Alexitimia, ansiedade e depress?o em portadores de glaucoma / Depression, anxiety and alexithymia among glaucoma patients

To study the relationship of depression, anxiety and alexithymia symptoms of glaucoma patients. To evaluate patients with glaucoma at an ambulatory of ophthalmology on a Private Hospital, regarding their levels of depression, anxiety and alexithymia. To evaluate demographic aspects such as gender, age, scholarship, social and economic level and health items related to items studied. Subjects: There were 39 Glaucoma Patients, from ophthalmology ambulatory of Celso Pierro Hospital, whose voluntarily had been done their agreement to participate of the present research. Their age vary from 18 to 75 years old, both gender, different levels of scholarship and economic status. The data were collected consecutively, applying Toronto Alexithymia Scale, Beck Depression and Anxiety Inventories and Social and Demographic Questionnaire in an initial Exploratory Study. There were no differences among the sample except there was more female (66%) than male participants compounding the total sample. Regarding other aspects such as scholarship, marital state, social and economic level they were really similar, therefore homogeneous as group. The majority of the glaucoma patients have been presented alexithymia and minimal levels of depression and anxiety, which were expressed as high correlation between BAI and BDI scores. There was relationship between TAS-26 and BAI scores, and as lesser correlation, between TAS-26 and BDI. The hypotheses whose direction pointed to high level of depression and anxiety for glaucoma patients than people in general, and eventually alexithymia too, was not confirmed. In reality, there was alexithymia at this sample, and low level of anxiety and depression. However these low levels seem to be connected to long experience with their partial loss vision or their experience as normal viewer. / A presente pesquisa foi realizada para avaliar alexitimia, depress?o e ansiedade em portadores de glaucoma sob atendimento ambulatorial e como estas vari?veis encontram-se associadas. Avaliar tamb?m aspectos demogr?ficos tais como g?nero, idade, escolaridade,situa??o s?cio-econ?mico e itens relacionados a s?ude, considerando-se as vari?veis estudadas. Participantes: 39 pacientes do ambulat?rio de oftalmologia de Hospital e Maternidade Celso Pierro, com diagn?stico de glaucoma, idade entre 18 e 75 anos de idade, de ambos os g?neros (13 homens e 26 mulheres), diversos n?veis de escolaridade. Amostra ? Consecutiva do ponto de vista de sele??o. Trata-se de estudo explorat?rio sobre o tema. As medidas foram obtidas com a Escala de Alexitimia de Toronto TAS-26, o Invent?rio Beck de Depress?o- BDI e Invent?rio Beck de Ansiedade- BAI e o Question?rio S?cio-Demogr?fico. Com exce??o da vari?vel sexo, em que predominaram as mulheres, sob os demais aspectos a amostra revelou-se bastante homog?nea o que permitiu o estudo das vari?veis para todo o grupo. Os portadores de glaucoma apresentaram em sua maioria n?veis de alexitimia ( M=73,18, Dp= 11,56 ) compat?veis com os observados em amostras cl?nicas e com baixo n?vel de escolaridade. Foram observados baixos n?veis de ansiedade ( M=12,34, Dp=11,23) e depress?o ( M=12,31, Dp= 11,67) expressos em altos ?ndices de associa??o entre estas vari?veis. Estes baixos n?veis parecem relacionar-se com a estabilidade da situa??o visual desta amostra: perda visual parcial est?vel h? certo tempo, ou conserva??o de vis?o normal . Houve correla??o significante da TAS-26 com os escores da BAI ( r = 0, 49 , p< 0,01 bicaudal para amostra total e r = 0,55 , p<0,01 bicaudalpara amostra feminina ) e, em menor escala , com o BDI ( r =0,35 para amostra total ,p<0,05 bicaudal). E entre BAI e BDI verificou-se forte correla??o : r= 0, 68 para amostra total (p<0,001), r= 0,65 para amostra masculina e r= 0, 67 para amostra feminina (p <0,001). A hip?tese segundo a qual a amostra apresentaria ?ndices em m?dia mais elevados de depress?o, ansiedade, e eventualmente de alexitimia, que a popula??o geral, n?o se confirmou.

