Nutrition and neurodevelopment of the preterm and term infant

Introduction Optimal nutrition is vital in the management of infants born preterm. Dietary fat in infancy is fundamental for the provision of energy for growth and development. Essential fatty acids, specifically Long Chain Polyunsaturated Fatty Acids (LC-PUFAs) such as docosahexaenoic acid (DHA), have been under investigation by several international research groups in the past decade. Essential fatty acids are critical in neurodevelopment as DHA is found in high proportions in structural lipids of cell membranes, particularly in the central nervous system (CNS). The accumulation of essential fatty acids and particularly DHA in the brain and retina occurs most rapidly during the perinatal period, therefore preterm infants are of particular concern (Singer, 2001). Current scientific consensus is that the optimum growth rate for preterm infants is equal to the in utero growth rate throughout the last trimester, however, failure to achieve the optimum intrauterine growth rate is common in preterm infants (Olhager and Forsum, 2003). Preterm infants require large amounts of energy and nutrients with which many infants are not provided or are not able to absorb, due to immature gastrointestinal and metabolic systems and other medical complications (Olhager and Forsum, 2003). There are a number of unresolved issues regarding optimal growth rate and total energy requirements (ER) for preterm infants. Hypotheses/Objectives This study is a “side study” to a double blind randomised controlled trial (RCT) of DHA supplementation in preterm infants. The hypothesis of this “side study” is that increased DHA during the neonatal period would increase total energy expenditure (TEE) and improve neurodevelopmental outcome. Specifically, at term postconceptual age (PCA) it was hypothesised that preterm infants receiving higher intake of DHA would have higher TEE’s due to the acceleration in brain maturation. Also, it was hypothesised that preterm infants receiving high levels of DHA would have TEE’s equivalent to term born infants due to their same brain maturation status. Other hypothesised effects of DHA supplementation include an accelerated maturation of the visual cortical pathways, and accelerated white matter (WM) tract development aiding in brain maturation. The first objective of this study was to measure TEE and ER in very preterm infants when they reached an age of 31-33 weeks post conceptional age (PCA). The effects of DHA supplementation on TEE, at simulated in utero levels, in very preterm infants (born < 33 weeks PCA), when assessed at term equivalent (40 weeks PCA) were studied. Another objective was to compare WM brain tissue volume at term PCA between two preterm groups and then with the term born infants. Visual latency was also compared between the two preterm infant groups and then with the term born infants. Methods TEE was measured using the doubly labelled water (DLW) method which is based on the differential elimination of 2H (deuterium) and 18O from the body subsequent to a loading dose of these isotopes. TEE was measured at the preterm age between 31-33 weeks PCA and again at term PCA. TEE measurements are made at term PCA in a term born control group. Brain assessment was by Magnetic Resonance Imaging and (MRI) and Visual Evoked Potential (VEP). Magnetic resonance imaging quantitatively measured brain volumes and WM. Visual evoked potential would provide information on visual latency and amplitude. Results The cohort consisted of 38 infants. The TEE of the very preterm infant group was measured at 31-33 weeks PCA. The mean (±standard deviation) (SD) TEE was calculated at 80(±27) kcal/kg/d, and using data in the literature for foetal energy accretion of 28kcal/kg/d, the mean ER was calculated to be 108(±27) kcal/kg/d. At term PCA TEE was calculated for the preterm DHA supplemented group to be 56(±19) kcal/kg/d and for the non-DHA supplemented group 70(±39) kcal/kg/d. These measurements were not statistically different. Flash VEP conducted on preterm given different amounts of DHA tested at term PCA found no statistically different measurements. When combining these results and comparing them to measurements of term born infants at term PCA, the right eye measurements showed that preterm infants had statistically greater latencies than term infants. When combining the left and right eye measurements the latencies no statistical significance was found. Amplitude was also not statistically significant between the groups. MRI measures at term PCA were not statistically different DHA supplemented and the non-DHA supplemented preterm infant group. When the preterm infant cohort was combined and compared to the term born infant group, the results showed that preterm infants imaged at term PCA had reduced WM development in a number of frontal lobe projections, and anterior and posterior commissarial pathways of the corpus callosum and corona radiata. Discussion The TEE and ER measurements in this study represent the largest preterm infant cohort to date. The ER values reported here are of value in allowing the calculation of appropriate feeding and nutritional strategies for preterm infants. Although no differences in TEE between the DHA and non DHA supplemented groups were found this may have been due to the small sample size. With regard to the latency outcomes, it can be speculated that if measurements were conducted at a later PCA the correlations may have been stronger and significant. Several other factors may have also affected the results, including alertness of the infant at the time of testing, thickness of the cranium, and other health factors could not be controlled for. This study contains the youngest cohort to be compared via Flash VEP. The MRI data did not find significant differences in brain volume and WM between the DHA supplemented and the non-DHA supplemented groups. The infant CNS is rapidly developing and there are multiple environmental factors which may have affected outcomes. The data did however find differences in WM development between the preterm and term infants. The reduced WM development found in the preterm infants compared to term born infants may provide some explanation for the correlation between preterm birth and poorer cognitive and functional outcomes. Larger studies which extend beyond the first months of life are recommended in order to investigate the long-term relationships between DHA supplementation, TEE and brain maturation.

