Barium enemas : the patients' perspective : are we satisfying their psychological needs?

Quality of patient care has received much publicity with patients becoming more informed about their rights in the hospital. Consequently they are making greater healthcare demands. Increasing consideration needs to be taken of the quality of care given to the patients, and of their feelings about the services provided. The research investigates patients experiences of barium enemas, with the aim of gaining a holistic view of the examination, and addresses the area of patient information in this context. The underlying purpose of the research is to employ professional knowledge to address the psychological needs of patients. The research has tracked the patient's experience and perception of the barium enema examination throughout its whole process. This involved three main phases using both qualitative and quantitative methods, each focusing on patient care and satisfaction. Using interviews it was established that patients attending for barium enema were anxious. Following this the relationship between information and anxiety was addressed using a computer assisted learning package and a specially designed information leaflet. It was found that patients who had received the information leaflet had statistically significant reduced levels of anxiety. Furthermore, patients receiving the leaflet had a stronger feeling of having sufficient information than those who received only the standard hospital information. Giving the patient information relating to the procedures and likely sensations has been shown to reduce their anxiety level. The computer package showed potential for knowledge gain and usability. Implications of the results are in the area of professionalism, role development and standards for care of the patient in the diagnostic imaging department. Clinical implications are at a patient management level, with attention to continuity of care. It is suggested that high quality patient care and increased patient satisfaction can result when a more holistic and patient centred approach is adopted by radiographers.

However, the quality of patient information recording is a major problem being experienced by health services institutions in many countries. In Burundi there is a lack of accurate information recording and processing of patient information. The quality of patient information recorded is unreliable for decision making and management of healthcare delivery. Aim: This study investigated the quality of patient information record-keeping among nurses in selected hospitals in Burundi. The objectives were as follows: (a) to describe the knowledge of nurses on the benefits of the quality of patient record-keeping in selected hospitals in Burundi, (b) to describes nurses’ attitudes on the benefits of the quality of patient record-keeping in selected hospitals in Burundi, c) to determine the association between sociodemographic characteristics and nurses’ knowledge and attitudes on the benefits of the quality of patient record-keeping in selected hospitals, and (d) to identify barriers influencing the quality of record-keeping in selected hospitals in Burundi. Methodology: The study employed a quantitative research approach using a descriptive survey design. The target population for this study was N=121 nurses; an all-inclusive sampling technique was used to include all 121 qualified nurses who were working in the selected hospitals. A self-administered questionnaire was used to collect the data. The data were analysed using SPSS version 24. Statistical variables such as knowledge, attitudes and barriers were analysed using descriptive statistics to generate frequency, and cross tabulation using chi square test was also adopted to determine the association between sociodemographic characteristics and nurses’ knowledge and attitudes on the benefits of the quality of patient record-keeping. The reliability and validity of the instrument were established and a pre-test was conducted to validate the questions. All ethical principles regarding the study were adhered to. The findings from this study provide relevant information about the benefits of quality of patient record-keeping for the management of the hospitals and all relevant stakeholders in Burundi.

Syfte: Syftet med denna litteraturstudie var att undersöka hur information når fram till patienter med hjärt-kärlsjukdomar. Metod: Systematisk litteraturstudie. 15 artiklar, både kvalitativa och kvantitativa, granskades till resultatsdelen efter sökningar genom databaserna Academic Search Elite, CINAHL och PubMed, samt sökmotorn Elin@Dalarna. Artiklarna som använts är från 2002 och framåt. De hade en kvalitet som var antingen medel eller hög efter en granskning med hjälp av granskningsmallar för kvalitetsbedömning.Huvudresultat: Det fanns behov för patienter och deras anhöriga att få information om patientens tillstånd och kommande besvär. Riskuppfattning, riskbearbetning, känslor, personliga värderingar, social press, miljö och ekonomiska förutsättningar bidrog som barriärer för att få informationen att nå fram till patienterna. Patienterna uppgav att det behövdes förbättringar angående information och undervisning. Det fanns många områden där förbättringar kunde göras angående information och undervisning vid hjärt- och kärlsjukdomar. Ibland ges inte den informationen som patienterna tycker att de behöver.

