Establishing evidence-based training of cognitive behavioural therapy treatments : empirical and theoretical guidance for dissemination

The need to provide effective Cognitive Behavioural Therapy (CBT) treatments in routine clinical practice necessitates effective methods of CBT training, which are scalable and can be disseminated in populations without ready access to specialist trainers or large scale funding. This thesis seeks to add theoretical and empirical guidance to the development of evidence-based CBT training. The first chapter reviews the current literature on effective CBT training, and suggests theoretical and methodological guidance for future research on evidence-based training practices. Cohort studies (Chapters 2 – 4) examine data from former trainees for guidance on accurate CBT competence measurement and effective CBT training components. The fifth and sixth chapters report randomised-controlled trials examining the effects of internet-based training (IBT) on therapists’ competence and patient outcomes (Chapter 6). Findings from the literature review (Chapter 1) and Chapter 2 suggested the need for training effectiveness trials using treatment trial methodology, which includes ‘blind’ observer ratings of therapist behaviour as the measure of competence. Chapter 3 found that extensive training led to significant improvement in competence for trainees with a range of individual baseline characteristics. Since such extensive training is not accessible to the majority of practicing therapists, Chapter 4 examined which elements of extensive training were perceived by trainees as having the greatest positive effect on competence; thus providing empirical guidance in planning more scalable forms of training, such as IBT. Findings from the first four chapters provided guidance in planning the two randomised controlled trials outlined in Chapters 5 and 6, resulting in rigorous empirical data showing significant positive effects of IBT on therapists’ competence and on their patients’ clinical outcomes. Finally, the results of this thesis are related to the framework for developing evidence-based training, which was suggested in Chapter 1 and suggestions for future research are made.

Vykdant šalies švietimo reformą profesinės kompetencijos plėtojimas laikomas vienu iš prioritetinių tikslų, todėl jis kelia konkrečius reikalavimus tiek moksleiviui, tiek pedagogui. Kompetencijų ugdymas padės siekti Lietuvos tolimesnės plėtros strateginių tikslų — suteikti visiems jos žmonėms vienodas galimybes panaudoti ir plėtoti savo gebėjimus, gauti gerą visuomenės poreikius atitinkantį išsilavinimą ir pagal išsilavinimą atitinkamą darbą. Švietimas šiandien suvokiamas kaip vie¬nas kertinių ES vidaus politikos ramsčių, o profesinės kompeten¬cijos — kaip raktas nuo Europos ateities, nes pasta¬ruoju metu sėkminga asmens veikla priklauso ne tiek nuo įgytų baigus atitinka¬mą mokymo programą formalių kvalifikacijų, kiek nuo jo gebėjimo adaptuotis kintančioje aplinkoje, kurioje es¬minis faktorius yra asmens kompetencija ir jos plėtojimo potencialas. / In this Master paper a model for the pedagogues‘ attitude toward the training of competence is formed and professional improvement as the essence of competence change consistent patterns is predicated. While implementing the reform of the national education system, the development of professional competence is one of the primary goals, therefore it raises particular requirements both for a schoolchild and pedagogue. Competences‘ training will help to labour the strategic goals of the further development of Lithuania – to enable its people equally in employing and developing their capabilities, receiving a proper education that answers the demands of society and speciality adequate employment. Education today is perceived as one of the cornerstones of the internal policy of the EU and the professional competences as a key to the European future, for the successful personal activity recently depends not oly from the formal qualifications acquired from the respective curriculum, but from the ability to adapt in the changing environment, where the main factor is personal competence and its potential of development.

Pagrindinis darbo tikslas- ištirti mokyklos vadovo vadovavimo reikšmę pedagogų naujų kompetencijų įgijimo ir tobulinimo procese. Tikslui pasiekti keliami šie uždaviniai: analizuojant mokslinę literatūrą išsiaiškinti vadovo vadovavimo, bei mokytojų kompetencijų sampratas; apibrėžti mokyklos vadovo vadovavimo reikšmę pedagogų naujų kompetencijų ugdymo aspektu; išanalizuoti pedagogų kompetencijų tobulinimo tendencijas, bei jų požiūrį į vadovo sukuriamus veiksnius tobulinimosi procese; ištirti įstaigos vadovo požiūrį į pedagogų naujų kompetencijų plėtojimo galimybes. Darbą sudaro 3 dalys. Pirmoje darbo dalyje yra nagrinėjamos vadovo samprata ir aplinka, įvairiais aspektais aptariamas sėkmingas vadovavimas mokytojų tobulinimo procesui, jį veikiančios socialinės sąveikos, pedagogų kompetencijų sampratos bei jų tobulinimo galimybės. Antroje dalyje yra pateikta tyrimo metodika, bei tyrimo algoritmas. Trečioje darbo dalyje apžvelgiami atlikto tyrimo rezultatai. Darbo apimtis 81 puslapiai. Darbas iliustruotas 33 paveikslais, 7 lentelėmis, 1 schema. Darbe panaudota 66 šaltinių. / The main aim of the work is to investigate the importance of the director’s superintendence in the process of acquirement and perfection of the educationalists’ new expertise. To meet the aim these tasks have to be considered: to ascertain the conceptions of the schoolmaster’s superintendence and the teachers’ expertise while analyzing the scientific literature; to define the importance of the schoolmaster’s superintendence in the education of the educationalists’ new expertise; to analyze the tendencies of the educationalists’ expertise perfection and their opinion towards the factors that the schoolmaster creates in the process of perfection; to explore the opinion of the institution’s director towards the educationalists’ new expertise development. The final master’s work consists of three main parts, 81 pages, 33 pictures, 7 frames, 1 schema.

