Homeopathy in the prevention of upper respiration tract infections in children

The aim of this thesis is to explore why parents bring their children to homeopaths and to investigate the effect of homeopathic treatment for prevention of upper respiratory tract infections (URTI) in children. The reason for doing studies on this is that there has been a nearly threefold increase in the proportion of children among patients visiting Norwegian homeopaths. This raised the question of why it is so. Furthermore, recurrent respiratory complaints are a main reason why child patients consult homeopaths. This raised the question of the effect of homeopathic treatment in this patient group, because there is very little research on this. The thesis builds on four different studies conducted between August 2002 and June 2004. Parents of nine children that recently had been to a homeopath for the first time were interviewed to explore why parents take their children to homeopaths. All parents had been to a medical doctor before consulting the homeopath. It was the experiences with conventional medical treatment that led the parents to look for alternatives. The reasons were that 1) the parents did not want to give the medication prescribed by the doctor, 2) they wanted treatment while waiting for a problem to be assessed, 3) they did not want to continue to use the prescribed medication, 4) they stopped taking conventional medication due to side effects or 5) they were not offered any treatment by the medical doctor. The parents would consult a medical doctor if they felt insecure about the health conditions of the child and would visit a homeopath when they felt that the situation was clarified. There are parents who take their child to homeopaths despite not understanding or having belief in whether ultramolecular homeopathic medicines can have effects. One hundred and sixty-one children who had been diagnosed with an URTI by a medical doctor were recruited to participate in a trial on the effect of treatment by homeopaths for prevention of URTI in children. The children were randomly allocated to two groups. One group received an appointment immediately with one of five homeopaths who treated the patients as they do in their everyday practice. The other group (control) got such treatment after three months. The occurrence of URTI judged by the parents were significantly lower among those treated immediately by homeopaths (median 8 days in three months) compared to the control group who used self-selected conventional health care (median 13 days) (p=0.006). Homeopathic medicines are frequently used for self-treatment (over the counter-OTC). It is not known if the choice of the patient is the same, as a homeopath would have prescribed. A study was therefore conducted to explore if there can be developed indications for homeopathic medicines that facilitate that parents can chose the same medicine as a homeopath would prescribe for children with URTI. Firstly, data from a survey was used to find three medicines Calcarea carb, Pulsatilla and Sulphur that accounted for 60% of all prescription made by Norwegian homeopaths for children with URTI. Simplified constitutional indications for these medicines were developed and tested by comparing the choices of 70 parents with the prescription of eleven homeopaths. The parents were able to choose the same homeopathic medicine as homeopaths prescribed for 55% of the children. Two hundred and fifty-nine children who had been diagnosed with an URTI by a medical doctor were recruited to participate in a trial on the effect of one of three self-selected ultramolecular homeopathic medicines for prevention of URTI in children. The indications developed were used. The children was randomly allocated to receive either ultramolecular homeopathic medicine (C-30) or placebo. There was no difference in the occurrence of URTI judged by the parents among getting ultramolecular homeopathic medicine compared to those getting placebo (median 9 days in three months for both groups) (p=0.531). / Hensikten med denne avhandlingen er å undersøke hvorfor foreldre tar sine barn med til homøopat og å undersøke effekten av homøopatisk behandling i forebygging av øvre luftveisinfeksjoner (ØLI) hos barn. Bakgrunnen for de undersøkelsene som er gjort, er at det nesten er en tredobling i andelen barn blant pasienter hos homøopat. Dette utløste spørsmål om hvorfor det er slik. Videre er gjentatte luftveisplager en hovedårsak til at barn oppsøker homøopat. Fordi det er lite forskning på dette temaet ble spørsmålet om effekten av homøopatisk behandling i denne pasientgruppen også utløst. Avhandlingen bygger på fire ulike undersøkelser som er gjennomført mellom august 2002 og juni 2004. Homøopatisk medisin brukes internasjonalt i stor grad til selvbehandling. Man vet ikke om pasientens eget valg av homøopatisk medisin er lik det en homøopat ville foreskrevet. Det ble derfor gjennomført en undersøkelse av om det kan utvikles beskrivelser for indikasjoner for homøopatiske medisiner som gjør at foreldre kan velge samme medisin som en homøopat foreskriver for barn med ØLI. Først ble det funnet fram til tre medisiner, Calcarea carb, Pulsatilla og Sulphur som homøopater i Norge foreskriver til 60% av barn med ØLI. Så ble det utviklet indikasjoner for disse tre medisinene som ble testet ut ved at valgene til 70 foreldre ble sammenlignet med foreskrivingen til 11 homøopater. Foreldrene valgte samme medisin som homøopaten for 55% av barna. To hundre og femtien barn som hadde vært til lege på grunn av en øvre luftveisinfeksjon ble rekruttert til å være med på en undersøkelse av effekten av en av tre selvvalgte homøopatiske medisiner i forebyggingen av ØLI hos barn. Indikasjonene som ble utviklet ble brukt. Barna ble tilfeldig fordelt til enten å få homøopatisk medisin eller placebo. Det var ingen signifikant forskjell i forekomsten av ØLI mellom de som fikk homøopatisk medisin sammenlignet med de som fikk placebo (median 9 dager på tre måneder i begge grupper) (p=0,531).