No / Many service commissioners are demanding a reduction in pressure ulcer prevalence and regard pressure ulceration as a key indicator of care quality. Within our area of practice, local commissioners have indicated that all health care providers in the district should work together to reduce pressure ulceration across the local health care economy. Health care professionals clearly have a critical role to play in patient assessment, risk categorisation, care planning and equipment provision. However, this alone will not be sufficient to achieve the reduction targets which will involve effective patient engagement. National and International guidelines all recognise the importance of patient education in care and recognise the significance of patient involvement in personalised care planning and service provision. Hartigan et al have demonstrated the value of education leaflets in supporting pressure ulcer prevention in an elderly population. Patient support applications running on mobile phones and tablets are also available to assist in pressure ulcer prevention and patient education but are not widely used in a hospital setting. This study examines how effective standard verbal and written information is at delivering patient education for pressure ulcer prevention. Local hospital policy is that all patients identied as being at risk of developing pressure ulceration are provided with information on what a pressure ulcer is, what constitutes risk and how to assist staff in pressure ulcer prevention. The policy includes patient and carer involvement in care planning, and encouragement to report skin changes and pain to staff.

Information ska ses som en integrerad del i god omvårdnad av den patient som befinner sig i det akuta skedet av en hjärtinfarkt och är en förutsättning för en god vård. Sjuksköterskans bemötande och kommunikation är en viktig del i omvårdnaden av patienten. Det är av betydelse att sjuksköterskan har en förmåga att förmedla kunskap, förståelse under de stressade förhållanden som ibland kan råda på en akutmottagning. På så sätt ges förutsättningar för att patienten att kunna vara delaktig och ha inflytande över sin egen vård och behandling. Syftet med studien är att belysa informationens betydelse för patienten i den akuta fasen av en hjärtinfarkt. Metoden som använts är en litteraturstudie och resultatet består av 12 artiklar, 10 kvalitativa och 2 kvantitativa. I resultatet finns fem kategorier: patientens upplevelse av information, sjuksköterskans roll vid information, kommunikation mellan sjuksköterska och patient, sjuksköterskans stöd till patienten samt patientens oro och rädsla. Resultatet i litteraturstudien belyser att bra information som talas i klartext minskar patientens oro, reducerar ångest och ökar patientens förståelse för sin sjukdom. Patienter som får relevant och individuell information känner även tillfredställelse med vården och upplever ökat välbefinnande. / Information should be seen as an integrated part in caring for patients´ that suffer from acute myocardial infarction. The method that has been used is a literature study and the result comprizes in 12 articles, 10 qualitative and 2 quantitative. In the result, five categories has shown. These are: The patient´s experience of the information, the nurse´s roll when giving information, communication between the nurse and the patient, the nurses support to the patient and the patient´s anxiety and fear. The result in the literature study shows that good information in plain language reduces the patient´s worries and anxiety and increases the patient´s understanding of his/her condition. Patients who are given relevant and individual information feel satisfaction with the care and experience an increased well-being.

The perception of information needs of acute myocardial infarction patients and intensive care nurses: a comparative study

(Nursing), Faculty of Health Sciences, University of the Witwatersrand, 2011 / In this present study, the instrument cardiac patients learning needs inventory (CPLNI) was assessed for validity and reliability by four cardiology doctors, two critical care nurses, and one patient with a history of myocardial infarction (stage one of the study). The instrument was not modified as a result of non modified CPLNI used in the pilot study which consisted of thirty seven items of information grouped into eight clusters (constructs). The aim of the study was to describe and compare intensive care nurses and patients perceptions of information needs of acute myocardial infarction patients at a public sector tertiary hospital in Johannesburg. The study has made recommendations for clinical practice and education of intensive care nurses. A quantitative, non experimental, descriptive, prospective two-part design using questionnaires was utilized in this study to find out what information out of that commonly given following myocardial infarction, patients and critical care nurses rated as being most and least important (stage two). These results were then compared with the results obtained from patients and nurses who were given the same instrument to complete. Seventy six subjects were recruited. Results indicated that some similarities existed between the patients and the nurses in terms of what they perceived as the most and least important clusters/constructs of information. The item one (what to do if I get chest pain)was ranked first with a frequency of seventy five(98,68%). The scores for some informational clusters included on the instrument were significantly different between the patient and the nurses groups (p<0.05). The constructs miscellaneous (p=0.0054), physical activity (p=0.0022) and symptom management (p=0.0284) were statistically significant. These findings and others are discussed, and recommendations are made for improving the information given on post myocardial infarction,
