Thesis (M Med (Psychiatry))--University of Limpopo (Medunsa Campus), 2010. / The aim of the study was to establish a profile of psychiatric illnesses that are encountered during consultation-liaison (C-L) psychiatry and to describe interventions undertaken. The objective was to identify referral patterns, establish the referral rate, and to profile the diagnoses & describe the actions taken by the consulting doctor. This was a retrospective cross-sectional descriptive study conducted on all inpatients that were referred to psychiatry from other disciplines in the hospital for the 12 month period January to December 2009. The study was conducted at Dr George Mukhari Hospital, which is a tertiary psychiatric unit which caters for mental health care users in the Garankuwa , Soshanguve and Mabopane region. The study found that the referral rate to psychiatric C-L services was lower than at other institutions. Amongst the referred patients, there was a high incidence of organic brain syndrome diagnoses, but a low rate of depression. The study proposed further investigation into the reasons behind the low rate of referral and low rate of depression, so as to improve C-L service delivery in the hospital.

Realizou-se um estudo retrospectivo e descritivo da população de pacientes internados na Unidade de Internação Psiquiátrica do Hospital de Clínicas da Unicamp que foram a óbito no período de 18 anos. Objetivo: Descrever o perfil sócio demográfico e clínico dos casos de óbitos ocorridos na Unidade de internação Psiquiátrica do Hospital de Clinicas da Unicamp, comparando-os com uma amostra dos que receberam alta hospitalar. Método: Foi realizada uma análise descritiva de todos os casos de óbito num período de 18 anos, e uma comparação entre esses casos e um grupo sorteado de casos que receberam alta hospitalar. Resultados: Verificou-se que os pacientes que foram a óbito tinham o seguinte perfil: masculinos (58%), média de idade de 47,2 (DP=14,6) anos, procedentes da Região Metropolitana de Campinas (92%). Os diagnósticos psiquiátricos mais freqüentes no grupo de óbito foram de transtornos de humor e transtornos decorrentes do uso de álcool e drogas, e as causas de óbitos mais freqüentes foram doenças cardio-respiratórias (54%). Conclusões: Esses resultados sugerem que pacientes com transtornos mentais graves parecem morrer mais cedo que a população da Unidade Psiquiátrica do Hospital de Clínicas da Unicamp e a concentrarem-se nos grupo masculino, com transtornos do humor e transtornos relacionados a álcool e drogas / Abstract: Introduction: Current studies show that individuals with severe mental illness have lower life expectancy when compared to the general population. This is a retrospective and descriptive study of the inpatient population who died at the Psychiatric Unit of the University Hospital of Unicamp in an 18-year period. Objective: Describe the sociodemographic and clinical profile of the deceased patients hospitalized at the Psychiatric Unit of the University Hospital of Unicamp, comparing them to a sample of those who were discharged from the hospital. Method: A descriptive analysis of mortality and a comparison between these cases and a randomly allocated group of patients among those who were discharged. Results: It was found that the patients who died had the following profile: males (58%), mean age 47.2 (SD=14.6) years, single (54%), from Campinas great metropolitan area (92%). The most frequent psychiatric diagnoses among the patients who died were affective and alcohol/drug disorders, while the most frequent cause of death were cardiorespiratory diseases (54%). Conclusions: These outcomes suggest that patients with severe mental disorders tend to die earlier than the population from Psychiatric Unit of the University Hospital of Unicamp. Patienters erfarenheter av att vårdas på slutna psykiatriska avdelningar : En litteraturstudie / Patien´s experience of inpatient care in closed pysychiatric wards : A literature study

Bakgrund: Varje år drabbas 38,2% av människorna i Europa av någon form av psykisk ohälsa. De vanligaste tillstånden är depression och ångestsyndrom. Den vanligaste anledningen i Sverige till sluten vård är psykisk ohälsa i åldern 15–44 år. Syfte: Syftet var att beskriva erfarenheter av att bli vårdad på slutna psykiatriska avdelningar bland personer med psykisk ohälsa. Metod: En litteraturstudie genomfördes, baserad på åtta kvalitativa studier. Studierna återfanns i två databaser, APA PsycInfo och Cinahl. Analysen gjordes med inspiration av Fribergs femstegsmodell. Resultat: Analysen resulterade i fyra huvudkategorier och sexton underkategorier. Huvudkategorierna var:’ Att inte vara sedd skapar ett utanförskap’, ’Brister i vårdstrukturen skapar lidande’, Att bli stigmatiserad’ och ’Att uppleva ett ljus i mörkret’. Konklusion: Patienter inom psykiatrisk slutenvård upplever ett stort lidande, som kan förklaras genom att det finns brister i deras vård. Vårdpersonal inom psykiatrisk slutenvård är i behov av professionell kunskap om psykisk ohälsa och psykiatrisk vård. Vidare forskning bör genomföras om hur den psykiatriska vården kan förbättras för personer med psykisk ohälsa, då detta ses vara bristande i den slutna psykiatriska vården. / Background: Every year, 38.2% of people in Europe suffer from some form of mental illness. The most common conditions are depression and anxiety disorders. The most common reason in Sweden for inpatient care is mental illness at the age of 15–44. Aim: To describe experiences of being cared for in closed psychiatric wards among people with mental illness. Methods: A literature study was conducted, based on eight qualitative studies. The studies were found in two databases, APA PsycInfo and Cinahl. The analysis was inspired by Friberg's five-step model. Results: The analysis resulted in four main categories and sixteen subcategories. The main categories were: ‘Not being seen creates an exclusion’, ‘Deficiencies in the care structure creates suffering’, ‘Being stigmatized’ and ‘Experiencing a light in the dark’. Conclusion: Patients in psychiatric inpatient care experience a great deal of suffering, which can be explained by the fact that there are shortcomings in their care. Nursing staff in psychiatric inpatient care are in need of professional knowledge about mental illness and psychiatric care. Further research should be carried out on how psychiatric care can be improved for people with mental illness, then this have been seen deficient in the inpatient psychiatric care.

