Molecular interactions between gastric stem cells and their niche upon Helicobacter pylori infection

Infektionen mit H. pylori führen zu Veränderungen im Aufbau und der zellulären Zusammensetzung des Magenepithels. Aktuelle Studien haben gezeigt, dass Stromazellen, die sich in unmittelbarer Nähe der epithelialen Stammzelle befinden, die funktionelle Nische für dieses Kompartiment bilden und wichtige Faktoren für die Regulierung des Stammzellumsatzes und Differenzierung darstellen. Daher konzentriere ich mich bei dieser Arbeit auf die Rolle der Myh11+ Myofibroblasten, die sich sowohl unter als auch zwischen den Epitheldrüsenzellen befinden. Ich fand heraus, dass die Myofibroblasten in der Homöostase einen BMP-Gradienten entlang der Drüsenachse mit erhöhter Expression von Bmp2 im oberen Teil erzeugen, während die Drüsenbasis von Zellen umgeben ist, die Bmp-Inhibitoren produzieren. Basierend auf der Funktionsanalyse mit 3D-Organoidmodellen ist der Bmp-Signalweg ein zentraler Faktor für die rasche Differenzierung von Stammzellen in faveoläre mukusproduzierende Zellen. Darüber hinaus wurde ein auto-parakriner Signalweg gefunden, der zur Bildung von Bmp2 in Epithelzellen führt. Meine Untersuchungen Daten zeigten eine Verringerung des BMP-Signalwegs in H. pylori-infizierten Myofibroblasten. Darüber hinaus führte eine Infektion mit H. pylori nicht nur zum Verlust der stromalen, sondern auch der epithelialen Bmp2-Expression. Diese Beobachtung ging mit einer Zunahme von IFNγ einher, was auf eine Verbindung zwischen beiden Signalwegen hinwies. Tatsächlich zeigten anschließende Experimente mit Organoiden, dass IFNγ den Bmp-Signalweg hemmt und dadurch die terminale Differenzierung blockiert. Zusammenfassend zeigen meine Untersuchungen, dass das Schicksal einer Zelle, die zur Oberfläche der Magendrüse wandert, eine Induktion des BMP-Signalwegs erfährt. Die Reduktion dieses Signals durch IFNγ deutet auf einen Mechanismus hin, der Veränderungen in der zellulären Differenzierung und die Entwicklung von prämalignen epithelialen Läsionen im Verlauf einer H. pylori-Infektion bewirkt. / Infection with Helicobacter pylori (H. pylori) leads to alterations of the topology and cellular composition of the gastric epithelium. Recent studies have shown that stromal cells located in close proximity to the epithelial stem cell are creating the niche for this compartment and provide crucial factors that regulate stem cell turnover and fate decisions. Therefore, this thesis focuses on of Myh11+ myofibroblasts that are located beneath and between the epithelial gland cells. I found that during homeostasis, the myofibroblasts generate a BMP gradient along the gland axis with strong expression of Bmp2 in the upper part, whereas the base is surrounded by cells producing BMP inhibitors. Based on functional analysis with 3D organoid models, the BMP gradient occurs to be a main factor responsible for rapid differentiation of stem cells into pit surface mucous cells. Moreover, experiments led me to identify an auto-paracrine feed-forward BMP2 loop in epithelial cells, which further stabilizes BMP signaling once it is activated and induces terminal differentiation. Data presented in this study demonstrated a reduction of BMP signaling in H. pylori-infected myofibroblasts. Furthermore, infection with H. pylori resulted in loss of not only stromal, but also epithelial Bmp2 expression. This observation was accompanied with increase of Interferon γ (IFNγ), which indicated a link between both pathways. Consistently, stimulation of organoids with IFNγ impairs BMP signaling and the BMP2 feed-forward loop, and thereby blocks terminal differentiation. Together, this study shows that the fate of a cell migrating into the surface of the gastric gland is determined by an induction of BMP signaling and stabilized by an auto-paracrine BMP signaling enhancement. Reduction of this signaling by IFNγ revealed a mechanism which contributes to altered cellular differentiation and development of premalignant epithelial alterations in the context of H. pylori infection.

