Exploratory study of the factors that influence nutrition interventions in the United Arab Emirates' healthcare system

Non-communicable diseases are on the increase worldwide, causing more than 36 million deaths each year. Evidence of the link between the role of nutrition and reducing non-communicable diseases is predominant in the literature. The factors influencing intervention strategies/policies and activities, however, need attention. AIM: The study aims to examine the factors that influence nutrition interventions within the United Arab Emirates’ healthcare system. METHOD: This research adapts an interdisciplinary approach where a triangulation mixed methodology is applied. Both qualitative and quantitative methods are used, through the analysis of ten interviews with policy makers, four case studies and 161 questionnaires. Furthermore, the research framework, which emerged from the literature search and qualitative analysis, is tested and validated by rigorous quantitative analysis using SPSS. The statistical analysis, using factor analysis, MANCOVA and ranking analysis aims to provide solid support for the resulting factors. MAIN FINDING: The study identifies five factors that influence nutrition interventions in a healthcare system, and could enhance the effectiveness of nutrition interventions. The factors are 1) quality and processes, 2) training and use of technology, 3) senior management involvement and responsibility, 4) patient diversity, and 5) multidisciplinary teams. CONCLUSION: This study contributes to the emerging literature on management in nutrition interventions and the theory and importance of preventative measures in relation to nutrition. This study provides a roadmap for policy makers to adopt in order to enhance the role of nutrition interventions in healthcare settings.

Sexual harassment of academic staff at higher education institutions in South Africa / Pierre André Joubert.

The efforts to create an equal non-discriminatory South African society should also manifest in the workplace and, more specifically, in the academic arena. Academics are regarded as the leaders of society and the shapers of the future of a country. Their conduct should be of the highest ethical and moral standards, and no form of discrimination should be allowed by or against them. In terms of the Employment Equity Act, sexual harassment is a form of unfair discrimination and carries a substantial penalty should an employer be found guilty of vicarious liability. The purpose of this study was to determine the perceived incidence of sexual harassment of academic staff at higher education institutions in South Africa, as well as their awareness of the policies dealing with sexual harassment. The sufficiency of the grievance procedures designed to deal with complaints of sexual harassment was also evaluated. A cross-sectional survey design was used to reach the research objectives. The Sexual Harassment Questionnaire (SHQ) was randomly distributed amongst a sample of 710 academic staff members from 10 higher education institutions in South Africa. A response rate of 22,8 percent (n = 162) was achieved. The statistical analysis was carried out using the SPSS 15.0 program (SPSS 2007), a program that is used to conduct statistical analysis regarding reliability and validity of the measuring instruments, descriptive statistics, /-tests, analysis of variance, correlation coefficients and multiple regression analysis. Article 1 focuses on the perceived incidence of sexual harassment. In this article, five categories of sexual harassment were used as indicators of the incidence thereof, namely verbal, non-verbal, physical, gender and quid pro quo harassment. A statistically significant correlation coefficient with a large effect was found between verbal and non-verbal harassment. A practically significant correlation of a medium effect was also found between physical, verbal, non-verbal and quid pro quo harassment and sexism, as well as between the control item of sexual harassment and physical, verbal, non-verbal and quid pro quo harassment. Analyses of variance were performed on the different demographic groups using various variables and the findings indicate no practically significant effect of gender, age, population group or years of service on sexual harassment. In Article 2, the awareness of sexual harassment policies and procedures were determined. Various aspects of policies were investigated, such as content, development, types and implementation. The results show that despite indications that sexual harassment policies do exist and that they are regarded as effective tools in addressing sexual harassment, the implementation of such policies is not effective. In addition, few academic staff members receive training/guidance on the utilisation of these policies. Significant correlation coefficients were found between the elements of an effective policy and between population groups and some of the elements. Article 3 reports on findings regarding the sufficiency of grievance procedures in dealing with complaints of sexual harassment. The results show a positive correlation between confidence in the grievance procedure, the amount of attention that supervisors pay to grievances, regular feedback to employees regarding the progress of grievances, willingness of supervisors to take decisions, the amount of confidence in supervisors and the effectiveness of the procedure. Utmanande beteende kan ofta innebära aktivitetsinskränkningar för individer som lider av beteendet men även för omgivningen. Detta innebär färre möjligheter till aktivitet och delaktighet och riskerar att gå ut över livskvalitén. Idag saknas ett fokus för att utveckla arbetsterapeutiska metoder inom området och forskningen som finns är begränsad. Syftet med examensarbetet var att visa vilka arbetsterapeutiska metoder som beskrivs i litteraturen för att minska utmanande beteende. Studiedesignen som användes var scoping review. Databaserna som användes var AMED, CINAHL, MEDLINE och PsycINFO. Inklusionskriterier för artiklarna var att de var tillgängliga i fulltext, skrivna på engelska och publicerade mellan 2003 och 2018. Sökning resulterade i 124 träffar varav 13 var dubbletter. Resultatet inkluderade 20 artiklar som motsvarade syftet varav 13 var publicerade i USA. Resultatet visade en stor bredd av metoder som delades in i fyra nyckelområden: aktivitetsbaserade metoder, miljöbaserade metoder, sensoriska metoder samt stöd och strategier. Detta är ett relativt outforskat område där det finns stora möjligheter för arbetsterapeuter att bidra med sin kompetens. / Challenging behaviour often results in occupational limitations for the individual that suffer from the behaviour, but also for their surroundings. This entails reduced possibilities to activity and participation which will affect their quality of life. There is a lack of focus on developing occupational therapeutic methods within this area today, also the current research within the field is limited. The aim of this bachelor thesis was to explore occupational therapeutic methods described in the literature to reduce challenging behaviour. Scoping review was used as study design. In AMED, CINAHL, MEDLINE and Psycinfo were 124 articles found, 13 of these were doublets. The result included 20 articles corresponding to the aim, 13 of these were published in USA. and 20 articles which corresponded to the aim was included in the results. All included articles were available in full text, written in english and published between 2003- 2018. A wide range of methods were shown in the results. These were categorized into four key issues: activity based methods, environmental methods, sensory methods and support and strategies. This field is relatively unexplored and there are large possibilities for occupational therapists to contribute with competence.

