Cuidador de idosos com doença de Alzheimer : efeitos de grupos psico-educacionais e suporte domiciliar individualizado

Family eldercare providers are central figures in guaranteeing the wellbeing of their elderly relatives who develop dementia. However, when such a person becomes dependent on other family members, in addition to an increase in the caregiver s domestic chores, the behavior problems that arise can have a negative impact when caregivers are not well prepared for this situation, making the caregivers more susceptible to depression and contributing to the deterioration of their own physical and psychological wellbeing, making the caregiving context even more difficult. The objective of this study was to analyze the effects of psycho-educational support groups (PEG) for people caring for a relative with Alzheimer s disease (AD), associated with individual, home visits that were either training-oriented (Experimental Group EG) or listening-oriented (Control Group CG) in nature, with respect to the participants perceptions of burden, depressive symptoms and quality of life. The nine participants were randomly assigned to either the EG and CG. The EG participants received encouragement and assistance in operationalizing the concepts learned in the PEG, which addressed strategies for managing stress and depression and provided training in some of the social skills that are useful in establishing new routines (praise, constructive criticism, and asking for help). The CG received visits of the same duration, bus focused only on listening. The participants were interviewed about their situation, but did not receive individual assistance in operationalizing the strategies presented in the PEG. At the end of the first phase of the intervention, each group received the other type of home-based intervention. The data were analyzed using nonparametric tests (Mann-Whitney and Wilcoxon). The psycho-educational groups were effective in transmitting information, but did not reduce the caregivers perceptions of burden, reporting of depressive symptoms or change their quality of life ratings. After the caregivers received the training-oriented home visits (TV), their perceptions of burden significantly decreased and their quality of life ratings increased. The listeningbased visits had no significant effects on the caregivers. Six months later, an analysis of the follow-up measures indicate that improvements in perceptions of burden, frequency of depressive symptoms and quality of life scores were maintained by those who experienced the PEG coupled with TV. In conclusion, it is clear that professionals who offer orientation programs for family members caring for elderly relatives who have AD should further invest in evaluating the influence of home visits that focus on assisting these caregivers in adjusting their managerial and coping strategies, so as to increase our knowledge of how to blend group-based and individual psycho-educational supports in order to maximize the effects of such interventions. / A demência se caracteriza pela perda de funções cognitivas, comprometendo a execução normal das mais simples tarefas da vida diária. Os cuidadores familiares são pessoas chaves para garantir o bem-estar do seu familiar idoso com demência. Mas, além do aumento da carga de tarefas domésticas que ocorre quando um idoso se torna dependente, os distúrbios de comportamento do idoso dementado provocam um impacto negativo na vida dos cuidadores quando estes não são bem orientados. Isso os tornam mais vulneráveis ao desenvolvimento de quadros depressivos o que contribui para a deterioração de suas próprias condições físicas e psicológicas, complicando ainda mais a tarefa de cuidar. O presente trabalho teve como objetivo analisar os efeitos de um grupo psico-educacional (GPE) para cuidadores de idosos com doença de Alzheimer (DA), associado com um atendimento domiciliar individualizado que tinha como característica uma intervenção terapêutica com o cuidador (Grupo Experimental GE) ou a simples escuta deste (Grupo Controle GC). Os efeitos investigados incluíram as percepções dos participantes de burden, sintomas de depressão e qualidade de vida. Os nove participantes foram alocados aleatoriamente no GE e GC. No GE, os participantes receberam incentivo e suporte na operacionalização de conceitos aprendidos no GPE, os quais envolveram o manejo do estresse e da depressão e o treino de algumas habilidades sociais que são úteis no estabelecimento de rotinas novas (elogios, críticas construtivas e pedir ajuda). O grupo controle recebeu visitas com a mesma duração, mas focando apenas a escuta. Os participantes responderam a um roteiro de entrevista sobre sua situação, sem receber apoio individual na operacionalização das estratégias tratadas no GPE. Após a conclusão da primeira fase da intervenção, cada grupo recebeu o outro tipo de visita domiciliar. Análises feitas com os testes não-paramétricos Mann-Whitney e Wilcoxon mostraram que os grupos terapêuticos psico-educacionais foram eficazes para passar informação aos cuidadores, mas não aliviaram a sobrecarga ou afetaram a pontuação destes cuidadores nas medidas de sintomas depressivos ou qualidade de vida. Porém, quando os cuidadores obtiveram acompanhamento terapêutico individualizado, houve uma diminuição significativa do burden e um aumento da qualidade de vida. As visitas focando a escuta não levaram a nenhuma alteração por parte dos cuidadores. Os sintomas depressivos não foram afetados imediatamente por nenhum tipo de intervenção. Seis meses após o término das intervenções, foi feita outra avaliação (follow-up) a qual mostrou que a associação entre GPE a VTD foi efetiva na manutenção das melhorias nos escores de burden, depressão e qualidade de vida. Conclui-se que os profissionais que oferecem programas de orientação a cuidadores de idosos com demência deveriam investir mais na avaliação dos efeitos de visitas domiciliaras para ajudar cuidadores no aperfeiçoamento de suas estratégias de gerenciamento e enfrentamento do estresse e da sobrecarga. Assim, poder-se-ia conhecer melhor estes efeitos da combinação de grupos psico-educativos e de visitas domiciliares individualizadas, o que por fim, poderia ajudar a maximizar o potencial de impacto das intervenções com cuidadores de idosos com DA.

