Hur sjuksköterskor och patienter med fibromyalgi upplever vårdrelationen : En litteraturstudieJohnsson, Cecilia, Malmstedt, Johanna January 2015 (has links)
Fibromyalgi (FMS) är ett syndrom som främst karaktäriseras av kroniska smärtor i olika delar av kroppen. Patienter med FMS upplever ofta skepsis och misstro i mötet med vården på grund av att deras symptom är osynliga. Samtidigt kan det vara svårt för sjuksköterskor och övrig vårdpersonal att veta hur de ska förhålla sig till en sjukdom som inte uppvisar några kliniska fynd och vars etiologi fortfarande är relativt okänd. Dessa omständigheter gör vårdrelationen komplex. Syftet med denna studie är därför att utforska hur vårdrelationen upplevs mellan patienter med FMS och sjuksköterskor. 11 artiklar har analyserats, ur vilka ett huvudtema, fyra teman och två subteman identifierades. Resultatet visar att brist på tid, kunskap och förståelse resulterar i en problematisk vårdrelation. Betydelsen av att vårda dessa patienter i ett multiprofessionellt team framgår även. I diskussionen utvecklas på vilka sätt patienter med FMS och sjuksköterskor upplever vårdrelationen olika. Sjuksköterskornas känslor av maktlöshet kan från patientens sida istället uppfattas som ointresse och brist på engagemang, vilket kan medföra att patienterna tar med sig negativa känslor in i nya vårdmöten.

the treatment of orofacial pain in patients with fibromyalgia , at the city of João
Pessoa - PB . In the first stage of the study, a questionnaire was applied to
participants , which allowed us to assess the location and intensity of the initial
orofacial pain research. The second stage began with the random division of the
patients into two different groups for treatment : Group A, treated with anesthetic
infiltration into four sessions, and Group B , treated by lasertherapy in 12 sessions
(with Laser AsGaAl 780nm and ED=50J/cm2). In the third stage, after completion
of treatment in both groups, patients were submitted to a new questionnaire,
proceeding to data analysis. Both treatments were quite effective as the goal of
reducing and even extinguish the pain in the head and neck. The t test was
performed to compare two paired samples, considering the treatment of Group A,
results in a statistically significant difference from the ' Degree of Orofacial Pain
intensity ' in the moments Before x After, with t=103.422 with degrees of freedom
(df)=32 and p-value=0.000<α=0.05. Considering the B treatment, resulted in
statistically significant differences in relation of the ' Degree of Orofacial Pain
intensity ' in the moments Before x After, with t=12,178 with degrees of freedom
(df)=32 and p - value=0.000< α=0.05. For comparison of two independent
samples, t=0.742 with (df)=64 and significance p - value=0.461>α=0. These
results indicate no significant difference in mean intensity of Orofacial Pain after
each type of treatment, however, due to the question " improved well being " , we
validate the additional properties of the Low Level Lasertherapy ( primary,
secondary and therapeutic ) as better as infiltration with anesthetics. Showing up
lasertherapy as a promising treatment for headaches and neck in patients with
Fibromyalgia Syndrome. / Este ensaio clínico se propôs a avaliar o efeito da Laserterapia e das infiltrações
anestésicas no tratamento da dor orofacial de pacientes portadores de
fibromialgia, no município de João Pessoa PB. Na primeira etapa do estudo, foi
aplicado um questionário aos participantes, que permitiu avaliar a localização e a
intensidade da dor orofacial inicial nos sujeitos da pesquisa. A segunda etapa
iniciou-se com a divisão aleatória dos pacientes em dois grupos, para tratamento:
Grupo A, tratado com infiltrações anestésicas em quatro sessões; e Grupo B,
tratado através da Laserterapia, em 12 sessões (com Diodo Laser AsGaAl 780nm
e DE=50J/cm2). Na terceira etapa, logo após o término do tratamento nos dois
grupos, os pacientes foram submetidos a um novo questionário, procedendo-se a
análise dos dados. Ambos os tratamentos mostraram-se bastante eficazes quanto
ao objetivo de diminuir e até extinguir as dores na região de cabeça e pescoço. O
teste t foi realizado para comparação de duas amostras emparelhadas,
considerando o tratamento do Grupo A, resultou em diferença estatisticamente
significativa em relação ao Grau de Intensidade da dor Orofacial nos instantes
Antes x Pós, apresentando t=103,422 com graus de liberdade (g.l.)=32 e
significância p-valor=0,000<a=0,05. Considerando-se o tratamento B, resultou em
diferença estatisticamente significante em relação ao Grau de Intensidade da dor
Orofacial nos instantes Antes x Pós, apresentando t=12,178 com graus de
liberdade (g.l.)=32 e significância p-valor=0,000<a=0,05. Para comparação das
duas amostras independentes, t = 0,742 com (g.l.)=64 e significância pvalor=
0,461>a=0. Esses resultados indicam não haver diferença significativa
quanto à intensidade média da dor Orofacial após cada um dos tipos de
tratamentos, contudo, devido ao quesito melhoria do bem estar , validamos as
propriedades adicionais do Laserterapia (primária, secundária e terapêutica), em
detrimento às infiltrações com anestésicos. Mostrando-se a Laserterapia bastante
promissora para o tratamento das dores de cabeça e pescoço em pacientes
portadores de Síndrome da Fibromialgia (SF).