Paths to Tier 1 Genomics Implementation: A Survey of Chronic Disease Directors

Although evidence is currently available for population-based genetic screening and testing of individuals and their family members for certain hereditary chronic disease conditions (Tier 1), few states have integrated these genomic applications into chronic disease prevention programs. State and territorial chronic disease directors (CDDs) could provide the leadership needed to deliver these applications in more states. The purpose of this study was to determine whether an association exists between current chronic disease genomics funding or specific state genomic activities and the level of knowledge and interests in genomics by these directors. Rogers's diffusion of innovations (DIT) theory was used to explain the current climate of state chronic disease genomics and the need for an innovation champion to promote these evidence-based applications both in and out of the state health departments. A nonexperimental, cross-sectional, correlational survey of CDDs (N = 58) was performed using the Chronic Disease Director's Survey and results were analyzed using chi-square, independent t test, ANOVA, logistic regression, and Pearson's correlation coefficient. Results showed CDDs knowledge of genomics is unrelated to current state funding; however, CDD knowledge and interest in genomics was associated with inclusion of genetics in cancer control and cardiovascular health action plans, Tier 1 condition education, privacy and nondiscrimination laws, Behavioral Risk Factor Surveillance System (BRFSS) genomics questions, and frequent collaborations with outside entities. These results provide clear ideas to increase CDDs knowledge and interest in chronic disease genomics and potentially impact Tier 1 genomics implementation in more states.

The purpose of this study is to describe and analyse how school and social service in the city of Malmö cooperate regarding children at risk and what they do in order to meet their needs. We also aim to explore the view and knowledge of the personnel and their experience of the guardians’ impact during collaboration. The empirical data was collected by personal interviews with personnel from the school and social service. The results have been analysed from a theory of organizational culture and symbolic interaction.The study shows that the organizations have various forms of cooperation, whereof the majority occur in individual cases, and that preventative work is not a priority due to reorganisations and changes in society. One of the central aspects regarding collaboration is intervention in an early stage to meet the childrens needs. We question if this could be done if preventative work would be a low priority. Both parties experiencing frustration from the personnel of the other organization, which mainly occurs from lack of trust and knowledge about the working situation of the other. The personnel from school and social service consider themselves to have an understanding for the personnel of the other organization. However, this is not a comprehension, which the personnel are experiencing from the personnel of the other organization. The view of the personnel between school and social service differs, which could be explained by “language differences” seen from a perspective of symbolic interaction. The differences might decrease and result in a better understanding between the parties if preventative work were to be prioritized for creating networks and strengthen their relations. The results also show that the guardians have a significant impact concerning the organizations to be able to cooperate in individual cases. This is because the confidentiality needs to be removed in order to enable cooperation between the parties. One conclusion based on the results is that visions and guidelines are not conformed in what is possible to integrate in the daily practical work of the personnel. Furthermore, several political decisions have to be made in order to give the personnel in school and social service adequate conditions for cooperation.

