A Multiscale in Silico Study to Characterize the Atrial Electrical Activity of Patients With Atrial Fibrillation. A Translational Study to Guide Ablation Therapy

[ES] La fibrilación auricular es la arritmia cardíaca más común. Durante la fibrilación auricular, el sustrato auricular sufre una serie de cambios o remodelados a nivel eléctrico y estructural. La remodelación eléctrica se caracteriza por la alteración de una serie de canales iónicos, lo que cambia la morfología del potential de transmembrana conocido como potencial de acción. La remodelación estructural es un proceso complejo que involucra la interacción de varios procesos de señalización, interacción celular y cambios en la matriz extracelular. Durante la remodelación estructural, los fibroblastos que abundan en el tejido cardíaco, comienzan a diferenciarse en miofibroblastos que son los encargados de mantener la estructura de la matriz extracelular depositando colágeno. Además, la señalización paracrina de los miofibroblastos afecta a los canales iónicos de los miocitos circundantes. Se utilizaron modelos computacionales muy detallados a diferentes escalas para estudiar la remodelación estructural inducida a nivel celular y tisular. Se realizó una adaptación de un modelo de fibroblastos humanos a nivel celular para reproducir la electrofisiología de los miofibroblastos durante la fibrilación auricular. Además, se evaluó la exploración de la interacción del calcio en la electrofisiología de los miofibroblastos ajustando el canal de calcio a los datos experimentales. A nivel tisular, se estudió la infiltración de miofibroblastos para cuantificar el aumento de vulnerabilidad a una arritmia cardíaca. Los miofibroblastos cambian la dinámica de la reentrada. Una baja densidad de miofibroblastos permite la propagación a través del área fibrótica y crea puntos de salida de actividad focal y roturas de ondas dentro de esta área. Además, las composiciones de fibrosis juegan un papel clave en la alteración del patrón de propagación. La alteración del patrón de propagación afecta a los electrogramas recogidos en la superficie del tejido. La morfología del electrograma se alteró dependiendo de la disposición y composición del tejido fibrótico. Se combinaron modelos detallados de tejido cardíaco con modelos realistas de los catéteres de mapeo disponibles comercialmente para comprender las señales registradas clínicamente. Se generó un modelo de ruido a partir de señales clínicas para reproducir los artefactos de señal en el modelo. Se utilizaron electrogramas de modelos de dos dominios altamente detallados para entrenar un algoritmo de aprendizaje automático para caracterizar el sustrato fibrótico auricular. Las características que cuantifican la complejidad de las señales fueron extraídas para identificar la densidad fibrótica y la transmuralidad fibrótica. Posteriormente, se generaron mapas de fibrosis utilizando el registro del paciente como prueba de concepto. El mapa de fibrosis proporciona información sobre el sustrato fibrótico sin utilizar un valor único de corte de 0,5 milivoltios. Además, utilizando la medición del flujo de información como la entropía de transferencia combinada con gráficos dirigidos, en este estudio, se siguió la dirección de propagación del frente de onda. La transferencia de entropía con gráficos dirigidos proporciona información crucial durante la electrofisiología para comprender la dinámica de propagación de ondas durante la fibrilación auricular. En conclusión, esta tesis presenta un estudio in silico multiescala que proporciona información sobre los mediadores celulares responsables de la remodelación de la matriz extracelular y su electrofisiología. Además, proporciona una configuración realista para crear datos in silico que pueden ser usados para aplicaciones clínicas y servir de soporte al tratamiento de ablación. / [CA] La fibril·lació auricular és l'arrítmia cardíaca més freqüent, en la qual el substrat auricular patix una sèrie de remodelacions elèctriques i estructurals. La remodelació de tipus elèctric es caracteritza per l'alteració d'un conjunt de canals iònics que modifica la morfologia del voltatge transmembrana, conegut com a potencial d'acció. La remodelació estructural és un fenomen complex que implica la relació entre diversos processos de senyalització, interaccions cel·lulars i canvis en la matriu extracel·lular. Durant la remodelació estructural, els abundants fibroblasts presents en el teixit cardíac comencen a diferenciar-se en miofibroblasts, els quals s'encarreguen de mantenir l'estructura de la matriu extracel·lular dipositant-hi col·lagen. A més, la senyalització paracrina dels miofibroblasts amb els miòcits circumdants també afectarà els canals iònics. Es van utilitzar models computacionals molt detallats a diferents escales per estudiar la remodelació estructural induïda a nivell tissular i cel·lular. Es va fer una adaptació a nivell cel·lular d'un model de fibroblasts humans per reproduir-hi l'electrofisiologia dels miofibroblasts durant la fibril·lació auricular. A més, l'exploració de la interacció del calci amb l'electrofisiologia dels miofibroblasts va ser avaluada mitjançant l'adequació del canal de calci a les dades experimentals. A nivell tissular es va estudiar la infiltració de miofibroblasts per tal de quantificar l'augment de vulnerabilitat que això conferia per patir una arrítmia cardíaca. Els miofibroblasts canvien