The research protocol was designed to collect data about infertility s emotional aspects and how women deal with them. The members of the group participated in a semi-structured interview and in a test using the Rorschach method. This study covered questions about the feminine principle, the initial phases of the human development and showed, briefly, some aspects of the role the woman have been playing in our society. The theory utilized in this study was based on the Analitical Psychology by Carl Gustav Jung, and the depressed and disphoric content, as well as the stress manifestation, were verified by using the projective test of Rorschach method, developed by Hermann Rorschach. The analisys of the results provided by the Rorschach method reinforced former studies results that pointed to the presence of stress and depression in women under assisted reproduction procedures. The categorization of the semi-structured interview data highlighted painful aspects and the strong impact these women suffered / Esta pesquisa foi realizada com um grupo de 15 mulheres inscritas no programa de Reprodução Assistida do Hospital Perola Byington, e objetivou investigar o psiquismo de mulheres inférteis através do método de Rorschach (Sistema Compreensivo). O protocolo de pesquisa foi elaborado de forma que pudesse coletar dados da vivência emocional da infertilidade e seu enfrentamento. As participantes foram submetidas a uma entrevista-semidirigida e ao método de Rorschach. Este estudo abordou questões acerca do princípio feminino, das primeiras fases de desenvolvimento humano, e apresentou de forma sucinta aspectos do papel que a mulher vem ocupando ao longo da história em nossa sociedade. O corpo teórico do presente estudo foi elaborado a partir dos pressupostos da Psicologia Analítica de Carl Gustav Jung, os conteúdos disfóricos e depressivos, e também a manifestação de estresse foram averiguados através do método de Rorschach, teste projetivo, elaborado por Hermann Rorschach. A análise dos resultados encontrados no método de Rorschach corroborou estudos anteriores que apontam a manifestação de estresse e depressão em mulheres submetidas a procedimentos de reprodução assistida. A categorização dos dados da entrevista semidirigida evidenciou aspectos dolorosos dessa vivência e o grande impacto sofrido por estas mulheres

This statement gives an overall summary of the aims and achievements of the research work and scholarship carried out by the author during her time at The University of Manchester (and UMIST - now part of The University of Manchester) for which the publications presented give evidence. The research has been about exploring the design process, the activities and issues, and elements involved - from both an industry and student point of view. The publications explore design pedagogy, the skills required by designers and how these might fit into a curriculum for design today.In three parts it summarises the publications presented, reviews the main aspects of design and the current state of knowledge and research in design and summarises the core aspects as distilled from over 36 years practice, research and scholarship.The driver for much of the research undertaken has been to gain a better understanding of the core aspects of design - what key knowledge and skills are required by designers to allow the consistent design of better products and services which enhance the experiences of users. The work presented investigates design and design methods: the activities and processes and the elements involved. It considers responses to designs, the emotional aspect of design - why some designs are preferred over others, why some colour combinations are more desirable, and why repetition is so important to the human psyche. Underpinning the work presented are three research questions. • Are design rules and processes generic for whatever is being designed? • Can a better understanding of design theory and the emotional response to designs ensure a more effective process and thus lead to stronger designs? • Can students be educated to be better design thinkers and ultimately better designers? It concludes that: • 'design' is a process; • design is a problem-solving process and problem-solving is a design process; • for the most effective outcomes a creative and structured approach is required; • this process is based on generic rules and principles which are applicable across all discipline areas; • collaborative/cross disciplinary elements reinforce the concept that there are processes involved that are not unique to individuals or discipline specific; • a greater understanding of the process is of benefit to all individuals and organisations; • any design/problem solving activity will normally result in more than one solution option. The results of the research have informed the author's teaching practice and have been disseminated through publications to benefit the wider education arena. The work presented aims to inform students and design education practitioners.

Paauglių fizinio aktyvumo, subjektyvaus sveikatos vertinimo ir emocinių bei elgesio ypatumų sąsajos / The link between teenagers’ physical activeness and the subjective health evaluation and the emotional, behavioural aspects

