O acelerado processo de envelhecimento tem trazido não só mudanças no perfil demográfico, mas também no perfil epidemiológico da população. Entre os problemas de saúde encontrados em idosos, merecem destaque os transtornos mentais, visto que acometem cerca de um terço dessa população, sendo os transtornos depressivos e a demência os problemas de saúde mental mais prevalentes. O transtorno depressivo maior (TDM), bem como o comprometimento cognitivo leve (CCL), tem sido associado prejuízo das funções cognitivas, além de aumentar o risco de desenvolvimento de demência, principalmente doença de Alzheimer (DA). Sendo assim, é importante que haja identificação preventiva de pessoas com maior risco para o desenvolvimento de DA a fim de proporcionar um tratamento precoce. Dessa maneira este estudo objetivou comparar os marcadores biológicos envolvidos na cascata amiloide, expressos em leucócitos e líquor, nas condições clínicas TDM, CCL, DA e controles saudáveis. Para tanto, foi determinada a expressão proteica das secretases ADAM10, BACE1 e PSEN1 e do peptídeo A? em leucócitos e líquor pelos métodos de Western Blotting, ELISA e Luminex. Ao analisarmos a expressão das secretases e do peptídeo Abeta em leucócitos não observamos diferenças estatisticamente significantes entre os grupos, exceto pela expressão da proteína PSEN1, que apresentou menores níveis em DA em relação à TDM. Em relação ao líquor, observamos uma significativa diminuição do peptídeo A? no grupo DA quando comparado aos grupos CCL, TDM e também a controles saudáveis; porém não foram observadas diferenças de expressão desse peptídeo entre CCL, TDM e controles saudáveis. Esses resultados sugerem que a matriz leucocitária, apesar de expressar os componentes desta via, sugerindo dispor da maquinaria enzimática necessária para o processamento da proteína precursora do amiloide (APP), não é a ideal para estudos sobre a cascata amiloide. Além disso, os resultados obtidos em amostras de líquor, de acordo com a casuística analisada, reforçam os dados da literatura que estabelecem a redução da concentração do peptídio Abeta no líquor como um biomarcador da DA, sem termos detectado alterações nos demais grupos estudados. Nossos resultados (negativos) em relação à expressão das APP-secretases no líquor acrescentam dados a este domínio ainda controverso da literatura. Assim sendo, mais estudos devem ser realizados para que possamos compreender melhor as vias relacionadas ao desenvolvimento da DA / The accelerated aging process has brought not only changes in the demographic profile, but also in the epidemiological profile of the population. Among the health problems found in the elderly, mental disorders are worth mentioning, since they affect about a third of this population, being the depressive disorders and dementia the most prevalent mental health problems. Major depressive disorder (MDD), as well as mild cognitive impairment (MCI), have been associated with higher levels of cognitive symptoms and increased risk of developing dementia, especially Alzheimer\'s disease (AD). Therefore, it is important to identify individuals at greater risk for the development of AD in order to provide treatment at early stages of the disease process. In this way, this study aimed to compare the biological markers involved in the amyloid cascade under the clinical conditions TDM, CCL, DA and healthy controls expressed in leukocytes and CSF. We determined the protein expression of ADAM10, BACE1 and PSEN1 and A? peptides in leucocytes and CSF using the Western Blotting, ELISA and Luminex methods. When analyzing the expression of the secretases and the A? peptide in leukocytes, we did not observe statistically significant differences between the groups, except for the expression of the PSEN1 protein, which presented lower levels in AD in relation to MDD. In relation to CSF, we observed a significant reduction of the Abeta peptide in AD when compared to MCI, TDM and also to healthy controls, but no differences in expression of this peptide were observed between MCI, MDD and healthy controls. These results suggest that the leukocytes, although expressing the key components and the enzymatic machinery necessary for the proteolytic processing of the amyloid precursor protein (APP), may not represent an adequate biological matrix for studies addessing the amyloid cascade. In addition, results obtained in CSF samples, according to the analyzed series, reinforce the literature data that establish the reduction of A? peptide concentration in CSF as a biomarker of AD, without detecting alterations in the other groups studied. Our (negative) results in relation to the expression of APP-secretases in CDescriptors: MSF add data to this still controversial domain of the literature. Therefore, more studies should be done so that we can better understand the pathways related to the development of AD

