Avaliação da acurácia da ressonância magnética no diagnóstico das lesões traumáticas do plexo braquial / Evaluation of magnetic resonance imaging accuracy in the diagnosis of traumatic brachial plexus injuries

A lesão do plexo braquial é considerada a alteração neural mais grave das extremidades. A principal causa é o trauma de alta energia, especialmente acidentes envolvendo veículos a motor. Por este motivo, as lesões traumáticas do plexo braquial são cada vez mais frequentes. O presente estudo avaliou a acurácia da ressonância magnética (RM) no diagnóstico das lesões traumáticas do plexo braquial no adulto, utilizando o achado intraoperatório como padrão-ouro. Também foi avaliada a acurácia da neurografia pesada em difusão (neurografia DW) em relação à RM convencional e a capacidade de diferenciação dos três tipos de lesão: avulsão, ruptura e lesão em continuidade. Trinta e três pacientes com história e diagnóstico clínico de lesão traumática do plexo braquial foram prospectivamente estudados por RM. Os achados obtidos pela RM sem e com o uso da neurografia DW, e os achados de exame clínico foram comparados com os achados intraoperatórios. A análise estatística foi feita com associação de significância de 5%. Observou-se alta correlação entre a RM com neurografia DW e a cirurgia (rs=0,79), e baixa correlação entre a RM convencional e a cirurgia (rs=0,41). A correlação interobservador foi maior para a RM com neurografia DW (rs = 0,94) do que para a RM sem neurografia DW (rs = 0,75). Os resultados de sensibilidade, acurácia e valor preditivo positivo foram acima de 95% para as RM com e sem neurografia DW no estudo de todo o plexo. As especificidades foram, em geral, maiores para a neurografia DW (p < 0,05). Em relação à diferenciação dos tipos de lesão, a RM com neurografia DW apresentou altas acurácias e sensibilidades no diagnóstico da avulsão/rotura, e alta especificidade no diagnóstico da lesão em continuidade. A acurácia da RM (93,9%) foi significativamente maior que a do exame clínico (76,5%) no diagnóstico das lesões de todo o plexo braquial (p < 0,05). / Brachial plexus injury is considered the most severe neural disorder in the extremities and in general resulting from high-energy trauma in young patients, usually involving motor vehicles. For this reason, traumatic brachial plexus injuries are becoming more frequent. This study evaluated the accuracy of magnetic resonance imaging (MRI) in the diagnosis of traumatic brachial plexus injuries in adults, using surgical findings as the gold standard method. We also evaluated the accuracy of diffusion weighted image neurography (DW neurography) compared to conventional MRI and the ability to differentiate the three types of injuries by MRI: avulsion, rupture and lesion-in-continuity. Thirty-three patients with clinical history and diagnosis of traumatic brachial plexus injury were prospectively studied by MRI. Impact d'un repas méditerranéen complet sur les fonctions métaboliques et endothéliales postprandiales en comparaison à un repas riche en acides gras saturés chez des hommes sains.

Contexte L’américanisation des habitudes de vie, notamment de l’alimentation, semble être en grande partie responsable de l’augmentation incessante de l’incidence élevée des maladies cardiovasculaires. La période postprandiale, où l’homéostasie vasculaire et métabolique est mise à l’épreuve, est d’une importance considérable dans le développement ou la prévention de l’athérosclérose et représente maintenant la majeure partie du temps d’éveil. La compréhension de l’influence de la composition d’un repas sur son impact postprandial est donc essentielle dans notre lutte dans la prévention de ces maladies. Objectifs L’objectif principal de ce projet de recherche était d’étudier les réponses endothéliale et métabolique à un repas de type méditerranéen mixte (MMM), puisqu’elles sont inconnues. Ce projet avait aussi pour objectifs d’évaluer l’impact microvasculaire d’un tel repas et de caractériser la composition postprandiale des acides gras plasmatiques. À titre comparatif, ces éléments ont aussi été étudiés suite à un repas riche en gras saturés (HSFAM). Méthodes Vingt-huit (28) hommes sains, exempts de facteurs de risque de maladies cardiovasculaires ont reçu de façon randomisée les deux repas à l’étude. Le MMM, composé de saumon frais et de légumes cuits dans l'huile d'olive, contenait 7.87g de SFA et 2.29g d’acides gras polyinsaturés oméga-3, tandis que le HSFAM, composé d'un sandwich déjeuner avec œuf, saucisse et fromage, contenait 14.78g de SFA. Les mesures de la fonction endothéliale mesurée par échographie brachiale (FMD), de la fonction microvasculaire mesurée par spectroscopie proche de l’infrarouge (NIRS) et de la composition des acides gras