Blood glucose management in Type 1 diabetes is crucial in preventing several diabetic complications. Blood glucose management is a complex task requiring diabetics too carefully administer the correct dosages of insulin by taking their blood glucose levels, food consumption, exercise, stress, illnesses and several other factors into account. Improved bolus calculation greatly aids in controlling blood glucose levels within a tight range. This study investigates how the ets-concept (Equivalent Teaspoons Sugar-concept) can be used to develop products to calculate insulin boluses. A cellular phone based software application was developed to calculate insulin boluses using the ets-concept. This product was tested in a clinical trial. A blood glucose characterization procedure was also developed to characterize the blood glucose response of a Type 1 diabetic to carbohydrate ingestion and insulin administration. The family is fundamental in the treatment of the patient: It represents the foundation that will influence in the acceptance or not of the disease by the carrier. Because of this, the objectives of this study were described, the health beliefs of parents of carrying children with diabetes mellitus type 1 and understanding behaviors and psychics change that will possibly influence in the relation of the treatment. 13 people were investigated, 11 month to 10 years old children’s parents with Diabetes Mellitus Type 1, through an interview for attainment the health beliefs. The analyses of the data had been carried through the basis of the model of “health beliefs”. This model analyzed: impact of the diagnosis, susceptibility, severity, benefits, barriers, self-efficacy and present expectations of the future. The results had shown that the parents had difficulties, fears and unreliabilities, for the illness of their children. When telling the situations lived since the diagnosis until the current moment, in all the stages, the parents disclose intense suffering. They constantly are invaded from fear of loss, in the present as in the future in function of the complications of the illness. From these results is recommend that the parents receive a support from a team of health professionals with specific knowledge and with purpose to inform on the illness and how to minimize the fears and unreliabilities that create obstacles for the adhesion to the treatment. We expected with this type of support improve the quality of life of the patient and their family. / O Diabetes Mellitus tipo 1 geralmente ocorre na infância ou adolescência e repercute de forma dramática na vida dos pais. A família é fundamental no tratamento do paciente: representa o alicerce que influenciará na aceitação ou não da enfermidade por parte do portador. Por isso, os objetivos deste estudo foram descrever as convicções de saúde de pais de crianças portadoras de diabetes mellitus tipo 1 e compreender mudanças comportamentais e psíquicas que possam influenciar na conduta em relação ao tratamento. Investigou-se 13 pessoas, pais de crianças de 11 meses a 10 anos portadoras de Diabetes Mellitus Tipo 1, por intermédio de uma entrevista para levantamento e descrição de fatores de convicção de saúde. Os dados foram avaliados com base em um modelo de “convicção de saúde”. Esse modelo avaliou: impacto do diagnóstico, suscetibilidade, severidade, benefícios, barreiras, eficácia própria e expectativa de futuro de cada um dos pais. Os resultados mostraram que os pais experimentam dificuldades, medos e inseguranças, pela doença do filho. Ao relatarem as situações vividas desde o diagnóstico até o momento atual, em todas as etapas, os pais revelam intenso sofrimento. Eles são constantemente invadidos por medo de perda tanto no presente como no futuro em função das complicações da doença. A partir desses resultados recomenda-se que os pais recebam atendimento de uma equipe multidisciplinar com conhecimento específico e com a finalidade de informar sobre a doença e aplacar os medos e inseguranças que criam obstáculos para a adesão ao tratamento. Espera-se com este tipo de atendimento melhorar e a qualidade de vida do paciente e de sua família.