Tyrime dalyvavo 283 tiriamieji (138 vaikinai ir 145 merginos). Tiriamąją imtį sudarė Kauno m. ir Kauno rajono vidurinių mokyklų 9 -11 klasių moksleiviai. Paauglių fizinio aktyvumo lygis buvo vertinamas naudojant „Modifikuotą tarptautinį fizinio aktyvumo klausimyną“ (IPAQ- International Physical Activity Questionnaire; Ainsworth, Levy, 2004; Volbekienė, 2006), emociniams bei elgesio ypatumams nustatyti naudotas „11-18 metų jaunuolio savęs vertinimo klausimynas“ (ASEBA – YSR 11/18, Achenbach System of Empirically Based Assesment - Youth Self Report; Achenbach, 2001; Žukauskienė, 2006). Sveikata vertinta darbo autoriaus sudarytu klausimynu. Tyrimo rezultatai parodė, kad paauglių fizinis aktyvumas susijęs su sveikata ir kai kuriais emociniais bei elgesio ypatumais: fiziškai aktyvesni paaugliai geriau vertino savo sveikatą, jų nerimo/ depresijos išreikštumas buvo žemesnis lyginant su fiziškai pasyvesnių paauglių grupe. Didėjant vaikinų fiziniam aktyvumui jų agresyvumas didėja. Fizinio aktyvumo lygis neturėjo ryšio su merginų agresyvumu, paauglių delinkventiniu elgesiu ir užsisklendimu. / The study was conducted among 283 teenagers (138 boys and 145 girls). The selection was made from the 9-11th form students in Kaunas City and District secondary schools. The level of physical activeness was measured according to The International Physical Activity Questionnaire (Ainsworth, Levy, 2004; trans by Volbekiene, 2006). Emotional and behavioural aspects were evaluated using The Questionnaire of Self Assessment for 11-18 year olds (ASEBA- YSR 11/18, Achenbach, 2001, trans by Zukauskiene, 2006). Health levels were assessed using a questionnaire constructed by the author of the study. The results of the study have indicated the link between teenagers’ physical activeness and their health, some emotional and behavioural aspects: more physically active teenagers evaluated their health better; their anxiety/depression indicators were lower compared to those of the less physically active. Whilst the aggression in boys grew respectively with rising physical activeness, such a link wasn’t found in girls. No connection between physical activeness and teenagers’ delinquent behaviour or shyness was determined.

The family is fundamental in the treatment of the patient: It represents the foundation that will influence in the acceptance or not of the disease by the carrier. Because of this, the objectives of this study were described, the health beliefs of parents of carrying children with diabetes mellitus type 1 and understanding behaviors and psychics change that will possibly influence in the relation of the treatment. 13 people were investigated, 11 month to 10 years old children&#146;s parents with Diabetes Mellitus Type 1, through an interview for attainment the health beliefs. The analyses of the data had been carried through the basis of the model of &#147;health beliefs&#148;. This model analyzed: impact of the diagnosis, susceptibility, severity, benefits, barriers, self-efficacy and present expectations of the future. The results had shown that the parents had difficulties, fears and unreliabilities, for the illness of their children. When telling the situations lived since the diagnosis until the current moment, in all the stages, the parents disclose intense suffering. They constantly are invaded from fear of loss, in the present as in the future in function of the complications of the illness. From these results is recommend that the parents receive a support from a team of health professionals with specific knowledge and with purpose to inform on the illness and how to minimize the fears and unreliabilities that create obstacles for the adhesion to the treatment. We expected with this type of support improve the quality of life of the patient and their family. / O Diabetes Mellitus tipo 1 geralmente ocorre na infância ou adolescência e repercute de forma dramática na vida dos pais. A família é fundamental no tratamento do paciente: representa o alicerce que influenciará na aceitação ou não da enfermidade por parte do portador. Por isso, os objetivos deste estudo foram descrever as convicções de saúde de pais de crianças portadoras de diabetes mellitus tipo 1 e compreender mudanças comportamentais e psíquicas que possam influenciar na conduta em relação ao tratamento. Investigou-se 13 pessoas, pais de crianças de 11 meses a 10 anos portadoras de Diabetes Mellitus Tipo 1, por intermédio de uma entrevista para levantamento e descrição de fatores de convicção de saúde. Os dados foram avaliados com base em um modelo de &#147;convicção de saúde&#148;. Esse modelo avaliou: impacto do diagnóstico, suscetibilidade, severidade, benefícios, barreiras, eficácia própria e expectativa de futuro de cada um dos pais. Os resultados mostraram que os pais experimentam dificuldades, medos e inseguranças, pela doença do filho. Ao relatarem as situações vividas desde o diagnóstico até o momento atual, em todas as etapas, os pais revelam intenso sofrimento. Eles são constantemente invadidos por medo de perda tanto no presente como no futuro em função das complicações da doença. A partir desses resultados recomenda-se que os pais recebam atendimento de uma equipe multidisciplinar com conhecimento específico e com a finalidade de informar sobre a doença e aplacar os medos e inseguranças que criam obstáculos para a adesão ao tratamento. Espera-se com este tipo de atendimento melhorar e a qualidade de vida do paciente e de sua família.