Tato práce se zabývá studiem fluorescenčně značeného hyaluronanu, konkrétně rhodaminylamino hyaluronátu sodného (Hya-Rh, 40 kDa), pomocí dvouohniskové fluorescenční korelační spektroskopie (2f-FCS). Nejdříve byla prostudována literatura týkající se využití FCS techniky v koloidní chemii a při studiu polymerů, přičemž následně byly shrnuty veškeré poznatky o využití 2f-FCS metody. Na základě prvotních měření byl zjištěn vhodný postup přípravy a způsob uchovávání vzorku Hya-Rh používaném pro následující experimenty. Záhy byly prostudovány možné vlivy koncentrace Hya-Rh na jeho difúzní charakteristiky jak ve vodě, tak ve fyziologickém roztoku. Následně bylo studováno chování Hya-Rh a vliv koncentrace solí alkalických kovů ve vodných roztocích těchto solí a fluorescenčně značeného hyaluronanu. Poté bylo sledováno chování Hya-Rh v závislosti na koncentraci velmi nízkomolekulárního hyaluronanu (VLMW HA, 404 kDa) v čisté vodě i ve fyziologickém roztoku, přičemž získané výsledky byly mezi sebou porovnány. Nakonec bylo využití 2f-FCS metody celkově zhodnoceno a popsáno z hlediska studia chování fluorescenčně značeného hyaluronanu v roztocích.

La complejidad de la información nutricional en el etiquetado de superalimentos andinos necesita de conocimientos matemáticos y nutricionales para poder interpretarse según las necesidades del consumidor. Esta investigación tiene como objetivo analizar los elementos del diseño de información en el etiquetado de granos andinos empacados y su comunicación sobre los beneficios para la salud nutricional. Para ello, se plantea como hipótesis que los elementos del diseño de información facilitan la comprensión mediante códigos visuales estandarizados como la iconografía y la tipografía. Por consiguiente, se eligió un diseño de investigación descriptivo, con el fin de examinar el contenido y los elementos gráficos de forma cualitativa, para comprender de qué manera los elementos del diseño de información exponen esta data y finalmente, contrastarlo con estudios previos. Entre los principales hallazgos se observó el limitado uso de iconografía que alegue beneficios nutricionales, la cual se expresó con frases cortas. También, se manifestó que la legibilidad en el diseño no solo depende de la tipografía, sino también del tratamiento del color y la tridimensionalidad del etiquetado aplicado. Finalmente, se concluyó que los elementos del diseño de información logran comunicar un concepto de manera íntegra sin utilizar texto, mientras que organizan y proyectan una data compleja de forma sintetizada apta para la comprensión del consumidor. / The complexity of the nutritional information in the labeling of Andean superfoods requires mathematical and nutritional knowledge for the customers to interpret them according to their needs. The aim of this research is to analyze the elements of the information design in the labeling of packaged Andean grains and their communication on the benefits for nutritional health. The study hypothesized that the elements of information design facilitate understanding through standardized visual codes such as iconography and typography. The study used a descriptive design and data analysis centered on the visual and content aspects of the labeling to understand how the elements of the information design present this information and discuss previous research. The main findings were that there is limited use of iconography claiming nutritional benefits and it was expressed under short sentences. Also, it was stated that exceptional legibility in the design not only depends on the typography but also the colors and the three-dimensionality of the labeling. Finally, it was concluded that the elements of information design manage the communication of a concept comprehensively without using text while organizing and synthesizing complex data suitable for the consumer understanding. / Trabajo de investigación

Advanced signal processing techniques for multi-target tracking

