Stöd och omhändertagande vid suicid : En kvalitativ litteraturstudie om efterlevandes erfarenheter / Support and care after suicide : A qualitative literature study concerning experiences after bereavement

Background: Suicide has a negative impact on those bereaved by suicide. Early intervention has been shown to be beneficial. However, there is limited knowledge as to what specific kind of intervention should be offered in the immediate aftermath. Aim: To explore the experiences and demands of the suicide bereaved in the immediate aftermath of the loss of a relative due to suicide. Method: Literature study with thematic analysis. Results: In the immediate aftermath of suicide, the bereaved have a need to be acknowledged, to be able tom normalise emotions, get help in mobilizing their informal support networks, and be allowed to express anger. Furthermore, the bereaved need to be regarded and treated as in need of help and to be offered continued help without having to actively ask for and seek out help. Finally, the bereaved need information, both orally and in writing. They need psychoeducation regarding the grief process as well as information about practicalities and how to contact the health system. Conclusions: The suicide bereaved have an extensive and multifaceted need for help, support and information during the immediate aftermath of the loss of a relative due to suicide. Given the results of this study, there is reason to critically examine the current lack of routines for a structured care of the bereaved in emergency and prehospital care. / Bakgrund: Efterlevandes hälsa påverkas menligt vid suicid. Tidiga insatser till stöd för de efterlevande kan påverka förloppet, men kunskapsläget är begränsat vad gäller vilka specifika insatser som bör erbjudas i direkt anslutning till suicidiet. Syfte: Att belysa efterlevandes erfarenheter och vilket stöd de efterfrågar i direkt anslutning till förlusten av en anhörig i fullbordat suicid. Metod: Litteraturstudie med tematisk analys. Resultat: Under den första perioden efter en förlust av närstående i fullbordat suicid har efterlevande behov av att bli uppmärksammade, att få hjälp att normalisera känslor, hjälp att mobilisera det egna stödnätverket och att bli lyssnade på och få ge uttryck för sin ilska. Vidare har de efterlevande behov av att bli aktivt uppsökta, att bli betraktade och bemötta som hjälpbehövande och att bli erbjudna fortsatt hjälp utan att själva behöva be om det. Avslutningsvis har de efterlevande behov av information, såväl muntligen som i skrift. De har behov av psykoedukation beträffande sorgeprocessen men även information kring praktikaliteter och kontaktvägar i vården. Slutsatser: Efterlevande har ett stort och mångfacetterat behov av hjälp, stöd och information på flera olika plan under den första tiden efter en förlust av närstående i fullbordat suicid. Givet denna studies resultat finns anledning att kritiskt granska den rådande bristen på rutiner för ett strukturerat omhändertagande av efterlevande i akut- och prehospital vård.