Assoziation zwischen der Besiedlung mit Helicobacter pylori im Kindesalter und dem Body-Mass-Index

Diese Dissertation befasst sich mit den möglichen Folgen einer Kolonisation mit Helicobacter pylori (H. pylori) im Kindesalter. Von besonderem Interesse ist dabei die Entwicklung des Body-Mass-Index (BMI). Dazu werden die Daten einer populationsbezogenen Querschnittsstudie, welche 1998 und 2006 unter Leipziger Schulanfängern bzw. Achtklässlern durchgeführt wurde, analysiert. Insgesamt konnten 1349 bzw. 1161 Kinder, deren H. pylori-Status mittels 13C‑Harnstoff-Atemtest untersucht wurde, in die Untersuchung inkludiert werden. Dabei bestätigte sich, dass die Besiedlung mit H. pylori u.a. signifikant mit einem geringeren sozialen Status und einer geringeren Körpergröße der Kinder assoziiert ist. Der BMI der untersuchten Kinder unterscheidet sich jedoch nicht signifikant in Abhängigkeit von einer Kolonisation mit H. pylori. Auf diesen Ergebnissen basierend werden zwei Hypothesen diskutiert. Zum einen wird analysiert, inwieweit ein Einfluss einer Infektion mit dem Erreger auf den BMI der Kinder nicht sicher auszuschließen ist und dieser eventuell aufgrund der Komplexität der Gewichtsentwicklung in den Ergebnissen dieser Untersuchung nicht erkennbar ist. Zum anderen wird die geringere Körpergröße H. pylori‑besiedelter Kinder nicht auf mit der Infektion in Verbindung stehende Ernährungsstörungen in Zusammenhang gebracht, da sonst vor einer entscheidenden Größenminderung das Gewicht bzw. der BMI der Studienteilnehmer zurück bleiben müsste.

Frecuencia de Helicobacter Pylori en muestras histopatológicas gástricas malignas y premalignas, servicio de anatomía patológica -Hospital Regional Lambayeque, 2016-2019

Objetivo: Determinar la frecuencia de Helicobacter pylori en muestras histopatológicas gástricas malignas y premalignas del servicio de anatomía patológica del Hospital Regional Lambayeque durante los años 2016 al 2019. Material y métodos. Estudio censal, observacional descriptivo, retrospectivo y transversal. La población de estudio estuvo conformada por los pacientes cuyas muestras histológicas gástricas fueron recibidas y estudiadas en el servicio de anatomía patológica del Hospital Regional Lambayeque. Resultados. Cumplieron con los criterios de selección 566 pacientes. La edad media fue 57,3; rango de 8 a 97 años. Hubo un predominio de pacientes entre la quinta (23.9%), sexta (24%) y séptima (17%) década de la vida. La frecuencia global de Helicobacter Pylori en las muestras seleccionadas fueron de 70.5%, con un predominio en el sexo masculino (71.2%). Los hallazgos anatomopatológicos se distribuyeron de la siguiente manera: 147 pacientes (26%) con gastritis crónica atrófica (GCA), 333 (58.8%) con metaplasia intestinal (MI), 60 (10.6%) con displasia y 26 (4.6%) con cáncer gástrico. Metaplasia intestinal fue el hallazgo más común; el mismo que se encontró en 166 personas (49.85%) con MI completa, 73 (21.92%) con MI incompleta y 94 (28.22%) con MI mixta. Conclusiones La frecuencia de Helicobacter Pylori (HP) fue alta en las lesiones premalignas, mientras que no hubo diferencias en la presencia o ausencia en muestras gástricas malignas. Conforme avanzaba la edad disminuía la frecuencia del HP pero aumentaban la gravedad de las lesiones. / Objective: Determine the frequency of Helicobacter pylori in malignant and premalignant gastric histopathological samples from the pathological anatomy service of the Hospital Regional Lambayeque during the years 2016 to 2019. Material and methods. Census, observational, descriptive, retrospective and cross-sectional study. The study population was made up of patients whose gastric histological samples were received and studied in the pathological anatomy service of the Hospital Regional Lambayeque. Results. 566 patients fulfilled the selection criteria. The mean age was 57.3; range from 8 to 97 years. There was a predominance of patients between the fifth (23.9%), sixth (24%) and seventh (17%) decades of life. The overall frequency of Helicobacter Pylori in the selected samples was 70.5%, with a predominance in the male sex (71.2%). The anatomopathological findings were distributed as follows: 147 patients (26%) with chronic atrophic gastritis (CAG), 333 (58.8%) with intestinal metaplasia (IM), 60 (10.6%) with dysplasia and 26 (4.6%) with gastric cancer. Intestinal metaplasia was the most common finding; the same that was found in 166 people (49.85%) with complete MI, 73 (21.92%) with incomplete MI and 94 (28.22%) with mixed MI. Conclusions The frequency of Helicobacter Pylori (HP) was high in premalignant lesions, while there were no differences in the presence or absence in malignant gastric samples. As age advanced, the frequency of PH decreased but the severity of the lesions increased.