The multi-target tracking problem essentially involves the recursive joint estimation of the state of unknown and time-varying number of targets present in a tracking scene, given a series of observations. This problem becomes more challenging because the sequence of observations is noisy and can become corrupted due to miss-detections and false alarms/clutter. Additionally, the detected observations are indistinguishable from clutter. Furthermore, whether the target(s) of interest are point or extended (in terms of spatial extent) poses even more technical challenges. An approach known as random finite sets provides an elegant and rigorous framework for the handling of the multi-target tracking problem. With a random finite sets formulation, both the multi-target states and multi-target observations are modelled as finite set valued random variables, that is, random variables which are random in both the number of elements and the values of the elements themselves. Furthermore, compared to other approaches, the random finite sets approach possesses a desirable characteristic of being free of explicit data association prior to tracking. In addition, a framework is available for dealing with random finite sets and is known as finite sets statistics. In this thesis, advanced signal processing techniques are employed to provide enhancements to and develop new random finite sets based multi-target tracking algorithms for the tracking of both point and extended targets with the aim to improve tracking performance in cluttered environments. To this end, firstly, a new and efficient Kalman-gain aided sequential Monte Carlo probability hypothesis density (KG-SMC-PHD) filter and a cardinalised particle probability hypothesis density (KG-SMC-CPHD) filter are proposed. These filters employ the Kalman- gain approach during weight update to correct predicted particle states by minimising the mean square error between the estimated measurement and the actual measurement received at a given time in order to arrive at a more accurate posterior. This technique identifies and selects those particles belonging to a particular target from a given PHD for state correction during weight computation. The proposed SMC-CPHD filter provides a better estimate of the number of targets. Besides the improved tracking accuracy, fewer particles are required in the proposed approach. Simulation results confirm the improved tracking performance when evaluated with different measures. Secondly, the KG-SMC-(C)PHD filters are particle filter (PF) based and as with PFs, they require a process known as resampling to avoid the problem of degeneracy. This thesis proposes a new resampling scheme to address a problem with the systematic resampling method which causes a high tendency of resampling very low weight particles especially when a large number of resampled particles are required; which in turn affect state estimation. Thirdly, the KG-SMC-(C)PHD filters proposed in this thesis perform filtering and not tracking , that is, they provide only point estimates of target states but do not provide connected estimates of target trajectories from one time step to the next. A new post processing step using game theory as a solution to this filtering - tracking problem is proposed. This approach was named the GTDA method. This method was employed in the KG-SMC-(C)PHD filter as a post processing technique and was evaluated using both simulated and real data obtained using the NI-USRP software defined radio platform in a passive bi-static radar system. Lastly, a new technique for the joint tracking and labelling of multiple extended targets is proposed. To achieve multiple extended target tracking using this technique, models for the target measurement rate, kinematic component and target extension are defined and jointly propagated in time under the generalised labelled multi-Bernoulli (GLMB) filter framework. The GLMB filter is a random finite sets-based filter. In particular, a Poisson mixture variational Bayesian (PMVB) model is developed to simultaneously estimate the measurement rate of multiple extended targets and extended target extension was modelled using B-splines. The proposed method was evaluated with various performance metrics in order to demonstrate its effectiveness in tracking multiple extended targets.