O suicídio é um fenômeno complexo que constitui um grave problema de saúde pública, mobilizando estudos e programas de prevenção pelo mundo. O tabu impede que o suicídio seja tratado abertamente pela sociedade, ocasionando consequências negativas. O aumento da consciência é uma das maneiras de diminuí-lo. Partindo de grupos focais com sobreviventes enlutados pelo suicídio, objetivou-se compreender como o tabu, o estigma, a comunicação sobre o suicídio e a conscientização podem colaborar ou prejudicar a prevenção e a posvenção; com base nesses aspectos propomos procedimentos que possam aumentar a consciência sobre o assunto. Essa pesquisa, a partir da Teoria Fundamentada nos Dados (TFD), apresenta reflexões e sugestões de atuação para a ampliação da consciência da problemática do suicídio. Por meio da análise dos dados, três grandes temas foram encontrados: a compreensão (condição), a conscientização (interação) e a ação (consequência) que integram a ideia central de que o suicídio deve ser um problema de todos. O aumento da consciência pode gerar aumento da demanda por atendimento qualificado, sendo que é tarefa essencial encontrar locais acessíveis e habilitados para atender pessoas com ideação, tentativa de suicídio e familiares enlutados. Aumentar a consciência pública não é só prevenir suicídio, mas também promover mudança de atitude; para muitos, o suicídio continua sendo ficção até acontecer. Campanhas constituem uma das várias formas de atuação, mas devem ter objetivos claros. O aumento da conscientização (awareness) juntamente com o desenvolvimento da competência (suicide literacy) e a promoção do diálogo compõem os pilares necessários para a tríade da mudança, que deve ser multifatorial, multissetorial e multidisciplinar. Foram apresentadas propostas envolvendo os 6Cs: Conscientização; Campanhas; Capacitação; Competência; Conversa e Conexão e as sugestões de intervenção foram separadas em: a) Conteúdo; b) Veículos para comunicação; c) Como; d) Quando; e) Por quem; f) Para quem; g) Provedores ou interessados; h) Desafios; i) Facilitadores; j) Outras ações. Essas propostas devem ser culturalmente adaptadas, planejadas e integradas com um plano abrangente de prevenção, sendo avaliadas por indicadores, além das taxas de mortalidade. Os colaboradores evidenciaram que ainda há muito tabu relacionado ao assunto, o que pode tornar-se um impeditivo para o cuidado e um multiplicador de estigmas e de preconceito, que estão em concordância com pesquisas internacionais sobre o tema. Além das propostas supracitadas, também sugerimos a utilização das caixas de medicamento como veículo para o aumento de consciência, o desenvolvimento dos grupos de apoio, o empoderamento das pessoas com a experiência vivida (lived experience), a criação de uma Associação Brasileira de Sobreviventes do Suicídio e de uma revista especializada no tema. Identificamos que aumentar a comunicação responsável, que faça sentido para as pessoas, tem importância fundamental para a sociedade e para aqueles que foram tocados por essa tragédia. The taboo prevents suicide from being openly treated by society, leading to negative consequences. Raising awareness is one way to reduce it. Starting from focus groups with suicide-bereaved survivors, the aim was to understand how taboo, stigma, suicide communication and awareness can collaborate or impair prevention and postvention; based on these aspects we propose procedures that can increase awareness on the subject. This research, based on the Grounded Theory (GT), presents reflections and suggestions for action to broaden the awareness of the problematic of suicide. Through the analysis of the data, three main themes were found: understanding (condition), awareness (interaction) and action (consequence) that integrate the central idea that suicide is everyone business. Increased awareness can generate increased demand for qualified care, and it is essential to find accessible and qualified places to assist people with ideation, suicide attempt and suicide loss survivors. Increasing public awareness is not only preventing suicide, but also promoting attitude change; for many, suicide remains fiction until it happens. Campaigns are one of several forms of action, but they must have clear objectives. Increasing awareness along with the development of the suicide literacy and the promotion of dialogue constitute the necessary pillars for the \"triad of change\", which must be multifactorial, multisectoral and multidisciplinary. Proposals were presented involving the 6Cs: Consciousness; Campaigns; Coaching; Competency; Conversation and Connectedness and the intervention suggestions were separated into: a) Content; b) Vehicles for communication; c) How; d) When; e) By whom; f) For whom; g) Providers or interested parties; h) Challenges; i) Facilitators; j) Other actions. These proposals should be culturally adapted, planned and integrated with a comprehensive prevention plan and evaluated by indicators, in addition to mortality rates. The collaborators showed that there is still a lot of taboo related to the subject, which can become an impediment to care and a multiplier of stigmas and prejudice, which are in agreement with international researches on the subject. In addition to the aforementioned proposals, we also suggest the use of medicine boxes as a vehicle for raising awareness, developing support groups, empowering people with lived experience, creating a Brazilian Suicide Survivors Association and a specialized journal in the subject. We have identified that increasing responsible communication, which makes sense to people, is of fundamental importance to society and to those who have been touched by this tragedy. The moment society understands and engages in changing this phenomenon, new strategies of prevention and of postvention can be developed

Utah Mental Health Professionals' Recommendations for Working with Youth at Risk for Suicide