Efeitos do treinamento aeróbio na cognição e no metabolismo cerebral em repouso em sujeitos com comprometimento cognitivo leve / Effects of aerobic training on cognition and resting-state metabolism in subjects with mild cognitive impairment

Introdução: Apesar de ser uma intervenção potencialmente promissora para melhorar a cognição em sujeitos com comprometimento cognitivo leve (CCL), os efeitos do treinamento aeróbio (TA) no metabolismo regional de glicose (MRG) no cérebro ainda são pouco conhecidos. Objetivo: Avaliar os efeitos do TA na cognição e no MRG cerebral em sujeitos com CCL, usando tomografia por emissão de pósitrons com 18F - fluordesoxiglicose (PET-18FDG). Métodos: Sessenta e cinco sujeitos com CCL realizaram um programa de TA de intensidade moderada, duas vezes por semana, durante 24 semanas. Uma avaliação cognitiva com a escala Alzheimer\'s Disease Assessment Scale - Cognitive Subscale (ADAS-Cog) e outra do metabolismo cerebral em repouso com o PET-18FDG foram realizadas antes e depois da intervenção. A capacidade aeróbia foi comparada utilizando o consumo máximo de oxigênio (VO2 máx.) medido em mL/kg/min. Os dados de PET-18FDG foram analisados voxel por voxel com o programa SPM8 (limiar: p < 0,001 não corrigido, tamanho de agrupamento >= 50 voxels). Uma técnica de modelação de equações estruturais foi usada para criar um modelo para explorar as mudanças no MRG cerebral em repouso depois do programa de TA. Resultados: Comparações com testes t pareados revelaram melhora nos valores da escala ADAS-Cog (diferença média: -2,4 (3,5), p < 0,001) e no VO2 máx. (diferença média: 1,4 (2,3) mL/kg/min, p < 0,001). A análise metabólica do cérebro constatou uma redução bilateral do MRG em ambos os polos temporais anteriores, no giro frontal inferior esquerdo, no giro do cíngulo anterior esquerdo, no hipocampo direito, no giro frontal médio esquerdo e nos núcleos caudados bilaterais. Em contraste, houve um aumento do MRG no precuneus direito e no giro frontal inferior esquerdo. Um modelo de confirmação de fatores foi usado para criar variáveis latentes que refletem a rede de saliência (RS) e a rede de modo padrão (default mode network (DMN)) com base em regiões com mudanças no MRG encontradas depois do TA. Uma análise de caminhos foi utilizada para testar a hipótese de que o TA poderia induzir mudanças (diminuição) do MRG em regiões da RS. Essa redução pode refletir um aumento da eficiência da rede e, consequentemente, um melhor controle sobre outras redes. Em última instância, essa mudança induziria um aumento do MRG no precuneus, um importante nó da DMN. Os valores de adequação do modelo atingiram significância estatística, demonstrando que esse modelo explica a variância dos dados medidos. Conclusão: O TA melhorou a cognição e alterou o MRG em regiões da DMN e da rede de saliência em sujeitos com CCL / Background: Despite being a potentially promising intervention to improve cognition in subjects with mild cognitive impairment (MCI), the effects of aerobic training (AT) on regional brain glucose metabolism (rBGM) are not yet entirely understood. Objective: To evaluate the effects of AT on cognition and rBGM in subjects with MCI using 18FDG-PET. Methods: Sixty-five subjects with MCI performed a twice-a-week, moderate intensity, AT program for 24 weeks. Assessment with the ADAS-Cog (Alzheimer\'s Disease Assessment Scale - Cognitive Subscale) and evaluation of rBGM with 18FDG-PET were performed before and after the intervention. Aerobic capacity was compared using the relative maximal oxygen consumption (VO2 max) measured in mL/Kg/min. 18FDG-PET data were analyzed on a voxel-by-voxel basis with SPM8 (threshold: uncorrected p < 0.001, cluster size >= 50 voxels). Structural equation modeling was used to create a model to explore the changes of resting-state rBGM after the AT program. Results: Comparisons using paired t-tests revealed improvements in the ADAS-Cog (mean difference: -2.4 (3.5), p < 0.001) and VO2 max scores (mean difference: 1.4 (2.3) mL/kg/min, p < 0.001). Brain metabolic analysis revealed a bilateral reduction of rBGM in both anterior temporal poles, left inferior frontal gyrus, left anterior cingulate cortex, right hippocampus, left middle frontal gyrus and bilateral caudate nuclei. In contrast, there was an increase in rBGM in the right precuneus and left inferior frontal gyrus. Confirmatory factor analysis was used to create latent variables reflecting the salience network (SN) and default mode network (DMN) based on regions with rBGM changes found after the AT. A path model analysis was used to test the hypothesis that AT would induce rBGM changes (decrease) in regions of the SN. This decrement may reflect improved efficacy and consequently, better control over other networks. Ultimately, these changes would induce an increment in rBGM in the precuneus, an important node of the DMN. Values of model fit reached statistical significance, demonstrating that this model explain the variance of the measured data. Conclusion: AT improved cognition and changed rBGM measured with 18FDG-PET in nodes od the DMN and SN in subjects with MCI