Body composition and energy expenditure in men with schizophrenia

There is an increase in the prevalence of obesity among people with schizophrenia thought to be due in part to the weight enhancing side-effects of medications commonly used to treat the symptoms of schizophrenia. Despite the deleterious health effects associated with obesity and its impact on quality of life and medication compliance, little is known about body composition and energy expenditure in this clinical group. The primary purpose of this thesis was to enhance understanding of body composition and energy expenditure, particularly resting energy expenditure in men with schizophrenia who take atypical antipsychotic medications. Unique to this investigation is the evaluation of clinical tools used to predict body composition and energy expenditure against reference methodologies in men with schizophrenia. Further, given the known links between obesity and physical activity, an additional but less comprehensive component of the thesis was a consideration of total and activity energy expenditure in addition to the interaction between psychiatric symptoms, side-effects of antipsychotic medications and physical activity also occurred as part of this thesis. Collectively, the goals of this thesis were addressed through a series of studies – the first two studies were related to the measurement and characteristics of body composition in men with schizophrenia, while the third and fourth studies were related to the measurement and characteristics of resting energy expenditure in men with schizophrenia. The fifth and sixth studies the utilised doubly labelled water technique to quantify activity and total energy expenditure in a small group of men with schizophrenia and explored the use of accelerometry in this cohort. The final study briefly considered the impact of psychiatric symptoms and self-reported medication side-effects on objectively measured physical activity. In the first study, thirty-one male adults previously diagnosed with schizophrenia and sixteen healthy male controls were recruited. Estimates of body composition derived from an anthropometry-based equation and from bioelectric impedance analysis (BIA) using deuterium dilution as the reference methodology to determine total body water were compared. The study also determined the validity of equations commonly used to predict body composition from BIA in the men with schizophrenia. A further aim was to determine the superiority of either BIA or body mass index (BMI) as an indicator of obesity in this cohort. The inclusion of the control group, closely matched for age, body size and body composition demonstrated that there was no difference in the ability of body composition prediction methods to distinguish between fat and fat-free mass (FFM) in controls and men with schizophrenia when both groups had similar body composition. However this study indicated that an anthropometry-based equation previously used in people with schizophrenia was a poor predictor of body composition in this cohort, as evidenced by wide limits of agreement (25%) and systematic variation of the bias. In comparison, the best predictor of percentage body fat (%BF) in this group was gained when impedance values were used to predict percentage body fat via the equation published by Lukaski et al (1986). Although percentage body fat was underpredicted using the Lukaski et al. (1986) equation, the mean magnitude was relatively small (1.3%), with the limits of agreement approximately 13%. Linear regression analysis revealed that %BF predicted using the Lukaski et al. (1986) equation explained 25% more of the variance in percentage body fat than BMI. Further, this study also indicated that BIA was more sensitive than BMI in distinguishing between overweight and obesity in this cohort of men with schizophrenia. Because of the almost exclusive use of BMI as an indicator of obesity in people with schizophrenia, the level of excess body fat may be in excess of that previously indicated. The second study extended the examination of body composition in men with schizophrenia. In this study, the thirty-one participants with schizophrenia (age, 34.2 ± 5.7 years; BMI, 30.2 ± 5.7 kg/m2) were individually matched with sedentary controls by age, weight and BMI. Deuterium dilution was used to distinguish between FFM and fat mass. The previous study had indicated that while BIA was a suitable group measure for obesity, on an individual level the technique lacked the precision required for investigating body composition in men with schizophrenia. Waist circumference was used as an indicator of body fat distribution. The findings of this study indicated that in comparison with healthy sedentary controls of similar body size and age, men with schizophrenia had higher levels of body fat which was more centrally distributed. Percentage body fat was on average 4% higher and waist circumference, on average 5 cm greater in men with schizophrenia than the sedentary controls of the same age and BMI. Further, this study indicates that the use of BMI to predict body fat in men with schizophrenia will result in greater bias than when it is used to predict body fat in other sedentary men. Commonly used regression equations to predict energy requirements at rest are based on the relationships between