A queda e suas consequências para o idoso : aspectos psicológicos e emocionais

The falls among the elderly constitute an important clinical and public health problem due to its high incidence with consequent elevated mortality and morbidity on this population, causing a great impact on their life, on their relatives lives and on the public health system, due to the high costs of assistance. This study aimed at investigating the fall and its consequences for the elderly people, considering the psychological and emotional aspects. Thirty two people were interviewed, 25 women and seven men, aging 60 to 98 years-old, all residents in the city of Uberaba - MG, selected at four different places: two rehabilitation centers, physiotherapy clinic, aquatic academy and two long permanence institutions. Most of the group was retired, single, with low education level and lived predominantly in multigeneration families. The participants were interviewed about their personal data and health habits. The questionnaire was composed by 20 questions about the fall event under several aspects. The data received statistical treatment and they were analyzed quantitatively and qualitatively. In the results, it was verified that the average of falls by person was 3,21; women presented a bigger relative value than men. The falls occurred mostly in the external areas of the homes, and during the day. Most of the elders had a positive conduct in front to the event, adopting preventive measures. The psychological and emotional consequences most related were the fear (of falling, of leaving alone and other fears), the increase of the attention and care. The results suggest there is an enormous need to produce information about some risk factors for falls which the elderly people are exposed daily; health professionals should provide more orientation concerning posture; and there is a need of developing significant, but low costs practices in the primary level of attention to the elder's health. / As quedas entre pessoas idosas constituem um importante problema clínico e de saúde pública, devido a sua alta incidência com conseqüente mortalidade e morbidade elevadas nesta população, causando um grande impacto na vida dos idosos, familiares e sistema público de saúde, gerando altos custos assistenciais. O presente estudo teve como objetivo principal investigar a queda e as suas conseqüências para o idoso, sob o enfoque dos aspectos psicológicos e emocionais. Para realização deste estudo foram entrevistados 32 idosos, 25 mulheres e sete homens, com idades de 60 a 98 anos, todos residentes na cidade de Uberaba MG, selecionados em seis locais diferentes; dois centros de reabilitação, consultório de fisioterapia, academia aquática e duas instituições de longa permanência. A maioria do grupo era de aposentados, solteiros, com baixa escolaridade e residiam predominantemente em famílias multigeracionais. Os participantes foram questionados acerca de seus dados pessoais e hábitos de saúde. Além disso, foram feitas 20 perguntas que abordavam vários aspectos do evento queda. As respostas a essas questões foram categorizadas com base em estudos gerontológicos sobre envelhecimento normal e patológico, sobre a queda e suas conseqüências biopsicossociais e na psicologia do desenvolvimento life-span. Os dados receberam tratamento estatísticodescritivo e foram analisados qualitativamente. Nos resultados pode ser verificado que a média de quedas por idoso foi igual a 3,21, sendo que as mulheres apresentaram um valor relativo maior que os homens. As quedas ocorreram, na maiora das vezes, nas áreas externas dos domicílios e no período diurno. Os idosos, na sua maioria, tiveram uma conduta positiva frente ao evento, adotando medidas preventivas. As conseqüências psicológicas e emocionais mais relatadas foram o medo (de cair, de sair sozinho e outros medos), o aumento da atenção e do cuidado. Os resultados encontrados sugerem que há uma grande necessidade de se produzir informações sobre alguns fatores de risco para as quedas, aos quais os idosos estão expostos diariamente, sobre a necessidade de uma postura orientadora por parte dos profissionais da saúde e do desenvolvimento de práticas preventivas significativas de baixo custo, no nível primário de atenção à saúde do idoso. / Mestre em Psicologia Aplicada

The New Labour discourse of Social and Emotional Learning (SEL) across schools in England and Wales as a universal intervention : a critical discourse analysis