Rôle de la prokinéticine-2 dans le tissu adipeux / Role of prokineticin-2 in adipose tissue

L’obésité est un facteur de risque pour de nombreuses maladies telles que le diabète de type 2 et les maladies cardiovasculaires. La prokinéticine-2 a été caractérisée comme une hormone anorexigène qui joue un rôle dans la régulation de l’appétit et le métabolisme énergétique via une action sur le récepteur PKR2 au niveau centrale.Cette étude consiste à étudier l’implication de la PK2 et de PKR1 dans le tissu adipeux et dans la physiopathologie de l’obésité. Les souris PKR1-/- dans lesquelles le gène codant pour PKR1 est totalement inactivé, présentent une obésité par hyperplasie, du fait de la prolifération des préadipocytes. Les souris PKR1-/- présentent un état diabétique, avec une insensibilité à l’insuline accrue et une forte intolérance au glucose. Les souris aP2-PKR1-/- dans lesquelles PKR1 est spécifiquement inactivé dans le tissu adipeux, présentent, elle aussi, une obésité hyperplasique, due de la prolifération des préadipocytes. In vitro, nos résultats montrent que la PK2 est capable d’inhiber la différenciation des préadipocytes en inhibant la prolifération des préadipocytes lors de la phase d’expansion clonale mitotique. Nos résultats permettent d’envisager un rôle de la PK2 et de son récepteur PKR1 dans le traitement de l’obésité. / Obesity is a risk factor for various disorders such as type 2 diabetes and cardiovascular diseases. The prokineticins, prokineticin-1 and prokineticin-2 bind two similar G protein-coupled receptors, PKR1 and PKR2. Prokineticin-2 is an anorexigenic hormone that plays a role in appetite regulation and energy metabolism, via a direct hypothalamic mechanism. Since adipocytes express mainly PKR1, we investigated the role of PKR1 in adipocyte functions. PKR1-null mutant mice exhibit increased body weight that is due to an increased visceral fat mass. Mutant adipose tissue is characterized by adipocyte hyperplasia due to an increase in number of proliferating preadipocyte. Mutant adipocytes exhibit downregulation of insulin signaling that is associated with glucose and insulin tolerance. Adipocyte-specific aP2-PKR1 knockout mice present also an increased visceral adipose tissue that lead to a slight increased body weight. Fat mass is also characterized by an hyperplasia and an increased preadipocyte proliferation. This mice present slight metabolic changes. Utilizing 3T3-L1 murine preadipocytes, our study reveals that prokineticin-2 exerts an antiadipogenic function in murine cells. Inhibition of adipogenesis mediated by prokineticin-2 involved PKR1. Prokineticin-2 also inhibits proliferation of preadipocytes. These results suggest that prokineticin-2 via PKR1 signaling plays a crucial role in adipogenesis and adipose tissue hyperplasia.

A síndrome metabólica (SM) e o diabete melito (DM) caracterizam-se por uma série de alterações no metabolismo de lipoproteínas, entre elas a hipertrigliceridemia e a redução nas concentrações de HDL-colesterol (HDL-C). Em estudo prévio demonstramos que indivíduos saudáveis, não obesos, com concentração de HDL-C abaixo de 40mg/dL, quando comparados àqueles com concentração de HDL-C acima de 60mg/dL, apresentam, no plasma, esteróis marcadores de absorção intestinal de colesterol alimentar diminuídos, e de síntese de colesterol aumentados. Achados semelhantes foram descritos por outros autores em portadores de SM e DM, sugerindo que a resistência à insulina participa na origem do distúrbio, embora não se saiba por quais mecanismos. Considerando nossos achados prévios, os objetivos deste estudo são investigar quais os mecanismos moleculares envolvidos nas alterações do metabolismo de colesterol presentes em indivíduos com concentrações alta (HIPERALFA, HDL-C > 60mg/dL, n = 36) ou baixa (HIPOALFA, HDL-C < 40mg/dL, n = 37) de HDL-C por meio de medida de: 1) conteúdo celular de colesterol e expressão gênica de enzimas e receptores críticos para a regulação intracelular de colesterol, tendo como modelo células linfomononucleares de sangue periférico; 2) parâmetros que regulam o metabolismo de lipoproteínas no plasma e que são influenciados pela insulina, tais como lecitina-colesterol aciltransferase (LCAT), lipoproteína lipase (LPL) e lipase hepática (LH) pós-heparina, proteína de transferência de colesterol esterificado (CETP), proteína de transferência de fosfolípides (PLTP) e pré- beta1 HDL. Os critérios de exclusão foram: diabete melito, IMC 30Kg/m2, tabagismo, consumo elevado de álcool e uso de fármacos capazes de interferir no metabolismo de lipoproteínas. Mostramos que, quando comparado ao grupo HIPERALFA, o grupo HIPOALFA apresentou maiores concentrações de insulina, triglicérides, ALT(TGP), índice HOMA, atividade de LCAT e LH, e menor atividade de LPL e concentração de pré-beta1 HDL. Não houve diferença entre os grupos com relação ao conteúdo celular de colesterol, à expressão dos genes estudados (ABCA1, ABCG1, SR-BI, LDLR, HMG CoA redutase, SREBP-1c e LXR alfa), às atividades de CETP e PLTP no plasma e à ultrassonografia de carótidas. Nossos resultados mostram que indivíduos com concentração alta ou baixa