weight and resting energy expenditure (REE) and in such equations, weight acts as a surrogate measure of FFM. The objectives of study three were to measure REE in a small group of men with schizophrenia who were taking the antipsychotic medication clozapine and to determine whether REE can be predicted with sufficient accuracy to substitute for the measurement of REE in the clinical and/or research settings. Body composition was determined using deuterium dilution and REE was measured using a Deltatrac Metabolic Cart via a ventilated hood. The male participants, (aged 28.0 ± 6.7 yrs, BMI 29.8 ± 6.8 kg/m2) were weight stable at the time of the study and had been taking clozapine for 20.5 ± 12.8 months, with doses of 450 ± 140 mg/day. Of the six prediction equations evaluated, the equation of Mifflin et al. (1990) with no systematic bias, the lowest bias and the lowest limits of agreement proved to be the most suitable equation to predict REE in this cohort. The overestimation of REE can be corrected for by deducting 160 kcal/day from the predicted REE value when using the Mifflin et al. (1990) equations. However, the magnitude of the error associated with the prediction of REE for an individual is 370 kcal/day. The findings of this study indicate that REE cannot be predicted with sufficient individual accuracy in men with schizophrenia, therefore it was necessary to measure rather than predict REE in subsequent studies. In the fourth study, indirect calorimetry (Deltatrac Metabolic Cart via ventilated hood) and deuterium dilution were used to accurately determine REE, respiratory quotient (RQ) and FFM in 31 men with schizophrenia and healthy sedentary controls individually matched for age and BMI. Data from this study indicated that gross REE was lower in men with schizophrenia than in healthy sedentary controls of a similar age and body size. However, there was no difference between the groups in REE when REE was adjusted for FFM using the mathematically correct method (analysis of covariance with FFM as the covariate). There was however a statistically and clinically significant difference in resting, fasted RQ between men with schizophrenia and controls, suggesting that RQ rather than REE may be an important correlate worthy of further investigation in men with schizophrenia who take antipsychotic medications. Studies five and six involved the application of the doubly labelled water (DLW) technique to accurately determine total energy expenditure (TEE) and activity energy expenditure (AEE) in a small group of men with schizophrenia who had been taking the atypical antipsychotic medication clozapine. The participants were those who took part in study three. The purpose of these studies was to assess the validity of a commercially available tri-axial accelerometer (RT3) for predicting free-living AEE and to investigate TEE and AEE in men with schizophrenia. There was poor agreement between AEE measured using DLW and AEE predicted using the RT3. However, using the RT3 to measure inactivity explained over two-thirds of the variance in AEE. This study found that the relationship between current AEE per kilogram of body weight and change from baseline weight in men taking clozapine was strong although not significant. The sedentary nature of the group of participants in this study was reflected in physical activity levels, (PAL, 1.39 ± 0.27), AEE (435 ±352 kcal/day) and TEE (2511 ± 606 kcal/day) that fell well short of values recommended by WHO (2000) for optimal health and to prevent weight gain. Given the increasing recognition of the importance of sedentary behaviour to weight gain in the general community, further examination of the unique contributing factors such as medication side effects and symptoms of mental illness to activity levels in this clinical group is warranted. The final study used accelerometry (RT3) to objectively measure activity in a group of 31 men with schizophrenia who had been taking atypical antipsychotic medications for more than four months. The purpose of this study was to explore the relationships between psychiatric symptomatology, side-effects of medication and physical activity. Accelerometry output was analysed to provide a measure of inactivity and moderate intensity activity (MIA). The well-validated and reliable standardised clinical interview, the Positive and Negative Syndrome Scale (PANSS) was used as a measure of psychiatric symptoms. Perceived side-effects of medication were assessed using the Liverpool University Neuroleptic Rating Side-Effects Scale (LUNSER). Surprisingly, there was no relationship reported between any measures of negative symptoms and physical inactivity. However, self-reported measures of medication side-effects relating to fatigue, sleepiness during the day and extrapyramidal symptoms explained 40% of the variance in inactivity. This study found significant relationships between some negative symptoms and moderate intensity activity. Despite the expectation that as symptoms of mental illness reduce, inactivity may diminish and moderate intensity activity will increase, it may not be surprising that in practice this is an overly simplistic view. It may be that measures of social functioning and possibly therefore cognition may be better predictors of physical activity than psychiatric symptomatology per se.