plasmatique ont été effectuées à jeun et en période postprandiale. Résultats Deux sous-groupes de répondeurs aux repas à l’étude se sont dégagés de ces travaux. Un premier sous-groupe de sujets ayant une triglycéridémie à jeun élevée, mais normale (hnTG) a démontré des altérations endothéliales seulement suivant le repas HSFAM. Un second sous-groupe de sujets ayant une triglycéridémie plus faible et normale (lnTG) n’a quant à lui pas subi d’altérations endothéliales suivant les deux repas à l’étude. Les sujets hnTG ont aussi démontré une charge triglycéridémique postprandiale (iAUC) plus importante et qui était de surcroît enrichie en acide stéarique suivant la HSFAM et en acide gras polyinsaturés oméga-3 suivant le MMM. L’évaluation par NIRS de la fonction microvasculaire nous révèle un ralentissement de la réoxygénation post-ischémique qui suggère une altération postprandiale du recrutement capillaire chez les sujets hnTG seulement. De telles altérations, qui semblent être plus importantes suivant le HSFAM, pourraient être en partie responsables de l’impact endothélial de ce repas. Conclusions Cet essai clinique démontre donc de façon novatrice qu’un repas MMM n’a pas d’effet endothélial délétère et que cette neutralité est indépendante de la triglycéridémie à jeun. De plus, une triglycéridémie à jeun élevée mais normale semble indiquer des dysfonctions endothéliales et métaboliques à des épreuves nutritionnelles tel un repas HSFAM. La consommation de repas méditerranéens par des individus sains à la triglycéridémie marginale serait bénéfique, peut-être même davantage que pour des individus de triglycéridémie plus faible, dans la prévention de l’athérogénèse et des maladies cardiovasculaires. / Background The Westernization of lifestyles, notably dietary habits, seems to be largely responsible for the ongoing increase of incidence of cardiovascular diseases. The postprandial period, where vascular and metabolic homeostatic regulating processes are under pressure, is of considerable importance in the development or prevention of atherosclerosis and now represents the majority of waking hours. The understanding of the postprandial effects of meals of varying composition is therefore essential in our effort to prevent such diseases. Objectives The main objective of this research project was to study the endothelial and metabolic responses to a mixed Mediterranean meal (MMM), since they are unknown. This project also aimed to evaluate the microvascular impact of such a meal and to characterize the composition of postprandial plasma fatty acids. These elements were also evaluated following a saturated fatty meal (HSFAM) for comparison. Methods Twenty-eight (28) healthy men free of cardiovascular diseases and risk factors were randomly assigned one of two test meals. The MMM, consisting of fresh salmon and vegetables cooked in olive oil, contained 7.87g of SFA and 2.29g of omega- 3 fatty acids, while the HSFAM, consisting of a breakfast sandwich with egg, sausage and cheese contained 14.78g of SFA. Evaluations of endothelial function by brachial ultrasound (FMD), of microvascular function by near-infrared spectroscopy (NIRS) and of plasma fatty acids composition were done in the fasted and postprandial states. Results Two subgroups of responders to the test meals emerged from this work. A first subgroup of subjects with high-normal fasting triglyceridemia (hnTG) experienced impairments of endothelial function following the HSFAM while a second subgroup of subjects with low-normal triglyceridemia (lnTG) did not experience any endothelial alterations following this meal. Interestingly, the MMM had no deleterious endothelial impact in any of those subgroups. Moreover, the hnTG subjects demonstrated greater postprandial triglyceridemic load (iAUC) that was in addition enriched with stearic acid after HSFAM and omega-3 PUFA following the MMM. Assessment of microvascular function revealed postprandial prolongation of post-ischemic reoxygenation only in hnTG subjects suggestive of alterations of capillary recruitment. These changes, which seemed to be more important after the HSFAM, could be partly responsible for its negative impact on FMD. Conclusions This clinical trial demonstrates in innovative ways that a MMM has no deleterious effects on endothelial function irrespective of triglyceridemia in normal ranges. In addition, high but normal fasting triglyceride levels suggest metabolic and endothelial dysfunctions following nutritional challenges such as a HSFAM. Consumption of Mediterranean meals by healthy individuals with marginal triglyceride levels would be beneficial, perhaps even more so than for individuals with lower triglyceride levels, in preventing atherogenesis and cardiovascular diseases.

