“Prevalencia de enteroparásitos en niños de 8 a 13 años de edad de la Institución Educativa N° 6041 “Alfonso Ugarte” del distrito de San Juan de Miraflores”

Las enteroparasitosis intestinales constituyen un problema de salud pública en Perú, debido a que estos parásitos pueden ingresar al organismo por vía oral y hábitos higiénico-sanitarios deficientes que facilitan su transmisión y conservación. Cuando la carga de dichos parásitos es considerablemente alta o se acompaña de alteraciones en la inmunidad del hospedero, se pueden producir complicaciones que comprometen seriamente la salud del paciente. Sabemos que el control farmacológico de las parasitosis es efectivo y seguro. No obstante, sin autocuidado y mantenimiento sostenible de buenas condiciones higiénico-sanitarias, no es posible su erradicación. Considerando lo mencionado anteriormente, desarrollaré la presente investigación en una población escolar de la Institución Educativa N° 6041 “Alfonso Ugarte” del distrito San Juan de Miraflores para determinar la prevalencia de parasitosis intestinales. Se eligió una muestra representativa conformada de 116 niños de 8 a 13 años. Las muestras fecales obtenidas fueron analizadas utilizando: examen macroscópico, método directo, método de Parodi Alcaraz y test de Graham. El 85.3% de los alumnos examinados resultaron parasitados. La incidencia parasitaria fue mayor en mujeres (86.8%) comparado a los hombres (83.6%). La frecuencia parasitaria de acuerdo al Monoparasitismo de los grupos taxonómicos fueron 35.3% del Phylum Amoebozoa, 3.4% del Phylum Metamonada, 3.4% del Phylum Platyhelminthes, 0.9% del Phylum Bigyra y 0.9% del Phylum Nematoda, con las especies Entamoeba coli, Giardia lamblia, Hymenolepis nana, Blastocystis hominis y Enterobius vermicularis, respectivamente. La mayor frecuencia correspondiente al Biparasitismo fue la asociación de los Phyla Metamonada y Amoebozoa con 32.8%. La mayor frecuencia correspondiente al Triparasitismo fue la asociación de los Phyla Metamonada, Amoebozoa y Platyhelminthes con 1.7%. The intestinal enteroparasites constitute a public health problem in Peru, due to these parasites can enter the body by mouth and hygienic habits-poor health that facilitate its transmission and conservation. When the burden of such parasites is considerably high or is accompanied by alterations in the immunity of the host, it can produce complications which seriously compromise the health of the patient. We know that the pharmacological control of the parasitosis is effective and safe. However, self-care and sustainable maintenance of good hygienic and sanitary conditions, it is not possible to its eradication. Considering the above, I will develop this research in a school population of the Educational Institution N° 6041 "Alfonso Ugarte" of the district San Juan de Miraflores to determine the prevalence of intestinal parasitism. I chosen a representative sample consisted of 116 children 8 to 13 years old. Stool samples obtained were analyzed using: macroscopic examination, direct method, Parodi Alcaraz’s method and Graham’s test. The 85.3% of the students examined were parasitized. Some organisms similar to B. hominis have been described in a range of non-human hosts and are denominated Blastocystis sp., but their zoonotic potential still needs clarification. The objective of this work was to investigate the occurrence of natural infection by Blastocystis sp. in three species of domesticated birds sold at cooperative markets (with individual vendor stalls) in two districts of the city of Rio de Janeiro and to characterize the morphology of these organisms found in bird droppings, to enable a precise diagnosis. A total of 214 birds were purchased: 35 ducks (Anas platyrhynchos) from market A and 38 from market B, 35 quails (Coturnix japonica) from market A and 36 from market B and 35 chicks (Gallus gallus) from market A and 35 from market B. Of the birds studied from market A, 8 chicks (22.9%), 15 quails (42.9%) and 13 ducks (37.1%) presented biological forms of Blastocystys sp. in their droppings, while at the market B, the respective figures were 15 chicks (42.9%), 2 quails (5.56%) and 21 ducks (55.3%). There was no statistically significant difference in relation to the occurrence of infection in ducks and chicks between the