<p>Bakgrund: Antalet patienter i behov av njurtransplantation per år är cirka 400-500, varav endast cirka 350 kommer att bli transplanterade. Bristen på organ är tydligt framträdande. I väntan på transplantation behöver patienten dialysbehandling, vilket ofta upplevs vara tidskrävande och utmattande. Sjuksköterskor bör ha kunskap om patienters upplevelser i samband med väntan på transplantation för att kunna utvecklas i sin roll som sjuksköterska och få en djupare förståelse för deras situation.</p><p>Syfte: Syftet var att beskriva kroniskt njursjuka patienters upplevelser av att leva med dialysbehandling och väntan på transplantation.</p><p>Metod: Studien är en litteratursammanställning av tio kvalitativa och kvantitativa vetenskapliga artiklar inom området, vilka har granskats, analyserats och sammanställts.</p><p>Resultat: I resultatet framkom flera olika upplevelser, så som längtan efter frihet, utmattning, höga förväntningar, frustration och rädsla inför framtiden i samband med dialysbehandlingen och väntan på transplantationen.</p><p>Slutsats: Kroniskt njursjuka patienters upplever sin situation som psykiskt, fysiskt och socialt påfrestande. En av sjuksköterskans viktigaste uppgifter i samband med bemötandet av patienter under dialysbehandling är att vara tillgänglig. Med en tillgänglig och öppen inställning till patienterna blir sjuksköterskan mer mottaglig för patienternas individuella behov.</p> / <p>Background: The number in need of kidney transplants per year is approximately 400-500, of which only about 350 will be transplanted. Nurses should have knowledge of patients' experiences in connection with awaiting transplantation to be able to evolve in her role as a nurse and get a deeper understanding of the patients’ situation.</p><p>Aim: The aim of this study was to describe patients’ experiences of living with chronic kidney failure with dialysis and awaiting transplantation.</p><p>Method: The study is a literature compilation of ten qualitative and quantitative scientific articles in the field. The articles have been reviewed, analyzed and compiled. Results: The results revealed several different experiences, such as yearning for freedom, fatigue, high expectations, frustration and fear for the future in connection with the dialysis treatment and awaiting transplantation.</p><p>Conclusion: Chronic kidney disease patients perceive their situation as mentally, physically and socially stressful. One of the nurse's most important tasks in connection with the treatment of patients in dialysis is to be available. With an accessible and open approach to the patient in general, a nurse becomes more responsive to patient's individual needs.</p>

The sexual assault and rape of male offenders and awaiting-trial detainees

In this study the nature and extent of the sexual assault and rape of male sentenced offenders and awaiting-trial detainees in Pretoria Local Correctional Centre were investigated. Prison gangs, overcrowding of correctional centres, the involvement of correctional officials and the sexual orientation of the potential victim were highlighted as risk factors in the sexual assault and rape of male offenders and awaiting-trial detainees. As this study focused on both the victim and the offender of male-on-male sexual assault and rape, both victimological and criminological theories were used as a theoretical basis. Creswell’s dominant-less-dominant model of combination was used in this study, with the dominant model being the qualitative methodology, and the less-dominant model the quantitative methodology. Non-purposive sampling was used, as the researcher could not identify the victims and/or perpetrators of male-on-male sexual assault and rape individually. In order to obtain in-depth information on the research participants’ experiences of sexual activities and rape in the correctional centre, face-to-face structured interviews were conducted with one hundred research participants. After the analysis and interpretation of the data, it became evident that male-on-male sexual assault and rape, as well as consensual sexual activities, do occur in this correctional centre. Research participants highlighted four types of sexual engagements in this correctional centre, namely the need for emotional sex, survival sex, compliant sex and forced sexual acts. The reasons offered for the occurrence of rape in this correctional centre include that an agreement had not been reached between inmates and because of that the rape will take place, corruption by correctional officials, the involvement of prison gangs and the use of deception by the perpetrators. Six research participants revealed that they had been the victims of rape. All the victims were raped within days or weeks after their arrival at the correctional centre. Five of the victims did not receive medical treatment after the rape, and none of the victims received counselling or therapy after the rape. Five of the research participants reported that they had sexually assaulted and/or raped other inmates. Four of the perpetrators were awaiting trial for an aggressive offence (armed robbery) and one for a sexual offence (rape). The perpetrators forced their victims to engage in oral sex, inter-femoral sex and anal sex. The aims of the study were reached and recommendations for further research were also made. Emanating from the feedback of the participants as well as the literature review, the researcher developed an Offender Sexual Assault Protocol. Vårda patienter vid vårdplatsbrist : Sjuksköterskors upplevelser på en akutmottagning / Caring for patients when there is a lack of hospitalbeds : Nurses' experiences in an emergency department