Bakgrund Trycksår definieras som en lokal skada i huden och/eller i underliggande vävnad som uppstår till följd av ett kraftigt eller långvarigt tryck med eller utan kombination av skjuv. Skadan har en negativ inverkan på personens livskvalité, såväl fysisk, psykiskt och socialt. Trots att riskfaktorer och förebyggande åtgärder är väl kända så minskar inte prevalensen av trycksår. Sjuksköterskan är ansvarig för den trycksårsförebyggande omvårdnaden. För att säkerställa att omvårdnaden som ges är patientsäker och av god kvalité ansvarar sjuksköterskan för att ordinera evidensbaserade omvårdnadsåtgärder anpassade efter patientens behov och förutsättningar. Syfte Syftet var att belysa faktorer som påverkar den trycksårsförebyggande omvårdnaden för äldre personer inom slutenvården. Metod En icke-systematisk litteraturöversikt genomfördes för att besvara studiens syfte. Från litteratursökningar i databaserna PubMed och CINAHL framkom 19 vetenskapliga artiklar med både kvantitativ samt kvalitativ metod. Kvalitén av artiklarna granskades enligt Sophiahemmet Högskolas bedömningsunderlag. Genom en integrerad dataanalys formulerades tre huvudkategorier med tillhörande subkategorier. Resultat Från litteraturöversiktens resultat framgick det att trycksårsförebyggande omvårdnad för äldre inom slutenvården utfördes i varierande grad. Faktorer som påverkade utvecklingen samt uppkomsten av trycksår inkluderade sjuksköterskans utbildning, patientens medverkan, utförandet av förebyggande åtgärder samt användningen av riskbedömningar, hjälpmedel och dokumentation av trycksårsarbetet. Slutsats Genom att utföra trycksårsförebyggande omvårdnad och ta i beaktande de faktorer som kan påverka omvårdnaden, framkom det att antalet trycksår minskade. För att uppnå en lyckad trycksårsprevention krävs evidensbaserad kunskap och utbildning hos de kliniskt verksamma sjuksköterskorna. Det krävs även ett stöd från verksamheten för att minska sjuksköterskornas arbetsbelastning och motverka tidsbristen för omvårdnaden. / Background Pressure ulcers are defined by a local damage in the skin and/or underlying tissue that occurs as a result of heavy and/or prolonged pressure with or without the combination of sheer. The damage has a negative impact on the person's quality of life both physically, mentally and socially. Despite riskfactors and preventative interventions being well known, the prevalence of pressure ulcers does not decrease. The nurse is responsible for the nursing care given to prevent pressure ulcers. To ensure that the care provided is patient-safe and of good quality, the nurse is responsible for prescribing evidence-based interventions according to the patients’ needs and condition. Aim The aim of this literature review was to shed light on factors affecting the pressure ulcer prevention care for older people in inpatient care. Method A non-systematic literature review was conducted. Searches in the databases PubMed and CINAHL resulted in 19 scientific articles with both quantitative and qualitative design. The quality of the articles was assessed according to Sophiahemmet university’s evaluation basis. Through an integrated data analysis, three main categories were formulated with associated subcategories. Results The results showed that the pressure ulcer prevention care for the older people in inpatient care was carried out in varying degrees. Factors that affected the development and onset of pressure ulcers included the nurse’s education, patients’ participation, the performance of preventative measures and the use of risk assessments, aids and documentation of the pressure ulcer care. Conclusions By performing pressure ulcer prevention care and considering the factors that can affect the care, it was found that the number of pressure ulcers decreased. To achieve successful pressure ulcer prevention, evidence-based knowledge and education of the clinically active nurses is required. Support from the organization is required to reduce the nurses workload and counteract the lack of time for nursing.

Electronic Strategies to Enhance Venous Thromboprophylaxis in Hospitalized Medical Patients: A Randomized Controlled Trial

<p>Venous thromboembolism (VTE), which encompasses deep vein thrombosis (DVT) and pulmonary embolism (PE), is the most preventable cause of death in hospitalized patients. Due to its high mortality, morbidity, and cost, health care providers are obligated to not only effectively diagnose and treat VTE, but also to prevent it if possible. This has been reinforced by a number of national and international quality initiatives to prevent hospital-acquired VTE. Despite the existence of well-accepted clinical practice guidelines on VTE prophylaxis, 1 in 3 hospitalized medical patients receives an inappropriate VTE prophylaxis strategy. Both underuse of prophylaxis in patients with VTE risk, and overuse of prophylaxis in patients without VTE risk are problems. The use of inappropriate VTE prophylaxis strategies is likely due to the complexity and heterogeneity of hospitalized medical patients, and the difficulty of applying “one size fits all” practice guidelines to this group. Institution-wide knowledge translation strategies are required to close the gap between evidence and practice, and promote evidence-based VTE prophylaxis strategies in hospitalized medical patients. The objective of this thesis is to design a cluster randomized controlled trial to determine if a standardized electronic order set, with an embedded computerized decision support system and audit and feedback component, affects the use of appropriate VTE prophylaxis in hospitalized medical patients. The unit of randomization in this study is the hospital, which serves as the cluster. The unit of observation in this study is the individual patient. The primary outcome of this study is the proportion of in-hospital days during which appropriate VTE prophylaxis is administered, in intervention versus control hospitals. Secondary outcomes are the rates of hospital-acquired VTE, major bleeding and mortality, in intervention versus control hospitals. Design, analytic and ethical challenges unique to cluster randomized trials will also be discussed. Strategies to overcome them in this trial will be presented.</p> / Master of Science (MSc)