This thesis reports on a critical discourse analysis (CDA) of the SEL policy makers’ conversations taking place in England and Wales during the New Labour period. The research sets out to offer a critical explanation of Welsh and English SEL policy thinking and doing and how the SEL policy discourse worked to privilege certain ideas and topics and speakers and exclude others. Thinking with theory and building on the work of Apple (2007) and Ball (2012) I draw on the contemporary tenets of critical theory to examine the (dominant) English and (often subjugated) Welsh discourse(s) to historically locate and contextualise the mainstream SEL literature within the ideological discourse of neoliberalism (Harvey, 2005). This neoliberalism is one which unequivocally drives policy in the direction of markets and propounds a thorough marketization of educational provisions and practices (Lynch, 2006).Drawing on data from a series of eight semi structured interviews with key national level policymakers, alongside documentary analysis, I argue that New Labour in England, particularly in its second term, through a particular policy network and the SEAL programme, adopted SEL as a tool of managerialism designed to shape and govern a self-managing, entrepreneurial, placid subject in the service of the neoliberal economic model. Alternatively I contend that the Welsh assembly adopted SEL as a practical and progressive tool for developing a more equal society and a more egalitarian and democratic modus operandi of social justice (rooted in normative precepts of the collective and of community cohesion). This “Welsh” approach was powerfully intertwined with the devolution programme and notions of the child as a democratic citizen with agency and rights. In both England and Wales this understanding and application of SEL was intimately connected with national identity and notions of nationhood. This work was undertaken using a CDA approach. It employed Fairclough’s Three Dimensional Model (1992) of Critical Discourse Analysis and engaged with the subject and data through the three lenses of text (the written and spoken word), discursive practice (the production, distribution and consumption of the text) and social practice (the wider social, political and economic forces shaping the discourse). By illuminating through CDA the ideologically infused discursive claims to truth and value, which underpinned the rhetoric and substance of the UK (Anglo-centric) Government’s version of SEL in schools and that of the devolved Welsh Government, my findings reveal the broader scale ideas and political-ontological truth claims which drove the development of SEL across England and Wales during the New Labour period; the research therein unveils the implicit but reified notions of childhood and children’s wellbeing which were central to SEL development at both the national and devolved levels; it identifies the unspoken and latent ideological projects which were core to the production of divergent SEL discourses in each of the countries; and finally, it reveals the influence which national tradition, domestic power structures, cross-societal inequities and the subjugation of certain identities have had on the conceptualisation and practical delivery of SEL in England and Wales. The study concludes that the relationship between language and political ideology in England and Wales during the New Labour years powerfully shaped the SEL policy discourse. In England the result of this was a thin version of SEL co-opted into the service of the neoliberal marketplace. In Wales a similar outcome occurred but only after a very different contextualised and transformative version of SEL was relinquished due to the invasive neoliberal forces attacking Welsh education.

Devido ao aumento da população idosa ou com limitações físicas/mentais é possível observar o crescimento de estudos na área de Internet das Coisas com o objetivo de monitorar a saúde e auxiliar no gerenciamento e melhora na qualidade de vida dessa parte da população. Nesse sentido, a abordagem baseada em Internet das Coisas aplicada em ambientes médicos e casas inteligentes tem o objetivo de fornecer conectividade entre o paciente e o ambiente ao seu redor, provendo mecanismos para ajudar em diagnósticos e prevenção de acidentes e/ou doenças. Nesse contexto surge a oportunidade de explorar sistemas computacionais para identificar o estado físico e emocional, em tempo real, de indivíduos com alguma limitação para monitorar a saúde; por exemplo, identificar o comportamento da rotina do usuário e emitir alertas aos familiares e/ou equipe médica sobre algum evento anormal ou identificar indícios de distúrbios emocionais. Ainda, com base na Inteligência Artificial é possível que sistemas computacionais possam aprender e se adaptar ao contexto que se encontra, por exemplo aprender e se adaptar a quantidade de exercícios e/ou estado emocional do usuário em determinadas situações, combinando conceitos tanto de Internet of Things quanto de Inteligência Artificial. Assim, este projeto tem como objetivo desenvolver e avaliar um modelo que possa: i.) identificar o estado físico e emocional do usuário; ii.) prover um mecanismo que possa monitorar de maneira inteligente as atividades do cotidiano do usuário e; iii.) explorar a abordagem de integração de dados com a utilização de múltiplos sensores IoT para uma melhor interação entre dispositivos computacionais no ambiente Health Smart Homes. / Due to the increase in the elderly population or with physical/mental limitations it is possible to observe the growth of studies in the field of Internet of Things with the objective of monitoring health and assisting in the management and improvement of the quality of life of this part of the population . In this sense, the Internet of Things approach applied in medical environments and smart homes aims to provide connectivity between the patient and the environment around them, providing mechanisms to aid in diagnosis and prevention of accidents and/or diseases. In this context the opportunity arises to explore computational systems to identify the physical and emotional state, in real time, of individuals with some limitation to monitor health; for example, identifying the behavior of the users routine and issuing alerts to family members and/or medical staff about some abnormal event or identifying evidence of emotional disturbances. Also, based on Artificial Intelligence, it is possible for computational systems to learn and adapt to the context they are in, for example learning and adapting the amount of exercises and/or emotional state of the user in certain situations, combining concepts both of Internet of Things and of Artificial Intelligence. Thus, this project aims to develop and evaluate a model that can: i) identify the physical and emotional state of the user; ii) provide a mechanism that can intelligently monitor the daily activities of the user and; iii) explore the data integration approach with the use of multiple IoT sensors for better interaction between computing devices in the Health Smart Homes environment.
