I denna kvalitativa studie har åtta semistrukturerade intervjuer genomförts med yrkesverksamma undersköterskor inom hemtjänst. Syftet med studien är att undersöka hemtjänstpersonalens erfarenheter av äldres psykiska ohälsa inom ramen av sitt arbete. Genom en tematisk analys identifierades två huvudteman. I det första temat Människan som projekt framkommer att intervjupersonerna ser på mötet med äldre med psykisk ohälsa som komplext och lyfter vikten av ett gott bemötande och regelbunden kontakt som viktiga faktorer för att hantera mötet på bästa sätt. Detta presenteras vidare genom två underteman – Bemötande och Kontinuitet. I det andra temat Människan i verksamheten framkommer utmaningarna i mötet med äldre personer med psykisk ohälsa som samtliga ligger i verksamheten. Utmaningarna består enligt intervjupersonerna av tidsramar som är svåra att förhålla sig till, samt en kompetensbrist hos både de själva och i samhället när det kommer till äldre med psykisk ohälsa. Även detta presenteras vidare genom underteman – Tidsramen och Kompetensbrist. Avslutningsvis kopplas resultatet till tidigare forskning. Där framkommer likheter mellan forskning och studiens resultat avseende mötet med äldre personer med psykisk ohälsa som komplext och utmanande utifrån brist på tid, kunskap och kompetens. Komplexiteten och utmaningarna problematiseras vidare utifrån Lipskys teori om gräsrotbyråkrati och visar på balansgången i hemtjänstpersonalens arbete mellan individens behov och arbetets struktur. / In this qualitative study, eight semi-structured interviews were conducted with educated home care employees within the home care workforce. The aim of this study is to investigate home care employees’ experiences of elderly people with mental illness within the frame of their work. Through a thematic analysis two main themes where identified. The first theme, The individual as a project, presents how the home care employees perceive the meeting with elderly’s mental illness as complex, and experience that a good reception and regular contact are two important factors for being able to handle mental illness in the best way. This is further presented in two subthemes – Reception and Continuity. The second theme, The individual within the organization, presents the challenges that the home care employees encounter with working with elderly people with mental illness. These challenges consist of timeframes that the employees perceive as hard to work within but also a lack of competence, both within themselves and in the society when it comes to elderly’s mental illness. Furthermore, this is also presented in two subthemes – The timeframe and Lack of competence. Finally, the result is connected to previous research where similarities between the previous research and the result is revealed regarding the meeting with mental illness as complex and comprised of challenges, including lack of time, knowledge and competence. The complexity and challenges are further problematised from Lipsky’s theory of street level bureaucracy and emphasises the balance in home care employees’ work between the individual needs and the structure of the work.

The management of this study considers that the relationship between the family and care-dependent persons does not take place in a single or homogeneous way, since it derives from a lot of social elements, economic, cultural, demographic and biological elements, based on the social and cultural historicity of its actual construction. These elements configure the objective and subjective conditions for the act of caring. The presence of the State is predominant, since it is responsible for guaranteeing the society's standards of civility, and the establishment of a relationship between the State and the family, based essentially on complementarity, not on the substitution of one or the other. Little submission of the family to the state. In order to identify the interfaces of protection and social deprotection in this process, the family's protective capacity, composed by the assessment of the objective conditions of the families, living territories and social ties, was used as the matrix. The qualitative research had as a locus the Home Care Service of the municipality of São Bernardo do Campo, called PID SBC and carried out the study of multiple cases through a home visit and interview. The complexity of the reality confronted by the families in care and, insofar as their protective capacity is concerned, reveals that the interactions between territory, links and objective conditions may reveal a tendency of the triangular relationship to present itself with more than one dependence be faced. The first one is due to the imperative biological condition of those who are ill, and the second concerns the social violations that the family caregivers are subjected to in this process / Este estudo se dedica ao exame da relação triangular entre o sujeito enfermo – aqui um dependente de cuidados múltiplos –, a atenção domiciliar e a atenção familiar. Considera-se que a relação da família com pessoas dependentes de cuidados não se concretiza de modo único ou homogêneo, pois decorre de vários elementos sociais, econômicos, culturais, demográficos e biológicos, tendo por base e premissa a historicidade social e cultural de sua construção real. Tais elementos configuram as condições objetivas e subjetivas para o ato de cuidar. Tem-se como preponderante a presença do Estado, visto que é o responsável por garantir padrões de civilidade da sociedade, e o estabelecimento de uma relação entre Estado e família fundamentalmente de complementariedade, não de substituição de um ou de outro, tampouco de submissão da família para com o Estado. Para a identificação das interfaces de proteção e desproteção social nesse processo, utilizou-se como matriz de análise a capacidade protetiva da família, composta pela apuração das condições objetivas das famílias, dos territórios de vivência e dos vínculos sociais. A pesquisa de caráter qualitativo teve como locus o Serviço de Atenção Domiciliar do município de São Bernardo do Campo, denominado PID SBC, e realizou o estudo de casos múltiplos, através de visita domiciliar e entrevista. Constata-se a complexidade da realidade enfrentada pelas famílias em atendimento e, no que tange à sua capacidade protetiva, verifica-se nas interações entre território, vínculos e condições objetivas tendência de a relação triangular apresentar-se com mais de uma dependência a ser enfrentada. A primeira pela imperativa condição biológica de quem é enfermo, e a segunda diz respeito às violações sociais a que são submetidos os cuidadores familiares nesse processo

