Os parâmetros estudados foram o controle da dor nos períodos de 1, 2 e 3 dias após a instituição do tratamento da alveolite diagnosticada. A terapêutica constou de anestesia local, cuidadosa limpeza cirúrgica e irrigação intra-alveolar com solução fisiológica, seguida da aplicação da pasta preenchendo todo o alvéolo. Com base nos resultados foi possível concluir que o emprego tópico da pasta proporcionou redução da dor e não proporcionou reações adversas locais e/ou sistêmicas ao material empregado fatos que indicam o seu uso como um medicamento eficaz sendo uma opção segura para o tratamento de feridas de extrações acometidas pela alveolite e dos desconfortos causados por esta patologia. / It is proposal of this work to evaluate the properties of the ointment compounded by 10% metronidazole and 2% lidocaine in dental alveolus attacks for dry sockets, in 25 patients attended in discipline of Integrated Clinic and Surgery and Buco-Maxillo-Facial Traumatology of the Dentistry College of Araçatuba – Unesp. The studied parameter had been the control of pain, in the periods of 1, 2 and 3 days after the instituion of the treatment of the installed osteitis. The therapeutics were local anesthesia, careful surgery cleanness and alveolar irrigation with physiological solution, followed of the application of the ointment filling al the alveolus. Based on the results, it was possible to conclude that the topic use of ointment, produced positive results that recommend its use as an effective medice being a safe option on the treatment of wound extractions attacks harmed by dry socket and the discomforts caused for this pathology.

Development of Biomimetic Human Lung Alveolus Chip

The potential of physiologically relevant in vitro cell culture models for studying physiological and pathophysiological phenomena has been widely recognized as replacements for animal and conventional in vitro models. To create models that accurately replicate the structure and function of tissues and organs, it is essential to comprehend the biophysical and mechanical features of the extracellular matrix (ECM) and incorporate them into the in vitro cell culture models. Therefore, we first aimed to investigate how nanotopography can modulate cell behaviors by studying cell behaviors on nanostructures of various aspect ratios on a cobalt-chromium-molybdenum (CoCrMo) alloy surface. We also explored the impact of nanofibrous membranes on the formation of alveolar epithelium, which is critical for lung alveolar interstitium chips. In addition, we investigated the effect of mechanical stretch on cell behaviors and focused on how the dimensionality of the stretch affects cell behaviors. To create physiologically relevant in vitro models based on our findings, we engineered a stem cell niche using a combination of nanofibrous membranes, mechanical stretch, and a soft substrate, and evaluated its impact on stem cell behaviors. Finally, we created a biomimetic human lung interstitium chip for application in physiological and pathophysiological in vitro studies.

Reconstruction of the alveolar process in cleft patients

Background. The treatment of patients born with cleft lip and palate has been gradually modified over the years as the surgical procedures have developed and improved. Multidisciplinary team care has evolved and provided improved care with enhanced results. Clefts in the alveolus can be reconstructed by alveolar bone grafting or by periosteoplasty. The main goal is to repair and close the alveolar cleft and create a continuous alveolar processes so that the teeth can erupt. Aims. This thesis has several aims: to investigate the impact of dental status and initial cleft width on the outcome of Secondary alveolar bone grafting (SABG) in patients born with unilateral cleft lip and palate (UCLP) at the 10-year follow-up (Studies I and II); to compare the outcomes of primary periosteoplasty (PPP) with those of SABG in patients born with unilateral cleft lip and alveolus (CLA) (Study III); to evaluate clinical and radiographic conditions and identify factors important for the final treatment outcomes after SABG ( Study IV); to evaluate two radiographic methods, i.e. occlusal radiographs and cone beam tomography (CBCT)) for assessing alveolar bone height ( study IV). Results. In UCLP patients, SABG achieved excellent results in terms of bone height; tended to reduce with time, correlated with dental status and dental restoration factors. Occlusal radiographs correspond well with the CBCT, for evaluating alveolar bone height in cleft area. The width of the initial cleft does not seem to affect the success of SABG. Finally, patients with CLA treated with PPP at the time of lip repair have inferior bone formation outcomes in the cleft area compared with patients treated with SABG at the time of mixed dentition. Conclusion.  Poor dental status and malpositioning negatively affect the long-term survival of bone in the alveolar cleft. The initial cleft width affects certain dental status factors. In adults with UCLP, the alveolar bone height in the cleft was correlated to the presence of gingival inflammation and restorations at 20 years follow-up. Specially designed maintenance therapy is beneficial, after complex dental restorations in the cleft area. SABG is preferred to PPP for the reconstruction of alveolar clefts.