la dinàmica de la reentrada, i presentar-ne una baixa densitat permet la propagació a través de la zona fibròtica, tot creant punts de sortida d'activitat focal i trencaments d'ones dins d'aquesta àrea. A més, les composicions de fibrosi tenen un paper clau en l'alteració del patró de propagació, afectant els electrogrames recollits en la superfície del teixit. La morfologia dels electrogrames es va veure alterada en funció de la disposició i la composició del teixit fibròtic. Per comprendre els senyals clínicament registrats es van combinar models detallats de teixits cardíacs amb models realistes dels catèters de cartografia disponibles comercialment. Es va generar un model de soroll a partir de senyals clínics per reproduir-hi els artefactes de senyal. Es van utilitzar electrogrames de models de bidominis molt detallats per entrenar un algoritme d'aprenentatge automàtic destinat a caracteritzar el substrat fibròtic auricular. Les característiques que quantifiquen la complexitat dels senyals van ser extretes per identificar la densitat i transmuralitat fibròtica. Posteriorment, es van generar mapes de fibrosi mitjançant la gravació del pacient com a prova de concepte. El mapa de fibrosi proporciona informació sobre el substrat fibròtic sense utilitzar un sol valor de tensió de tall de 0,5 mV. A més, utilitzant la mesura del flux d'informació com l'entropia de transferència combinada amb gràfics dirigits, en aquest estudi es va fer un seguiment de la direcció de propagació de l'ona. L'entropia de transferència amb gràfics dirigits proporciona informació crucial durant l'electrofisiologia per entendre la dinàmica de propagació d'ones durant la fibril·lació auricular. En conclusió, aquesta tesi presenta un estudi multi-escala in silico que proporciona informació sobre els mediadors cel·lulars responsables de la remodelació de la matriu extracel·lular i la seva electrofisiologia. A més, proporciona una configuració realista per crear dades in silico que es poden traduir a aplicacions clíniques que puguen donar suport al tractament de l'ablació. / [EN] Atrial fibrillation is the most common cardiac arrhythmia. During atrial fibrillation, the atrial substrate undergoes a series of electrical and structural remodeling. The electrical remodeling is characterized by the alteration of specific ionic channels, which changes the morphology of the transmembrane voltage known as action potential. Structural remodeling is a complex process involving the interaction of several signalling pathways, cellular interaction, and changes in the extracellular matrix. During structural remodeling, fibroblasts, abundant in the cardiac tissue, start to differentiate into myofibroblasts, which are responsible for maintaining the extracellular matrix structure by depositing collagen. Additionally, myofibroblasts paracrine signalling with surrounding myocytes will also affect ionic channels. Highly detailed computational models at different scales were used to study the effect of structural remodeling induced at the cellular and tissue levels.At the cellular level, a human fibroblast model was adapted to reproduce the myofibroblast electrophsyiology during atrial fibrillation. Additionally, the calcium handling in myofibroblast electrophysiology was assessed by fitting calcium ion channel to experimental data. At the tissue level, myofibroblasts infiltration was studied to quantify the increase of vulnerability to cardiac arrhythmia. Myofibroblasts alter the dynamics of reentry. A low density of myofibroblasts allows the propagation through the fibrotic area and creates focal activity exit points and wave breaks inside this area. Moreover, fibrosis composition plays a key role in the alteration of the propagation pattern. The alteration of the propagation pattern affects the electrograms computed at the surface of the tissue. Electrogram morphology was altered depending on the arrangement and composition of the fibrotic tissue. Detailed cardiac tissue models were combined with realistic models of the commercially available mapping catheters to understand the clinically recorded signals. A noise model from clinical signals was generated to reproduce the signal artifacts in the model. Electrograms from highly detailed bidomain models were used to train a machine learning algorithm to characterize the atrial fibrotic substrate. Features that quantify the complexity of the signals were extracted to identify fibrotic density and fibrotic transmurality. Subsequently, fibrosis maps were generated using patient recordings as a proof of concept. Fibrosis map provides information about the fibrotic substrate without using a single cut-off voltage value of 0.5 mV. Furthermore, in this study, using information theory measurements such as transfer entropy combined with directed graphs, the wave propagation direction was tracked. Transfer entropy with directed graphs provides crucial information during electrophysiology to understand wave propagation dynamics during atrial fibrillation. In conclusion, this thesis presents a multiscale in silico study atrial fibrillation mechanisms providing insight into the cellular mediators responsible for the extracellular matrix remodeling and its electrophysiology. Paleomagnetic and Thermomagnetic Studies on Rock Samples from COSC - 1 Drilling Project / Paleomagnetiska och termomagnetiska studier av stenprover från COSC - 1   djupborrningen