The side-effects of chemotherapy treatment are an increasingly important research focus as more cancer patients are reaching survivorship. While treatment allows for survival, it can also lead to problems which can significantly affect quality of life. Cognitive impairments after chemotherapy treatment are one such factor. First presented as anecdotal patient reports, over the last decade empirical evidence for this cognitive concern has been obtained. Much attention has been focused on post-chemotherapy research, yet little attention has been granted to these same patients’ cognition before treatment commences. Breast cancer (BC) patients face many obstacles before chemotherapy treatment such as: surgery and side-effects of anesthesia, increased cytokine activity, stress of a new disease diagnosis and upcoming challenges, and emotional burdens such as depression and anxiety. Many of these factors have independently been shown to affect cognitive abilities in both healthy populations as well as other patient groups. Therefore, the pre-treatment (or baseline) BC patient status warrants systematic study. This would then reduce mistakenly attributing carried-over cognitive deficits to side effects of chemotherapy. As well, it is possible that certain confounding variables may have neural manifestations at baseline that could be exacerbated by chemotherapy agents. The following thesis first presents a review paper which critically describes the current literature examining chemotherapy-related cognitive impairments (CRCIs), as well as possible confound variables affecting this population. Subsequently, three original research papers present pre-chemotherapy data showing significant neuroanatomical and neurofunctional differences in BC patients compared to controls. In particular, these neural differences are present in brain regions that have been reported in post-chemotherapy papers. This, as well as the effects of variables such as the number of days since surgery, depression and anxiety scores and more, support the initiative that research attention should increase focus on these patients at baseline in order to better understand their post-chemotherapy results.

Reconnaissance mnésique dans le vieillissement normal, le trouble cognitif léger et la maladie d’Alzheimer : impact du matériel et caractérisation des processus impliqués

Résumé: L’objectif général de la thèse était de caractériser les déficits de reconnaissance mnésique dans la maladie d’Alzheimer et le trouble cognitif léger. La thèse comprend trois articles. Le premier article propose une recension des écrits portant sur les déficits cognitifs dans le trouble cognitif léger, alors que les deux articles suivants rapportent les résultats d’études expérimentales portant sur la reconnaissance. Le but de la première étude empirique était d’évaluer l’impact du type de matériel sur la reconnaissance à long terme et la reconnaissance à court terme dans la maladie d’Alzheimer en comparant l’atteinte pour des stimuli verbaux et musicaux. Nos analyses de groupe ont révélé que les atteintes des personnes avec maladie d’Alzheimer s’étendaient à toutes les épreuves et que les déficits étaient d’une ampleur comparable en reconnaissance musicale et verbale. Les analyses corrélationnelles appuient, bien que partiellement, une certaine spécificité d’atteintes par domaine, particulièrement en reconnaissance à long terme, mais suggèrent également que les deux domaines puissent partager certains mécanismes. L’objectif de la seconde étude était de caractériser les processus utilisés en reconnaissance dans le vieillissement normal et le trouble cognitif léger en fonction de la nouveauté et du type de matériel. L’étude évaluait la recollection et la familiarité à l’aide de la méthode remember/know. Les tâches étaient composées d’items connus et d’items nouveaux faisant partie du domaine verbal ou du domaine musical. Les résultats ont révélé que la recollection était atteinte dans le vieillissement normal et le trouble cognitif léger, mais uniquement pour la reconnaissance de stimuli connus, ce qui est compatible avec le fait que les deux groupes ont de la difficulté à encoder l’information de façon élaborée. D’autre part, la familiarité était compromise dans le vieillissement