Strategies for de novo DNA sequencing

The development of improved sequencing technologies hasenabled the field of genomics to evolve. Handling andsequencing of large numbers of samples require an increasedlevel of automation in order to obtain high throughput andconsistent quality. Improved performance has lead to thesequencing of numerous microbial genomes and a few genomes fromhigher eukaryotes and the benefits of comparing sequences bothwithin and between species are now becoming apparent. Thisthesis describes both the development of automated purificationmethods for DNA, mainly sequencing products, and a comparativesequencing project. The initially developed purification technique is dedicatedto single stranded DNA containing vector specific sequences,exemplified by sequencing products. Specific capture probescoupled to paramagnetic beads together with stabilizing modularprobes hybridize to the single stranded target. After washing,the purified DNA can be released using water. When sequencingproducts are purified they can be directly loaded onto acapillary sequencer after elution. Since this approach isspecific it can be applied to multiplex sequencing products.Different probe sets are used for each sequencing product andthe purifications are performed iteratively. The second purification approach, which can be applied to anumber of different targets, involves biotinylated PCR productsor sequencing products that are captured using streptavidinbeads. This has been described previously, buthere theinteraction between streptavidin and biotin can be disruptedwithout denaturing the streptavidin, enabling the re-use of thebeads. The relatively mild elution conditions also enable therelease of sensitive biotinylated molecules. Another project described in this thesis is the comparativesequencing of the 40 kbcagpathogenicity island (PAI) in fourHelicobacter pyloristrains. The results included thediscovery of a novel gene, present in approximately half of theSwedish strains tested. In addition, one of the strainscontained a major rearrangement dividing thecagPAI into two parts. Further, information about thevariability of different genes could be obtained. Host-bacteria interactions : Host cell responses and bacterial pathogenesis

Helicobacter pylori colonizes the human stomach, where it causes gastritis that may develop into peptic ulcer disease or cancer when left untreated. Neisseria gonorrhoeae colonizes the urogenital tract and causes the sexually transmitted disease gonorrhea. In contrast, Lactobacillus species are part of the human microbiota, which is the resident microbial community, and are considered to be beneficial for health. The first host cell types that bacteria encounter when they enter the body are epithelial cells, which form the border between the inside and the outside, and macrophages, which are immune cells that engulf unwanted material.       The focus of this thesis has been the interaction between the host and bacteria, aiming to increase our knowledge of the molecular mechanisms that underlie the host responses and their effects on bacterial pathogenicity. Understanding the interactions between bacteria and the host will hopefully enable the development of new strategies for the treatment of infectious disease. In paper I, we investigated the effect of N. gonorrhoeae on the growth factor amphiregulin in cervical epithelial cells and found that the processing and release of amphiregulin changes upon infection. In paper II, we examined the expression of the transcription factor early growth response-1 (EGR1) in epithelial cells during bacterial colonization. We demonstrated that EGR1 is rapidly upregulated by many different bacteria. This upregulation is independent of the pathogenicity, Gram-staining type and level of adherence of the bacteria, but generally requires viable bacteria and contact with the host cell. The induction of EGR1 is mediated primarily by signaling through EGFR, ERK1/2 and β1-integrins. In paper III, we described the interactions of the uncharacterized protein JHP0290, which is secreted by H. pylori, with host cells. JHP0290 is able to bind to several cell types and induces apoptosis and TNF release in macrophages. For both of these responses, signaling through Src family kinases and ERK is essential. Apoptosis is partially mediated by TNF release. Finally, in paper IV, we showed that certain Lactobacillus strains can reduce the colonization of H. pylori on gastric epithelial cells. Lactobacilli decrease the gene expression of SabA and thereby inhibit the binding mediated by this adhesin. / <p>At the time of the doctoral defense, the following papers were unpublished and had a status as follows: Paper 2: Manuscript. Detección de Helicobacter pylori y organismos "Helicobacter heilmannii - like" en mucosa gástrica de perros, a través de la reacción en cadena de la polimerasa (PCR)

Memoria para optar al Título Profesional de Médico Veterinario / Con el fin de detectar, por primera vez en Chile, la presencia de Helicobacter pylori y organismos Helicobacter heilmannii-like en el estómago de perros, se realizó la reacción en cadena de la polimerasa (PCR) a biopsias gástricas de 10 perros que fueron positivos a la prueba de ureasa. Para la detección de H. pylori se utilizó una secuencia génica de la citotoxina vacuolizante (vac A) y para la detección de H. heilmannii-like se utilizó una secuencia génica de 112 pares de bases (pb) de ARN 16S, específico para este grupo de bacterias. Se obtuvo positividad en 5/10 perros para H. pylori y positividad en 6/10 perros para H. heilmannii-like. Dos individuos fueron positivos a ambas bacterias y un individuo negativo para ambas bacterias. A partir de estos resultados y con el fin de diagnosticar bacterias H. pylori y/o bacterias H. heilmannii-like en biopsias gástricas endoscópicas de perros, se recomienda la utilización de PCR, idealmente antecedido por la prueba rápida de ureasa. / Proyecto FIV no. 4602019