Associação dos achados morfofuncionais cardíacos, renais e vasculares com as alterações do índice tornozelo-braço em pacientes hipertensos diabéticos / Association of cardiac, renal and vascular morphological and functional findings with changes in ankle brachial index in diabetic hypertensive patients

Introdução: Inúmeros estudos estabeleceram correlações entre o índice tornozelo-braço (ITB), um marcador de aterosclerose subclínica, e o prognóstico cardiovascular em diferentes populações. No entanto, poucos estudos avaliaram a correlação entre os valores do ITB e lesões cardiovasculares e renais, exclusivamente, em pacientes com hipertensão arterial e diabetes. Objetivo: Estudar a prevalência de alterações morfofuncionais cardíacas, carotídeas, retinianas e renais de acordo com a presença ou não de valores de ITB alterados (ITB <= 0,9 ou ITB > 1,4) em pacientes hipertensos com diabetes tipo 2. Métodos: Foram incluídos no estudo 99 pacientes hipertensos diabéticos com idade entre 50 e 80 anos. A aferição do ITB foi realizada em todos os pacientes por método validado e estes foram classificados em Grupo 1 (ITB normal, n = 49) ou Grupo 2 (ITB alterado, n =50). Todos os pacientes foram submetidos, em até 06 meses, à realização de ecodopplercardiograma, ultrassonografia de carótidas, retinografia colorida, aferição da taxa de filtração glomerular (TFG) e da albuminúria de 24h. Os pacientes foram analisados para a ocorrência ou não de um desfecho-composto ecocardiográfico que incluiu alterações morfológicas e funcionais cardíacas relevantes para a prática clínica. Os pacientes dos grupos 1 e 2 foram também comparados quanto à prevalência de placas carotídeas com ou sem repercussão hemodinâmica, TFG < 60 ml/mim/m2, albuminúria de 24h > 30mg e presença ou não de retinopatia. Por fim, foram comparadas as frequências médias das seguintes lesões de órgãos-alvo de ambos os grupos, considerando-se valor unitário para a presença de cada uma delas: hipertrofia do ventrículo esquerdo, retinopatia hipertensiva, TFG < 60 ml/min/m2 e estenose da artéria carótida interna > 50% do seu diâmetro. Resultados: A média de idade dos pacientes foi 65,4 ± 7 anos, sendo 61,6% deles do sexo feminino. A presença de níveis elevados de pressão arterial sistólica (153,4 ± 18 versus 170 ± 26 mmHg), de albuminúria de 24h > 30mg (55,3% versus 82,6%) e de TFG < 60 ml/min/m2 (12,8% versus 33,3%) foi significativamente maior (p < 0.05) entre os pacientes do Grupo 2. O desfecho-composto ecocardiográfico foi mais prevalente no grupo 2 (84,0% versus 59,2%; p = 0,006) e a frequência média de lesões de órgãos-alvo também foi maior nos pacientes do grupo 2 (0,36 ± 0,31 versus 0,19 ± 0,19; p = 0,001). Análise por regressão logística binária revelou que o ITB foi uma das variáveis preditoras independentes para o desfecho-composto ecocardiográfico (OR = 3,43; IC 95% = 1,07 - 11,0; p = 0,04). A partir da análise por regressão linear obteve-se um modelo final no qual o ITB foi uma das três variáveis preditoras independentes para a estimativa da frequência média de lesões de órgãos-alvo com coeficiente beta = 13,22 (1,81 - 24,63), ao lado da idade e do infarto prévio. Conclusão: Nossos dados mostram que valores de ITB alterados estão associados à maior prevalência de lesões em órgãos-alvo, principalmente alterações ecocardiográficas, em pacientes com hipertensão arterial e diabetes / Introduction: A lot of studies have established strong correlations between the ankle-brachial index (ABI), a marker of subclinical atherosclerosis and cardiovascular prognosis in different populations. However, few studies have assessed the correlation between the values of the ABI and cardiovascular and renal lesions in patients with hypertension and diabetes. Objective: To study the prevalence of cardiac, carotid, renal and retinal morphological and functional changes according to the presence or not of altered ABI values (ABI <= 0.9 or ABI > 1.4) in hypertensive patients with type 2 diabetes. Methods: It was included 99 diabetic hypertensive patients aged between 50 and 80 years. The measurement of the ABI was performed in all patients by validated method and they were classified in Group 1 (normal ABI, n = 49) or group 2 (altered ABI, n = 50). All patients were submitted, up to 6 months, to Doppler echocardiography, carotid ultrasound, color retinography, assessment of glomerular filtration rate (GFR) and 24h albuminuria. Patients were analyzed for the occurrence or not of a composite echocardiographic outcome which included morphological and functional cardiac alterations relevant to clinical practice. Patients in groups 1 and 2 were compared regarding the prevalence of carotid plaques with or without hemodynamic repercussion, TFG < 60 ml/min/m2, 24h albuminuria > 30 mg and the presence or not of retinopathy. Finally, we compared the prevalence of mean frequency of the following end-organ lesions of both groups, considering unit value for each one: left ventricular hypertrophy, hypertensive retinopathy, TFG < 60 ml/min/m2 and internal carotid artery stenosis > 50%. Results: The mean age of the patients was 65.4 ± 7 years, with 61.6% of them female. The presences of elevated levels of systolic blood pressure (153.4 ± 18 versus 170.0 ± 26 mmHg), of 24h albuminuria > 30 mg (55.3% versus 82.6%) and TFG < 60 ml/min/m2 (12.8% vs. 33.3%) were significantly greater (p < 0.05) among the patients of Group 2. The composite echocardiographic outcome was more prevalent in Group 2 (84.0% versus 59.2%, p = 0.006) and the average frequency of subclinical injury of target organs was also greater in patients of Group 2 (0.36 ± 0.31 versus 0.19 ± 0.19; p = 0.001). Binary logistic regression analysis revealed that the ABI was one of the independent predictors of composite echocardiographic outcome (OR = 3.43; IC 95% = 1.07 - 11.0; p = 0.04). From the linear regression analysis it was obtained a final model in which the ABI was one of three independent predictors for the estimation of the average frequency of end-organ damage with ? coefficient = 13.22 (1.81-24.63), besides age and previous myocardial infarction. Conclusion: Our data demonstrates that changed ABI values are associated with higher prevalence of subclinical end-organ lesions, principally changes in echocardiographic parameters, in patients with hypertension and diabetes