The COSC-1 borehole located in the Lower Seve nappe of the Central Scandinavian Caledonides is intended to study the tectonic evolution of the Paleozoic orogen. The drilling project reached a depth of 2495.8m and obtained samples from different lithologal units of mica schist, amphibolite, gneiss, and mylonitic gneisses.     To test if the samples are suitable for paleomagnetism, laboratory measurements of natural remanent magnetism are performed on sub-samples of the COSC-1 drill core in the paleomagnetic laboratory with alternating field demagnetization, IRM acquisition as well as the temperature dependence of susceptibility measurements. Important properties of the samples such as coercivity, Curie temperature, inclination, and declination are determined with the measurements.     Most of the samples carry a remanent magnetization that can be readily demagnetized with alternating field demagnetization up to 100 mT. Some of the samples may be suitable for paleomagnetism, with the major ferromagnetic mineral compositions of magnetite, hematite, and FeTi oxides. However, low coercivities and median destructive fields suggest that the magnetite-bearing samples will be prone to remagnetization, making them more challenging to use in terms of retrieving a primary magnetization originating from the Caledonian orogeny. The thermomagnetic results of some samples reflect the metamorphic conditions experienced by the rocks and the mineral assemblage changed irreversibly during step-wise heating experiments. These results can possibly be used as an indicator for the metamorphic temperature experienced by the different lithologal units. Although further measurements are needed to get a clearer understanding of the suitability of the COSC-1 sample for paleomagnetic reconstructions (including thermal demagnetization experiments), this thesis indicates that the COSC-1 rocks contain interesting paleo- and rock magnetic features that are worth further study.

Breaking Myths! : Unveiling the storytelling processes in the reception of Hilma af Klint from the 1980s and 2010s

This dissertation studies the critical reception of Hilma af Klint trough three exhibitions: Spiritual in Art – Abstract Painting 1890-1985 (Los Angeles, 1986), Secret Pictures by Hilma af Klint, (Helsinki, 1988) and Hilma af Klint – A Pioneer of Abstraction (Stockholm, 2013), addressing the crucial prominent figures and voices in the discursive field around af Klint. The aim is carried out through the Critical Discourse Analysis by Norman Fairclough, coupled with Pierre Bourdieu's concept of the cultural field and habitus. Judith Butler's theory of gender performance completes the theoretical framework by addressing issues of gender in both af Klint's practice as well as in the analysed critical reviews. The thesis examines how the discourses about af Klint changed during different periods of time in the 1980s and the 2010s. The central hypothesis is that this case study can be used as an example to see how the art field presents women artists as a marginal phenomenon instead of including them in the general art historical canon. But the reception of af Klint cannot be fully understood through the lens of gender; thus, e.g. the closeness to the occult is considered both as a leading mechanism in the artistic practice, creating interest towards af Klint, and as an identity which pushes her to the marginal. The results of this study lead to a better understanding of critical articles' role in the cultural phenomena's narrative creation.