Bakgrund: En akutmottagning har som mål att bedöma, vårda och stabilisera akut sjuka patienter. De patienter som behöver fortsatt vård kommer att läggas in på en vårdavdelning. Vårdplatsbristen leder till att patienter blir kvar på akutmottagningen. Detta leder till en hög arbetsbelastning och en ansträngd situation för sjuksköterskorna som arbetar på akutmottagningen. Syfte: Syftet är att undersöka sjuksköterskors upplevelser av att vårda inläggningsklara patienter som blir kvar på akutmottagningen vid vårdplatsbrist. Metod: Studien genomfördes med en kvalitativ metod med fenomenologisk ansats, där 10 sjuksköterskor intervjuades med öppna frågor. Datan analyserades med en innebördsanalys. Resultat: Resultatet av sjuksköterskors upplevelser av att vårda inläggningsklara patienter vid vårdplatsbrist kunde sammanställas i fem innebördsteman: Inläggningsklara patienter prioriteras bort, Brister i den praktiska omvårdnaden, Avsaknad av resurser till att bedriva avdelningsarbete på akutmottagningen, Att aldrig få ett avslut och En känsla av att inte ge god vård. Konklusion: Sjuksköterskorna upplever att de inläggningsklara patienterna oftast blir nedprioriterade. Vilket leder till att sjuksköterskorna inte möter de inläggningsklara patienternas vårdbehov som de hade önskat. De beskriver känslor som ångest, ilska, stress, irritation och ett dåligt samvete. / Background: An emergency department aims to care for, treat and stabilize acute ill patients. The patients who need continued care will be admitted to the hospital ward. The shortage of hospital beds leads to patients remaining in the emergency department. This leads to a high workload and a stressful situation for the nurses working in the emergency department. Aim: The purpose is to explore nurses' experiences of caring for patients awaiting admission who remain in the emergency department when there is a shortage of hospital beds. Method: The study was conducted using a qualitative method with a phenomenological approach. 10 nurses were interviewed with open-ended questions. The data were analyzed using a meaning analysis. Findings: The results of nurses’ experiencesof caring for patients awaiting admission who remain in the emergency department when there is a shortage of hospital beds could be compiled into five meaning themes: Patients awaiting admission are not prioritized, Shortcomings in practical nursing, Lack of resources to provide ward care in the emergency department, Never getting a closure and A feeling of not provide for good care. Conclusion: Nurses experience of the patients that are ready for hospital ward regularly getting down prioritized. Which leads to the nurses not meeting the standards for the patients and their need of care as the nurses would have wanted. They are describing feelings like anxiety, anger, stress, irritation and a guilty conscience.

Com base na abordagem da pesquisa qualitativa de cunho etnográfico, nosso trabalho procurou demonstrar que, devido à interação das mulheres entre si e delas com a equipe de saúde, as clientes parecem conviver melhor com seus conflitos biopsicossociais. Observamos que a efetividade dos tratamentos está também relacionada à escuta interessada das clientes com papilomavirose, estando sujeitas a agravos em conseqüência de práticas curativas e automatizadas, cotidianamente exercidas por profissionais do sistema público de saúde. Assim sendo, realizamos com base em Freire, o desenvolvimento de um processo socioeducativo que suscitou, como produto, a criação de uma sala de espera interativa no Setor de Ginecologia de um hospital universitário. O principal objetivo foi converter um espaço de espera em uma sala com a preocupação não só de acolhimento, com também de um constante processo de construção compartilhada de saberes. Ao mesmo tempo, práticas ligadas à promoção da saúde e à prevenção do câncer são desenvolvidas, conforme reivindica a atual Política Nacional de Humanização da Saúde. / The lack of scientific culture, and educational activities appropriate to increase it, seem to contribute to morbidity and mortality of cervical cancer by human papillomavirus (HPV) in women of low educational level. Based on the approach of qualitative ethnographic research, our study sought to demonstrate that, due to the interaction of women among themselves and with their health care team, clients seem to live better with their conflicts biopsicossociais. We found that the effectiveness of treatment is also related to hearing from interested customers with papilomavirose, and subject to damages as a result of healing practices, automated daily performed by professional public health system. Therefore, we based on Freire, the development of a process which led childcare as a product, creating a waiting room in the interactive industry of Gynecology of a university hospital. The main objective was to convert a room waiting in a room with a concern not only the host, with also a constant process of shared construction of knowledge. At the same time, practices related to health promotion and cancer prevention are developed, as claims the current National Policy of Humanization of Health.

The goal of the research is to determine if the Gestalt groupworkprogram have a influence on the self-image and general satisfaction of the awaiting-trail adolescent. To reach the above goal the researcher used applied research. Standardised measure-instruments as well as observation was used. Gestaltgroupwork can be implemented with great success in residential institutions like secure care facilities for children. / In hierdie studie is Gestalttegnieke binne 'n groepwerkprogram benut as intervensie, om die verhoorafwagtende jeugdige in aanhouding weer in kontak te bring met homself en om sy bewustheidskontinuum te vergroot. Die doel van die navorsing was om te bepaal of die Gestaltgroepwerkprogram 'n invloed sal he op die selfbeeld en algemene tevredenheid van die verhoorafwagtende adolessent. Om bogenoemde te bereik het die navorser toegepaste navorsing onderneem en het van sowel waarneming as gestandaardiseerde meetinstrumente gebruik gemaak. Mathematical modeling of TB disease dynamics in a crowded population.

Philosophiae Doctor - PhD / Tuberculosis is a bacterial infection which is a major cause of death worldwide. TB is a curable disease, however the bacterium can become resistant to the first line treatment against the disease. This leads to a disease called drug resistant TB that is difficult and expensive to treat. It is well-known that TB disease thrives in communities in overcrowded environments with poor ventilation, weak nutrition, inadequate or inaccessible medical care, etc, such as in some prisons or some refugee camps. In particular, the World Health Organization discovered that a number of prisoners come from socio-economic disadvantaged population where the burden of TB disease may be already high and access to medical care may be limited. In this dissertation we propose compartmental models of systems of differential equations to describe the population dynamics of TB disease under conditions of crowding. Such models can be used to make quantitative projections of TB prevalence and to measure the effect of interventions. Indeed we apply these models to specific regions and for specific purposes. The models are more widely applicable, however in this dissertation we calibrate and apply the models to prison populations.
