Inzidenz gehäufter ventrikulärer Rhythmusstörungen bei Patienten mit automatischem Kardioverter - Defibrillator (ICD) / Electrical Storm in ICD - Patients

Ein elektrischer Sturm (rezidivierende VT-, oder VF - Episoden innerhalb eines kurzen Zeitraums) tritt in 50% - 70% bei Empfängern von implantierbaren Kardiovertern – Defibrillatoren auf. Die Ursachen dieses elektrophysiologischen Phänomens sind bisher nicht bekannt. In dieser Arbeit untersuchten wir das Auftreten, den Verlauf, therapeutische Möglichkeiten und die Prognose des elektrischen Sturmes. 415 konsekutive Patienten, die aufgrund eines überlebten plötzlichen Herztodes, der MADIT oder der MADIT II Kriterien einen ICD erhalten haben, wurden durchschnittlich für 893 ± 176 Tage begleitet. Der elektrische Sturm wurde als ventrikuläre Tachykardie oder ventrikuläres Flimmern definiert, bei der / dem die Notwendigkeit eines therapeutischen Einschreitens von mindestens drei Mal innerhalb von 24 Stunden gegeben war. 35 aller ICD Patienten (8.4%) entwickelten einen elektrischen Sturm. Gehäuft trat dieser bei Patienten mit Koronarer Herzkrankheit auf (12%). 195 Patienten (47%) erhielten einen oder mehrere adäquate Entladungen (Schockabgabe ± ATP). Bei den Patienten, bei denen eine Therapie notwendig wurde, lag der durchschnittliche Zeitpunkt dafür 238 ± 57 Tage nach der Implantation des ICD. Die 415 Patienten hatten eine durchschnittliche linksventrikuläre Ejektionsfraktion von 38 ± 15% (zwischen 12% minimale Ejektionsfraktion und 74% maximale Ejektionsfraktion). 18 der Patienten mit Koronarer Herzkrankheit (78%), die einen elektrischen Sturm erlitten haben, hatten eine linksventrikuläre Ejektionsfraktion, die kleiner als 30% war. Bei Patienten mit Koronarer Herzkrankheit und bei Patienten mit Herzinsuffizienz, die jeweils eine linksventrikulären Ejektionsfraktion kleiner 30% besaßen und einen Schrittmacher im VVI Modus implantiert hatten, trat ein elektrischen Sturm in 16 % der Fälle (17 von 107 Patienten) auf. Im Vergleich dazu entwickelten Herzinsuffizienz - Patienten, die einen biventrikulären Schrittmacher inklusive eines ICD implantiert hatten, einen elektrischer Sturm in nur 2% (p < 0.05). Der elektrische Sturm ist ein häufiges Ereignis bei Patienten, denen ein ICD implantiert wurde. Patienten mit Koronarer Herzkrankheit und einer linksventrikulären Ejektionsfraktion kleiner 30% sind am häufigsten davon betroffen (16%). Beta – Blocker, Amiodaron und die Behandlung einer kongestiven Herzkrankheit falls erforderlich, erscheinen als die erfolgsversprechensten Maßnahmen in der Therapie des elektrischen Sturmes. Patienten mit einer Herzinsuffizienz (EF < 30%) und einem kombinierten biventrikulären Herzschrittmacher / ICD - Gerät hatten signifikant weniger Episoden mit einem elektrischen Sturm (2%), als Patienten mit Herzinsuffizienz (EF < 30%) und einem konventionellen ICD – Gerät (16%). / Electrical storm, i.e. multiple episodes of ventricular tachycardia of fibrillation in a short period of time is reported to occur in 50 to 70% of recipients of implantable cardioverter/defibrillators (ICD´s). The reason for such conditions remain elusive. Here we investigated incidence, therapeutic options and prognosis of electrical storm. 415 consecutive patients who received and ICD because of survived sudden cardiac death or according to MADIT or MADIT II criteria were followed up for a mean of 893±176 days. Electrical storm was defined as ventricular tachycardia or fibrillation with the necessitiy of device interrogation ?3 times during 24h. 35 of all ICD patients (8.4%) developed electrical storm. Incidence was highest in patients with coronary artery disease (12%). 195 patients (47%) received one or more appropriate ICD therapie (ATP or shock delivery). In the patients with the necessity for therapy average time for onset of therapy was 238±57 days after implantation. In the 415 patients, mean left ventricular ejection fraction was 38±15% (range 12% to 74%). 18 of CAD patients (78%) with electrical storm had an EF<30%. Comparison of CAD patients with an EF <30% and CRT/ICD patients (EF<30%) showed that patients with back-up VVI pacing had electrical storm in 16% (17 out of 107 patients) and CRT/ICD patients had electrical storm in only 2% (p<0.05). Electrical storm is a frequent event in ICD patients. Patients with coronary artery disease and an EF<30% are most likely to be affected. ß-blockers, amiodarone and treatment of congestive heart failure if adverdent, seem to be most promising in the treatment of electrical storm. Heart failure patients (EF<30%) with a combined CRT/ICD device have significantly less episodes with electrical storm than heart failure patients (EF<30%) with conventional ICD therapy.