normal, sans impact additionnel du trouble cognitif léger, et seulement pour la reconnaissance de stimuli inconnus. Cette atteinte peut être associée aux difficultés des aînés dans les tâches d’amorçage perceptif impliquant des items inconnus. Les résultats découlant de ces études s’avèrent pertinents dans une perspective clinique, en plus de pouvoir contribuer à certaines questions d’ordre théorique. / Abstract: The main objective of this thesis was to characterize recognition memory deficits in Alzheimer’s disease and mild cognitive impairment. The thesis includes three articles. The first paper proposes a literature review on the cognitive deficits observed in mild cognitive impairment, whereas the following articles report results from experimental studies on memory recognition. The goal of the first empirical study was to assess the impact of the type of material on long-term recognition and short-term recognition in Alzheimer’s disease comparing deficits in the verbal and musical domains. Results indicated that musical long-term recognition and short-term recognition were impaired in Alzheimer’s disease. Furthermore, musical deficits were of the same magnitude as those found in the verbal domain. A positive correlation was however observed between musical and verbal short-term recognition, whereas it was not the case in long-term recognition. These data thus partially support the presence of domain-specific impairments, particularly in long-term recognition, but also suggest that musical and verbal memory could share some mechanisms. The goal of the second study was to evaluate the impact of the novelty and the type of material on recognition processes in normal aging and mild cognitive impairment. The study assessed two processes that contribute to recognition memory: recollection and familiarity. These processes were measured using the remember/know procedure. Recognition tasks were composed of well-known stimuli and novel stimuli in the verbal domain and in the musical domain. Results revealed that recollection was impaired by normal aging and mild cognitive impairment, but only for the recognition of well-known items. This is compatible with the fact that both groups have difficulty encoding information in an elaborate manner. In turn, familiarity was impaired by normal aging, with no additional impact of mild cognitive impairment, and only for the recognition of novel items. This deficit could be associated with impaired perceptual priming effects for novel stimuli that are reported in normal aging. The results reported in this thesis are relevant from a clinical perspective, and could also contribute to theoretical issues.

Working memory in Alzheimer’s disease and mild cognitive impairment (MCI) : assessment and intervention

L’objectif principal de cette thèse est d’examiner et d’intervenir auprès des déficits de la mémoire de travail (MdeT) à l’intérieur de deux populations cliniques : la maladie d’Alzheimer (MA) et le trouble cognitif léger (TCL). La thèse se compose de trois articles empiriques. Le but de la première expérimentation était d’examiner les déficits de MdeT dans le vieillissement normal, le TCL et la MA à l’aide de deux versions de l’empan complexe : l’empan de phrases et l’empan arithmétique. De plus, l’effet de «l’oubli» (forgetting) a été mesuré en manipulant la longueur de l’intervalle de rétention. Les résultats aux tâches d’empan complexe indiquent que la MdeT est déficitaire chez les individus atteints de TCL et encore plus chez les gens ayant la MA. Les données recueillies supportent également le rôle de l’oubli à l’intérieur de la MdeT. L’augmentation de l’intervalle de rétention exacerbait le déficit dans la MA et permettait de prédire un pronostic négatif dans le TCL. L’objectif de la deuxième étude était d’examiner la faisabilité d’un programme d’entraînement cognitif à l’ordinateur pour la composante de contrôle attentionnel à l’intérieur de la MdeT. Cette étude a été réalisée auprès de personnes âgées saines et de personnes âgées avec TCL. Les données de cette expérimentation ont révélé des effets positifs de l’entraînement pour les deux groupes de personnes. Toutefois, l’absence d’un groupe contrôle a limité l’interprétation des résultats. Sur la base de ces données, la troisième expérimentation visait à implémenter une étude randomisée à double-insu avec groupe contrôle d’un entraînement du contrôle attentionnel chez des personnes TCL avec atteinte exécutive. Ce protocole impliquait un paradigme de double-tâche composé d’une tâche de détection visuelle et d’une tâche de jugement alpha-arithmétique. Alors que le groupe contrôle pratiquait simplement la double-tâche sur six périodes d’une heure chacune, le groupe expérimental recevait un entraînement de