de HDL-C no plasma diferem com relação à sensibilidade à insulina, além de parâmetros envolvidos na regulação do metabolismo de lipoproteínas. Estes achados não se relacionaram com o metabolismo celular de colesterol no modelo estudado, mas sugerem que este quadro metabólico, cuja origem é desconhecida, precede o aparecimento, no decorrer da idade, de outras alterações típicas da síndrome metabólica no grupo com baixas concentrações de HDL-C no plasma / The metabolic syndrome (MS) and the diabetes mellitus (DM) are characterized for a series of alterations in lipoprotein metabolism as hypertriglyceridemia and reduced HDL-cholesterol (HDL-C) concentration. In previous study we demonstrated that healthy non obese subjects with HDL-C concentration below 40mg/dL, when compared with those with HDL-C above 60mg/dL, present low plasma sterol markers of alimentary cholesterol intestinal absorption and high plasma sterol markers of cholesterol synthesis. Similar findings have been described by others in subjects with MS and DM, suggesting that insulin resistance participates in the origin of the disorder, although by unknown mechanisms. Considering our previous findings, the objectives of this study are to investigate the molecular mechanisms involved in alterations of cholesterol metabolism in subjects with high HDL-C (HYPERALPHA, HDL-C > 60mg/dL, n = 36) or low HDL-C (HYPOALPHA, HDL-C < 40mg/dL, n = 37) concentration by means of measuring: 1) cellular cholesterol content and gene expression of critical enzymes and receptors involved in the intracellular cholesterol regulation, having peripheral blood mononuclear cells as model; 2) parameters that regulate the plasma lipoproteins metabolism and that are influenced by insulin, such as lecithin: cholesterol acyltransferase (LCAT), post-heparin hepatic (HL) and lipoprotein lipase (LPL), cholesteryl ester transfer protein (CETP), phospholipid transfer protein (PLTP) and pre-beta1 HDL. The exclusion criteria were: diabetes mellitus, body mass index 30Kg/m2, smoking, heavy drinking and use of medications that interfere with lipoprotein metabolism. We demonstrated that, as compared with HYPERALPHA, the HYPOALPHA group presented higher insulin, triglycerides and alanine aminotransferase concentrations, HOMA index, LCAT and HL activities and lower LPL activity and pre-beta1 HDL concentration. There was no difference between the groups in the cellular cholesterol content, expression of genes (ABCA1, ABCG1, SR-BI, LDLR, HMG CoA reductase, SREBP-1c and LXR alpha), plasma CETP and PLTP activities and carotid ultrasonography. Our results show that subjects with high or low plasma HDL-C concentration differ in relation to insulin sensitivity and in parameters involved in lipoprotein metabolism regulation. These findings were not related to the cellular cholesterol metabolism in the studied model, but they suggest that this metabolic disturbance, whose origin is unknown, precedes the appearance, in the course of the human life of other typical alterations of metabolic syndrome in the low HDL-C concentration group

Custos de procedimentos de saúde e associação com nível de atividade física e estado nutricional de idosos hipertensos e diabéticos: análise do Estudo SABE - Saúde, Bem-estar e Envelhecimento, 2010 / Health care costs and association with physical activity level and nutritional status among hypertensive and diabetic elderly: analysis of SABE Study Health, Wellbeing and Aging, 2010

Introdução O nível de atividade física (NAF) insuficiente e estado nutricional (EN) inadequado conferem risco de desenvolvimento de hipertensão arterial e diabete, bem como dificultam o controle destas doenças. Assim, infere-se que os custos despendidos pelo SUS com medicamentos, internações e consultas de hipertensos e diabéticos apresentem relação inversa com NAF, incluindo a prática de caminhada e EN. Entretanto, estudos epidemiológicos que descrevam estes custos e analisem essas associações na população idosa são inexistentes no Brasil, o que dificulta a fundamentação para a implementação de políticas publicas para a economia de recursos. Objetivo Descrever os custos com procedimentos de saúde de idosos hipertensos e diabéticos e verificar qual a sua associação com NAF e EN, segundo sexo e grupos etários. Métodos A amostra foi constituída por 806 idosos com autorreferência à hipertensão e/ou diabete ( 60 anos) residentes no município de São PauloSP, participantes das três coortes do Estudo Saúde, Bem-estar e Envelhecimento SABE - em 2010. A variável dependente custo total anual (em Reais), foi estimada com base nos dados autorreferidos sobre uso de medicamentos, uso dos serviços ambulatoriais e internações hospitalares, retroativos a um ano da coleta de dados. A variáveis explanatórias: i) NAF foi estimada a partir de entrevista utilizando o International Physical Activity Questionnaire (IPAQ, versão curta), classificando os idosos segundo duração da realização de atividades físicas moderada, em ativos ( 150 minutos/semana) e insuficientemente ativos (< 150 minutos/semana); ii) Prática de caminhada, categorizada segundo frequência semanal: a) 4 dias/ semana; b) 1 a 3 dias/semana; c) não caminha. iii) EN, identificado pelo índice de massa corporal (IMC), classificando os idosos em dois grupos: a) IMC < 28 kg/ m²; b) IMC 28 kg/ m² (excesso de peso); as variáveis de controle foram o sexo, grupos etários (a. 70 anos; b. 65 a 69 anos; c. 60 a 64 anos); estado civil (a. casado; b. outros) e, escolaridade (a. sem escolaridade; b. 1 ano). A descrição dos custos segundo as NAF e EN foi representada pelos valores de média e IC95 por cento , mediana e P25 P75, valores mínimos e máximos. Modelos de regressão logística múltipla foram empregados para analisar as associações entre variáveis dependentes e explanatórias. O nível de significância foi estabelecido em 5 por cento e todas as análises foram realizadas considerando amostras complexas, por meio do software Stata, 13.0. 9 Resultados: A média de custo total anual por pessoa foi de R$ 732,54 e a soma dos custos relativa a 12 meses para os 806 idosos foi de R$ 609.587,20, sempre superiores para idosos em excesso de peso, com NAF insuficiente e para idosos que não caminham. Idosos em excesso de peso apresentaram chance 50 por cento superior de estarem no grupo de maior custo total anual (OR 1.49, IC95 por cento 1.01 2.18) e mais de 70 por cento superior de maior custo com medicamentos (OR 1.71, IC95 por cento 1.18 2.47). A ausência de caminhada significou a chance superior para maiores custos anuais com medicamentos (OR 1.63, IC95 por cento 1.06 2.51) e custos totais (OR 1.82, IC95 por cento 1.17 2.81). Todas as análises ajustadas por sexo e idade. O NAF não se associou aos custos totais e custo com medicamentos (p>0.05). Conclusão: Os custos para o controle de HAS e DM em idosos são altos e se associam inversamente à prática de caminhada e ao estado nutricional, especialmente em relação ao custo com o uso de medicamentos antihipertensivos e hipoglicemiantes. / Background- The insufficient physical activity level (PAL) as well the inadequate nutritional status (NS) increases the risk of hypertension and the development of diabetes, and difficult the control of theses diaseases. Thus, we can infer that the costs spent by the public health system to the management of hypertension and diabetes in the population would shows an inverse relationship with physical activity level, including engagement in walking, and nutritional status. However, epidemiological studies describing these costs and analyzing these associations among elderly population are non-existent in Brazil, which makes difficult the implementation of public policies for the economy of resource. Aim - To describe the health care costs among hypertensive and diabetic elderly people and to analyze the associations with PAL and NS according to sex and age groups. Methods - The sample consisted of elderly people with self-reported hypertension and/or diabetes ( 60 years-old) living in São Paulo-SP, participants of the three cohorts SABE Study (Health, Wellbeing and Aging), in 2010. The dependent variable (total annual cost - in Reais, R$) was estimated based on self-reported use of medication, use of outpatient services and hospital admissions, retroactive to one year of data collection. The explanatory variables: i) PAL was estimated from interview using the International Physical Activity Questionnaire (IPAQ, short version), classifying the elderly accorind to the time performing in moderate physical activity ( 150 minutes / week) and insufficiently active (< 150 minutes / week). ii) Engagement in walking, categorized according to the weekly frequency: a) 4 days / week; b) 1 to 3 days / week; c) does not walk. a) BMI < 28 kg/m2; b) BMI 28 kg / m² (overweight); the control variables were sex and age groups: a) 70 years; b) 65-69 years; c) 60 to 64 years. The description of the costs according to the PAL and NS was represented by mean values and 95 per cent , median and P25 -P75, minimum and maximum values. Multiple logistic regression models were emplyed to analyze associations. The level of significance was set at 5 per cent and all analyzes were performed considering complex samples, using the Stata software, 10.0. Results - The average of total annual cost was R$ 732.54 and the sum of costs on 12 months was R$ 609,587.20, always higher for elderly with excess weight, insufficient PAL and the elderly who do not engage in walking. The group of elderly with excess weight showed a 50 per cent chance of 11 being in the highest annual total cost group (OR 1:49, 95 per cent 1:01 to 2:18) and 70 per cent higher for the costs with medicine use (OR 1.71, IC95 per cent 1.18 2.47). The absence of walking had higher chance for highest costs with medication (OR 1.63, 95 per cent CI 1:06 to 2:51) and total costs (OR 1.82, 95 per cent CI 1.17 - 2.81). All analyzes adjusted for sex and age. The PAL was not associated to the total costs and cost with medication. Conclusion- The costs related to the control of hypertension and diabetes in the elderly are high and were inversely associated to the engagement in walking and nutritional status, especially in relation to the costs with the use of antihypertensive and hypoglycemic medication.