O tratamento baseia-se na irradiação e destruição do tumor, preservando os órgãos normais quanto ao excesso de radiação. Radionuclídeos emissores &beta;- como 90Y, 131I, 177Lu e 188Re, são úteis para o desenvolvimento de radiofármacos terapêuticos e, quando associados a AcM como o Anti-CD20 são importantes principalmente para o tratamento de Linfomas Não Hodgkins (LNH). 188Re (E&beta;- = 2,12 MeV; E&gamma;= 155 keV; t1/2 = 16,9 h) é um radionuclídeo atrativo para RIT. O Centro de Radiofarmácia do IPEN possui um projeto que visa a produção do radiofármaco 188Re-Anti-CD20. Com isso,este estudo foi proposto para avaliar a eficácia desta técnica de marcação para tratamento em termos compartimentais e dosimétricos. O objetivo deste trabalho consistiu na compararação da marcação do AcM anti-CD20 com 188Re com a marcação do anticorpo com 90Y, 131I, 177Lu e 99mTc (pelas suas características químicas similares) e 211At, 213Bi, 223Ra e 225Ac. Através do estudo de técnicas de marcação relatadas em literatura, foi proposto um modelo compartimental para avaliação de sua farmacocinética e estudos dosimétricos, de alto interesse para a terapia. A revisão de dados publicados na literatura, possibilitou demonstrar diferentes procedimentos de marcação, rendimentos de marcação, tempo de reação, impurezas e estudos de biodistribuição. O resultado do estudo mostra uma cinética favorável para o 188Re, pelas suas características físicas e químicas frente aos demais radionuclídeos avaliados. O estudo compartimental proposto descreve o metabolismo do 188Re-anti-CD20 através de um modelo compartimental mamilar, que pela sua análise farmacocinética, realizada em comparação aos produtos marcados com emissores &beta;-: 131I-antiCD20, 177Lu-anti-CD20, o emissor &gamma; 99mTc-anti-CD20 e o emissor &alpha; 211At-Anti-CD20, apresentou uma constante de eliminação de aproximadamente 0,05 horas-1 no sangue do animal. A avaliação dosimétrica do 188Re-Anti-CD20 foi realizada através de duas metodologias: pelo método de Monte Carlo e pelo uso de uma fonte pontual &beta;- através da Fórmula de Loevinger via programa Excel. Através da Fórmula de Loevinger fez-se a validação do método de Monte Carlo para a dosimetria do 188Re-Anti-CD20 e dos demais produtos. As doses e as taxas de doses obtidas pelos dois métodos foram avaliadas em comparação à dosimetria do 90Y-Anti-CD20, 131I-Anti-CD20 e do 177Lu-Anti-CD20, obtidas pela mesma metodologia. O estudo de dose foi realizado utilizando modelos matemáticos considerando um camundongo nude de 25g, simulando diferentes tamanhos de tumor e diferentes formas de distribuição do produto dentro do animal. De acordo com os resultados obtidos, pela energia de emissão &beta;-, 188Re-Anti-CD20 apresenta maior deposição de energia para tumores volumosos em relação aos demais produtos avaliados. Em uma simulação com 100% do produto captado pelo tumor, 89% da dose total manteve-se absorvida pelo tumor, preservando a integridade de ógãos críticos como coração (2%), pulmões (5%), coluna (4%), fígado (0,014%) e rins (0,0007%). Em uma simulação onde há uma biodistribuição do produto no organismo do animal, 38% da dose total é absorvida pelo tumor e >3% é absorvida pela coluna. Nessa situação mais próxima da realidade, a extrapolação dos dados para um humano de 70kg, mostrou que a dose absorvida no tumor corresponde a cerca de 33%; na coluna 7% e o coração receberia uma dose de 35% do total. No. of bitstreams: 0 / Neste trabalho foi transfectada uma linhagem de células embrionárias de rim humano (HEK293) com os genes das subunidades &alpha; e &beta; da tireotrofina humana (hTSH), hormônio glicoproteico secretado pela hipófise. Após 5 dias de cultivo obteve-se uma concentração de hTSH no meio condicionado de 0,95&mu;g/mL. O material foi concentrado e purificado utilizando uma estratégia envolvendo duas etapas, uma cromatografia de troca catiônica e uma cromatografia líquida de alta eficiência (HPLC) de fase reversa, que permitiu uma recuperação de 55% e uma pureza >90%. O produto purificado (hTSH-HEK) foi analisado e comparado a uma preparação comercial obtida em células CHO (hTSH-CHO) e a uma preparação hipofisária (hTSH-Pit). A identidade e a pureza do hTSH-HEK foram avaliadas por métodos físicoquímicos