Bridging birth: the Birth Sisters as an adaptation to hospital birth

This study was designed to explore the mediated experience of supported birth in a hospital setting by examining the role of the Birth Sisters, with a focus on how the individual Birth Sisters perceived the impact of their role. The goal of my research is to understand the ways in which having a Birth Sister assist women in emerging from birth empowered and achieving better psychosocial and medical outcomes. I hope to contribute to the growing research on the role of doula support by providing a focused examination of the lived experiences of the women providing this support as part of a hospital setting. As such, this study examines the ways in which Birth Sisters act as a mitigating force to bridge the biocultural, cultural and structural aspects that emerge in a U.S. hospital birth.

Utilization of peripartum doulas for improvement of maternal mental health and social support of at-risk Hispanic women

Peripartum depression is the most common complication associated with childbearing yet there are many barriers to diagnosis and treatment. Motherhood is an especially vulnerable time for Hispanic women who experience additional barriers that prevent them from seeking maternal mental health care. Due to these barriers, researchers have increased their interest in prevention. They have found that low social support is a modifiable risk factor with one of the strongest predictive values. Doulas are specifically trained to provide physical, social, and emotional support to mothers during pregnancy, labor, and/or postpartum periods. In this context they may be considered a powerful tool to lower barriers and improve maternal psychosocial wellbeing. However, significant limitations from prior studies have prevented the acceptance of using doulas routinely for at-risk mothers. By identifying and incorporating components of other successful social support interventions in my literature review, I hope to eliminate some of these limitations. This study will select participants with low social supports and other risk factors to be randomized into home-visiting doula intervention group or standard of care control group. Maternal mental health and perceived social support will be evaluated from the first trimester to 6 months postpartum. If there are significant differences between these groups, there is a potential to increase doula training and recruitment for prevention of postpartum depression for minorities. Hopefully the results will guide law makers to increase insurance coverage and guide practitioners to routinely offer doula services for at-risk mothers who need additional supports to receive adequate mental health care. Finally, I aim to increase the perceived value of doulas in the setting of improving maternal mental health/ social wellbeing and increase their acceptance within the medical community.

As questões relativas ao parto e sua assistência apresentam-se como problemas relevantes de saúde pública. O enfrentamento das adversidades presentes na assistência à saúde materna tem envolvido diversas iniciativas resultando em avanços em várias direções. Os estudos sobre as práticas assistenciais em saúde vêm tematizando questões como a influência considerável da dimensão relacional na qualidade dos cuidados prestados. Este trabalho aborda tal dimensão no contexto da assistência ao parto. Objetivo: Descrever e analisar pontos de conexão entre as vivências das parturientes durante o trabalho de parto e parto e o suporte oferecido. Aspectos metodológicos: Estudo descritivo, qualitativo, utilizando técnica de entrevista semi-estruturada com parturientes e provedoras de cuidados (doulas) e observação de rotinas em maternidade de São Paulo, em 2004. Resultados: Verificou-se a relevância e a valorização das relações interpessoais no processo da parturição. Processo este referido pelas parturientes como experiência de elevado grau de estresse com vivências de dor, medo, angústia e ansiedade. Identificou-se também disposições subjetivas, entre as doulas, de solicitude, acolhimento e diálogo no exercício de provimento dos cuidados. / Issues related to childbirth and its appropriate care represent a relevant part of the public health concerns. When dealing with the various adversities that appear in the practice of the maternal health care, professionals of the area have launched many initiatives that resulted in advancements in various directions. Studies carried out on health care practices have been focusing questions such as the considerable influence of the dimension of interpersonal relationships on the quality of the services rendered. This paper approaches such dimension in the context of the childbirth care. Hypothesis: the beneficial effects coming from the emotional/psychological support provided for women during labor and birth depend on the quality of the interpersonal relationships established among the agents involved in the situation. Objective: The target of the present study is to describe and analyze the existing connecting points between the childbearing women\' s experiences during labor and birth and the suport they are provided for. Methodological aspects: This paper comprehends a qualitative/descriptive study that uses semi-structured interview techniques among with childbearing women and caregivers (doulas), besides observation of routine practices in maternity hospitals of São Paulo city. Results: Both the relevance and valorization concerning interpersonal relationships during the process of parturition were verified, such process being described by childbearing women as an experience that brings about a high degree of stress, with pain, fear, anguish, and anxiety playing important roles. During the provision of care, subjective dispositions of solicitude, acceptance, and dialogue were also detected among doulas. Final considerations: Data analysis enabled us to consider the intersubjective space of relationships as the point where interlocutions take place, being marked by the listening of the childbearing women\' s demands and the acceptance and reassurance of the processes they are experiencing, thus characterizing it as a resource equivalent to those prescribed as technologies appropriate to the birth care.

"It's About the Patient" : Perspectives of Intrapartum Nurses on Working in Collaborative Practice with Birth Doulas

Birth doulas offer continuous physical, emotional, and informational support before, during, and after birth. There is strong evidence of the maternal health benefits associated with continuous labour support. Nurses are the most common intrapartum care provider and navigate the overlapping labour support role with doulas. While there is evidence of mutual respect, conflicts can occur with negative consequences. This study aimed to explore collaborative practice between nurses and doulas from the perspective of intrapartum nurses in Ontario, Canada. Thorne's qualitative interpretive description (ID) methodology was used. Nurses experienced in working with doulas were recruited via email invitations and postings on a nursing online forum. Semi-structured 1:1 interviews were conducted with six nurses. Experience ranged from 4 to 35 years across nine hospitals. Interviews were recorded and transcribed verbatim. Data analysis was informed by the ID approach. Findings comprised two main themes, each with four sub-themes. First, participants described influences on nurses' readiness to collaborate with doulas. These influences include the culture clash between medicalized and natural birth, support from management and hospital policies and previous professional experiences. These led to the nurses' approach, "it's about the patient". Second, participants' responses reflected a continuum of nurse-doula collaboration, beginning with being open-minded, a necessity for getting on the patient's team, building trust and working together towards shared goals. These findings provide a framework for interventions to facilitate collaborative practice, including in-service training and enactment of appropriate institutional policies. Improved collaboration enhances both nurses' and doulas' practice while promoting better childbirth outcomes.

The study highlights the significant role that cultural doulas play in supporting and guiding migrant women during pregnancy, childbirth, and the postpartum period in Sweden. The Swedish healthcare system is understaffed, overburdened, and medically dominated, making it difficult for nurses and midwives to provide continuous and culturally-sensible care to their clients. Cultural doulas can significantly support their clients and the maternal healthcare system. However, improvements must be made in the working conditions of cultural doulas to create a sustainable project that can serve many migrant families in the future. The study emphasizes the need to recognize the significant contribution of cultural doulas to improving equitable maternal healthcare in ethnically diverse Sweden.

The thesis examines the importance of psychological support from a supplementary person provided to mothers during their pregnancy and mainly during the process of childbirth. Such person may be a partner, relative, midwife, doula, doctor, etc. The aim of this thesis is to find out why the mother chooses to give birth in the presence of the specific person, how important is the psychological (and sometimes also physical) support from an accompanying person for such woman during delivery, or what specific action the mother considers psychologically supportive during the pregnancy and childbirth. Theoretical part describes a psyche of the mum-to-be and her need for psychological support during the pregnancy and the delivery and names the types of support in relation to a person, who is supporting. Practical part will be based on qualitative research carried out as half-structured interviews with mothers, who delivered a baby with accompaniment.