A incidência de diabete melito (DM) tem aumentado, principalmente em idosos. Evidências epidemiológicas mostram que obesidade e gordura abdominal constituem fator de risco para o desenvolvimento de DM. Objetivo: Verificar a associação da incidência de DM referida com obesidade e gordura abdominal, em coorte de idosos domiciliados no município de São Paulo/Brasil - 2000 e 2006. Casuística e métodos: Foram utilizados dados do Estudo SABE Saúde, Bem-estar e Envelhecimento, longitudinal, epidemiológico, de base domiciliar, realizado no município de São Paulo, em 2000 (2.143 idosos), e em 2006 (1.115 idosos). A população de estudo foi constituída por idosos ( 60 anos), de ambos os sexos, selecionados por amostra probabilística, e que não referiram DM, em 2000, e que apresentaram todos os dados necessários a este estudo. As variáveis analisadas foram: DM referida, com alternativa de resposta dicotômica (sim ou não), obesidade, pelo índice de massa corporal - IMC 30 kg/m², gordura abdominal (circunferência da cintura CC 88 cm, para mulheres, e 102 cm, para homens; e razão cintura/quadril RCQ 1, para homens, e 0,85, para mulheres) e características sociodemográficas (sexo, grupo etário, escolaridade, companhia no domicílio). Para verificar a associação entre as variáveis, utilizou-se teste Rao Scott, para amostra complexa, e regressão logística múltipla (p<0,05) e o programa Stata/SE 10.0 for Windows. Resultados: Dos 1.115 idosos, foram reavaliados 914, sendo que 72 constituíram os novos casos de Manuela F. de Almeida DM (7,7 por cento /1.000 pessoa/ano). Verificou-se que a maior proporção (58 por cento ) de indivíduos que referiram DM, era composta por mulheres. Ainda que a obesidade tenha se associado, positivamente, com referência de DM, constatou-se que a RCQ (OR=2,32; IC=1,47-3,67) e CC (OR= 2,33; IC=1,44-3,77) foram preditoras para DM referida. Conclusão: De todas as variáveis analisadas, apenas a gordura abdominal constituiu fator de risco para incidência da doença, no período de 6 anos / The incidence of diabetes mellitus (DM) has increased, mainly in aged persons. Epidemiological evidences show that obesity and abdominal fat constitute risk factor for development of DM. Objective: To verify the association the incidence of DM with obesity and abdominal fat, in cohort of elderly domiciled in São Paulo / Brazil - 2000 and 2006. Casuistic and methods: It were analized data of the SABE Survey, a longitudinal, epidemiologic and household based study, carried in the city of São Paulo, in 2000 (2,143 elderlies) and 2006 (1,115 elderlies). The study population was constituted by aged ( 60 years), of both gender, selected by probabilist sample, who did not reported DM, in 2000, with all necessary data to this study. The analized variables were: reported DM, with dicotomic answer (yes or no), obesity, by body mass index - BMI 30 kg/m², abdominal fat (waist circunference - WC 88 cm, for women, and 102 cm, for men; and waist-to-hip ratio WHR 1, for men, and 0.85, for women) and socio-demographics characterstics (gender, age group, educational status and home company). To verify the association among the variables, it was applied the Rao Scott test, for complex samples, multiple logistic regression (p<0.05) and statistical software Stata/SE 10.0 for Windows. Results: Out of the 1,115 elderlies, it were re-assessed 914, being 72 as new cases of DM (7.7 per cent /1,000 people/year). It was verified that a greater proportion (58 per cent ) of the subjects who reported DM, was Manuela F. de Almeida composed by females. Even though obesity was positively associated to reported DM, WHR (OR=2.33; IC=1.47-3.77) and WC (OR= 2.32; IC=1.44-3.67) were predictors for DM. Caractérisation et étude d'un élément régulateur du gène codant pour le récepteur à la vasopressine de type 2

Thèse réalisée en cotutelle avec l'Université Pierre et Marie Curie, Paris VI, France / Le contrôle de la transcription constitue le principal niveau de la régulation de l’expression des gènes dans les cellules eucaryotes. Dans le génome de ces derniers, les éléments régulateurs peuvent être localisés à de très grandes distances du gène qu’ils régulent. Le laboratoire a identifié 6 familles indépendantes avec un diabète insipide néphrogénique (DIN) lié à l’X portant de grandes délétions en amont du gène de l’AVPR2. Dans chacune de ces familles, les gènes AVPR2 et AQP2 ont été retrouvés intacts et les hommes sont atteints de DIN lié à l’X dans sa forme rénale « classique ». Le séquençage et l’analyse de 30 et 31 kilobases en amont et en aval de l’AVPR2 ont permis l’identification de 6 zones délétées chez 6 familles indépendantes, dont 5 zones de taille supérieure à 7 kilo bases, et une zone, de 102 paires de bases, commune à l’ensemble des délétions. Chez