The Creation of a Pacemaker Clinic at a Federally-Funded Patient-Centered Medical Home: A Quality Improvement Project

It is common for clinicians to implant medical devices, such as permanent pacemakers and implantable defibrillators, for cardiac diseases. These medical devices require follow-up care at regular intervals to ensure proper device function and optimal outcomes. Currently, many individuals without insurance or financial resources lack access to recommended follow-up care after implantation of a cardiac device. The purpose of this project was to determine the number of individuals who have had a medical device implanted without insurance coverage over a 3-year period, and then to establish a clinic that provides this service. The standard of care and operating procedure for the pacemaker clinic was established using evidence-based guidelines from the Heart Rhythm Society and the American Heart Association. Complexity science was the theoretical model used to guide this project's design and implementation. This quality improvement initiative was non-experimental, descriptive, and quantitative. Data were extracted from the ICD Registry and United States Census Bureau to determine the number of residents, insurance status, and number of implants over a 3-year period. These data were used to estimate the number of individuals with devices. The data revealed that 40 individuals with low power cardiac devices and 15 individuals with high power devices lacked access to care. The model developed estimates a growth rate of 7 to 10% annually. The pacemaker clinic will provide access to over 70 individuals lacking care for their pacemakers, thereby resulting in improved healthcare outcomes, fewer preventable complications, and optimal device performance.