two markets studied, but there was between the quails at the two markets. Considering the number of infected birds at the two markets irrespective of species, the difference was statistically significant. From the observations of the stained fecal smears of the animals that were positive for Blastocystis sp., it was possible to characterize six forms of organisms, namely vacuolar, granular, ameboid, cystic, avacuolar and multivacuolar. It was also possible to characterize the reproductive forms of Blastocystis sp. as binary division, endodyogeny, budding, plamotomy and schizogeny. Even though there are no studies confirming the zoonotic and pathogenic potential of Blastocystis sp., either to human or animal hosts, we suggest that environmental contamination can be occurring at these markets, due to the poor hygienic and sanitary conditions, thus posing a risk to both the animals and the people who circulate there. The knowledge of evaluative forms that were found and characterized in this work is important to gives a precise and faithful diagnosis of Blastocystis sp. in dropping. / Blastocystis hominis ? um dos parasitos de maior preval?ncia no trato gastrintestinal de humanos, por?m seu papel patog?nico ainda permanece incerto. Alguns organismos semelhantes ? B. hominis t?m sido descritos em uma variedade de hospedeiros n?o-humanos sendo denominado Blastocystis sp., mas seu potencial zoon?tico n?o est? esclarecido. O objetivo desse trabalho foi verificar a ocorr?ncia de infec??o natural de Blastocystis sp. em tr?s esp?cies de aves dom?sticas comercializadas em dois mercados municipais do Rio de Janeiro, caracterizando morfologicamente esses organismos encontrados em material fecal para a emiss?o de um diagn?stico preciso. Foram adquiridas 214 aves, sendo 35 patos (Anas platyrhynchos) do mercado A e 38 do mercado B, 35 codornas (Coturnix japonica) do mercado A e 36 do mercado B, e 35 pintos (Gallus gallus)do mercado A e 35 do mercado B. Das aves pesquisadas no mercado A, 8 (22,9%) pintos, 15 (42,9%) codornas e 13 (37,1%) patos apresentaram formas biol?gicas de Blastocystis sp. nas fezes, enquanto que no mercado B, 15 (42,9%) pintos, 2 (5,56%) codornas e 21 (55,3%) patos apresentaram tais formas nas fezes. N?o houve uma diferen?a estatisticamente significativa em rela??o ? ocorr?ncia de infec??o em patos e pintos dos dois mercados municipais pesquisados, ocorrendo essa diferen?a somente entre as codornas dos dois mercados. Ao considerarmos o n?mero de aves infectadas nos dois mercados, independentemente da esp?cie, observou-se que n?o houve uma diferen?a estatisticamente significativa entre os dois mercados. Das observa??es de esfrega?o de fezes coradas com Giemsa daqueles animais que apresentaram positividade para Blastocystis sp., foi poss?vel caracterizar seis formas do organismo, sendo estas: vacuolar, granular, ameb?ide, c?sticas, avacuolar e multivacuolar. Tamb?m foi poss?vel caracterizar as formas reprodutivas de Blastocystis sp. como: divis?o bin?ria, endodiogenia, brotamento, plamatomia e esquizogonia. Introducción. El estado nutricional de los individuos es considerado un valioso indicador del estado de salud de la población, así como también del accionar de factores socio-económicos y ambientales. Objetivos: Analizar la relación entre el estado nutricional, las enteroparasitosis y las condiciones socioambientales en niños de Aristóbulo del Valle, Misiones. Métodos. Estudio antropométrico transversal y parasitológico en niños de ambos sexos y entre 6 a 11 años de escuelas públicas del Municipio de Aristóbulo del Valle. Para el análisis de desnutrición se utilizó como referencia CDC/NCHS y para exceso de peso IOTF. El análisis parasitológico se realizó mediante la toma de muestras seriadas de materia fecal y escobillado anal. El relevamiento de las condiciones socio-ambientales se realizó mediante encuestas estructuradas. Resultados. El estado nutricional indicó mayor desnutrición crónica (7,5%) que global y aguda y mayor sobrepeso (9,8%) respecto a obesidad. Los niños presentaron alto porcentaje de parasitismo (86%) y las especies más prevalentes fueron Blastocystis hominis, Enterobius vermicularis y Giardia lamblia. La mayoría de las familias de estos niños habitaban viviendas de chapa y madera, con limitado acceso a servicios públicos, altos