Retinoic Acid Metabolism Blocking Agents and the Skin : In vivo and in vitro Studies of the Effects on Normal and Diseased Human Epidermis

Retinoic Acid Metabolism Blocking Agents (RAMBAs) increase the endogenous levels of all-trans retinoic acid (RA) by inhibiting CYP26 enzymes. Thus they are believed to mimic the effects of retinoid treatment. Their mechanism of action and effects on vitamin A metabolism in keratinocytes are however uncertain. To explore this and the function of CYP26 in human skin was the main purpose of the project. The effects of two RAMBAs (talarozole and liarozole) on the expression of retinoid biomarkers in epidermis were studied in vivo and in vitro. Normal human skin (n=16) exposed to topical talarozole for 9 days showed similar response as previously reported for topical RA, even though no skin inflammation occurred. Lamellar ichthyosis patients (n=11) treated systemically with liarozole showed variable clinical improvement after 4 weeks with only mild effects on the retinoid biomarkers and the expression did not always correlate at the protein and mRNA levels. In these studies the proinflammatory transcripts IL-1α and TNFα were down-regulated by RAMBAs. In vitro, using an organotypic epidermis model we first studied how the RA metabolism was affected by adding RA and/or RAMBAs. We next examined the effects of the same agents on the expression of vitamin A metabolising enzymes in monolayer cultures of proliferating and differentiating keratinocytes. The results show among other things that CYP26 A1 and B1 are both involved in the catabolism of RA, and that talarozole potently increases the level of endogenous RA, primarily by inhibiting CYP26B1. However the drug´s biological effects cannot be solely attributed to increased RA levels. In conclusion, RAMBAs are promising new drugs for treatment of skin disorders, but further studies on their mechanism of action are needed.

Kallikrein-related peptidases in human epidermis : studies on activity, regulation, and function