Medical students’ perceptions were gathered in order to evaluate the usefulness and relevance of the Clinical Skills module and the different components thereof as experienced by the students themselves. This in the end led to valuable feedback that were given to the course coordinators which in turn may lead to curricula changes being made to improve the overall teaching and learning experience for future medical students rotating through the CSC.

Job satisfaction of health professionals in Kigali University Teaching Hospital

Magister Public Health - MPH / Job satisfaction is important for the delivery of quality health care and health worker retention. This study aimed to identify the extent of job satisfaction among University Teaching Hospital of Kigali (UTHK) health workers and to describe the variables related to job satisfaction. These included working conditions, remuneration, patient care, development opportunities, supervision, time pressure and staff relations. A cross- sectional survey of UTHK health workers was conducted using a standardized instrument to identify health worker job satisfaction with related key work factors. A self-administered questionnaire was used to collect data from 274 health workers selected using a proportional stratified random sampling method and which included 21 medical doctors, 159 nurses, 19 midwives, 37 paramedic and 37 administrative staff. Respondents provided written consent to participate in the study. Analysis consisted of both descriptive statistics of overall satisfaction and various satisfactionrelated factors. For determining whether there is a significant relationship between job satisfaction and its independent variables, data were categorized and Chi-square or Fisher Exact test performed. Results showed a moderate overall job satisfaction level with 79.1% of respondents rating their satisfaction between 6 and 8 (mean: 6.7) on a scale of 1-10. A majority of respondents (82.6%) reported being dissatisfied with work income and 85.6% believed that their pay was not comparable to the work done. Over four fifths of the respondents (83.3%) reported feeling overwhelmed by responsibilities at work while a big percentage (96.5%) reported their job to be demanding physically, emotionally as well as mentally. However, respondents reported strong satisfaction (between 80% and 95%) with respect to work meaning, professionalization, training and orientation variables. Factors significantly associated with job satisfaction were adequate training to fulfill responsibilities (p value<0.001), feeling unvalued by the hospital (p=0.037) and dissatisfaction with supervisor care for patients and employees (p=0.034). In conclusion, improvement of remuneration, working conditions and hospital management in Kigali University Teaching Hospital would be expected to increase the level of job satisfaction of hospital health workforce.

Fortbildning inom psykisk ohälsa : En kvalitativ intervjustudie med fritidsgårdspersonal

Psykisk ohälsa är ett omfattande folkhälsoproblem, särskilt bland barn och ungdomar. Det kan därför vara av betydelse att fritisgårdspersonal som dagligen arbetar med barn och ungdomar också kan ge stöd och vägledning till barn och ungdomar som mår dåligt psykiskt. Att fritidsgårdspersonal får fortbildning om psykisk ohälsa kan även vara en främjande faktor för fritidsgårdspersonalen att motverka ohälsosamma stressorer inför svåra situationer. Fortbildningen kan således stärka personalens förmåga att hantera utmanande situationer gällande psykiska besvär bland barn och ungdomar.  Föreliggande studie genomfördes för fritidsgårdsverksamheternas enhet i Västerås stad. Syftet med studien var att undersöka fritidsgårdspersonalens upplevda behov av fortbildning gällande psykisk ohälsa bland ungdomar.  En kvalitativ ansats valdes för studien och datainsamlingen genomfördes utifrån semistrukturerade intervjuer. Urvalet av informanter var målinriktat och fem personer intervjuades. Därefter transkriberades och analyserades intervjuerna utifrån en manifest innehållsanalys. Resultaten visar att fritidsgårdspersonalen upplever svårigheter kring hantering av psykisk ohälsa bland barn och ungdomar på grund av att de är i behov av verktyg som hjälper dem att hantera svåra situationer i allmänhet.  Men de upplevde dessutom ett behov av fortbildning kring psykisk ohälsa i synnerhet. Personalen upplever också att de har bristande kunskap kring olika tillstånd och diagnoser som psykisk ohälsa omfattar. Resultatet påvisar även att fritidsgårdspersonalen uttryckte en känsla av meningsfullhet för sitt arbete. Slutsatsen från studien tyder på att det finns ett behov av fortbildning gällande psykisk ohälsa för fritidsgårdspersonalen. / Mental health is an increasing public health problem, especially among children and adolescents. It is therefore essential for the youth care staff who work with children and adolecents on a daily basis to know how to effectively handle young individuals who are suffering from mental health problems. The fact that the youth care staff receive further training on mental illness can be a stimulating factor for them to counter difficult situations appropriately. The continuing education can also strengthen the staff's ability to handle challenging situations regarding mental health problems among children and adolescents. A study was conducted for several youth care centres in Vasteras city, Sweden. The purpose of the study was to investigate the youth care staffs perceived need for competence training regarding mental illness among adolecents. A qualitative approach was chosen, and the data collection was thus carried out based on semi-structured interviews. Purposeful sampling was used and five youth care staff were interviewed. After which, their answers were transcribed and analysed based on a manifest content analysis.   The results show, that youth care staff experience difficulties in managing mental illness among children, and adolescents because they are in need of tools that help them handle difficult situations in general. But they also experienced a need for further education on mental illness in particular. This is largely due to a lack of knowledge about the various conditions and diagnoses that mental illness involves. The results also show that the staff expressed a sense of meaningfulness for their work. The conclusion from the study suggests that there is a need for continuing education regarding mental health for youth care staff.
