fold and medial part of upper arm after SLNB, with significantly different between the groups (p < 0.05). At cronaximetric proofs the sensibility was decreased in 56% in the axillary fold and 48% in the medial part of upper arm for ALND and 39% in the axillary fold and 14% at medial part of upper arm on patients who underwent SLNB. CONCLUSIONS: Arm morbidity is higher and function is lower after ALND compared to SLNB, in women submitted to breast cancer surgery

Ce travail présente notre contribution au domaine du raisonnement temporel (RT) en intelligence artificielle. Nous avons défini et mis en oeuvre un mécanisme de raisonnement abductif (génération d'hypothèses) pour le RT. Un tel mode de raisonnement présente en particulier l'intérêt d'être une alternative possible au raisonnement par défaut pour prendre en compte la non monotonie inhérente au RT. Une autre motivation est que le raisonnement abductif appliqué au RT est une approche possible pour la planification. Préalablement à la présentation de la procédure d'abduction, nous étudions l'intérêt pour le raisonnement temporel du principe de résolution avec contraintes de Bürckert. Ce principe s'avère être un cadre formel intéressant pour décrire l'intégration des systèmes de contraintes temporelles dans des systèmes déductifs utilisant le principe de résolution comme règle d'inférence. Dans la deuxième partie, nous proposons une procédure de génération d'hypothèses qui est basée conjointement sur des travaux de Kakas et Mancarella sur l'abduction en programmation logique, et sur l'idée de la résolution avec contraintes. La procédure que nous proposons possède des mécanismes originaux facilitant en premier lieu son application au raisonnement temporel, et permettant ensuite de ramener la conservation de la consistance des hypothèses déjà générées à des tests de satisfaction de contraintes temporelles. Nous présentons des exemples d'utilisation, en particulier pour la planification en utilisant le Calcul d'Evénements de Sergot et Kowalski comme formalisme de représentation. Le cadre de la programmation logique avec contraintes, sous jacent à notre travail, nous permet d'étendre notre procédure à d'autres systèmes de contraintes. Nous décrivons en particulier l'utilisation de contraintes sur domaines finis, ce qui permet de décrire et gérer des ressources finies en planification. L'implantation de la procédure utilise, en plus des contraintes temporelles et des contraintes sur domaines finis, des contraintes sur le typage des termes, ce qui nous permet de proposer un cadre logique avec types et sous-typage qui facilite la description du formalisme temporel et des problèmes à résoudre.

In the same breath, relationships are easily un-formed and the family fractured. The emotional impact of any breakup is usually  / tense, and it follows by implication that this will be exacerbated if a child is involved. For reasons that will be expounded on in the thesis, many caregivers, usually mothers have  / opted to abduct their children. Some have opted to apply formally to relocate. Relocation disputes are widely regarded as one of the most controversial and difficult issues in  / family law internationally. These disputes usually arise pursuant to a relational breakdown, when the resident parent (usually the mother) seeks to relocate with the children  / either  / domestically or internationally. This causes a significant impact  / on contact arrangements with the other parent (usually the father). Depending on the country in which they find themselves and the laws applicable to relocation with the child, mothers are faced with the decision to remove the child either lawfully or wrongfully. International family law  / jurisprudence has been developed over time to assist with custody and relocation disputes. &lsquo / Much has been written on the Hague Convention &ndash / its flaws and its successes, its  / effectiveness and utilization. This thesis aims to look specifically at relocation disputes within the context of international parental child abduction / more specifically, it sets out to  / explore whether there is a link between those phenomena, and whether the Hague Convention is sufficient for dealing with relocation disputes. I hope to make a convincing  / argument that if there were an international instrument regulating relocation, there would be uniformity and consistency. Self powered wrist extension orthosisSinger, Mathew Kyle January 2006 (has links)
One of the most devastating effects of tetraplegia is the inability to grasp and manipulate everyday objects necessary to living an independent life. Currently surgery is widely accepted as the solution to improve hand functionality. However, surgery becomes difficult when the user has paralysed wrists as is the case with C5 tetraplegia. The aim of this research was to develop a solution which provided controlled wrist flexion and extension which, when combined with surgery, achieves a 'key pinch' grip. This particular grip is critically important for people with C5 tetraplegia as it is used for countless grasping activities, necessary on a day-to-day basis. A systematic design process was used to evolve the solution to provide controlled wrist flexion and extension. Concept brainstorming identified four alternative solutions which were evaluated to find the preferred concept. The chosen solution was called the Self Powered Wrist Extension Orthosis, more commonly referred to as the 'orthosis'. This concept contained a shoulder harness which provided both energy and control to the wrist harness, which in turn changed the wrist position. The orthosis was developed with the use of a mathematical model which theoretically predicted the functional performance by comparing the required force needed to move the wrist harness to the achievable force supplied by the user's shoulders. Using these parameters, the orthosis was optimized using the matlab Nelder-Mead algorithm which adjusted the wrist harness geometries to maximize the functional performance. A prototype was constructed and tested with the help of two participants who when combined, achieved an average of 18.5° of wrist rotation. The theoretical model however predicted an average range of motion of 28.4°. The discrepancy found between the theoretical and experimental result can be contributed to incorrect assumptions in the theoretical model. This included unaccounted friction and inaccurate modeling of the orthosis dynamics. The feedback from potential users of the orthosis was enthusiastic and encouraging especially towards the simplicity, usability and practicality of the design.

