Soukromý sektor ve veřejném zdravotnictví / Private sector in public health care systems

This master thesis is trying to describe the situation of private sector in public health care systems. As a private sector we understand patients, private health insurance companies and private health care providers. The focus is placed on private health care providers, especially in ambulatory treatment. At first there is a definition of health as a main determinant of a health care systems, definition of public and private sectors in health care systems and the difficulties at the market of health care, including externalities, inperfect information and elasticity of supply and demand at this market. There is also the problem of financing health care as a mix of public and private payments, which is different in every health care system. The main focus is laid on the situation in the Czech republic. As a comparison there are described situations in Denmark, Germany, Great Britain and United States. The size of private sector in every health care system is different and it depends on different circumstances in particular state, including social-economic development, regulation of health care market from the state and the attitude of patients to private health care.

This is a qualitative, cross-sectional, exploratory research. This project subjects twenty ( 20 ) professionals in the health fields, Pronto Sorriso discipline participants in 2006 and 2007. After signing TCLE the informed began data collecting through individual interviews, using as a tool to search a roadmap that consists of seven semi-structured questions, as well as a biographical questionnaire containing seven socio items. This research allowed us to trace the profile of the respondents: the majority were students of medicine, female, aged between twenty and thirty years and completed the graduation between the years 2008 and 2011 . It was found that the majority of professionals reported as positive: the contribution in teaching and learning for academic training , changes and their effects on personal and professional life and changes in attitudes and attitude towards patients, colleagues and the team work. This author also improves the communication performance in multi coexistence , making a difference in their lives. We concluded that the Projeto Pronto Sorriso has provided new knowledge and experiences, and added values brought about the development of skills related to professional practice extremely important in building good health practices . / A pesquisa teve como objetivo analisar a percepção dos participantes da disciplina Pronto Sorriso da Faculdade de Medicina da Universidade Federal de Goiás (FMUFG), em relação à contribuição deste projeto em sua formação profissional, descrever as características individuais dos participantes, identificar por meio das entrevistas as principais mudanças na vida do profissional decorrente da sua participação no projeto, analisar as contribuições e repercussões em sua formação e identificar a motivação para participação no Pronto Sorriso. Trata-se de uma pesquisa qualitativa, transversal, de natureza exploratória, sendo os sujeitos da pesquisa vinte (20) profissionais das áreas de saúde, participantes da disciplina Pronto Sorriso nos anos de 2006 e 2007. Após a assinatura do TCLE iniciou-se a coleta de dados, através de entrevista individualizada, utilizando como instrumento de busca um roteiro composto por sete questões semi estruturadas, bem como um questionário sócio biográfico contendo sete itens. A pesquisa possibilitou traçar o perfil dos entrevistados: a maioria foram acadêmicos do curso de medicina, do sexo feminino, com idades entre vinte e trinta anos e concluíram a graduação entre os anos de 2008 e 2011. Constatou-se que a maioria dos profissionais relatou como pontos positivos: a contribuição no processo ensino e aprendizagem para a formação acadêmica, mudanças e repercussões na vida pessoal e profissional e mudanças de atitudes e postura em relação aos pacientes, com os colegas e com a equipe de trabalho. Refere ainda melhora no desempenho de comunicação, na convivência multiprofissional, fazendo um diferencial em suas vidas. Conclui-se que o Pronto Sorriso propiciou novos conhecimentos e vivências, acrescentou valores e proporcionou o desenvolvimento de habilidades referentes à atuação profissional de extrema relevância na construção de boas práticas de saúde.

Digitalisering inom vården : Effekter vårdpersonalen upplever efter implementering av ett IT-system / Digitizing in health care : Effects the health care staff experience after implementationof a IT-system

Rapportens syfte är att i en fallstudie undersöka vilka effekter personalen på ett vård och omsorgsboendeti Filipstads kommun upplever i sin vardag, efter att ett nytt IT-system implementerats, för att bedöma om upphandlingsprocessens genomförande ligger till grund för effekterna personalen upplever. Detta ämne är aktuellt, då digitalisering inom vården ingår allt mer i personalens vardag inom vård och omsorg. Effekter efter en implementering av ett IT-system uppstår alltid, men frågan är om effekterna påverkar vård och omsorgspersonalens vardagliga arbetssituation på ett positivt eller negativt sätt. Detta är en fallstudie med kvalitativ inriktning, där metodvalen är semistrukturerad intervju och deltagande känd observation. Utifrån den insamlade teorin utformades de intervjufrågor som användes vid intervju med säkerhetssamordnare (SS ) och upphandlingschef (UC) för Filipstads kommun som är IT-systemets beställare. Efter intervjun med SS och UC i sammanhållning med teori utformades de intervjufrågor som ställdes till gruppledaren för vård och omsorgsboendet samt fem stycken personal på vård och omsorgsboendet, varav fyrast jobbar dag och en jobbar natt. Resultatet av fallstudien visar att upphandlingsprocessen och implementeringen av IT-systemet utförts utan några större brister, men IT-systemet har en hög dysfunktionalitet. Respondenternas och gruppledarens syn är att kraven sett annorlunda ut om de fått medverka vid kravinsamlingen, denna syn stöds inte av det som framkommit under rapportens skrivande.Dock har personalens vardagliga arbetssituation försämrats efter det nya IT-systemet implementerats. Syftet med det nya IT-systemet var att personalen skulle känna sig tryggare och ha bättre koll över brukarna. Detta har inte uppnåtts då IT-systemet har en så hög dysfunktionalitet att en slutbesiktning varit omöjlig att genomföra.