Der implantierbare Defibrillator hat sich als wichtigstes Instrument bei der Prävention des plötzlichen Herztodes auf dem Boden maligner Arrhythmien erwiesen3. Dennoch gibt es Patienten, die niemals eine lebensrettende Therapie durch ihr ICD-Aggregat erfahren. Ziel der vorliegenden retrospektiven, monozentrischen Studie war es, den Nutzen eines ICD-Aggregatwechsels trotz ersten Aggregatlebens ohne adäquate Therapieabgaben zu eruieren. Mit dieser Zielsetzung wurde untersucht, welche Patienten von einem Aggregatwechsel profitieren und welche nicht. Eingeschlossen wurden 71 Patienten, die mindestens einen ICD-Aggregatwechsel durchlaufen und keine adäquate Therapie durch ihr erstes Aggregat erfahren hatten. Die statistische Auswertung der erhobenen Daten ergab mehrere signifikante Prädiktoren für die Notwendigkeit einer ICD-Therapie nach dem ersten Aggregatwechsel trotz ersten Aggregatlebens ohne adäquate Therapieabgaben: Hauptprädiktoren waren eine hohe Anzahl an kardiovaskulären Risikofaktoren, nicht therapierte, aber doch dokumentierte ventrikuläre Rhythmusstörungen, eine geringe linksventrikuläre Ejektionsfraktion (LVEF) zum Zeitpunkt des ersten Aggregatwechsels sowie eine kurze Lebensdauer des ersten ICD-Aggregats. Weitere signifikante Prädiktoren waren Adipositas Grad II, Niereninsuffizienz Grad III, nicht-insulinpflichtiger Diabetes mellitus, Apoplex, Hyperurikämie sowie Phenprocoumon-Therapie. Aktuell fehlen Daten und klare Leitlinien, die sich nicht nur wie bisher auf die Indikationsstellung bei Primärimplantation beziehen, sondern die Entscheidungshilfen geben, ob ein Aggregatwechsel durchgeführt werden sollte oder nicht. Die Ergebnisse dieser Arbeit unterstützen diese Forderung nach neuen Leitlinien. / Background Implantable cardioverter-defibrillator (ICD) therapy is exceptionally powerful preventing sudden cardiac death in several patient groups at high risk for malign tachyarrhythmia. However, several patients never experience life-saving-therapy by their ICD, and, in contrast, might even suffer from complications caused by the ICD such as device infections or inappropriate shocks, which have been shown associated with increased morbidity and mortality. There is currently a lack of data helping to decide whether an ICD-generator-exchange will be beneficial for every patient and thus should be performed or not. Objective The aim of this study was to reveal patients who might not benefit of ICD generator change when reaching elective replacement therapy. Methods and Results In this retrospective monocentric analysis, 71 patients who had at least one ICD generator exchange and did not experience any adequate and successful therapy by their first implantable defibrillator were included. Further inclusion criteria were a minimal follow-up-time after first generator-exchange of 1 year. 59 (83.1%) subjects were male. Mean age at the time of the first generator exchange was 63.5±11 years. 27 (38%) subjects had received the ICD for primary and 44 (62%) for secondary prevention. The average follow-up after primary ICD-implantation was 8.2±1.8 years and the average time after which the first generator exchange was performed was 5.0±1.4 years. Mean follow-up time after first generator exchange was 3.2±1.6 years. During this time 4 (5.6%) patients died. Only 11 (15.5%) of all study-patients received at least 1 adequate ICD-therapy, whereas 15 (21.1%) of the cohort received inadequate shocks. Main predictors of ventricular tachycardia with adequate ICD therapy were a high number of cardiovascular risk factors and previous non-sustained ventricular arrhythmias during initial generator life. Conclusions Patients who are at a low cardiovascular risk and did not experience sustained or non-sustained ventricular tachycardia over the initial ICD battery lifespan are at low risk for future need of ICD therapy. Therefore, indication for generator exchange for battery depletion should be carefully reconsidered in these patients.

In the United States, see recommendations for cardiac safety of sports arenas but also to health and fitness facilities. However, this does not exist in Sweden today. Objective: The purpose of this study is to identify the health- and fitness facilities today are equipped with respect to cardiac safety and specifically relating to preparedness for cardiac arrest. Method: Quantitative data are collected and compiled with the help of a questionnaire. The questionnaire contained 19 questions in total. Random selection of plants and a total of 263 facilities contributed 88 plants in the study. The loss was 67 %. Results: Of the 88 facilities was the 94 % who were trained and certified in cardiac cardiopulmonary resuscitation and 66 % in advanced cardiac cardiopulmonary resuscitation. Just over half of all plants had AED. Half of the establishments, which had more than 10 minutes to the nearest hospitals had no AED and of these, 75 % were privately owned. Conclusion: Preparedness for cardiac arrest on our health and fitness facilities seems to be good, for example, most of the facilities that responded to the questionnaire had AED and practical training of the staff. Most are also trained and certified in cardiac cardiopulmonary resuscitation. But there is room for improvement, for example, only about half of all holdings with &gt; 10 minutes to the hospital had no AED and three facilities lacked both AED and trained staff in CRP. Guidelines are needed on preparedness for acute cardiac events on health and fitness facilities in Sweden, just as there are in the United States today.

Patienters livskvalité och erfarenheter av att leva med implanterbar cardioverterdefibrillator (ICD) : En litteraturstudie

