Bakgrund: Att motivera deprimerade patienter till fysisk aktivitet upplevde sjuksköterskor många gånger var svårt på grund av patientens tillstånd. Syfte: Studien syftade till att undersöka och beskriva sjuksköterskors erfarenheter av att motivera patienter med depression inom vuxenpsykiatrisk öppenvård till att aktivera sig fysiskt. Metod: Författarna genomförde en semistrukturerad intervjustudie baserat på tolv intervjuer som analyserades genom en kvalitativ innehållsanalys. Resultat: I studien framkom vikten av att anpassa arbetet efter individens förutsättningar. Sjuksköterskorna använde sig inte av någon specifik metod, men utifrån intervjuernas innehåll kunde fyra kategorier avseende innehållet i kontakterna mellan sjuksköterska och patient identifieras: att informera, stödja, vägleda och att skapa en utvecklande miljö. Slutsats: Sjuksköterskorna fokuserade på hela patientens livssituation då de ansåg att många bitar spelar in för att man ska kunna motivera patienten till fysisk aktivitet. / Background: Nurses' experience of motivating patients with depression to physical activity was that motivational work with patients was often difficult due to the patient's condition. Objective: The study aimed to investigate and describe nurses' experiences of motivating patients with depression in adult psychiatric outpatient care to become physically active. Method: The authors conducted a semi-structured interview study based on twelve interviews that were analyzed through a qualitative content analysis. Results: The study revealed the importance of adapting their work to the individual's circumstances. The nurses did not use any specific method, but based on the content of the interviews, four categories regarding the content of the contacts between the nurse and the patient were identified: to inform, to support, to guide and to create a developing environment. Conclusion: The nurses focused on the entire patient's life situation as they considered many parts had a role to play in order to motivate the patient to physical activity. / <p>Godkännandedatum: 2020-05-25</p>

Sjuksköterskors erfarenheter av språkbarriärer i vården : En litteraturöversikt / Nurse’s experiences of language barriers in healthcare : A Literature review

Bakgrund: Språkbarriärer i hälso- och sjukvården har blivit alltmer förekommande då invandringen och migrationen ökat. Vikten av Kommunikation, jämlik vård, patientsäkerhet och patienters erfaranheter framförs. Syfte: Syftet med litteraturstudien var att belysa sjuksköterskors erfarenheter av språkbarriärer i vården. Metod: En allmän litteraturöversikt genomfördes enligt Fribergs (2017) analysmodell. 10 vetenskapliga artiklar som analyserades varav 9 var kvalitativa och en kvantitativ artikel. Artiklar hämtades från databaserna PUBMED, CINAHL Complete och Medline med följande sökord: nurse perception, nurse-patient-relations, nurse experience och language barriers. Resultat: Resultatet visade att vårdkvaliteten påverkades negativt av språkbarriärer, resultatet påvisade även risk för patientsäkerheten. Sjuksköterskor tillämpade olika sätt för att besegra språkhindren. Litteraturstudiens resultat presenteras utifrån dessa huvudkategorier: språkbarriärer leder till osäkerhet, frustration och oro, att kommunicera genom tolk, att tala samma språk och alternativa kommunikationsmedel. Sammanfattning: I denna litteraturöversikt har sjuksköterskors erfarenheter av språkbarriärer i vården belysts samt hur det påverkar omvårdnaden. För att uppnå jämlikhet och kunna ge personcentrerad vård till patienter så bör sjuksköterskan ha en kulturell kompetens samt förstå innebörden av kulturell omvårdnad. Det är viktigt att kunna utbilda framtida sjuksköterskor och etablerade sjuksköterskor om språkbarriärer samt om mångkulturell omvårdnad. / Background: Language barriers in health care have become increasingly common as immigration and migration have increased. The importance of communication, equal care, patient safety and patient’s experiences of language barriers are presented. Aim: The purpose of the literature study was to shed light on nurse’s experiences of language barriers in care. Method: A general literature review was conducted according to Fribergs (2017) analysis model. 10 scientific articles were analyzed of which 9 were qualitative and one quantitative article. Articles were retrieved from the databases PUBMED, CINAHL Complete and Medline with the following keywords: nurse perception, nurse-patient-relations, nurse experience and language barriers. Results: The results showed that the quality of care was negatively affected by language barriers and risked patient safety. Nurses applied different ways to overcome language barriers. The results of the literature study are presented based on these main categories: Language barriers lead to uncertainty, frustration and anxiety, to communicate through an interpreter, to speak the same language and alternative means of communication. Summary: In this literature review, nurses' experiences of language barriers in care have been highlighted, as well as how it affects nursing. In order to achieve equality and be able to provide person-centered care to these patients, the nurse should have a cultural competence and be able to understand the meaning of cultural nursing. It is important to be able to train future nurses and established nurses about language barriers and about multicultural nursing.

