Poskytování spirituální péče na jednotce intenzivní péče / Providing spiritual care in the intensive care unit

The rise in modern nursing and nursing processes using holistic philosophies. When providing care it should be taken into account the biological, psychlogical, social, but also the spiritual needs of patients. Caring for spiritual needs is an integral part of comprehensive care. Providing spiritual care and psychological support are among the main activities of hospital chaplains. Their work is of great importance in overcoming all the difficulties that illness brings. They are part of a multidisciplinary collaboration in the treatment of patients without a distinct faith. The aim of the thesis is to find out the level of awareness of patients in the sphere of spiritual care, their expectations and their contribution. For the thesis I chose to collect data from a hospital through quantitative research using an anonymous questionnaire. The target group of my research will be 100 patients who are hospitalised in an intensive care unit with a minimum hospitala stay of 7 days. The goal of the work was fulfilled. The result of the research was confirmation that the spiritual care provided was reflected in their psychological state and also fulfilled the patients expectations. In this scenario, many companies seek new ways to become and / or remain differentiated. This way, the importance of research in this area, where this thesis sought to an exploratory study in a multinational company subdivided into subsidiaries in the branch of service in the area of power generation that operate in the Amazon. The purpose of this research is to understand how the organization assesses the performance of internal logistics operations and compare the measurement systems used with those indicated by current literature. Thus, conditioning was continuously improve the measurement of processes, indicating a competitive advantage in its strategic planning in order to contribute to increasing competitiveness and raising the level of services in its segment. He also applied a semi-structured interview with key business people and an extensive documentary analysis. The study also revealed that the performance of logistics systems investigated have significant conceptual differences within the company investigated in relation to that indicated by the literature used and the persons responsible for undertaking the process itself is deeply unaware of organization and business, where he noted there is a lack of tools to assist them in making strategic decisions, only the prevailing conception of cognitive professional members. / Ações imprevisíveis no mercado, rápida mudança nos processos, variação do consumo e volatilidade nos níveis de demanda são alguns elementos que caracterizam incerteza no mercado atual. Neste cenário, muitas empresas procuram novas formas de se tornarem e/ou se manterem diferenciadas. Desta forma, cresce a importância de estudos nesta área, onde nesta dissertação buscou-se um estudo exploratório em uma empresa multinacional subdividida em subsidiárias na ramificação de prestação de serviços na área de geração de energia que atuam no Amazonas. O objetivo da pesquisa é compreender como esta organização avalia a performance das operações da logística interna e comparar os sistemas de medição utilizados com os indicados pela literatura vigente. Desta maneira, foi condicionado melhorar continuamente a medição dos processos, apontando vantagem competitiva em seu planejamento estratégico, de forma, a contribuir para o aumento da competitividade e a elevação do nível de serviços em seu segmento. Aplicou-se também uma entrevista semi-estruturada com as pessoas chaves do negócio e uma extensa análise documental. O estudo ainda revelou que os sistemas de desempenho logístico investigados possuem diferenças importantes conceituais dentro da empresa pesquisada em relação ao indicado pela literatura utilizada e as pessoas responsáveis pela empresa desconhecem profundamente o próprio processo organizacional e empresarial, onde se constatou há ausência de ferramentas para auxiliá-los à tomada de decisões estratégicas, prevalecendo apenas à concepção cognitiva profissional dos membros.

Experimental Investigation of Hydrodynamic Effects on a Vibrating Kaplan Runner

An experimental investigation of a vibrating Kaplan turbine runner was performed in order to understand the hydrodynamic effects and to obtain or confirm the mass and damping coefficients used for dimensioning at the design stage. Improved design can lead to increased efficiency and lifetime of hydropower stations. The method was based on the 90◦ phase shift between acceleration and velocity and their relationship with mass and damping respectively. The experiment examined frequencies between 1–9 Hz at displacements between 0.25–2.00 mm. Results showed a frequency dependent added mass which varied between 1.2 and 1.5 (neglecting the highest and lowest frequencies) and an added damping between 0.8 and 1.2 which became of importance at low frequencies. A mathematical interpretation of the fluid solid interactions (based on the constitutive equation for stresses in a Newtonian fluid) has been derived and connected to the obtained experimental data.