Values and long-term care decision-making for frail elderly people

This project explored the values considered by elderly people, their younger relatives, and health professionals in decisions about residential long - term care, aiming to contribute to the literature on prospectively held values. The mixed methods design utilised a medical record review of 60 frail elderly hospital patients, a stratified survey of 3,015 adults in the South Australian community, and interviews with 36 stakeholders ( 10 elderly people, 10 younger relatives, and 18 health professionals ). The medical record review confirmed that the hospital patients and their outcomes resembled those described internationally. It was used to develop a hypothetical vignette, used in the later studies. Survey responses suggested that when considering a hypothetical long - term care decision, community members put the elderly person ' s physical health and safety first. Situational variables ( the elderly person ' s autonomy, environmental adaptation, and caregiver burden ) appeared secondary, albeit less so with increasing age of the respondent. Thematic analysis of the interviews demonstrated that elderly stakeholders considering a hypothetical decision were more likely to mention autonomy values, and less likely to mention safety values, than were relatives or health professionals. However, elderly stakeholders were also more likely to suggest restrictive solutions, such as residential placement and proxy decision - making. This finding raised methodological issues concerning ' third person ' vignettes, in that respondents might be responding as proxy decision - makers, rather than as if the hypothetical decision applied to themselves. The project confirmed that, in this context, prospectively held values resembled the retrospectively described values identified by McCullough, Wilson, Teasdale, Kolpakchi and Shelly ( 1993 ). Hence, the retrospective literature could be applied. The project supported the importance and complexity of psychosocial predisposing factors when applying the Andersen Behavioral Model ( Andersen, 1995 ) to long - term care decisions. Additionally, the Ecological Theory of Aging ( Nahemow, 2000 ) and the MacArthur Model of Successful Aging ( Andrews, Clark, & Luszcz, 2002 ) were found to be relevant to long - term care decisions for individuals and populations. It was concluded that both clinically, and at a policy level, discussions of long - term care could be more effective if they focussed on maintenance of elderly people ' s autonomy and control, rather than on their physical health and safety. / Thesis (Ph.D.) -- University of Adelaide, School of Psychology, 2006.

Syftet med studien var att undersöka kommunsjuksköterskan omvårdnadsdokumentation om bensår i hemsjukvård. Som metod valdes journalgranskning. Resultatet visade att omvårdnadsdokumentationen brister på många sätt. / Background: In Sweden, approximately 50 000 people suffers from slow-healing leg ulcers. The number of municipal registered nurses ́ has decreased in recent years. The municipal registered nurses ́ often hold function as an advisory consultant for the nursing staff and without participating in nursing care. This documentation is based on second-hand information from the nursing staff. Purpose: The aim was to investigate how municipal registered nurse care documents about leg ulcers in patients in home care practice. Method: An audit of patient journals was chosen to investigate how municipal registered nurse ́ care documents about leg ulcers in patients in home care practice. Results: The results are presented in five themes: contact cause, health and medical history, skin functions alternatively skin / tissue, skin action alternatively action / prescription skin / tissue and Ulcer case record. Beneath these themes the assessment of documentation was into incomplete, partially complete and complete. The result shows that the municipal registered nurse often documents in chronological order and not applying to the nursing process. The results also showed that the old documentation was not completed in the journal, which meant that it was difficult to follow a course of treatment. Discussion: The result shows that documentation in home care failures in many ways. Municipal registered nurse works as a consultant and is not involved in daily care. The documentation is based on second-hand information from the nursing staff and resulting in the documentation of leg ulcers is poor. Patient journals documentation was divided into different keyword, which means that it was difficult to follow the nursing process.