The Lived Experience of Nurses in Caring for Patients with COVID-19

Limited research exists about the experiences of nurses’ caring for patients during the COVID-19 pandemic post hoc. To understand nurses’ realities of caring for patients with COVID-19, I aimed to understand the lived experience of nurses caring for patients with COVID-19 in the United States. A qualitative design with a hermeneutic phenomenological methodology was used. Sixteen participants were recruited via purposive sampling, augmented with snowball sampling. Data were collected through unstructured interviews and were analyzed using Braun and Clarke’s Reflexive Thematic Analysis. Nurses’ experiences of caring for patients with COVID-19 in the U.S. were consolidated into four themes: “a living hell”; “rationing patient safety”; “mental aftermath of the war zone”; and “post-pandemic pride”. Nurses provided patient care amidst challenging environments of limited to no resources, with extensive, large-scale critically ill patients, and patient deaths. Due to overwhelming patient care demands, nurses were unable to provide safe patient care to everyone in need, resulting in patient deterioration and death. These experiences caused nurses to endure maladaptive mental effects, such as compassion fatigue and moral suffering, which can threaten the safety of patients. Despite these undesirable results, nurses continued to provide patient care and expressed feelings of pride in the profession of nursing for surviving the COVID-19 pandemic. However, this study demonstrates the persistent impacts on nurses’ abilities to provide safe care after the crisis years. Support for nurses is recommended to preserve patient safety.

Bakgrund: En individanpassning efter kulturella referensramar förutsätter personcentrerad vård i transkulturella vårdmöten. Att omvårdnaden är kulturell kompetent är meningsfullt för att säkerställa jämlik och effektiv vård för alla patienter, oavsett deras kulturella bakgrunder. Med ett mångkulturellt samhälle som bakgrund och jämlik vård som mål inom hälso- och sjukvården, blir betydelsen av ökad kulturell kompetens alltmer accentuerad. Syfte: Att belysa sjuksköterskors upplevelser av transkulturella vårdmöten i Skandinavien. Metod: En litteraturstudie baserad på 12 kvalitativa vetenskapliga artiklar. Resultat: Fyra kategorier och 13 underkategorier identifierades. Kategorierna var: Upplevelser av utmaningar i transkulturella vårdmöten, Upplevelser av osäkerhet i transkulturella vårdmöten, Upplevelser av otillräcklighet i transkulturella vårdmöten, Upplevelser av svårigheter i transkulturella kommunikationen. Konklusion: Trots utmaningar i transkulturella vårdmöten visade sjuksköterskorna en god kulturell medvetenhet, en vilja att förvärva kulturell kunskap samt en adekvat kulturell sensitivitet, vilket bekräftade möjligheten till att utveckla kulturella kompetenser och ge kulturkongruent omvårdnad. / Background: An individual adaptation based on cultural frameworks presupposes person-centered care in transcultural care encounters. Ensuring cultural competence in care is meaningful to achieve equitable and effective healthcare for all patients, regardless of their cultural backgrounds. With a multicultural society as the backdrop and equitable care as the goal in healthcare, the importance of increased cultural competence becomes increasingly emphasized. Aim: To illustrate nurses' experiences of transcultural care encounters in Scandinavia. Method: A literature review based on 12 qualitative scientific articles. Results: Four categories and 13 subcategories were identified. The categories were: Experiences of challenges in transcultural care encounters, Experiences of uncertainty in transcultural care encounters, Experiences of inadequacy in transcultural care encounters, Experiences of difficulties in transcultural communication. Conclusion: Despite experiences of negative elements, nurses demonstrated good cultural awareness, a willingness to acquire cultural knowledge, and adequate cultural sensitivity, confirming their potential to develop cultural competencies and provide culturally congruent care.