Introduction. The outermost layer of the epidermis, the stratum corneum (SC), plays a fundamental role in our defense against microorganisms, chemicals, and dehydration. The SC is composed of tightly packed keratinized skin cells, corneocytes. For a functioning skin it is essential that corneocytes are constantly shed (desquamated). Kallikrein-related peptidase (KLK) 5 and KLK7 may be important in the desquamation process through degradation of desmosomal proteins. Severe hereditary diseases, where inhibition of KLK5 and/or KLK7 is missing, points to the importance of regulation of protease activity. KLKs may be regulated in various ways: tissue expression, activation of proforms, specific inhibitors, and physico-chemical properties like pH. Besides their involvement in desquamation, KLKs may also be important in immune defense and inflammation by processing of mediators and via activation of proteinase-activated receptors (PARs). Aims. 1. To identify and characterize previously unknown proteases in the SC. 2. To further characterize KLK5 and KLK7 with special focus on activation mechanisms. 3. To identify new inhibitors of KLKs in human SC. 4. To further characterize KLKs regarding effects of various inhibitors and substrates. 5. To study possible functions of KLKs in inflammation, in particular via activation of PAR-2. Methods. Plantar SC was used as a source for purification of proteins. Recombinant proteins were produced in different expression systems (insect cells, yeast cells, and bacteria). Different activity assays and kinetic studies were performed. Tissue expression was studied by immunohistochemistry, immunoblot and PCR. PAR-2 activation was studied by measurement of intracellular [Ca2+] and immunofluorescense in KNRK-PAR2 cells. Results. Active KLK14 was purified from extracts of plantar SC. KLK14 showed a superior catalytic efficiency as compared to KLK5 when measuring trypsin-like activity. This indicated that KLK14, despite being present in low amounts in skin, may have great relevance for skin physiology. Among enzymes tested only KLK5 showed autocatalytic activity and is so far the only enzyme found in SC that can activate proKLK7. KLK5 could also activate proKLK14. This together with studies of pH dependence on activation placed KLK5 as a possible key activating enzyme in a proposed proteolytic cascade in the SC. In plantar SC extracts we have also identified the novel Kazal-type serine protease inhibitor 9 (SPINK9). Our results indicate that SPINK9 is preferentially expressed in palmo-plantar skin and specific for KLK5. Differences found regarding substrate specificity and inhibition profile can be useful in evaluating the contribution of individual KLKs to the proteolytic activity in crude SC extracts. One interesting finding was that KLK8, present at high protein levels in the epidermis, could not be inhibited by any protease inhibitor found in the extracts. PAR-2 activation studies showed that KLK5 and 14 but neither KLK7 nor 8 can activate PAR-2. Immunohistochemistry preferentially detected KLK14 in intraepidermal parts of the sweat ducts and in dermal sweat glands but we could also show coexpression of KLK14 and PAR-2 in the SC and stratum granulosum of the epidermis in inflammatory skin disorders. To summarize, KLK involvement in desquamation may be dependent on a proteolytic activation cascade regulated by an intrinsic pH gradient and specific inhibitors present in SC. Another possible function of KLKs is as mediators of inflammation through activation of PAR-2.

Chronic Inflammation-Driven Tumor Promotion Asociated with CD8+ T Cells

Chronic inflammation is associated with carcinoma development in several clinical settings, and we sought to investigate the role of T cells in this phenomenon using the DMBA/TPA two-stage chemical carcinogenesis protocol. We demonstrate that, paradoxical to models of immunosurveillance, wild-type (WT) mice have a markedly higher rate of tumor formation relative to strains lacking CD8+ T cells. Adoptive transfers of antibody-coated magnetic bead-enriched peripheral CD8+ T cells into TCRáâ-/- mice confirmed that the increased mean tumor area and progression to carcinoma was attributable to the presence of CD8+ T cells. All analyzed strains of mice in which the CD8 compartment was intact (WT, CD4-/-) showed significant increases in tumor susceptibility. Putative tumor-promoting (T-pro) cells (TCRáâ+CD8+CD44+CD62L- tumor infiltrating lymphocytes, TILs) were directly compared to their phenotypic equivalents in peripheral blood lymphocytes (PBLs). In WT and CD4-deficient mice, CD8+ TILs consistently revealed a markedly higher relative expression, by RT-PCR, of IFNã, TNFá and COX-2, and a striking decrease in expression of perforin. Cytokine-bead analysis (CBA) comparison of CD8+ and CD4+ TIL in tumors from WT mice confirmed the increased expression by the CD8+ TIL of IFNã and TNFá. To our knowledge, this is the first demonstration of increased carcinogenesis attributable to CD8+ TILs, characterized by their high IFNã, TNFá, and COX-2 production and defective perforin production relative to phenotypically equivalent PBLs. These studies may have mechanistic implications for the role of T cells in inflammation-associated carcinogenesis.

Dermatological and musculoskeletal disorders of nursing home workers in Australia, Japan, South Korea and Taiwan