Previous issue date: 2009-12-17 / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / The aim of this study was to evaluate the presence of Temporomandibular Disorders (TMD) and the involvement of pain, as well as psychological and psychosocial factors, on a population with Fibromyalgia Syndrome (FMS). This is an observational, cross-sectional study with a direct inductive approach. The universe was composed of patients from the Acupuncture and Pain Ambulatory Facility of Hospital das Clínicas of the Federal University of Pernambuco (HC-UFPE) diagnosed with FMS, according to American College of Rheumatology criteria. The following were assessed: the sample variables (sex and age); classification and degree of TMD by the simplified anamnestic index; psychological and psychosocial factors involved using Research Diagnostic Criteria for TMD (RDC/TMD): Axis II and FMS according to the medical chart used in the ambulatory facility of HC-UFPE. The data were analyzed using descriptive and inferential statistical techniques and Pearson s chi-square or Fisher s Exact test. The significance level used was 5% and the intervals were obtained with 95% of confidence. The sample was composed of 50 female patients with mean age of 47.02 years and standard deviation of 10.27 years. Of these, 49 reported the presence of signs and symptoms related to TMD, which was classified as moderate (40.0%), severe (36.0%) and slight (22.0%). Seventy-six percent of the sample needed treatment for TMD. With respect to the signs and symptoms investigated the most frequent items with a positive response were those related to nape of the neck and neck pain (86.0%), self-assessment of tension (76.0%) and headache (60.0%). Most (80.0%) reported facial pain during the 4 weeks; all but 1 patient had chronic pain. Of the activities related to the quality of life of the patients, the items most compromised by facial pain, reported by 80.0% of the individuals, were: Maintaining normal facial expression, without signs of pain or sadness (68.0%); Eating hard food items (64.0%) and Masticating (60.0%). All the participants experienced body pain, mainly in the occipital region (92.0%). The most frequent complaints from body pain were: fatigue (98%), difficulty in concentrating (90%), anxiety (88%), numbness in the hands or feet (84%) and restless sleep (80%). A significant positive correlation was observed between the duration of facial pain and body pain. Mean visual analogue scale values were 5.22 and 8.7 for facial and body pain, respectively. Depression was found in 90.0% of the patients and most had somatization with pain (98.0%) and somatization without pain (96.0%). The degree of TMD showed a significant association (p < 0.05) with the level of depression and with limitations in mandibular function: yawning, speaking and maintaining normal facial expression without signs of pain or sadness. Facial pain showed a significant association (p < 0.05) with the variables: level of TMD, depression, teeth grinding, fatigue muscle pain from masticating, buzzing in the ear and headache. There was a significant correlation between the occurrence of depression and the following limitations: performing physical exercises; eating hard foods; smiling or laughing; sexual activity; speaking and maintaining normal facial expression, with no signs of pain or sadness. It was concluded that the fibromyalgia population studied exhibited a high prevalence of TMD. Depression was statistically associated to TMD, which was more intense in individuals with severe depression. A significant association was also found when the psychosocial factors involved in mandibular function limitations, degree of depression and severity