Bakgrund: Behandling med implanterbar cardioverter defibrillator (ICD) kan ges iprimärt- eller sekundärt syfte. ICD: n har till uppgift att upptäcka och behandlaallvarliga rytmrubbningar och vid behov avge en stöt/stötar. Patienter som lever med enICD kan få svårt att acceptera sina nya liv samt att deras livssituation kan påverkas.Sjuksköterskan har en viktig roll att individanpassa omvårdnaden för dessa patienter.Syfte: Syftet var att beskriva patienters livskvalité och erfarenheter av att leva medimplanterbar cardioverter defibrillator (ICD). Syftet var också att beskriva urval ochundersökningsgrupp i de inkluderade artiklarna.Metod: En deskriptiv litteraturstudie där 11 vetenskapliga artiklar inkluderades varavsex kvalitativa, fyra kvantitativa och en artikel hade mixad metod. Artiklarna har sökts idatabasen Pubmed med begränsning på 6 år.Resultat: Patienter som lever med en ICD beskriver en oro och rädsla för att ICD: n skaavge stötar. Erfarenhet som patienter betonar är att diskussion om avaktivering av ICD ilivets slutskede ansågs vara bristfällig eller helt utesluten. Då livskvalitén påverkas ochpatienterna beskrev sig bli begränsade i vardagen så kan coping vara ett verktyg för attfinna lösningar. Tio av resultatartiklarna angav tydligt urvalsmetoden. Samtligaredogjorde för antalet deltagare och hur många som var män respektive kvinnor.Medelåldern på deltagarna redovisades tydligt i alla artiklarna.Slutsats: Livskvalitén har visats påverkad hos patienter med en ICD, även oro ochrädslor för stötar. Sjuksköterskan bör ha ett holistiskt synsätt för att se hela patientenoch dennes behov.

Candida Endocarditis Associated With Cardiac Rhythm Management Devices: Review With Current Treatment Guidelines

Coincident with an increased use of cardiac rhythm management devices (CRMD) has been an increase in the number of pacemaker and cardioverter-defibrillator infections. CRMD endocarditis accounts for about 10% of all device-related infections, and cardiac infection caused by Candida sp. is a rare event. To date, only sporadic reports of this unusual and life-threatening event have been reported. By describing a case of CRMD-related Candida endocarditis and conducting a literature review, we provide a detailed characterisation of this unusual clinical entity with an emphasis on diagnosis, management and treatment. A case of CRMD-related Candida endocarditis is presented and a computer search for confirmed cases of CRMD-Candida endocarditis was conducted. Current recommendations for management and treatment were documented. From 1969 to 2009, 15 patients with CRMD-Candida endocarditis (12 pacemaker and three implanted cardioverter-defibrillator) were documented. All were males, non-albicans Candida sp. were frequently recovered, a major fungal embolus occurred in 27% of patients and two of 10 patients who received defined antifungal therapy and device explantation expired. CRMD Candida endocarditis is a rare and serious clinical event; isolates can include Candida albicans and other Candida sp., and treatment involves both targeted antifungal therapy and device removal.

Der implantierbare Kardioverter/Defibrillator wird zum Schutz vor einem plötzlichen Herztod aufgrund maligner Herzrhythmusstörungen eingesetzt. Detektionsalgorithmen sollen dabei den Herzrhythmus korrekt klassifizieren und bei ventrikulären Tachykardien eine Therapie mittels Überstimulation oder Schockabgabe auslösen, nicht jedoch bei tachykardem Vorhofflimmern. Die Vermeidung solcher inadäquater Therapieabgaben stellt eine Herausforderung an die Detektionsalgorithmen der Implantate da. Bei steigender Prävalenz des Vorhofflimmerns auch bei ICD-Trägern stellt sich daher die Frage nach dem Einfluss dieser Rhythmusstörung auf die Versorgung dieser Patientengruppe. In der vorliegenden Arbeit sind retrospektiv die Daten von ICD-Trägern mit Vorhofflimmern und ICD-Trägern ohne Vorhofflimmern bezüglich der gewählten Implantate, Häufigkeit adäquater und inadäquater Therapieabgaben und der daraus resultierenden Maßnahmen untersucht worden. In der Auswertung zeigt sich, dass Patienten mit Vorhofflimmern tendenziell mehr inadäquate Therapien erlitten und signifikant häufiger hospitalisiert werden mussten. Anhand der Daten zeigt sich, dass Vorhofflimmern maßgeblich die Versorgung dieser Patientengruppen beeinflusst. / The implantable cardioverter/defibrillator (ICD) offers protection against sudden cardiac death due to malign cardiac arrhythmia. Detection algorithms are used for correct classification of the cardiac rhythm. Upon correct detection of ventricular tachycardia ATP or shock therapy is delivered. One prerequisite is the correct distinction of atrial fibrillation (AF). Due to rapid and arrhythmic pacing it is a challenge for the detection algorithms to avoid inadequate therapies for AF. The prevalence of AF is increasing and therefore more patients with an ICD suffer of AF. This coincidence rises the question whether AF influences the treatment of patients with both AF and an ICD. In the following thesis the data of patients with an ICD and AF are retrospectively compared to patients with an ICD and without AF. The type of implants, frequency of adequate and inadequate therapies and the follow up activities were analyzed. The results show that patients with AF tend to have more inadequate therapies and significantly more hospitalizations. The data states that AF has an influence on the therapy of patients with an ICD.

