Compartimentation intracellulaire du métabolisme énergétique dans les différentes chambres cardiaques : quelle implication en rythmologie ? / Intracellular compartimentation in cardiac energetics in ventricles and atria : implications in cardiac electrical dysfunctions

Les pathologies cardiovasculaires sont responsables d’un grand nombre de décès dans le monde aujourd’hui, dont plus de la moitié trouve son origine dans des anomalies rythmiques. Les phénomènes de dépolarisation et de repolarisation rythmiques des cardiomyocytes, à l’origine de l’activité contractile rythmique du myocarde, mettent en jeu des processus cellulaires consommateurs d’énergie et les mitochondries,. Dans cette thèse, la régulation de la fonction mitochondriale a été étudiée lors de l’induction d’arythmies ventriculaires sur coeurs isolés perfusés de rats. Les résultats montrent que l'évolution de la consommation d’oxygène du myocarde est un paramètre important dans la réponse aux arythmies. L’inhibition des myosines ATPases par la blebbistatine induit sur la durée une diminution de la susceptibilité vis-à-vis des arythmies. Les acteurs impliqués dans les différents types de réponses mitochondriales ont été caractérisés sur fibres perméabilisées. Nous montrons que la régulation de la fonction mitochondriale varie en fonction de la chambre cardiaque considérée. Les mesures de Km de la respiration mitochondriale, ainsi que l’étude des principaux mécanismes de transfert énergétiques, mettent en évidence des différences marquées entre les différentes chambres cardiaques. .Les résultats de nos travaux suggèrent que le remodelage myocardique observé en réponse aux modifications de charge, et/ou lors du développement des pathologies du myocarde sont susceptibles de favoriser la constitution d’un substrat arythmogène, propre à chaque chambre, et impliquant des perturbations des mécanismes de transfert énergétique. / Cardiovascular disease is the first cause of mortality in the whole world. Half of this mortality is due to heart failure, a progressive deterioration of cardiac contraction, which can be caused by electrical dyssynchrony. Implication of heart energetics on susceptibility to arrhythmia is therefore of particular interest to better understood the overall effect as well as the relative importance of the potential partners on developed pathologies. In this thesis, energetic regulation of mitochondria was studied in perfused hearts paced to induce arrhythmia. Myocardial oxygen consumption was shown as a crucial parameter in the response to rhythm troubles. Inhibitions of ATPase myofibrils by blebbistatin or AK-phosphotransfer by Ap5a did not produce same responses of mitochondrial compartment. When inhibited by Blebb, most of hearts presented less ectopic beats highlighting an anti-arrhythmic profile in this case of myofibrils inhibition. Energetic parameters involved in mitochondrial responses were characterized in skinned fibres. Mitochondria regulation was different between ventricle and atria, with a less sensitivity of mitochondria in atria compared to ventricle. Mechanisms of energy transfer between supply and demand were different between ventricle and atria. The role of AK-phosphotransfer was described as a determining parameter for atria energy maintenance, whereas ventricle presented a functional efficacy of CK pathway to regulate adenine nucleotide turnover. As suggested by these results, the role of adenylic nucleotides channeling could be considered as a mechanism used to counteract altered cardiac energetics that potentially sustained electrical pathologies.

<p>The design, simulation, and analysis of a reference atrium using ComputationalFluid Dynamics (CFD) are presented. Atria geometries can be observed in manybuildings but their understanding from an energy perspective is not fully understood.Due to the many physical phenomena occurring within these atria, it is often difficult toassess the thermal comfort, energy consumption, and functionality of an atrium's design.The scale of an atrium’s structure coupled with dynamic physical phenomena creates acomplex problem to solve. One particular tool that is useful in solving for detailedenergy quantities is CFD. Validation studies have been conducted using previousexperimental atria data to ensure confidence in the predictions. These validation studieswere successful and also provided further insight on turbulence models, glazing systems,HVAC systems, thermal mass, and fluid flow and heat transfer behavior in atriageometries. A design for a reference atrium located in Toronto, Canada was thensimulated for typical summer and winter conditions using various configurations forglazing, solar heat flux, wall materials, occupant load, and HVAC. Effet de la galanine sur le système nerveux cardiaque intrinsèque.‎