Företagen samlar avsiktligt in personlig information och spårar vilka hemsidor och produkter eller tjänster de intresserat sig av genom så kallade webb-spårningssystem. I samma takt som tekniker så som webb-spårningssystem börjat användas har också etisk marknadsföring på Internet blivit ett diskuterat och aktuellt ämne. Etisk marknadsföring på Internet handlar om vad som uppfattas som accepterat beteende av konsumenterna och företag måste ta hänsyn till konsumentens etiska koder. Syfte: Syftet med studien är att undersöka konsumenters uppfattningar om webbspårningssystem utifrån etiska koder. Forskningsfråga: Vad är etiskt accepterat användande av webb-spårningssystem från konsumentens perspektiv? Metod: I studien har en kvalitativ forskningsstrategi samt semi-strukturerade intervjuer tillämpats. Slutsats: Slutsatsen i studien är att konsumenters uppfattningar om webb-spårningssystem inte alltid uppfyller konsumentens etiska koder. Konsumenter har generellt sätt en negativ uppfattning om webb-spårningssystem men uppfattningarna skiljer sig beroende på vilken situation som konsumenten påverkas av webb-spårningssystem. Konsumenter har enbart positiva uppfattningar om webb-spårningssystem när tekniken används så att konsumenten gynnas av den.

Experiences of teachers in SFI, Swedish for (adult) Immigrants, indicate that during their schooling earlier in life, SFI students have developed skills, abilities, values, ideas and expectations about teaching and learning that differ somewhat from the prevailing communicative oriented theory of second language teaching which is emphasized in SFI. In the thesis these aspects are referred to as different school cultural frames of reference. The aim of the thesis is to generate knowledge about SFI-students’ school cultural frames of reference of relevance for SFI teaching. The considerable number of immigrants from Iraqi Kurdistan during the 1990s has led to an empirical focus related to this geographical area. From a critical perspective, in some respects a research interest of this kind can be seen as contributing to a division between ‘us’ and ‘them’, in a wider sense a part of exclusion and a maintenance of the segregated Swedish society. In a special section is given an account of this research ethic question, together with arguments from intercultural pedagogy that support a focus on school cultural frames of reference. The theoretical platform for the thesis is sociocultural theory. The concepts of social representations, pedagogical code, classification, framing, power distance, diaspora and distinctions of knowledge also are used. The thesis is based on two data materials. The first consists of data from interviews and talks with students and teachers in SFI, all from Iraqi Kurdistan. The second consists of data gathered through observations, classroom observations, interviews and talks during two visits in the KDP-administrated region of Iraqi Kurdistan, each visit being for a period of about one month. This data material also includes text materials, mainly textbooks in EFL for grade five and six, and course books about EFL teaching used in teacher education. The thesis illuminates several aspects that provide an understanding as to why SFI students from Iraqi Kurdistan can have certain abilities, values, ideas and expectations about teaching, learning materials, learning, teacher and student roles that differ from the communicative oriented second language teaching emphasized in SFI. However, results from the study also underline the importance of a ‘weak’ use of this understanding in a SFI teaching context.

Equity perception and communication among Arab expatriate professionals in the Kingdom of Saudi ArabiaHijazy, Muhammad January 2017 (has links)
The research aims to study how the communication context within the Arab cultures influences the employees' perception of equity and reaction to inequity. Specifically, the study explores how employees from Arab cultural backgrounds communicate with each other within the Saudi working context; and how they collect, interpret and use the different contextual information - from the contexts in which they live and work - in order to make judgements about issues related to the perception of equity and reaction to inequity. In order to study the research topic, a conceptual framework is developed to reconcile between Equity Theory, Social Comparison Theory and Hall's Context Model; and as a base serving the process of designing/choosing the methods of collecting and analysing the data. Three main research questions are developed which are about (i) how the communication context is related to employees' willingness and ability to react to inequity (ii) how the communication context shapes the nature of inequity reactions executed by employees and (iii) how the communication context is related to the way equity is perceived among employees. A modified version of critical realism is adopted to focus on exploring the mechanisms, within the communication context, which influence the perception of equity and reaction to inequity. A combination of retroduction and abduction is developed in a sense that retroduction is used to direct the research toward exploring the structure and mechanisms within the research setting, while abduction is used to draw conclusions about how the phenomena studied in the research are evolving by the structure and mechanisms. A mixed methods approach is adopted in the research. The research includes data from thirty-five semi-structured interviews which are conducted in mainly three Saudi private-sector organisations located in Jeddah with twenty-nine male employees and six male managers of six different Arab nationalities. Template analysis is used to analyse the qualitative interview transcripts and field notes, while cluster analysis is used to group the research participants based on their quantitative responses. The research finds that there are no clear-cut areas separating the activities linked to the perception of equity and reaction to inequity. I also conclude that the perception of equity norms and equity comparison components can sometimes be separate activities. Some factors such as the religious interpretation, face-saving, and contextual norms and powers influence the employees' willingness to react to inequity by altering the way in which those employees perceive equity norms. Here, unwillingness decisions are often made not as a result of personal conviction but as a compromise based on the personal evaluation of the surrounding context, realising the inability of the self to react to such situations in the first place. Thus, it can be concluded that inability to react to inequity can reduce the employees' willingness to react against under-rewarded situations. The process of perceiving equity comparison components is found to be related to the type of reaction adopted to re-establish the equity; this relationship is represented by groups affiliated by a hidden factor or factors, which is more influential than the ethnicity/nationality of the group's members. The research makes a methodological contribution to knowledge by suggesting a new approach to study human relations through the communication context; a conceptual contribution by combining the concepts of equity perception, social comparison and communication context in one conceptual framework; and an empirical contribution by providing a fresh insight to contextual themes in the Saudi working environment.