In this essay, the children’s picture book Hilma af Klint: maps of invisibility by Ylva Hillström and Karin Eklund is analyzed with the aim of exploring the picture book as a medium and tool for art history writing. The analysis examines how the artist Hilma af Klint is portrayed and how art history is presented, as well as placing a particular focus on analyzing the interaction between image and text. The content is examined with semiotic image analysis, an inductive literature study as well as reception aesthetics analysis. The analysis concludes that Hilma af Klint as an artist is portrayed as fearless, curious and serious, and her artistic ambition is described as the desire to understand the universe. The design of the picture book and the ability to freely place and combine images and texts in different ways means that it is possible to convey the history of the art in several ways. With the picture book, sometimes several parallel stories are told; sometimes picture and text work together to complement each other, other tims the same thing is presented in both text and picture, or the pictures tell a story of their own or provide additional information that does not appear in the text. Depending on the experience of the viewer/reader, it is possible to derive different things from the picture book, and gaps can be filled as well as be left empty, which creates excitement and curiosity about the rest of the art world. Through the illustrations representation of af Klint's work and Eklund's way of illustrating the works the images in the book relate back to the children's/viewer's own world. The design of the picture book functions as a gateway to what it is like to look at art and art books and it works to create images within the viewer.

Nordmænd og svenskeres perception og forståelse af dansk udtale

Formålet med denne opgave er at undersøge nordmænd og svenskeres perception og forståelse af dansk udtale. Tidligere undersøgelser af nabosprogforståelsen i Skandinavien har nemlig vist at nordmænd præsterer bedre end svenskere når det kommer til at forstå talt dansk. Dette er blandt andet blevet forklaret med at der er større ligheder mellem det norske og det danske ordforråd. Spørgsmålet er om nordmændene også klarer sig bedre end svenskerne hvis de kun testes på ord der er samme på dansk, norsk og svensk. Dette undersøges gennem et eksperiment bestående af en ordforståelsestest hvor 171 norske og 147 svenske gymnasieelever lytter til danske ord som de skal oversætte til sit modersmål. I alt testes 30 ord som indeholder forskellige svækkede konsonanter, eller som af andre anledninger kan forventes at være svære at forstå. Ordene bliver testet isoleret, altså uden kontekst. Konklusionen er at nordmænd også forstår talt dansk bedre end svenskere når leksikale forskelle mellem sprogene er udlignede. Selvom der på overfladen er forskel på de norske og svenske informanters forståelse af talt dansk, kan der også ses en del ligheder i forhold til deres perception og forståelse af de enkelte ord.

L’étude présentée dans cette thèse relève de la mécanique des fluides expérimentale et numérique appliquée aux écoulements pariétaux de refroidissement de chambres de combustion aéronautiques. En présence de phénomènes thermo-acoustiques, comme les instabilités de combustion, il est important d’évaluer si les capacités de l’écoulement pariétal à protéger les parois de chambre restent suffisantes. C’est ainsi que nous nous sommes intéressés aux écoulements de paroi multiperforée soumis à une excitation acoustique. Dans ce but, le banc d’essais MAVERIC a été amélioré grâce à l’installation d’un système qui permet de forcer acoustiquement l’écoulement transverse dans lequel les jets pariétaux débouchent. Nous avons pu alors mettre en évidence la forte sensibilité de ce type d’écoulements à l’excitation acoustique. Le bon accord entre les résultats expérimentaux et les simulations numériques aux grandes échelles (LES) effectuées est très encourageant dans le cas d’un forçage par onde stationnaire. Le forçage par onde progressive, étudié uniquement par simulations numériques, s’est révélé être capable de modifier significativement la topologie de l’écoulement. Enfin, à partir de l’outil numérique AVBP-AVTP qui permet le couplage de calculs fluide-solide, nous avons réalisé une étude de l’influence de la présence d’une excitation acoustique sur le comportement thermique de l’écoulement autour d’une paroi multiperforée de chambre de combustion. / This experimental and numerical study in the field of fluid mechanics deals with jets-in cross flow configurations that are relevant for the cooling of aero engine combustion chambers. Indeed, in presence of instabilities it is important to determine to which extent the film cooling is able to do its job of preserving the combustion chamber walls from the thermal load. The test facility MAVERIC has been upgraded in order to acoustically force the crossflow in which the jets are discharging. The strong sensitivity of the overall flow unsteady properties to the presence of the acoustic forcing has been clearly evidenced. The agreement between the experimental results and large-eddy simulations proved to be quite encouraging for a stationary acoustic wave whereas the case of a propagating acoustic wave investigated only numerically reveals also quite a significant change of the flow topology. In this context, the effect of the acoustic forcing on the wall thermal behavior has been analyzed thanks to the use of the fluid-solid coupled AVBP-AVTP solver.