Although skin disease and musculoskeletal disorders are believed to be common among nursing home workers, to date there have been no coordinated international studies of these occupational issues. Therefore, it was considered appropriate to conduct one of the first cross-cultural investigations of occupational dermatology and ergonomic complaints among nursing home workers in Australia, Japan, South Korea and Taiwan using a standardised methodology. This thesis documents a 4-year investigation of skin disease and musculoskeletal disorders conducted among 465 nursing home staff in Australia, Japan, South Korea and Taiwan. Skin diseases were diagnosed by specialist physicians during medical examinations, while information on musculoskeletal disorders was collected by means of a self-reported questionnaire. There were major differences in both the location and type of skin disease between the 4 groups. Overall, the Australian group suffered a generally higher prevalence of skin disease than in the other three countries investigated, most likely due to their significantly higher rate of sun-induced skin damage. The high prevalence of cutaneous fungal disease seen within the Taiwanese subjects most probably arose from the comparatively higher temperature and relative humidity of Taiwan. Other potentially important skin disease risk factors included previous skin disease and a history of allergy, both of which are consistent with current knowledge. Although musculoskeletal disorders were found to be most prevalent among the Japanese nursing home staff at almost all body sites, the reasons for this are not clear. It may have related to a generally higher musculoskeletal rate, or a higher degree of self-reporting on their questionnaires. Individual MSD risk factors included moving patients, washing patients, working as an assistant nurse and daily alcohol consumption. Interestingly, MSD was found to be a co-factor for current skin disease. Overall, this study indicated that certain occupational health issues consistently affect nursing home staff in the 4 countries, but the prevalence and rank order varies from nation to nation. It was also shown that nursing home work incurs a reasonable degree of risk and that skin disease and musculoskeletal disorders are important occupational issues within these facilities.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Tratar sobre dimensões vivenciais que envolvem o adoecimento cutâneo é indispensável em uma sociedade onde as pessoas vivem sujeitas às pressões quanto à: modelos socioculturais, preocupações com crenças e valores e imposições estéticas, podendo assim sofrer distorções em sua autoimagem corporal e autoestima. Refletindo sobre a teoria de adaptação de Roy quanto à singularidade do ser humano para reagir, aprender e se adaptar na sua convivência no mundo, consigo e com os outros, indaga-se: quais são as repercussões do acometimento cutâneo na vida das pessoas? Assim, formulou-se o objeto de estudo - Repercussões do acometimento de afecções cutâneas na vida das pessoas. Teve-se como objetivos propor uma perspectiva de cuidar em enfermagem compatível com as necessidades humanas de pessoas com alteração de autoimagem e autoestima devido a afecções dermatológicas; identificar as características sociodemográficas e clínicas das pessoas com afecções dermatológicas; analisar a dimensão imaginativa de pessoas referente à sua convivência com afecções dermatológicas, destacando seus sentimentos quanto à autoimagem e à autoestima. Este trabalho se insere no Grupo de Pesquisa Concepções teóricas para o cuidar em saúde e enfermagem - CNPq, na Linha de pesquisa Fundamentos filosóficos, teóricos e tecnológicos do cuidar em saúde e enfermagem e no projeto denominado A perspectiva estética/sociopoética do cuidar em enfermagem: identificando necessidades de autocuidado para promoção da saúde da Bolsa de Produtividade em Pesquisa CNPq, período 2011-2014, e da Bolsa de Professor Visitante da Universidade do Estado do Rio de Janeiro. Escolheu-se como referencial teórico metodológico a sociopoética e as técnicas de pesquisa dinâmica do corpo como território mínimo e a narrativa da lenda da beleza. O Grupo-Pesquisador, (GP) dispositivo analítico da sociopoética foi composto por 18 clientes da Enfermaria de Dermatologia do Hospital Universitário Pedro Ernesto, que desenvolveram as fases do método sociopoético no período de maio a agosto de 2013. Os dados produzidos foram analisados conforme os estudos sociopoéticos transversal, classificatório e filosófico. No transversal se destacam as categorias - Investindo no cuidado para o autocuidado e o desejo por uma pele íntegra x Imagem corporal elevada. No classificatório, a categoria Percebendo e conhecendo a afecção dermatológica x Descuidado consciente com o corpo. No estudo filosófico, destacam-se as categorias: comprometimento da autoimagem frente às repercussões da afecção dermatológica e a beleza existente no ser humano amado. Os resultados permitiram responder a questão inicialmente formulada e alcançar os objetivos propostos. Conclui-se que, a sociopoética como uma construção coletiva revelou-se, nesta pesquisa, como uma importante ferramenta tanto no cuidar/educar/pesquisar em enfermagem como na abordagem humanista aos clientes com afecções dermatológicas. Sendo assim, ao analisar a dimensão imaginativa referente à convivência das pessoas com a enfermidade citada, esta dissertação contribuiu para uma ampliação da perspectiva da imagem e estima das mesmas, na tentativa de possibilitar o desenvolvimento do cuidado de enfermagem cada vez mais próximo ao cliente por meio da interação e do diálogo. / Dealing with experiential dimensions, which involve skin disease, is essential in a society where people live subject to pressures regarding: sociocultural models, concerns about values and beliefs and aesthetic impositions and can therefore suffer distortions in their body self-image and self-esteem. Reflecting on Roy's adaptation theory as the uniqueness of the human being to react, learn and adapt in their coexistence in the world, with themselves and others, asks themselves: what are the repercussions of skin conditions in people's lives? Thus formulated, this was the object of study - The Impact on involvement of cutaneous conditions in people's lives. Had as objective to propose a perspective of nursing care consistent with the human needs of people with changing self-image and self-esteem due to dermatologic diseases; identify the sociodemographic and clinical characteristics of people with dermatologic diseases; analyze the imaginative dimension of people referring to their experience with dermatologic diseases, highlighting their feelings regarding self-image and self-esteem. This work is part of the Research Group theoretical Concepts for the caring in nursing and health care CNPq. In search Line philosophical Foundations, theoretical and technological of caring in nursing and health care and the project called the prospect aesthetics/social poetics of nursing care, identifying needs for self-care for health promotion of Research Productivity Grant CNPq, period 2011-2014, and the Scholarship of Visiting Professor of the State University of Rio de Janeiro. It was chosen as a theoretical and methodological social poetic and the techniques of dynamic search of body as territory minimum and the narrative of the legend of beauty. The Group Researcher (GP), device analytical socio-poetic was composed of 18 customers Infirmary Dermatology, University Hospital Pedro Ernesto, who developed the method socio-poetical phases in the period May to August 2013. The produced data were analyzed according to the studies, transverse, socio-poetical, classificatory and philosophical. In the transversal are highlighted the categories - Investing in care for self-care and the desire for a healthy skin x raised body image. In classification, the category realizing and knowing the dermatological condition x conscious carelessness with the body. In philosophical study, we highlight the categories: Commitment to the impact of self-image towards the dermatological disease and the existing beauty in the beloved human being. The results made it possible to answer the question originally formulated and achieving the proposed objectives. It is concluded that, socio-poetics as a collective construction proved in this research as an important tool for all in care / education / research in nursing as a humanistic approach to clients with dermatologic diseases. Thus, when analyzing the imaginative dimension regarding the coexistence of people with the disease mentioned, this thesis has contributed to a broadening of the perspective image and esteem of the same in an attempt to enable the development of nursing care increasingly closer to the client through interaction and dialog.