le patient porteur de cette délétion, l’osmolalité urinaire ne répond pas au dDAVP. Contrairement à ce qui est observé chez les patients atteints de DIN avec mutations de l’AVPR2, celui-ci présente des réponses hémodynamiques et de coagulation, normales. Ceci indique que les récepteurs V2 ne sont pas exprimés dans le tubule collecteur mais le sont au niveau des cellules endothéliales. Le but de notre travail est donc de tenter de comprendre les mécanismes régulateurs du locus de l’AVPR2, et plus précisément d’étudier l’expression « tissu spécifique » de ce gène. Les études réalisées in vivo, dans le système Hprt, confirment le rôle activateur de la séquence de 102 pb : coloration intense des tubules collecteurs avec la construction comportant la zone délétée et absence avec la construction ne la contenant pas. Cependant, les expériences menées in vitro semblent indiquer que cet effet dépende du contexte extracellulaire, isotonique ou hypertonique, de la nature des cellules, du tubule proximal ou collecteur, ainsi que du promoteur de l’AVPR2. L’identification des protéines liant potentiellement l’une des extrémités de la délétion a révélé la présence, soit de protéines régulatrices, soit de séquences inconnues, toutes exprimées dans le rein. À terme, ces études, ainsi que celles en découlant, permettront de positioner l’AVPR2 comme une cible de choix dans le traitement des diabètes insipides, centraux et néphrogéniques, par thérapie génique. / Transcriptional control is the primary means of regulating genes expression in eukaryotes cells. In the genome of the latter, regulatory elements can be localised with very long distance from the gene which they control. The laboratory identified six independent families with X-linked nephrogenic diabete insipidus (NDI) bearing large deletions upstream of the AVPR2 gene leaving intact AVPR2 and AQP2 coding sequences. Males bearing these deletions have classical renal X-linked NDI. The sequencing and analysis of 30 and 31 kilo bases upstream and downstream, respectively, encompassing the AVPR2 gene had led to identify 6 deletions in 6 ancestrally independent families including, 5 larger than 7 kilo bases and one of 102 base paires shared by the other deletions. In male patient bearing the 102 bp upstream deletion, urinary osmolality was unresponsive to dDAVP but, unlike patients with mutations in the coding sequence, their coagulation and hemodynamic responses to dDAVP were normal. This suggests that V2 receptors are not expressed in renal collecting duct cells but normally expressed in endothelial cells. Our goal is thus to understand the regulatory mechanism controlling the AVPR2 locus and more precisely the tissu specific expression of this gene.. The studies carried out in vivo, in the Hprt system, confirm the enhancer role of the sequence of 102 bp: intense coloration of the collecting tubules with construction comprising the deleted zone and abscence with construction not containing it. However, in vitro undertaken experiments seem to indicate that this effect depends on the extracellular context, isotonic or hypertonic, of the nature of the cells, of the tubule proximal or collecting duct, as well as promoter of the AVPR2. The identification of proteins potentially binding one of the ends of the deletion revealed the presence, either of regulating proteins, or of unknown sequences, all expressed in the kidney. In the long term, these studies, like those while rising, will make it possible to position the AVPR2 gene like a target of choice in the treatment of the diabetes insipidus, central and nephrogenic, by genic therapy.

INTRODUÇÃO: acredita-se que o controle glicêmico inadequado, a duração do diabetes melito (DM) e a presença de hipertensão arterial e dislipidemia sejam os fatores de risco mais importantes para o desenvolvimento das complicações microvasculares no DM, contudo, existem inúmeras evidências sugerindo que uma predisposição genética participe da suscetibilidade para o desenvolvimento dessas complicações. Vários genes relacionados aos mecanismos dos danos induzidos pela hiperglicemia têm sido investigados. O papel do estresse oxidativo na patogênese das complicações crônicas do DM vem sendo demonstrado e os genes que codificam enzimas que participam dos mecanismos antioxidantes são candidatos a conferirem suscetibilidade ou proteção contra as complicações crônicas. A glutationa é um dos mais importantes antioxidantes endógenos; no entanto, a associação entre polimorfismos em genes que codificam enzimas que participam desse sistema e complicações crônicas do DM foi pouco explorada na literatura. OBJETIVOS: avaliar a associação de polimorfismos em três genes que codificam enzimas relacionadas ao metabolismo da glutationa com o desenvolvimento