of TMD were reported. / O objetivo deste estudo foi avaliar a presença de Disfunção Temporomandibular (DTM) e envolvimento de dor, fatores psicossociais e psicológicos em uma população com Síndrome Fibromiálgica (SFM). Foi realizado um estudo observacional transversal com abordagem indutiva direta. O universo foi composto por pacientes do Ambulatório de Acupuntura e Dor do Hospital das Clínicas da Universidade Federal de Pernambuco (HC-UFPE) com diagnóstico de SFM segundo os critérios do Colégio Americano de Reumatologia para SFM. Avaliaram-se: as variáveis de caracterização amostral (gênero e idade); classificação e grau da DTM pelo índice Anamnésico Simplificado; fatores psicológicos e psicossociais envolvidos utilizando-se os Critérios Diagnósticos de Pesquisa em DTM (RDC/TMD): Eixo II e a SFM através da ficha clínica utilizada no referido ambulatório do HC-UFPE. Os dados foram analisados através de técnicas de estatística descritiva e inferencial utilizando o teste estatístico Qui-quadrado de Pearson ou o teste Exato de Fisher. O nível de significância utilizado foi de 5% e os intervalos foram obtidos com 95% de confiança. A amostra foi composta por 50 pacientes do gênero feminino com idade média de 47,02 anos e desvio padrão de 10,27 anos, destas 49 relataram a presença de sinais e sintomas relacionados à DTM que foi classificada quanto a sua severidade como moderada (40,0%), severa (36,0%) e leve (22,0%). Desta amostra 76% mostraram-se com necessidade de tratamento para DTM. Quanto aos sinais e sintomas investigados os itens mais freqüentes com resposta positiva foram aqueles relacionados a dor na nuca ou pescoço (86,0%), auto-avaliação de tensão (76,0%) e dor de cabeça (60,0%). A maioria (80,0%) relatou dor na face durante as 4 semanas; com exceção de uma paciente todas tinham dor crônica. Das atividades relacionadas à qualidade de vida das pacientes os itens mais prejudicados pela dor na face, relatada por 80,0% delas, foram: Ficar com o rosto normal, sem aparência de dor ou triste (68,0%); comer alimentos duros (64,0%) e mastigar (60,0%). Todas as participantes apresentavam dor no corpo sendo a região occipital a mais citada (92,0%). As maiores queixas devido a dor no corpo foram: fadiga (98%), dificuldade de concentração (90%), ansiedade (88%), dormência nas mãos ou pés (84%) e sono com vários despertares (80%). Foi observada correlação positiva elevada entre os tempos da dor na face e dor no corpo sendo a média da escala visual analógica 5,22 e 8,7 para dor na face e no corpo, respectivamente. Em 90,0% das pacientes foi encontrado depressão e a maioria tinha somatização com dor (98,0%) e somatização sem dor (96,0%). O grau da DTM mostrou associação significativa (p < 0,05) com o grau da depressão e com as limitações da função mandibular: bocejar, conversar e ficar com o rosto normal sem aparência de dor ou triste. Quanto a associação da dor na face, mostraram-se significativas (p < 0,05) as variáveis: grau da DTM, depressão, bruxismo, cansaço ou dor muscular ao mastigar, zumbidos no ouvido e dor de cabeça. Houve associação significativa entre ocorrência de depressão com as limitações: fazer exercícios físicos ou fazer ginástica; comer alimentos duros; sorrir ou gargalhar; atividade sexual; conversar e ficar com o rosto normal, sem aparência de dor ou triste. Conclui-se que há uma alta prevalência de DTM na população fibromiálgica estudada. A depressão foi estatisticamente associada à DTM que mostrou-se mais intensa nos indivíduos com maior severidade de depressão. Associação significativa também foi encontrada quando os fatores psicossociais envolvidos nas limitações da função mandibular, grau de depressão e severidade da DTM foram relacionados.