Einfluss einer vorhergehenden Influenza A Virus Infektion auf die angeborene Immunität gegenüber der sekundären Pneumokokkenpneumonie in humanem Lungengewebe

Sekundäre bakterielle Infektionen im Verlauf oder in Folge einer Infektion mit Influenza A Viren (IAV) steigern oftmals die Schwere des Krankheitsverlaufes, was besonders während der IAV Pandemien von 1918, 1968 und 2009 deutlich wurde. Genaue mechanistische Ursachen, welche dieser gesteigerten Kopathogenität zugrunde liegen wurden überwiegend in Tierversuchsmodellen adressiert und sind immunologisch unvollständig. Aufgrund organstruktureller und immunfunktioneller Speziesunterschiede ist ungewiss, inwieweit eine Übertragbarkeit der Daten zwischen Mensch und Maus besteht. Fokus der Arbeit bildete die Analyse potentieller IAV assoziierter Änderungen der angeborenen Immunität, welche sekundäre Pneumokokkeninfektionen in humanem ex vivo Lungengewebe begünstigen. Dafür wurden zentrale Zyto - bzw. Chemokine als Reaktion auf Einzelinfektionen mit dem saisonalen IAV Pan/99(H3N2) sowie Streptococcus pneumoniae D39 mit denen subsequenter viral-bakteriellen Koinfektion verglichen. Ausgelöst durch die antivirale Interferonantwort erfolgte die Reduktion der pneumokokkeninduzierten Bildung von IL-1β und GM-CSF auf translationaler und transkriptioneller Ebene. Vermutlich beeinflussen Typ I und II Interferone die IL-1β Bildung, welches über parakrine Wechselwirkungen an der GM-CSF Regulation beteiligt ist. Auf zellulärer Ebene verursachte IAV die Freisetzung von Typ I, II und III Interferonen aus primären humanen Alveolarepithelzellen vom Typ II. In humanen Alveolarmakrophagen unterdrückten Typ I und II Interferone die pneumokokkeninduzierte IL-1β Freisetzung. Folglich unterblieb die IL-1β-regulierte GM-CSF Sekretion aus Alveolarepithelzellen vom Typ II. Die Ergebnisse zeigen, dass influenzainduzierte Interferone durch die Unterdrückung der IL-1β regulierten Bildung von GM-CSF in humanem Lungengewebe beitragen. Damit unterstützt sie das Verständnis immunologischer Faktoren, welche diesem Krankheitsbild im Menschen pathophysiologisch zugrunde liegen können. / Secondary bacterial infections, which occur during or following an IAV infection, exaggerate the severity of the course of disease up to a lethal outcome, clearly recognizable during the fatal IAV pandemics from 1918, 1968 or 2009. Particular mechanisms underlying this exaggerated viral-bacterial copathogenity were almost solely addressed using animal models and are immunologically incomplete. Due to structural and immunofunctional interspecies differences the transferability of data between human and mice remains indeterminate. The study mainly purposed to investigate IAV associated modulations of innate immunity, which potentially facilitates secondary pneumococcal pneumonia in primary human ex vivo lung tissue. Hence secretion of central cyto- and chemokines initiated by single infection with the seasonal IAV Pan/99(H3N2) or the bacterium Streptococcus pneumoniae D39 were compared to subsequent viral-bacterial coinfection. In context of an antiviral interferon response the pneumococcal induced translation and transcription of IL-1β and GM-CSF were reduced. Probably type I and type II interferons affect generation of IL-1β, which participates in the regulation of GM-CSF by paracrine interactions. On a cellular basis the infection of primary human alveolar epithelial cells type II (AECII) with IAV triggered the release of interferon type I, II and III. In human alveolar macrophages type I and II interferons suppressed the pneumococcal induced release of IL-1β. Consequently, the IL-1β regulated generation of GM-CSF in AECII was impeded. The present study indicates, that influenza related induction of interferons suppresses the IL-1β related release of GM-CSF in human lung tissue. Thereby it takes part in contributing to pathophysiological comprehension of immunological factors underlying this copathogenity.