de nefropatia e retinopatia em pacientes diabéticos tipo 1. Foram estudados: o polimorfismo -129C/T do gene GCLC, o número de repetições do trinucleotídeo GCG no exon 1 do gene GPX1 e o polimorfismo -65T/C do gene GPX3. CASUÍSTICA E MÉTODOS: 299 pacientes (139 do gênero masculino e 160 do gênero feminino) com DM tipo 1 com mais de 15 anos de diagnóstico e mau controle glicêmico foram divididos conforme presença ou ausência das seguintes complicações: nefropatia diabética (ND) avançada, ND, doença renal crônica (DRC) estágios 3 a 5 e retinopatia diabética proliferativa (RDP). Em cada grupo foram avaliadas as freqüências das variantes alélicas dos três genes estudados. RESULTADOS: a distribuição dos genótipos na população estudada foi consistente com o equilíbrio de Hardy-Weinberg para os três genes analisados. A presença de pelo menos um alelo T do polimorfismo -129C/T do gene GCLC conferiu risco independente para a presença de ND avançada (OR = 2,82 ; IC 95% = 1,13 - 7,05; p = 0,026), para ND (OR = 3,64; IC 95% = 1,27 10,36; p = 0,016) e para DRC estágios 3 a 5 (OR = 5,74; IC 95% = 2,17 15,1; p < 0,001) e a presença de pelo menos um alelo C do polimorfismo -65 T/C conferiu risco independente para a presença de ND avançada (OR = 2,62; IC 95% = 1,19 -5,72, p = 0,022) na população estudada. Não houve associação do número de repetições do trinucleotídeo GCG do gene GPX1 com nenhuma das complicações estudadas. O haplótipo CC_TT, composto pelos alelos selvagens dos genes GCLC e GPX3, foi negativamente associado com ND avançada (OR = 0,32, IC 95% = 0,15 0,66; p = 0,002) e DRC (OR = 0,25; IC 95% = 0,11 - 0,55; p = 0,001). CONCLUSÕES: a presença de pelo menos um alelo T do polimorfismo -129 C/T do gene GCLC e de pelo menos um alelo C do polimorfismo -65 T/C do gene GPX3, ambos associados a uma menor atividade transcricional do respectivo gene, conferiram risco para a presença de complicações renais na população de pacientes estudada. / INTRODUCTION: glycemic control, diabetes duration, systemic hypertension and dyslipidemia have been implicated as main risk factors for the development of diabetic microangiopathy, however there is evidence suggesting that genetic predisposition plays a role in the susceptibility to microvascular complications. Based on underlying pathogenesis, polymorphisms of several genes belonging to multiple pathways have been investigated, like the genes related to mechanisms of hyperglycemia-induced damage. The role of oxidative stress in the pathogenesis of diabetic complication has been increasingly demonstrated and genes coding enzymes involved in antioxidant defense are candidates to confer susceptibility or protection against these complications. Glutathione is one the most important endogen antioxidants, however, the association between polymorphisms in genes related to glutathione metabolism and diabetic complications has not been deeply investigated. OBJECTIVES: to study the association between polymorphisms in three genes which code enzymes related to glutathione metabolism and the development of nephropathy and retinopathy in type 1 diabetic patients: the polymorphism -129 C/T of GCLC, the number of trinucleotide GCG repeats at exon 1 of GPX1 and the polymorphism -65 T/C of GPX3. CASUISTIC AND METHODS: 299 type 1 diabetic patients (139 male and 160 female) with at least 15 years from diagnosis and poor glycemic control were studied. The patients were divided in two groups according to the presence or absence of diabetic complications: with and without diabetic nephropathy (DN), advanced DN, chronic kidney disease (CKD) stages 3 to 5 and proliferative diabetic retinopathy (PDR). RESULTS: The allelic distribution of the three studied polymorphisms was consistent with Hardy-Weinberg equilibrium. The presence of at least one T allele of GCLC 129 C/T was an independent risk factor for advanced DN (OR = 2.82 ; CI 95% = 1.13 -7.05; p = 0.026), for DN (OR = 3.64; CI 95% = 1.27 10.36; p = 0.016) and for CKD stages 3 to 5 (OR = 5.74; CI 95% = 2.17 15.1; p < 0.001) and the presence of at least one C allele of GPX3 -65 T/C was an independent risk factor for advanced DN (OR = 2.62; IC 95% = 1.19 -5.72, p = 0.022) in the studied population. There were no associations between GCG trinucleotide repeats of GPX1 and diabetic complications. The haplotype CC_TT, composed by GCLC and GPX3 wild type alleles, was negatively related to advanced DN (OR = 0.32, CI 95% = 0.15 0.66; p = 0.002) and CKD (OR = 0.25; CI 95% = 0.11 0.55; p = 0.001). CONCLUSIONS: the presence of at least one T allele of -129C/T polymorphism of GCLC and one C allele of -65 T/C polymorphism of GPX3, both associated to a lower transcriptional activity of its genes, conferred risk for renal complications in the studied population.