e imunológico (espectrometria de massa MALDI-TOF, HPLC de exclusão molecular e de fase reversa, SDS-PAGE e ensaio imunoradiométrico). A porção glicídica do hTSH-HEK foi avaliada pela análise do perfil dos N-glicanos e o comportamento biológico deste hormônio foi avaliado por bioensaio in vivo e estudo farmacocinético. As 3 preparações apresentaram pureza equivalente (97%) e a massa molecular relativa do hTSH-HEK foi 2,1% menor do que a do hTSH-CHO e 2,7% maior do que a do hTSH-Pit. A maior hidrofobicidade relativa, avaliada por RP-HPLC, foi a do hTSH-HEK. Os N-glicanos identificados no hTSH-HEK foram do tipo complexo, apresentando predominantemente estruturas tri-antenárias, enquanto no hTSH-CHO e no hTSH-Pit as estruturas bi-antenárias foram predominantes. Foram detectadas diferenças significativas relacionadas à composição dos carboidratos para estas preparações, um teor muito menor de ácido siálico e muito maior de fucose foram observados no hTSHHEK. Foi confirmada a atividade biológica das 3 preparações, sendo a bioatividade do hTSHHEK 39% e 16% inferior à do hTSH-CHO e hTSH-Pit, respectivamente. A meia-vida circulatória do hTSH-HEK foi menor (1,5 X) que a do hTSH-CHO e a do hTSH-Pit (1,2 X). Quantification of cerebral blood flow with 15O-water PET : A comparison study between PET/CT and PET/MR and two different blood sampling instruments

Cerebral blood flow quantification is a vital diagnostic tool for disease monitoring and used for diagnosing a variation of pathological conditions. The human brain requires roughly about 20 % of the total cardiac output to sustain normal functioning, hence the perfusion of blood is an important factor to deliver oxygenated blood. The golden standard for quantifying the cerebral blood flow follows by measurement with dynamic positron emission tomography of 15O-labelled water modelled by tracer kinetic compartments. For implementation, knowledge of an input function must exist which is in general being sampled through arterial cannulation of the radial artery with a continuous sampling instrument. The core of this thesis is to establish if two sampling instruments contradicts in comparison to each other when sampling the data to the input function.  In total 22 subjects underwent a 10-minute dynamic  15O-labeled water brain PET scan on two imaging modalities PET/CT and PET/MR. Continuous arterial sampling was performed either by a Veenstra on PET/CT or a Swisstrace on PET/MR during a baseline scan. In two subjects the two sampling instruments were coupled in series and imaged solely on the PET/CT.  Cerebral blood flow analysis was done comparing varying dispersion times, the two imaging modalities compared each other and comparing the calculated and measured blood flows obtained through this study with the values obtained prior. To be able to compare the values showing inconsistency to the values obtained through this thesis, a comparison between two different iterative reconstruction methods was done. Here the method of ordered subsets expectation maximum was compared to a Bayesian penalized-likelihood method. To further compare the two sampling instruments an image derived input function was constructed and compared with the blood sampled input function. The results showed that there was no significant difference between measured cerebral blood flow between the two imaging modalities with the currently used reconstruction method based on Bayesian penalized likelihood but presented in the earlier data there was an inconsistency. A dispersion analysis with variation on the external dispersion time shows that if the time was chosen to low or to high compared to the standard time used it introduced distorted fitted models of the activity curves. This distortion creates further errors in the calculation of the cerebral blood flow, however with the analysis the standard dispersion time could be confirmed as an accurate fit. Subjects imaged with the two sampling instruments in series showed no significant difference except for the measured values on Veenstra to be slightly higher. Lastly the correlation between the image derived input function and the blood sampled input function showed poorly performance. Only a R2 value of 0.42 was achieved on the