Les neurones cholinergiques et adrénergiques efférents du système nerveux cardiaque ‎intrinsèque (SNCI) sont impliqués dans la régulation de la fonction cardiaque chronotrope. ‎La galanine, peptide présent et libéré dans les terminaisons nerveuses cardiaques, est capable ‎de diminuer la libération de l’acétylcholine. Notre hypothèse est d’étudier la possibilité que ‎la galanine puisse agir comme un peptide neuromodulateur inhibiteur dans les neurones du ‎SNCI chez le rat. Les oreillettes et les tissus associés du SNCI sont isolés à partir de rats ‎Sprague-Dawley et perfusés en vitro. Les valeurs de la fréquence auriculaire mesurées d’un ‎battement à l’autre sont tirées à partir d’un enregistrement bipolaire auriculaire. La perfusion ‎de la nicotine en forme de bolus (avec des concentrations dans le bain de 20-250 µm) a ‎induit des réponses biphasiques composées d’une réponse chronotrope négative au début ‎suivie d’une réponse chronotrope positive. Au cours de la superfusion de la galanine ‎‎(170 nM), les réponses chronotropes positives à la nicotine sont réduites chez 12/14 ‎préparations (contrôle : 21,35 ± 17,4 à galanine : 5,46 ± 8,0 bpm, n = 14, p = 0,02), tandis que ‎les réponses chronotropes négatives sont réduites chez 8 /14 préparations et affichent une ‎tendance générale vers la réduction (contrôle : -16,09 ± 14,7, galanine : -12,53 ± 13,2 bpm). ‎On peut conclure que la galanine agit comme un peptide neuromodulateur inhibiteur dans les ‎SNCI chez le rat, en modifiant de préférence la stimulation nicotinique de neurones ‎adrénergiques.‎ / Cholinergic and adrenergic efferent neurons of the intrinsic cardiac nervous system ‎‎(ICNS) regulate cardiac chronotropic function. Galanin, a peptide present and released in ‎cardiac nerve endings, is able to reduce the acetylcholine release. Our aim was to investigate ‎the proposition that galanin can act as an inhibitory neuromodulatory peptide in the rat ICNS. ‎Atria and associated ICNS tissues were isolated from Sprague-Dawley rats and superfused in ‎vitro. Beat to beat atrial rate measurements were derived from an atrial bipolar recording. ‎Nicotine bolus infusion (bath concentrations of 20-250 µM) induced biphasic responses ‎consisting of an early negative followed by a positive chronotropic response. During galanin ‎superfusion (170 nM), positive chronotropic responses to nicotine were reduced in 12/14 ‎preparations (control: 21.35±17.4 to galanin: 5.46±8.0 bpm, n=14, p=0.02) whereas the ‎negative chronotropic responses were reduced in 8/14 preparations and displayed an overall ‎tendency towards reduction (control: -16.09±14.7, galanin: -12.53±13.2 bpm). It is concluded ‎that galanin may act as an inhibitory neuromodulatory peptide in the rat ICNS, preferentially ‎modifying nicotinic stimulation of adrenergic neurons.‎

Modélisation numérique de l'activité électrique dans les oreillettes et les veines pulmonaires / Numerical modeling of the electrical activity of the atria and the pulmonary veins