Patientens upplevelse av att erhålla samt leva med en implanterbar defibrillator : en litteraturöversikt / Patient's experiences of obtaining and living with an implantable cardioverter defibrillator : a literature review

En livshotande hjärtrytmrubbning kan inom ett par minuter leda till hjärtstopp. Patienter med en ökad risk för detta kan erbjudas behandling med en implanterbar defibrillator (ICD), som kan bryta dessa tillstånd och är en livräddande behandling. Att leva med en ICD kan innebära en del begränsningar i det fortsatta dagliga livet vilket kan innefatta livsstilsförändringar för dessa patienter. Syftet med studien var att undersöka patientens upplevelser av att erhålla samt leva med en implanterbar defibrillator (ICD). En litteraturstudie av tidigare gjorda undersökningar har genomförts för att besvara syftet. Datainsamlingen av studierna har gjorts i databaserna Cinahl Complete och PubMed och har kvalitetsgranskats av författarna. En integrerad analys genomfördes och fyra huvudteman togs fram och presenteras i resultatet. Resultatet visar att kunskap hos patienter är viktigt för en bra livskvalitet. För detta krävs god information från vårdpersonalen vilken i många fall anses otillräcklig. Patienterna tvingas göra förändringar i det dagliga livet och för många innebär detta en social isolering. Detta på grund av att de slutar utföra många av tidigare aktiviteter på grund av rädsla för att det ska framkalla ett elektriskt tillslag. Generellt framkommer det att ICD bärare är nöjda och tacksamma över att de fått erhålla enheten men att det finns en hög förekomst av rädsla och ångest i deras fortsatta liv. En satsning på personcentrerad information av sakkunniga sjuksköterskor skulle kunna öka dessa patienters psykiska välmående.

Faktorer som påverkar hälsorelaterad livskvalitet hos patienter med implanterbar defibrillator

<p>Då behandling med implanterbar defibrillator (ICD) enligt forskning</p><p>förlänger överlevnaden för hjärtsjuka har socialstyrelsen i sina nationella</p><p>riktlinjer rekommenderat att behandling av arytmier och hjärtsvikt med</p><p>ICD ska prioriteras. Detta kommer att leda till att antalet patienter med</p><p>ICD ökar i framtiden. Syftet med denna litteraturstudie var att beskriva</p><p>faktorer som påverkar ICD patienternas hälsorelaterade livskvalitet.</p><p>Studierna påvisade generellt sett att ICD patienter som grupp inte hade</p><p>lägre hälsorelaterad livskvalitet, men på individ nivå framkom det</p><p>påverkande faktorer. Dessa faktorer indelades i tre kategorier:</p><p>psykologiska faktorer, att uppleva defibrilleringchocker samt tid för</p><p>anpassning och betydelsen av patientens ålder.</p><p>Sjuksköterskan har en viktig roll i en omvårdnadssituation att inge</p><p>trygghet, stödja patienten och bidra till att patientens kunskap ökar. Det</p><p>ger patienten möjligheten att påverka sin hälsorelaterade livskvalitet</p><p>eftersom den är avgörande för patientens välmående och upplevelse av</p><p>ICD behandlingen.</p><p>Informanterna i resultatstudierna var övervägande män och ur ett</p><p>genusperspektiv begrundas att resultatet i litteraturstudien inte</p><p>representerar befolkningen i stort.</p><p>Med hänvisning till att antalet patienter med ICD kommer att öka i</p><p>framtiden, behövs mer forskning om kvinnor och deras upplevelse av</p><p>hälsorelaterad livskvalitet och ICD behandling.</p>