type priorité variable dans lequel les participants devaient gérer leur contrôle attentionnel en variant la proportion de ressources attentionnelles allouée à chaque tâche. Les résultats montrent un effet significatif de l’intervention sur une des deux tâches impliquées (précision à la tâche de détection visuelle) ainsi qu’une tendance au transfert à une autre tâche d’attention divisée, mais peu d’effets de généralisation à d’autres tâches d’attention. En résumé, les données originales rapportées dans la présente thèse démontrent un déficit de la MdeT dans les maladies neurodégénératives liées à l’âge, avec un gradient entre le TCL et la MA. Elles suggèrent également une préservation de la plasticité des capacités attentionnelles chez les personnes à risque de développer une démence. / The principal aim of this dissertation is to investigate and intervene upon working memory (WM) impairments in two clinical populations: Alzheimer’s disease (AD) and mild cognitive impairment (MCI). The dissertation is comprised of three empirical articles. The goal of the first study was to examine WM impairments in normal aging, MCI and AD using two versions of the complex span task: sentence span and operation span. In addition, the effect of forgetting was assessed by manipulating the length of the retention interval. Results indicate impaired WM on complex spans in MCI and, to a greater extent, in AD. Data also support a role for forgetting within WM. Increasing the retention interval augmented deficit in persons with AD, and showed potential in predicting a negative prognosis in those with MCI. The objective of the second article was to investigate the feasibility of a computerized cognitive training paradigm for the attentional control component of WM in healthy older individuals and those with MCI. Data from this experiment revealed positive effects of the intervention for both groups. However, the absence of a control group limited interpretation of results. Based on those data, the third article aimed to implement a double-blind randomized controlled study of training of attentional control in MCI with executive deficits. This involved a dual-task paradigm comprised of a visual detection task and a visual alpha-arithmetic judgment task. While the control group performed simple dual-task practice over six one-hour sessions, the experimental group received variable-priority training, in which participants managed their attentional control by varying the proportion of attentional resources allocated to each task. Results show a significant effect of intervention on one of the two tasks involved (accuracy on the visual detection task) and a trend for transfer to another task of divided attention, but little generalization to other tasks of attention. In summary, the original data reported in this thesis demonstrates WM impairment in age-related disorders, with a gradient between MCI and AD, and suggest preserved plasticity of attentional capacities in persons at risk of developing dementia.

Examen de la mémoire épisodique dans le trouble cognitif léger : effet modérateur des comorbidités vasculaires

Le fardeau vasculaire (présence de maladies vasculaires et/ou cérébrovasculaires) est associé à une augmentation des troubles cognitifs chez les personnes âgées, ainsi qu’à un plus haut risque de démence vasculaire (DV) et de démence de type Alzheimer (DTA). Un nombre restreint de travaux a porté sur l’impact du fardeau vasculaire sur la cognition des personnes avec trouble cognitif léger (TCL). Pourtant, les personnes avec TCL représentent une population d’intérêt puisqu’elles sont à haut risque d’évoluer vers une démence. Cette thèse comprend trois articles de revue qui visent à exposer les connaissances entourant la santé vasculaire et la cognition des personnes âgées et trois articles empiriques (Chapitres 5, 6, 7). La première étude empirique traite de l’impact du fardeau vasculaire sur la cognition des personnes TCL et a comme objectif de spécifier quelles fonctions cognitives sont affectées par le fardeau vasculaire et dans quelle mesure le fardeau vasculaire influence l’hétérogénéité des TCL (Chapitre 5). Dans le cadre de la deuxième étude, nous avons examiné l’intégrité des processus stratégiques et non stratégiques de mémorisation des TCL avec et sans fardeau vasculaire, afin d’évaluer si ces processus diffèrent quantitativement et qualitativement entre ces deux groupes (Chapitre 6). Enfin, dans la troisième étude nous avons évalué les capacités d’association (binding) ainsi que la résistance à l’interférence des personnes TCL, les liens entre ces processus mnésiques et différents marqueurs cérébraux en plus des facteurs permettant de prédire l’évolution vers une démence (Chapitre 7). Les résultats