Previous issue date: 2006-08-15 / Fibromyalgia is a syndrome of unknown aetiology with diffuse, widespread and
chronicle pain. The diagnosis of the fibromyalgia should be made in agreement with
the American College of Rheumatology (ACR) 1990 criteria. The patient should
present diffuse pain for more than three months and feel the touch of the physician in
at least 11 of 18 sensitive points, called "tending points" that characterizes the
syndrome. Most of the patients with it are women, and the pick of age incidence in
the occasion of the diagnosis is between 40 and 50 years. The chronic pain promotes
a quality impact in the people s life; reducing his/her self-esteem, isolating them of
the social relationship and allowing them loose the control of their thoughts and
feelings. Depressive symptoms can be the result of the organic dysfunction, associa-
ted with the limited conditions of daily activities. The objective of this study is to
investigate the presence of depressive symptoms and to analyze the impact of the
same ones about on the quality of the patients life with SF. Thirty women, in service
from October to November of 2005, were selected with diagnosis of SF and answe-
red two questionnaires: Inventory of Depression of Beck, to investigate the presence
of depressive symptoms and SF-36 - Medical Outcome Survey Short Form -36, to
evaluate the life quality.The statistical results obtained by the program SPS show that
the depressive symptoms are responsible for the quality impact in these people's life,
mainly in the following aspects: physical, social, mental health, vitality and general
health. The analysis of the data suggests that the depressive symptoms can influence
in the women life quality with fibromyalgia negatively, because it increases the
sensation of physical incapacity, reduces the quality of the social relationships and
increases the pessimism, the fatigue and the discouragement / Fibromialgia é uma síndrome de etiologia desconhecida com dor difusa, generalizada
e crônica. O diagnóstico da fibromialgia deve ser feito de acordo com o critério do
American College of Rheumatology (ACR) 1990. O paciente deve apresentar dor
difusa por mais de três meses e relatar dor à palpação em pelo menos 11 de 18 pontos
sensíveis, chamados de tender points , caracaterizando assim a síndrome. A maioria
dos pacientes acometidos são mulheres, e o pico de incidência etária na ocasião do
diagnóstico está entre 40 e 50 anos. A dor crônica promove um impacto na qualidade
de vida dessas pessoas, diminuindo sua auto-estima, isolando-as do convívio social e
fazendo com que percam o controle dos seus pensamentos e sentimentos. Sintomas
depressivos podem ser o resultado da disfunção orgânica, associada às condições de
limitação das atividades diárias.Este estudo tem como objetivo investigar a presença
dos sintomas depressivos e analisar o impacto dos mesmos sobre a qualidade de vida
das pacientes com SF. Foram selecionadas 30 mulheres com diagnóstico de SF, em
atendimento no período de outubro a novembro de 2005, as quais responderam a dois
questionários: Inventário de Depressão de Beck, para investigar a presença de sinto-
mas depressivos e o SF-36 Medical Outcome Survey Short Form -36, para avaliar a
qualidade de vida. Os resultados obtidos na estatística pelo programa SPS demons-
tram que os sintomas depressivos são responsáveis pelo impacto na qualidade de vida
dessas pessoas, principalmente nos domínios: aspectos físicos, aspectos sociais, saú-
de mental, vitalidade e saúde geral. A análise dos dados sugere que os sintomas
depressivos podem influenciar negativamente a qualidade de vida das mulheres
fibromiálgicas, pois aumenta a sensação de incapacidade física, diminui a qualidade
das relações sociais, aumenta o pessimismo, a fadiga e o desânimo

Cette étude cherche à 1) comparer la personnalité et l’adaptation des fibromyalgiques (FM) avec ceux ayant une polyarthrite (PR), une spondylarthrite (SA) ou un syndrome de Gougerot-Sjögren (SG) ; 2) identifier les liens et influences entre la personnalité et les variables agissant sur le retentissement et le coping et 3) observer l’existence de différences intra-groupes chez les FM. 48 FM, 47 PR, 47 SA et 23 SG ont répondu aux BFI (personnalité), GHQ (souffrance psychique), FIQ (douleur, retentissement), FSS (fatigue), EVE (événements de vie), CSQ (coping) et MINI (comorbidité anxio-dépressive). Les FM ont une élévation de l’agréabilité (A), de l’ouverture (O), du Névrosisme (N) et un retentissement plus sévère que les autres. Dans les maladies rhumatismales, l’anxiété influence la dépression, la douleur, la fatigue et la dramatisation. N influence majoritairement l’anxiété, le coping actif et modérément la dépression. Enfin, la douleur et N permettent de distinguer deux sous groupes dans la FM. La personnalité a une influence sur l’adaptation. A et O semblent être des défenses face au stress alors que N a un impact direct et néfaste dans l’adaptation / This study aims to 1) compare personality and adaptation of fibromyalgia patients (FM) with those having a rheumatoid arthritis (PR), a ankylosing spondylitis (SA) or Sjogren’s syndrome (SG); 2) identify the links and the influences between the personality and the variables acting on repercussions of the illness and coping; 3) observe the existence of intra-group differences in FM. 48 FM, 47 PR, 47 SA and 23 SG answered the BFI (personality), GHQ (psychic suffering), FIQ (pain, repercussions of the illness), FSS (fatigue), EVE (events of life), CSQ (coping ) and MINI (anxio depressive comorbidity). The FM show a rise in agreeableness (A), openness (O) and neuroticism (N) and a more severe impact than the other diseases. In the rheumatic diseases, anxiety influences depression, pain, fatigue and dramatization. N influences mainly anxiety, active coping and moderately depression. Finally, pain and N allow to distinguish two groups in FM. Personality has an influence on adaptation. A and O seem to be defenses in the face of stress while N has a direct and worse impact on the adaptation