Suicide has immediate and long-term negative effects on surviving family members and adverse effects in the school and community. Suicide rates for children and adolescents have increased in the United States since 2009. As part of suicide prevention efforts, information about risk factors and warning signs are typically addressed in schools, private organizations, and communities. In addition to academic literature for professional audiences, various therapeutic resources are available to assist those who grieve the suicide of a loved one. In particular, practical and easy-to-implement interventions are needed by those who offer support to suicidal individuals and survivors of suicide. Practical resources are needed to address suicide prevention, intervention, and postvention needs. This study is based on a questionnaire that was given to 250 registered individuals at an annual state suicide prevention training conference. Of those attending, 60 (24%) completed the questionnaire. Participants who did not have experience working with suicidal youth did not participate, so the actual participation rate was influenced by this factor. Questions explored the recommendations of mental health professionals (N=60) who worked with (a) suicidal youth, (b) siblings of youth who completed suicide, and/or (c) youth whose parent completed suicide. More specifically this study investigated specific strategies and resources for working with these three specific groups of survivors. Findings from this study indicated that mental health professionals recommend a variety of suicide prevention resources and strategies such as implementing evidenced-based prevention programs, accessing community resources, offering individual and group counseling for survivors, involving or creating support systems, and listening to the affected youth. Also, recommended therapeutic approaches should include an action plan where students are able to receive appropriate mental health services. Based on this study, mental health professionals may be more effective as they acquaint themselves with available resources such as counselors, school psychologists, and community services to comprehensively care for struggling individuals. Mental health professionals, educators, and staff members should obtain relevant information and utilize effective intervention models in order to better address the prevention, intervention, and postvention needs of surviving individuals.Recommendations are made for future research in identifying the combination of resources that are most helpful. Recommendations are also made regarding specific content and training strategies to more effectively prepare and equip professionals to engage more fully in effective and supportive suicide prevention efforts.

[pt] Esta pesquisa teve como objetivo geral investigar a experiência de pais enlutados por suicídio. Para tanto, foram realizadas entrevistas semiestrutadas com 3 pais e 4 mães que perderam um(a) filho(a) adulto por meio do suicídio. Nesse sentido, buscamos investigar as características específicas do processo de luto parental por suicídio; compreender como os pais vivenciam os rituais no processo de luto por suicídio do filho(a); e compreender a percepção dos pais a respeito dos suportes recebidos após a morte do filho(a). Os dados coletados foram submetidos à análise de conteúdo e emergiram cinco categorias: (1) Respostas para o suicídio; (2) Meu/ minha filho(a) para além do suicida; (3) Recursos de enfrentamento no processo de luto; (4) Deslocamento dos cuidados parentais e (5) Busca por sentido e transformações a partir da morte do filho (a). Constatou-se que essa perda é acompanhada por estigmas sobre o ato suicida em si e sobre os pais sobreviventes, foram identificados busca por respostas para a morte do filho (a), sentimentos de culpa, desejo de espaços para falar livremente sobre o(a) filho(a), desenvolvimento de recursos de suporte e transformações e busca por sentido como elementos presentes no processo de luto desses pais. Conclui-se que facilitar o acesso a informações sobre suicídio, sobre luto, e proporcinar espaços seguros e de cuidado para os pais enlutados são facilitadores para a elaboração durante o processo de luto de sobreviventes. / [en] This research aimed to investigate the experience of parents bereaved by suicide. Therefore, semi-structured interviews were carried out with 3 fathers and 4 mothers who lost an adult son or daugther through suicide. We seached to investigate the specific characteristics of the process of parental mourning for suicide; understand how parents experience the rituals in the process of mourning their child s suicide; and understand the perception of parents about the support received after the child s death. The collected data was analyzed thur content analysis and five categories emerged: (1) Answers to suicide; (2) My child beyond the suicide; (3) Coping resources in the grieving process; (4) Displacement of parental care and (5) Search for meaning and perceptions about the changes after the child s death. It was found that this loss is accompanied not only by the stigma of the death of a child, but also by the suicidal act. Therefore, the search for answers, feelings of guilt, desire for safe spaces to talk freely about their child, development of support resources, and transformations and the search for meaning are penetrated by these characteristics. It is concluded that facilitating access to information about suicide and about grief, as well as providing safe and caring spaces for these bereaved are important aspects in the grieving process of survivors.

Suicide postvention plans: Are schools prepared to adjust to the new normal

This project involves the review of suicide postvention plans across school districts in California. The goal is to identify which elements are meet the recommendation, address the recommendations or do not include the recommendations for best practices in suicide postvention. This has been done by analyzing the plans across five components: preparation, identification and support for immediate needs, triage, identification and support for long term needs and a return to a new normal. Consideration was given to the format in which the plans were developed: websites, school safety plans, emergency operations plans, crisis response plans and suicide response plans. Through analyzing the plans, this research identifies the lack of preparation of schools to engage in postvention activities after a suicide.