Imaging through a scanner, darkly : spectral imaging for sentinel lymph node biopsies

Breast cancer is the single most prevalent form of cancer in the UK today, accounting for around 16% of all diagnoses, and around 31% of diagnoses in women. The survival rates are good, however the prognosis is heavily dependent on the stage to which the cancer has progressed at diagnosis. In order to help accurately determine this stage, the sentinel lymph node of patients undergoing tumour resection surgery is removed and examined cytologically for the presence of cancerous cells. This examination of the lymph node is currently the rate-limiting step in the operation as a whole. There is evidence in the literature to suggest that cancerous tissue has a measurably different infrared spectrum from healthy tissue owing to chemical and morphological differences in the tissue. There is further evidence to suggest that in the visible and near infrared region, the spectra of healthy lymph node tissue is different from that of cancerous tissue. This thesis details a project, performed in collaboration with a surgical team at St Mary's Hospital, Newport, Isle of Wight, to analyse spectral images taken in the visible and near infrared, of biopsied lymph node tissue. In the course of the project, an unsupervised classificaton technique, based on an extension to the well establised 'spectral angle', was developed to analyse the spectral images. Psoriasis affects 2-3% of the UK population causing itchy and/or painful plaques on the skin. One of the main treatments for psoriasis is UV phototherapy, exposure to which is a risk factor for burning and the development of cancers. This thesis details an investigation into the possibility of developing a targeted UV phototherapy system based on spectral imaging to delineate the plaques and a proposed new UV laser for treatment.