El presente proyecto de investigación tiene como objetivo presentar el desarrollo de nuestra propuesta de negocio Chamberos, la cual se dedica a ofrecer diversos servicios de reparaciones como fontanería, electrodomésticos, construcción, etc. A través de una forma fácil y segura, por medio de un aplicativo, que lo ayudará a contactar con servicios para incidentes que se puedan presentar en sus hogares. Para dar a conocer sobre este nuevo emprendimiento contamos con dos canales, página web y redes sociales de esta formar lograr tener una comunicación directa con nuestros clientes. Es importante mencionar que los servicios brindados, se darán a través de la modalidad a domicilio, logrando que el cliente ahorre tiempo al momento de solicitar estos servicios, además que los recibirá en la comodidad de su hogar sin necesidad de salir. Para la validación de nuestra idea de negocio, se realizaron varias entrevistas y encuestas donde se obtuvieron las diferentes opiniones de usuarios y expertos acerca del aplicativo Chamberos, así como también elaboramos diversos experimentos, lo cual nos ayudó a determinar la aceptación de los usuarios peruanos con el aplicativo, además que validamos nuestras hipótesis planteadas en cada experimento. Por último, con la finalidad de determinar si el proyecto Chamberos es viable en el mercado peruano, se elaboró un plan financiero completo para determinar que si nuestra idea de negocio es sostenible en el tiempo proyectado de tres años. Con ello determinamos que el proyecto presentado Chamberos es viable, debido a que en nuestros resultados financieros contamos con rentabilidad. / The objective of this research project is to present the development of our Chamberos business proposal, which is dedicated to offering various repair services such as plumbing, electrical appliances, construction, etc. Through an easy and safe way, through an application, which will help you contact services for incidents that may occur in your homes. To publicize about this new venture we have two channels, a website and social networks, in order to achieve direct communication with our clients. It is important to mention that the services provided will be given through the home mode, achieving that the client saves time when requesting these services, in addition to receiving them in the comfort of their home without having to leave. For the validation of our business idea, several interviews and surveys were carried out where the different opinions of users and experts about the Chamberos application were obtained, as well as we developed various experiments, which helped us determine the acceptance of Peruvian users with the application, in addition that we validate our hypotheses raised in each experiment. Finally, in order to determine if the Chamberos project is viable in the Peruvian market, a complete financial plan was prepared to determine whether our business idea is sustainable in the projected time of three years. El siguiente trabajo de investigación tiene como finalidad presentar un plan de negocios, en el cual se desarrolló una aplicación que conecta a freelancers con usuarios finales que requieran o deseen una remodelación en su hogar. Para ejecutar de una manera óptima la aplicación se llevó a cabo validaciones tanto a expertos como usuarios mediante la realización de entrevistas, en el caso de los expertos, estos nos proporcionaron información técnica y los usuarios, nos confirmaron que existía una necesidad no cubierta en el mercado actual peruano. Asimismo, se logró validar la aceptación de la aplicación a través de nuestros canales, siendo el más importante las redes sociales, ya que por ese medio se realizaron las primeras ventas del proyecto, por esta razón, en el siguiente estudio se está analizando la posibilidad de tener un retorno de inversión positivo a partir de la concertación de servicios mediante nuestra aplicación, y se espera que un mediano plazo se pueda añadir más categorías y tener un posicionamiento estable en el sector. / The purpose of the following research work is to present a business plan, in which an application was developed that connects freelancers with end users who require or want a home remodeling. To optimally execute the application, validations were carried out on both experts and users through interviews, in the case of the experts, they provided us with technical information and the users confirmed that there was a need not covered in the current Peruvian market. El presente trabajo de investigación aborda la realización de un emprendimiento de servicio y asistencia del hogar