Qualidade de vida e mecanismos de defesa em pacientes femininas com fibromialgia com ou sem depressãoAguiar, Rogerio Wolf de January 2008 (has links)
OBJETIVOS Este estudo tem dois objetivos: 1) verificar a freqüência de depressão em pacientes femininas com fibromialgia; 2) comparar as pacientes com fibromialgia com ou sem depressão para verificar se diferem no uso de seus mecanismos de defesa e qualidade de vida. MÉTODOS Oitenta e seis pacientes femininas com fibromialgia foram avaliadas psiquiatricamente. O diagnóstico psiquiátrico foi feito por meio de entrevistas clínicas e confirmado pelo MINI (Mini International Neuropsychiatric Interview), uma entrevista psiquiátrica estruturada. O uso dos mecanismos de defesa foi avaliado pelo DSQ-40 (Defense Style Questionnaire), um questionário autoaplicável com 40 questões. A qualidade de vida foi avaliada pelo WHOQOL – Bref (World Health Organization Quality of Life Instrument – Short Version), também auto-aplicável, composto por 26 questões, dividido em quatro domínios: físico, psicológico, relações sociais e ambiental. A gravidade da depressão foi verificada pelo BDI (Beck Depression Inventory), um teste com 21 itens de múltipla escolha. Todos os instrumentos utilizados já se encontram validados para o português brasileiro. RESULTADOS Sessenta e duas pacientes (72,1%) tinham depressão atual (Depressão Maior ou Distimia), com qualidade de vida pior do que aquelas sem depressão (p<0,008). Quanto mais grave foi a depressão, avaliada pelo BDI, pior foi avaliada a qualidade de vida em todos os domínios (p<0,001). As pacientes com depressão usaram mais mecanismos de defesa imaturos, menos mecanismos maduros (p<0,001) e eram mais jovens, quando comparadas com aquelas sem depressão (46,06 anos, DP ± 10,03 vs. 52,29 ± 11,23, t= 2,09, p= 0,039). Entretanto, na regressão logística controlada para a idade, todos os domínios da qualidade de vida e os escores de defesas maduras e imaturas permaneceram associados à presença de depressão. CONCLUSÕES Neste estudo, a alta freqüência de depressão em pacientes com fibromialgia corrobora os dados da literatura científica internacional, em seus limites superiores. A presença de depressão está associada a pior qualidade de vida e maior uso de mecanismos de defesa mal-adaptativos. Esses resultados sugerem que a depressão, quando presente, deve ser tratada vigorosamente para melhorar a qualidade de vida dos pacientes com fibromialgia. / OBJECTIVE This study has two aims: 1) to verify how frequent depression among female patients with fibromyalgia is; 2) to compare the female patients with fibromyalgia with and without depression in order to verify whether their defense mechanisms and quality of life differ. METHODS Eighty-six female patients with fibromyalgia were assessed. The psychiatric diagnosis was conducted through clinical interviews and confirmed by MINI (Mini International Neuropsychiatric Interview), a structured psychiatric interview. The use of defense mechanisms was evaluated through the self-applied means DSQ- 40 (Defense Style Questionnaire). Quality of life was evaluated by WHOQOL – Brief (World Health Organization Quality of Life Instrument – Short Version), also self-applied, composed by 26 questions, divided in four domains: physical, psychological, social relations and environment. The level of depression was attained through BDI (Beck Depression Inventory), a 21-multiple choice test. All instruments employed have been previously validated to Brazilian Portuguese. RESULTS Sixty-two patients (72.1%) presented current depression (Major Depression or Dysthymia). The quality of life of these patients was worse than of the patients without current depression (p<0.008). The graver the depression, which was evaluated by BDI, the worse the quality of life was in all domains (p<0.001). The depressed patients employed more immature defense mechanisms and a lesser number of mature mechanisms (p<0.001), and they were younger when compared to the ones who were not depressed (46.06 years old, DP ± 10.03 vs. 52.29 ± 11.23, t = 2.09, p = 0.039). The controlled logistic regression for age was used in this study and all domains of the quality of life and the scores of mature and immature defenses remained associated to the presence of depression. CONCLUSIONS In this study, the high occurrence of depression in patients with fibromyalgia corroborates the data available in international scientific literature, though, in its higher limits. The presence of depression is associated to a worse quality of life and to a greater use of defense mechanisms which are maladapted. These results suggest that depression should be treated vigorously in order to improve the quality of life of patients with fibromyalgia.