niveles de desempleo y padres con un nivel educativo básico. / Introduction: The study of the growth and nutritional status of the individuals is considered an important sign of the population health, as well as the action of socio-economic and environmental factors. Objectives: To analyze the relationship among the nutritional status, enteroparasitoses and socio-environmental conditions in children from Aristóbulo del Valle, Misiones. Methods: A cross-sectional anthropometrical and parasitological study was made in children from both sexes between 6 to 11 years old of public school from Municipality of Aristóbulo del Valle. NCHS was employed as reference in the analysis of undernutrition, and IOTF for the excess of weight. The parasitological analysis was realized by faecal samples and anal brushes. Socio-environmental conditions were evaluated through structured interviews. Results: The nutritional status indicated higher chronic undernutrition (7.5%) than underweight and wasting, and higher overweight than obesity (9.8%). Children showed high percentage of parasitism (86%) and Blastocystis hominis, Enterobius vermicularis and Giardia lamblia were the more prevalent species. Most families of these children live in precarious constructions made out of wood and/or masonry, with limited access to public services, high unemployment levels and a basic educational level of parents. Conclusions: The results obtained indicate the impact of the socio-economic, educational and sanitary impairment, in the children health from a sector Aristóbulo del Valle population, that entails to the coexistence of undernutrition, overweight and parasitic infections.

Genetic subtypes in unicellular intestinal parasites with special focus on Blastocystis

The development of molecular tools for detection and typing of unicellular intestinal parasites has revealed genetic diversities in species that were previously considered as distinct entities. Of great importance is the genetic distinction found between the pathogenic Entamoeba histolytica and the non-pathogenic Entamoeba dispar, two morphologically indistinguishable species. Blastocystis sp. is a ubiquitous intestinal parasite with unsettled pathogenicity. Molecular studies of Blastocystis sp. have identified 17 genetic subtypes, named ST1-17. Genetically, these subtypes could be considered as different species, but it is largely unknown what phenotypic or pathogenic differences exist between them. This thesis explores molecular methods for detection and genetic subtyping of unicellular intestinal parasites, with special focus on Blastocystis. We found that PCR-based methods were highly sensitive for detection of unicellular intestinal parasites, but could be partially or completely inhibited by substances present in faeces. A sample transport medium containing guanidinium thiocyanate was shown to limit the occurrence of PCR inhibition. The prevalence of Blastocystis in Swedish university students was over 40%, which is markedly higher than what was previously estimated. Blastocystis ST3 and ST4 were the two most commonly found Blastocystis subtypes in Sweden, which is similar to results from other European countries. Blastocystis sp. and Giardia intestinalis were both commonly detected in Zanzibar, Tanzania, each with a prevalence exceeding 50%. Blastocystis ST1, ST2, and ST3 were common, but ST4 was absent. While G. intestinalis was most common in the ages 2-5 years, the prevalence of Blastocystis increased with increasing age, at least up to young adulthood. We found no statistical association between diarrhoea and Blastocystis sp., specific Blastocystis subtype or G. intestinalis. Metagenomic sequencing of faecal samples from Swedes revealed that Blastocystis was associated with high intestinal bacterial genus richness, possibly signifying gastrointestinal health. Blastocystis was also positively associated with the bacterial genera Sporolactobacillus and Candidatus Carsonella, and negatively associated with the genus Bacteroides. Blastocystis ST4 was shown to have limited intra-subtype genetic diversity and limited geographic spread. ST4 was also found to be the major driver behind the positive association between Blastocystis and bacterial genus richness and the negative association with Bacteroides.