La qualité de vie et la capacité fonctionnelle chez les patients atteints de fibrillation auriculaire et d'insuffisance cardiaque congestive

De déterminer si une stratégie de contrôle du rythme améliore la qualité de vie et / ou la capacité fonctionnelle par rapport à une stratégie de contrôle de la fréquence cardiaque chez les patients atteints de fibrillation auriculaire et d'insuffisance cardiaque congestive. Méthode: Pour évaluer la qualité de vie, le questionnaire SF-36 a été administré à l'inclusion et à 4 mois chez 749 patients de l’étude AF-CHF. Les paramètres de capacité fonctionnelle évalués ont été la classe fonctionnelle NYHA (1376 patients) et la distance de marche de six minutes (1099 patients). Résultats: Le type du traitement assigné n'a pas eu un impact significatif sur la qualité de vie ou la capacité fonctionnelle. Conclusion: La qualité de vie et la capacité fonctionnelle sont similaires chez les patients randomisés au contrôle du rythme par rapport au contrôle de la fréquence. Les hommes non-obèses avec moins de comorbidités semblent plus susceptibles de s'améliorer. / To determine if a rhythm control strategy improves quality of life and/or functional capacity compared to a rate control strategy in patients with atrial fibrillation and congestive heart failure. Methods: To assess QoL, the Medical Outcomes Short Form-36 (SF-36) was administered to 749 patients included in the AF-CHF study at baseline and at 4 months. Functional capacity was assessed by NYHA class determined at baseline, 3 weeks, 4 months, and at 4-month intervals thereafter in 1376 patients and by 6 minutes walk test conducted at baseline, 3 weeks, 4 months, 1 year, and annually thereafter in 1099 patients. Results: The type of the assigned treatment had no significant impact on quality of life scores nor on functional capacity. Conclusion: Quality of life and functional capacity improved to a similar extent in patients randomised to rhythm versus rate-control strategies. Non-obese male patients with less comorbidities seem more likely to improve.

Nonparametric estimation of the off-pulse interval(s) of a pulsar light curve / Willem Daniël Schutte

The main objective of this thesis is the development of a nonparametric sequential estimation technique for the off-pulse interval(s) of a source function originating from a pulsar. It is important to identify the off-pulse interval of each pulsar accurately, since the properties of the off-pulse emissions are further researched by astrophysicists in an attempt to detect potential emissions from the associated pulsar wind nebula (PWN). The identification technique currently used in the literature is subjective in nature, since it is based on the visual inspection of the histogram estimate of the pulsar light curve. The developed nonparametric estimation technique is not only objective in nature, but also accurate in the estimation of the off-pulse interval of a pulsar, as evident from the simulation study and the application of the developed technique to observed pulsar data. The first two chapters of this thesis are devoted to a literature study that provides background information on the pulsar environment and -ray astronomy, together with an explanation of the on-pulse and off-pulse interval of a pulsar and the importance thereof for the present study. This is followed by a discussion on some fundamental circular statistical ideas, as well as an overview of kernel density estimation techniques. These two statistical topics are then united in order to illustrate kernel density estimation techniques applied to circular data, since this concept is the starting point of the developed nonparametric sequential estimation technique. Once the basic theoretical background of the pulsar environment and circular kernel density estimation has been established, the new sequential off-pulse interval estimator is formulated. The estimation technique will be referred to as `SOPIE'. A number of tuning parameters form part of SOPIE, and therefore the performed simulation study not only serves as an evaluation of the performance of SOPIE, but also as a mechanism to establish which tuning parameter configurations consistently perform better than some other configurations. In conclusion, the optimal parameter configurations are utilised in the application of SOPIE to pulsar data. For several pulsars, the sequential off-pulse interval estimators are compared to the off-pulse intervals published in research papers, which were identified with the subjective \eye-ball" technique. It is found that the sequential off-pulse interval estimators are closely related to the off-pulse intervals identified with subjective visual inspection, with the benefit that the estimated intervals are objectively obtained with a nonparametric estimation technique. / PhD (Statistics), North-West University, Potchefstroom Campus, 2014