Darbe nagrinėjamas kompleksinės pagalbos vaikams su įgimtais lūpos ir / ar gomurio nesuaugimais vertinimas tėvų požiūriu. Atlikta teorinė vaikų, turinčių įgimtą lūpos, alveolinės ataugos, gomurio nesuaugimus pagrindinių aspektų analizė. Tyrimo tikslas - ištirti tėvų požiūrį apie medicininės ir logopedinės pagalbos teikimą vaikams, turintiems įgimtų lūpos, alveolinės ataugos ir/ar gomurio nesuaugimų. Darbe aprašytos kompleksinės pagalbos vaikams su įgimtais lūpos ir / ar gomurio nesuaugimais strategijos, išnagrinėti logopedinės pagalbos ypatumai dirbant su tokiais vaikais. Panaudotas anketinės apklausos metodas, interviu ir atlikta statistinė duomenų analizė. Tyrime dalyvavo 105 tėvai, auginantys vaikus su įgimtais lūpos ir / ar gomurio nesuaugimais: 97 šeimos dalyvavo anketinėje apklausoje ir 8 – interviu. Svarbiausios empirinio tyrimo išvados: 1. Vaikams, turintiems lūpos, alveolinės ataugos ir gomurio nesuaugimų, reikalinga kompleksinė įvairių sričių specialistų pagalba. Tokiems vaikams pagalba teikiama iki 18 m. amžiaus. Visame šiame procese labai svarbu komandinis specialistų darbas ir aktyvus bendradarbiavimas su vaiko šeima. 2. Tėvų, auginančių vaikus su įgimtais lūpos, alveolinės ataugos ir/ar gomurio nesuaugimais, nuomonė apie teikiamą pagalbą jų vaikams yra skirtinga ir prieštaringa. There was carried out the theoretical analysis of the main aspects related to children with congenital cleft lip, alveolus and palate. Aim of the study – to research parents’ opinion about the treatment and logopedic help support for children with congenital cleft lip, alveolus and palate. In the work, the strategies of professional help for children with congenital cleft lip and / or palate are described, and the peculiarities of logopedic help in work with such children are researched. It was used the method of questionnaire, interview, and it was performed the analysis of statistical data. In the study, there were involved 105 parents having children with congenital cleft lip and / or palate: 97 questionnaire respondents and 8 interview participants. The main conclusions of the empirical study: 1. For children with congenital cleft lip and / or palate, the complex course for rehabilitation of functions is applied until the age of 18 years. In order to achieve good results, the teamwork of surgeon, orthodontist, speech therapist with other specialists in cooperation together with children parents is useful. 2. Computational Fluid Dynamics in a Terminal Alveolated Bronchiole Duct with Expanding Walls: Proof-of-Concept in OpenFOAM

Mathematical Biology has found recent success applying Computational Fluid Dynamics (CFD) to model airflow in the human lung. Detailed modeling of flow patterns in the alveoli, where the oxygen-carbon dioxide gas exchange occurs, has provided data that is useful in treating illnesses and designing drug-delivery systems. Unfortunately, many CFD software packages have high licensing fees that are out of reach for independent researchers. This thesis uses three open-source software packages, Gmsh, OpenFOAM, and ParaView, to design a mesh, create a simulation, and visualize the results of an idealized terminal alveolar sac model. This model successfully demonstrates that OpenFOAM can be used to model airflow in the acinar region of the lung under biologically relevant conditions.