présentés dans cette thèse appuient l’hypothèse selon laquelle le fardeau vasculaire influence le profil cognitif des TCL. Dans un premier temps, nous avons montré que le fardeau vasculaire est associé à une atteinte exécutive plus importante chez les TCL (Chapitre 5). De plus, nos résultats suggèrent que le fardeau vasculaire influence la classification clinique de ces derniers, puisque ceux dont le fardeau est élevé répondent davantage aux critères de TCL amnestique domaine multiple (trouble de mémoire plus au moins un autre déficit cognitif) tandis que ceux sans fardeau répondent davantage aux critères de TCL amnestique domaine unique (trouble isolé de la mémoire). Dans un deuxième temps, nous avons montré des différences dans la nature des processus mnésiques atteints chez les TCL avec et sans fardeau vasculaire (Chapitre 6). Alors que les premiers présentent une atteinte prédominante des processus stratégiques de mémorisation, les seconds présentent une atteinte des processus stratégiques et non stratégiques de mémorisation. Lorsque seuls les résultats des TCL ayant évolué vers une démence sont analysés, le patron d’atteinte est similaire puisque les TCL vasculaires sont quantitativement moins touchés que les TCL non-vasculaires au niveau des processus non-stratégiques de mémorisation. Enfin, tant les TCL qui progressent vers une démence que les TCL qui restent stables après un suivi de trois ans éprouvent tous des difficultés de mémoire associative et sont sensibles à l’interférence proactive (Chapitre 7). De plus, le fardeau vasculaire est associé à la sensibilité à l’interférence alors que le volume hippocampique est associé aux difficultés de mémoire associative. Enfin, nos résultats indiquent que les TCL qui éprouvent des difficultés de mémoire associative sont plus à risque d’évoluer vers une démence que les TCL ne présentant pas ces mêmes difficultés. De façon globale, les résultats de cette thèse révèlent que le fardeau vasculaire joue un rôle important dans l’hétérogénéité des TCL. / Vascular burden (presence of vascular diseases and/or cerebrovascular diseases) increase cognitive deficits in older adults and have been associated with vascular dementia and Alzheimer’s diseases. However, only a few studies have examined the impact of vascular burden on cognitive functioning in persons with mild cognitive impairment (herein referred to as MCIs). Individuals with MCI are a target population for research since they are at high risk of developing dementia. Understanding the factors that influence MCIs cognition is thus a priority. This thesis aims to identify the impact of vascular burden on MCI’s cognition. First, a summary of the literature concerning vascular health and cognitive functioning in the elderly is presented here (Chapters 1, 2, 3 and 4). Then, three studies that represent the core of this thesis are exposed. The first one aims to identify which cognitive functions are affected by vascular burden in MCIs (Chapter 5). In the second one, we assess strategic and non strategic memory processes in MCI with and without vascular burden (Chapter 6). Finally, in the last study, we assess binding and sensitivity to proactive interference in MCIs who progress to dementia, and MCIs who remain stable in a three-year follow-up (Chapter 7). This latter study also assesses which brain changes influence binding and interference capacity in MCIs and which factors predict progression to dementia. Results of this thesis first revealed that vascular burden plays an important role in cognitive heterogeneity of MCIs. First, MCIs who present a high vascular burden have more executive deficits. Second, those same MCIs run a greater risk of being clinically classified as amnestic multiple domain MCIs (memory impairment plus impairment in at least one cognitive domain, whereas MCIs with no vascular burden run a greater risk of being classified as amnestic single domain MCIs (isolated memory impairment) (Chapter 7). In the second article of this thesis, we showed that MCIs with vascular burden have memory impairment restricted to strategic memory processes. When only the results of MCIs who progressed to dementia were analysed, a similar pattern of memory impairment was found, since MCIs with vascular burden were less impaired in non strategic than strategic processes, whereas both processes were impaired in MCIs with no vascular burden (Chapter 8). Finally, MCIs who progressed to dementia, and those who remained stable after a three-year follow-up, showed binding difficulties and vulnerability to proactive interference. This difference was particularly evident when only MCI progressors were compared to healthy controls. In summary, vascular burden plays a role in the cognitive heterogeneity of MCI.