Le travail présenté dans ce manuscrit s’articule en trois axes distincts. Dérivation de modèles mathématiques de phénomènes électrophysiologiques en cardiologie Nous utilisons des méthodes d'analyse asymptotique pour dériver un modèle simplifié à partir d'un modèle de tissu auriculaire tridimensionnel, tout en contrôlant l'erreur d'approximation. Ces méthodes ont permis de dériver un modèle bisurfacique qui permet de simuler des comportements tridimensionnels dans les oreillettes pour un coût numérique bidimensionnel afin d'étudier des phénomènes entrant en jeu lors d'arythmies auriculaires, tels que la dissociation électrique ou des hétérogénéités transmurales. La preuve de la convergence du modèle bisurfacique est apportée, et une stratégie d'optimisation du modèle en dehors du régime asymptotique est formalisée. Une méthode d’homogénéisation est également utilisée pour construire un modèle continu homogénéisé de l'activité des myocytes incluant le comportement non linéaire des gap junctions. Processus déclencheurs d'arythmie Des preuves de concepts de mécanismes arythmogènes sont apportées à l'aide de modèles numériques des veines pulmonaires. Le premier mécanisme repose sur un bloc de conduction unidirectionnel engendré par une discontinuité dans la structure fibreuse. Le second est basé sur une dynamique différente lors de la dépolarisation et de la repolarisation lorsque deux couches de fibres de directions différentes sont superposées. Perpétuation des arythmies auriculaires Un modèle bicouche des oreillettes est construit à partir d'une méthode semi-automatique de construction des fibres que nous avons développées. Nous étudions avec l'influence d'hétérogénéités transmurales de fibrose sur la perpétuation des arythmies. Plusieurs protocoles d'ablation sont ensuite testés. Enfin, une méthode de personnalisation du modèle auriculaire est formalisée. / Three axes are explored.Derivation of mathematical models of electrophysiological phenomena applied to cardiology Asymptotic analysis methods allow to derive simplified models from three-dimensional complex atrial ones, while controlling approximation errors. We construct a bilayer surface model that allows to simulate three-dimensional phenomena for a bi-dimensional computational load, and to investigate 3D atrial patterns involved in atrial arrhythmia such as electrical dissociation or transmural heterogeneities. We prove the convergence of the bilayer model, and an optimization strategy to improve the model outside the asymptotic regime is formalised. Homogeneisation methods are also used to construct a homogenized continuous model of the electrical activity of the myocytes that includes the non linear behavior of gap junctions. Triggers of atrial arrhythmia Proofs of concept of arrhythmogenic mechanisms are given by using numerical models of the pulmonary veins. The first mechanism is based on a unidirectional conduction block triggered by a discontinuity of the fibre distribution. The second one comes from a different propagation pattern during the depolarization and the repolarization when two layer of fibres are superimposed. Atrial arrhythmia perpetuation A bilayer model of the atria is constructed from a semi automatic method that we developed. We investigate the influence of transmural heterogeneities of the distribution of fibrosis on the perpetuation of atrial arrhythmia. Several ablation protocols are assessed. Finally, a method of personalization of the model is given.

Intenzivní dům v Brně / Intenzive Urban Housing Brno

The subject of this thesis is the architectural study of revitalization of a city block and its spatial structure. Selected part of city was solved already in previous project that preceded own thesis. Merger proposal will allow a larger number of different functions within the complex, which will ensure the residents to various forms of housing, services and employment and leisure activities. The studied area is located in the city of Brno-Veveří. Plan shape of the object based on the area in which the three parties connected to the access road. Mass and spatial design respects the surrounding buildings and uses the sidewalks along the whole of the area. The matrix of the object is divided by two axes passing through the object into three blocks linked by a courtyard. This creates a square with a water surface with the possibility of recreation and sitting under the trees. The shape of these three blocks is further formed in several height terraced layers. Do terraces are placed in the atrium and individual residential and office units have large windows and balcony available. This solution brings natural light into the interior while allowing illumination of the courtyard and surrounding objects within the chosen area. This spatial structure is horizontally divided by function. There are proposed underground public garage, arcade with shops, services, administration and housing. The exterior of the building is designed by combining brick facades and white plaster, which works like optically lighter impression of the building.

Dům krátké cesty v Brně / 5-Minutes Neighbourhood

The territory is located in the city of Brno, district Trnitá on the embankment of the river Svitava. The subject of the diploma thesis is the architectural study of the design of the urban spatial structure with intensive use of city urban area, designed in pre-diploma project, which preceded diploma thesis. The design will allow mixing of greater amount of different functions on a relatively small built-up area, which will reduce the requirements on transport and provide the inhabitant with various forms of housing, services and employment. The shape of object's ground plan is based on area, which is connected from three sides to access roads. The mass and spatial solution respects the surrounding buildings and uses existing bike trails and sidewalks in the southeastern part of the area. The basic matter of object is divided by three axes passing through the building, into three blocks linked by common internal block. This creates a square with a tree-lined atrium with seating under the trees. The shape of these three blocks is further formed by removing of the mass, which results into forming of atriums, terraces and balconies, which brings natural light into the interior while allowing illumination of the patio and surrounding buildings within the territory. This spatial structure is horizontally divided by functions. Designed here are underground public garages, passages with shops, services, administration / commerce and housing. Despite the diversity of functions, for the facade was chosen a unified character of white horizontal lines contrasting with the dark gray lines of windows and facade cladding.