Influência do diabete melito na morbidade e mortalidade da insuficiência renal aguda em unidade de terapia intensiva

Despite the development in treatment, mortality remains high, varying beetween 50 and 70%. In hospitalised patients the incidence is about 5% but in respect to intensive care units it varies from 10 to 30%. In the last three decades, the characteristics of patients who suffer from acute renal failure changed dramatically. Before the advent of dialytic treatment, the main causes of mortality were uraemia, hyperkalaemia and the cardiac complications arising from volume overload. Nowadays, the causes are sepsis, cardiopulmonary failure, nephrotoxic drugs, and post renal transplantation complications. Multiple organ dysfunction, disseminated intravascular coagulation and diabetes mellitus are morbid conditions that can aggravate the prognosis of acute renal failure in intensive care units. Due to the high prevalence of diabetes mellitus in the population, this study intends to evaluate its influence in the morbidity and mortality of patients suffering from acute renal failure in the intensive care unit of Hospital de Base of São José do Rio Preto, Brazil was made in the period from January 1997 to December 2000. A total of 255 (25%) of the patients were diabetic and 765 (75%) were not. Demographic data, the presence of underlying diseases, aetiology, types, the clinical features and complications of acute renal failure were evaluated. Besides these, the presence of multiple organ failure syndrome was observed. In the study population 64% were male, 46% were more than 60 years old and 85% had one or more concomitant diseases. The ischaemic aetiology predominated in 53% of cases and a clinical cause was the most common type seen at 57%. The means and standard deviations of the Apache II score and creatinine levels (mg/dL) were 20.5 + 6.7 and 3.7 + 2.0 respectively. The prevalence of disseminated intravascular coagulation, shock, liver failure and respiratory failure were 32%, 69%, 15% and 79% respectively. Among the observed complications hyperkalaemia was seen in 35%, acidosis in 70%, sepsis in 61%, systemic arterial hypertension in 14%, bleeding in 22%, central nervous system disfunction in 44% and mortality in 71% of the cases. The demographic data, clinical features, morbidity and mortality due to acute renal failure of diabetic and non-diabetic individuals were compared. The hyperkalaemia, acidosis, respiratory failure, shock, central nervous system dysfunction, hypervolaemia and the bleeding were similar in both groups. A logistic regression analysis did not demonstrate a significant association between diabetes mellitus and mortality. An ischaemic aetiology, the failure of three or more organs, hyponatraemia and acidosis exhibited significant association between mortality and acute renal failure. In conclusion, the diabetic patients were older involving fewer men, with less oliguria, disseminated intravascular coagulation, hyponatraemia and liver failure than the non-diabetic individuals. Diabetes mellitus had no influence on the mortality due to acute renal failure in the intensive care unit. / A insuficiência renal aguda pode ser definida como uma redução abrupta e sustentada da taxa de filtração glomerular com conseqüente retenção de produtos nitrogenados. Apesar da evolução terapêutica, a sua mortalidade ainda continua elevada, variando entre 50 e 70%. Em pacientes hospitalizados, a sua incidência está próxima de 5% e, especificamente, em unidades de terapia intensiva, varia entre 10 e 30%. Nas últimas três décadas, as características dos pacientes acometidos de insuficiência renal aguda alteraram-se profundamente. Antes do advento do tratamento dialítico, as principais causas de mortalidade eram a uremia, a hipercalemia e as complicações cardiológicas decorrentes da sobrecarga de volume. Atualmente, são a sepse, a insuficiência cardiopulmonar, drogas nefrotóxicas e complicações pós-transplante renal. Quanto ao prognóstico de insuficiência renal aguda em unidades de terapia intensiva, a disfunção de múltiplos órgãos, a coagulação intravascular disseminada e o diabete melito são condições mórbidas que podem piorar a sua evolução. Devido à alta prevalência de diabete melito na população, o presente trabalho propôs-se avaliar a sua influência na morbidade e mortalidade da insuficiência renal aguda em unidade de terapia intensiva. Para tal, foram estudados, retrospectivamente, 1020 pacientes com insuficiência renal aguda internados na unidade de terapia intensiva do Hospital de Base de São José do Rio Preto, Brasil, no período de janeiro de 1997 a dezembro de 2000, dos quais, 255 (25%) eram diabéticos e 765 (75%) não diabéticos. Foram avaliados dados demográficos, presença de doenças de base, etiologia, tipos, quadro clínico e complicações de insuficiência renal aguda e ainda a presença de síndrome de disfunção de múltiplos órgãos. Entre a população estudada, 64% eram do sexo masculino, 46% tinham mais de 60 anos de idade, e 85% tinham uma ou mais doenças concomitantes. Nota de Resumo A etiologia isquêmica predominou com 53%, e a causa clínica foi o tipo mais freqüente com 57%. As médias e os desvios padrão de apache II e creatinina (mg/dL) foram 20,56,7 e 3,7+2 O respectivamente. A prevalência de coagulação intravascular disseminada, de choque, de insuficiência hepática e respiratória foi 32%, 69%, 15% e 79%, respectivamente. Entre as complicações, observamos a hiperpotassemia em 35%, a acidose em 70%, a sepse em 61%, a hipertensão arterial sistêmica em 14%, os sangramentos em 22%, a disfunção do sistema nervoso central em 44% e a mortalidade em 71%. Foram comparados, entre os diabéticos e os não diabéticos, os dados demográficos, quadro clínico, morbidade e mortalidade de insuficiência renal aguda. A hipercalemia, a acidose, a insuficiência respiratória, a sepse, o choque, a disfunção do sistema nervoso central, a hipervolemia e os sangramentos foram similares em ambos os grupos. A análise de regressão logística não mostrou associação significante entre diabete melito e a mortalidade. A etiologia isquêmica, a presença de três ou mais insuficiências de órgãos, a hiponatremia e a acidose foram de forma significante associadas com a mortalidade de insuficiência renal aguda. Em conclusão, os diabéticos foram mais idosos, menor prevalência de masculinos, menos oligúria, coagulação intravascular disseminada, hiponatremia e falência hepática do que os não diabéticos. O diabete melito não teve influência na mortalidade da insuficiência renal aguda em unidade de terapia intensiva.