al que lo hemos llamado WASI que en quechua significa hogar. Debido a la actual coyuntura esta palabra a tomado mayor relevancia, dado a los diversos cambios que asume el individuo de hoy ante la actual crisis sanitaria, nuestro hogar se convierte en centro de estudios, oficina, salón de reuniones, clases, gimnasio, cine, etc. El cuidado, limpieza y mantenimiento del hogar es importante para prevenir enfermedades. Por ello, WASI es la solución que brindamos para asistencia, mantenimiento y servicio para el hogar, desde la aplicación móvil, el cliente tendrá la opción de selección del tipo de servicio que desea, el rango horario y el personal que se encuentra cercana al punto donde desea seleccionar el servicio. Este proyecto será dirigido, al nivel socioeconómico B y el grupo de distritos ubicado en zona 7 de Lima Metropolitana, los cuales son los distritos de La Molina, San Isidro, San Borja, Santiago de Surco y Miraflores, como primera etapa de nuestro proyecto con proyección de expandirnos en una segunda etapa provincias. El presente trabajo de investigación tiene el objetivo de evaluar la viabilidad de un modelo de negocio, tanto a nivel de oportunidad de mercado como sostenibilidad comercial, que consiste en una plataforma digital sobre servicios para el domicilio. En esta los proveedores independientes de servicios y las micro y pequeñas empresas que también pertenecen a dicho rubro pueden anunciar sus ofertas. Y, los consumidores interesados pueden encontrar con facilidad toda esa gran variedad de ofertas y compararlas hasta encontrar la opción que más se ajuste a sus necesidades. Así se ayuda a dinamizar el mercado. Se ha hecho el planteamiento inicial sólo sobre la categoría de muebles para el hogar, pero se tiene previsto una expansión escalable hacia otros rubros como gasfitería, servicios eléctricos, decorado y hasta construcción. Para sustentar el proyecto se realizaron diversos experimentos para validar todo el proceso de desarrollo desde la identificación del problema, pasando por los diversos aspectos del modelo de negocio, hasta la intención de compra y ventas realizadas en versiones demo. Todo ello sirve para justificar que el modelo de negocio planteado responde a una oportunidad real en el mercado. Bajo esa premisa, hacia el final del trabajo se plantea el plan de negocio que involucra aspectos operativos, de marketing y financieros. Todo lo necesario para efectivamente hacer realidad el proyecto. / This research work has the objective of evaluating the viability of a business model, both at the level of market opportunity and commercial sustainability, which consists of a digital platform on services for the home. Smart connected homes : concepts, risks, and challenges

The growth and presence of heterogeneous connected devices inside the home have the potential to provide increased efficiency and quality of life to the residents. Simultaneously, these devices tend to be Internet-connected and continuously monitor, collect, and transmit data about the residents and their daily lifestyle activities. Such data can be of a sensitive nature, such as camera feeds, voice commands, physiological data, and more. This data allows for the implementation of services, personalization support, and benefits offered by smart home technologies. Alas, there has been a rift of security and privacy attacks on connected home devices that compromise the security, safety, and privacy of the occupants. In this thesis, we provide a comprehensive description of the smart connected home ecosystem in terms of its assets, architecture, functionality, and capabilities. Especially, we focus on the data being collected by smart home devices. Such description and organization are necessary as a precursor to perform a rigorous security and privacy analysis of the smart home. Additionally, we seek to identify threat agents, risks, challenges, and propose some mitigation approaches suitable for home environments. Identifying these is core to characterize what is at stake, and to gain insights into what is required to build more robust, resilient, secure, and privacy-preserving smart home systems. Overall, we propose new concepts, models, and methods serving as a foundation for conducting deeper research work in particular linked to smart connected homes. In particular, we propose a taxonomy of devices; classification of data collected by smart connected homes; threat agent model for the smart connected home; and identify challenges, risks, and propose some mitigation approaches. / <p>Note: The papers are not included in the fulltext online.</p>