Experimental Investigation of Fluid-added Parameters on a Kaplan Runner

In order to reach climate and environmental goals, Sweden is increasing the implementation of intermittent renewable energy sources such as wind and solar power to the electricity grid. The increase of intermittent energy sources is rising power regulation requirement towards hydropower, which increasingly exposes the hydraulic turbines to high loads and fluctuating hydraulic forces. These conditions affect the turbine’s structural and rotor dynamic behavior, leading to fatigue in turbine components. Identifying the parameters that affect the dynamics of the water turbine is an essential part of analyzing and, if possible, avoiding these situations. Furthermore, accurate rotor dynamic models are necessary to design for a robust hydropower unit and improve the estimate of wear on turbine components. Added parameters (added mass, polar moment of inertia, and damping) are hydrodynamic effects occurring due to interaction between structural vibrations and surrounding fluid. Added parameters can modify the turbine’s natural frequencies and consequently its dynamic behavior. Therefore, it is of interest to study and quantify the impact of these parameters on the turbine for accurate rotor dynamic modeling and turbine design. The added parameters have been investigated by conducting experiments on a model Kaplan runner, for which the project has been divided into two consecutive parts. First, experiments were performed in a test rig, in which the runner was excited in a lateral movement to determine added mass and linear damping. Secondly, experiments were performed in a test rig similar to the first, except the runner was excited in a torsional movement to determine added polar moment of inertia and torsional damping. Force and displacement have been measured during both movements, with the runner placed in air and thereafter in quiescent water. The added parameters were quantified by comparing measurements conducted with the runner in air against those conducted in water. By varying the excitation frequency and amplitude, added parameters have been analyzed against excitation frequency, velocity, and acceleration to determine dependent variables. The dimensionless added mass ratio, γma, was investigated within a range of acceleration of 0.07m/s2 to 5.00 m/s2 and in an excitation frequency of 2-9 Hz. Results exhibited a frequency-dependent added mass ratio, leading to a mass addition variation of 1.00-1.49 times the test rig mass with a mean γma of 1.22. Similarly, the dimensionless added polar moment of inertia, γIp, was investigated within a range of angular acceleration between 2.4 rad/s2 to 29.6 rad/s2 and in an excitation frequency range of 2-10 Hz. The mean added polar inertia ratio, γIp, was obtained as 1.09 times the polar moment of inertia of the test rig, corresponding to an increase in polar inertia of about 9%, compared to the total dry polar inertia of the test rig. Results showed that the added polar inertia ratio varied by approximately 1.8% within the studied frequency range. Thus, no frequency dependence could be determined. Due to measurement uncertainties and limitations of the test rigs, added linear damping and torsional damping could not be determined in either of the existing test rigs (lateral and torsional movement).

Grid frequency stability from a hydropower perspective

Many AC grids suffer from decreased frequency stability due to less system inertia. This has increased the risk of large-scale blackouts. This thesis and its papers address the frequency stability problem from a hydropower perspective. Grid frequency stability assessments often require accurate system inertia estimates. One approach is to estimate the inertia of all individual power plants and sum up the results. We implemented three inertia estimation methods on a Kaplan unit and compared their results. The generator contributed with 92-96% of the unit inertia, which verified the results from previous studies. However, the three methods estimated slightly different values for the unit inertia, which raises the question of when to use which method. Hydropower often deliver frequency control, yet we found no studies which validate Kaplan turbine models for large grid frequency disturbances on strong grids. Therefore, we performed frequency control tests on a Kaplan unit, implemented three hydropower models, and compared the simulation results to the measurement data. The models overestimated the change in output power and energy delivered within the first few seconds after a large change in frequency. Thus, it is important to have sufficient stability margin when using these types of hydropower models to assess the grid frequency stability. The Nordic transmission system operators are updating their frequency control requirements. We used measurement data and simulation models to assess whether improved runner blade angle control could help a Kaplan unit fulfill the coming requirements. The results showed that improved runner control does not improve the performance sufficiently for requirements fulfillment. The requirements are based on an assumption on minimum system inertia and became easier to fulfill if they were implemented with more system inertia. Thus, more inertia could allow more participants to deliver frequency control in the Nordic grid.

The second chapter contains analyzes of external and internal environment of the company and the project. On their basis the third chapter creates a specific proposal for project solution. Due to requirements related to maintaining of all functions of the weir construction was decided to design small hydropower plant completely under the water surface, with flap mounted to the roof of the hydropower plant. Part of the thesis was also an economical assessment of designed hydropower project.

This thesis focuses on the comprehensive approach to human health. She attempts to shed more light on this topic, to document current circumstances related to the psychosomatic medicine and the so called holistic medicine, and to clarify the manner of understanding these concepts nowadays. The theoretical part of the diploma thesis constitutes a rough summary of the comprehensive approach to human treatment, a view of fundamental theories of psychosomatic medicine and its current position in the Czech health system. The thesis also includes an outline of applied terms which are connected to the comprehensive approach to human treatment, especially in the specialized literature. The practical part of this diploma thesis monitors the way in which the media use the terms of holistic medicine and psychosomatic medicine. Further, a general understanding of these concepts is demonstrated by the means of a questionnaire survey on a sample of 454 respondents. Moreover, the practical part is completed by interviews with two specialists of the Czech psychosomatic scene.