A psychiatric service at a community hospital

The purpose of this dissertation is to evaluate an attempt to use the facilities of a general hospital with minimal psychiatric cover to deal with acute psychiatric conditions. This is in keeping with current psychiatric opinion in South Africa as expressed by Dr. C.W. Allwood and Dr G.A.D. Hart. This dissertation will examine the needs structure, difficulties and results of the provision of a service dealing with acute psychiatric disorders in a peripheral General Hospital and make suqqestions should similar satellite facilities be established at other peripheral hospitals. Although in principle. the concept of part-time specialist cover at a peripheral general hospital is not new in medical. surgical and gynaecological departments. the previous involvement of private psychiatrists within the teaching department has been in the form of out-patient department sessions only at academic and state psychiatric hospitals as is practiced in Cape Town. This project however allowed for the evaluation of emergency cases with subsequent referral to appropriate treatment centres if needed from a peripheral hospital. A consultation-liaison service within the general wards and the opportunity to conduct seminars, lectures and in-house training for all level of professional staff were also encompassed.

Manliga sjuksköterskors motiv att söka sig till och arbeta inom sluten psykiatri samt akutmottagning : En kvalitativ intervjustudie

Könsroller i samhället påverkar män och kvinnors val av arbetsområde inom vården. Enligt statistik från Socialstyrelsen väljer manliga sjuksköterskor främst att specialistutbilda sig inom akutsjukvård samt psykiatri, medan vissa områden har brist på män. De patienter som önskar bli omhändertagen av en manlig sjuksköterska kan i dagsläget inte få detta uppfyllt överallt. Syftet med denna studie var att beskriva manliga sjuksköterskors motiv att söka sig till och att arbeta inom sluten psykiatri samt akutmottagning. Metoden för studien var kvalitativ deskriptiv, genom semistrukturerade intervjuer vilka analyserades med kvalitativ innehållsanalys. Undersökningsgruppen valdes ut genom ändamålsenligt urval och bestod av fyra sjuksköterskor inom psykiatri samt fyra inom akutmottagning, vid ett sjukhus i norra Sverige och ett i mellersta Sverige. I resultatet framkom att ett motiv för de manliga sjuksköterskornas arbetsval var att det inom akutmottagning samt sluten psykiatri fanns bra möjligheter att göra skillnad för patienten, vilket medförde att sjuksköterskorna upplevde att de verkligen kunde göra nytta för patienten och fick känna sig behövda. Sjuksköterskorna ansåg att de hade bra utvecklingsmöjligheter inom sitt yrke och många karriärvägar. Arbetsmiljön upplevdes som tillfredsställande i förhållande till självständighet, variation och arbetstempo. Deltagarna ansåg att deras arbete var ett intressant område med roliga arbetsuppgifter. Slutsatsen var att det bland manliga sjuksköterskor inom psykiatri och akutmottagning fanns en attraktion till arbete som upplevs som intressant, självständigt, varierande, bra med utvecklingsmöjligheter och där sjuksköterskan får känna att han gör ett bra arbete. Sjuksköterskorna hade en negativ föreställning om att arbetet på somatisk vårdavdelning inte motsvarade dessa viktiga faktorer. / Gender roles in society influence the choices of men and women regarding working area in healthcare. According to statistics from the Health and Human Services Department of Sweden, (Socialstyrelsen) men tend to mainly specialize in emergency care and psychiatry, while some other specialization areas have a shortage of men. Patients who wish to be tended to by a male nurse can not get this fulfilled everywhere. The aim of this study was to describe why male nurses most commonly choose to work in psychiatric wards and emergency rooms. The method, which has been used, is a qualitative interview study using a descriptive design, which later was analyzed through qualitative content analysis. The informants were chosen using purpose selection and consisted of four nurses within the field of psychiatry, and an additional four that worked in emergency rooms, distributed on one hospital in northern Sweden, and one in the central part of the country. In the result the reasons for the choice regarding work place of the male nurses were that at the emergency rooms and psychiatric wards, the possibility of making a difference for the patient and also having time for him/her was great, which made the nurses feel that they could be of good use for the patient, and also felt needed. In the work place it was considered that the nurses had good potential for professional development. The working environment was perceived as satisfactory in relation to independence, variation and working place. The participants considered that their field of work offered an interesting work place and providing enjoyable work tasks. The conclusion drawn in this study, showed that among male nurses within the fields of psychiatry and emergency care, found an attraction to their work because the work is perceived as interesting, independent, varying, providing good opportunities for further professional development, and where the nurse gets to feel as if he is doing a good job. The nurses had a negatory conception regarding somatic care, since they thought that it did not meet these important key factors.