PET/CT while a meagre R2 value of 0.18 was achieved on the PET/MR. Although the correlation was poorly, the plotted activity curves from the two functions showed a representable appearance between each other. / Kvantifiering av det cerebrala blodflödet är ett nödvändigt diagnostiskt verktyg som används för att kontrollera och diagnostisera en variation av patologiska sjukdomstillstånd. Den mänskliga hjärnan kräver kring 20 %av den totala produktionen från hjärtat för att upprätthålla normal funktion, följaktligen är perfusion av blod en viktig faktor för att distribuera syrerikt blod runt om i kroppen. Den gyllene standarden för kvantifiering av det cerebrala blodflödet följer som undersökning med dynamisk positron emission tomografi av 15O-märkt vatten, modellerat  med kinetisk kompartment teori. För att kunna implementera detta måste information om en input-funktion erhållas, generellt erhålls detta genom att blod tags genom arteriell kanylering av antingen den radiella artären med ett kontinuerligt samplings instrument. Målet med detta arbete är att fastställa om två samplings instrument motsäger varandra vid mätning av data till input-funktionen. Totalt deltagande är 22 patienter som genomgick en 10-minuters dynamisk 15O-märkt vatten PET undersökning av hjärnan på två bildtagningsmodaliteter PET/CT och PET/MR. Kontinuerlig blodtagning genomfördes antingen med en Veenstra sampler instrument på PET/CT eller en Swisstrace sampler instrument på PET/MR tillsammans med en baseline undersökning. Vid två undersökningar seriekopplades de två instrumenten och patienterna blev endast undersökta vid PET/CT. För ytterligare kunna utvärdera de två instrumenten, konstruerades en bild framtagen input-funktion som sedan kunde jämföras med den blod samplade input-funktionen. Cerebrala blodflödes analyser gjordes med olika dispersions tider, även för att kunna jämföra de två bildtagningsmodaliteterna mot varandra och jämföra erhållna värden framtagna under denna studie med en tidigare studie. För att kunna jämföra avvikelserna i de uppmätta värdena har även två olika rekonstruerings metoder studerats. Resultaten visar ingen signifikant skillnad mellan de uppmätta cerebrala blodflödena mellan de två bildtagningsmodaliteterna rekonstruerade med den nuvarande standarden. Dispersions analysen med varierande extern dispersions tid visar att om tiden är för kort eller för lång jämfört med standardtiden, introduceras en osann anpassning av aktivitets kurvorna. Denna förvrängning av datat resulterar till fler avvikelser i beräkningarna av blodflödet, likväl var det möjligt att bekräfta standardtiderna som används. Patienter som undersöktes med instrumenten i seriekoppling visade ingen signifikant skillnad förutom att det uppmättes en aningens högre värden hos patienter med Veenstra som blod sampler. Slutligen, korrelationen mellan den bild framtagna input-funktionen och den blod samplade input-funktionen visade ett dåligt resultat. Endast ett R2 värde på 0.42 erhölls för PET/CT medan endast ett R2 värde på 0.18 på PET/MR erhölls. Trotts att korrelationen var dålig, visade de plottade aktivitets kurvorna ett representativt utseende mellan de två typerna av input funktion.

A fin de diseñar e implementar lenguajes de programación que tengan en cuenta el paradigma borroso modificaremos el lambda cálculo clásico, adjuntando a cada término un grado, y redefiniendo la beta-reducción, obteniendo que para que el nuevo cálculo verifique la propiedad de Church-Rosser la transmisión de los grados debe hacerse por medio de una función que sea una t-norma o s-conorma. Utilizando esta nueva herramienta diseñamos un lenguaje no determinista que satisface los requerimientos de la programación con datos borrosos. / With the aim of designing and implementing programming languages that take into account the fuzzy paradigm we will modify the classical lambda calculus by adding a degree to each term and by redefining the b-reduction. Thus, for the new calculus to verify the Church-Rosser property, the degree computed with can be made through a function that is a t-norm or an s-conorm. With this new tool we design a nondeterminist language that satisfies fuzzy dataprogramming requirements, and an example of its behaviour is shown.
