Os Hospitais Gerais desenvolveram-se, ao longo da história da medicina, como ambientes em que não havia espaço para os loucos. A partir do século XX, começa a ocorrer uma mudança nesse paradigma, sobretudo nos EUA e na Europa, com a criação das UPHGs em diversos serviços por lá, e a consolidação da Interconsulta psiquiátrica como área de atuação da psiquiatria, que passa a estreitar o contato com o restante da medicina. Foi um movimento que rendeu diversos avanços tanto à psiquiatria quanto às outras áreas médicas. No Brasil, esse percurso começou na década de 1950 e ocorre de forma mais tímida. Apenas 1,1% dos Hospitais gerais contam com suporte da psiquiatria em nosso país atualmente. Contudo, houve espaço no Brasil para o desenvolvimento de projetos pioneiros nesse campo, cuja história não está registrada. A psiquiatria pernambucana foi protagonista na empreitada, inaugurando inclusive a integração da psiquiatria ao hospital geral no Brasil. Pernambuco também foi um dos pioneiros na introdução da clínica psicanalítica em hospitais gerais do serviço público. O presente trabalho pretende registrar a história dessas experiências. Para tanto, escolhemos os serviços da UFPE e do Hospital Barão de Lucena, sedes dos primeiros serviços de saúde mental em hospital geral de Pernambuco. Fez-se uma ampla pesquisa documental, além de serem entrevistados alguns dos principais atores da construção desses serviços. Apesar de difícil, a integração dos cuidados de saúde mental ao hospital geral na UFPE e no HBL conseguiu efetivar-se, a partir da psiquiatria na UFPE e da psicanálise no HBL. Foram espaços configurados e sustentados dentro de uma perspectiva de formação, para além da assistência, e acreditamos que sua longevidade tenha relação com isso. / The history of psychiatry, for the most part, is intertwined with the history of asylums. General Hospitals have developed mostly as environments in which there was no room for the insane. Since the early twentieth century, a paradigm shift takes place, specially in the US and Europe, with the creation of psychiatryc units in several General Hospitals there. It led to the consolidation of Consultation Liaison Psychiatry as an official sub-specialty of Psychiatry. Integration of Psychiatry and other medical specialties was a movement that yielded many advances in both psychiatry as other medical areas. In Brazil, this journey began in the 1950s and is more . Only 1.1% of brazilian general hospitals have psychiatric support nowadays. However, there was room in Brazil for the development of pioneering experiments in this field, whose history is not recorded. Pernambuco’s psychiatry had a key role in these experiments, inaugurating the integration of psychiatry to the general hospital in Brazil. Pernambuco was also one of the first places to have a psychoanalytic couch in public service facilities. This work aims to record the history of these pioneering experiences in the state of Pernambuco. Therefore, we chose the services of UFPE and the Hospital Barão de Lucena, headquarters of general Hospital Psychiatry in Pernambuco. An extensive literature research was done, along with statements collection from some of the major players in the construction of these services. A interface entre psiquiatria e literatura na obra de Lima Barreto / The interface between psychiatry and literature in the work of Lima Barreto

INTRODUÇÃO: As humanidades médicas têm sido cada vez mais valorizadas mundialmente. No entanto, são poucos os estudos psiquiátricos nessa área no Brasil. A obra de Lima Barreto (1881-1922) reveste-se de importância para o psiquiatra pelo fato de este escritor ter tido grande contato com o meio psiquiátrico de sua época, em virtude da doença mental de seu pai e de seu quadro de alcoolismo, que motivou duas internações no Hospício Nacional de Alienados do Rio de Janeiro. Neste trabalho, procuramos relacionar a obra de Lima Barreto ao contexto histórico da psiquiatria e à sua biografia, testando ainda a hipótese de que o autor tenha apresentado declínio cognitivo expresso na elaboração de sua última obra, em virtude do alcoolismo. MÉTODOS: A obra do autor foi pesquisada, sendo analisadas as representações de doença mental. Como referencial teórico, utilizou-se os conceitos de carnavalização e de cronotopo de Mikhail Bakhtin. Foi usado, ainda, um software (WordSmith tools 3.0) para encontrar variáveis linguísticas que pudessem ter relação com declínio cognitivo do autor. RESULTADOS: Nas primeiras obras, há tendência para caracterizar a doença mental de forma idealizada ou caricatural; nas últimas, de forma mais naturalista e objetiva. Os percursos dos personagens dos últimos romances têm tendências mais fatalistas, com menor grau de mudança psicológica dos personagens, quando comparados com os romances iniciais. Com os relatos do autor sobre sua internação, tem-se acesso a uma descrição das condições do Hospício Nacional de Alienados, de seus internos e do caráter das internações. Observou-se, também, menor diversidade lexical na última obra do autor, aferido pelo software. CONCLUSÕES: A obra de Lima Barreto é uma fonte representativa para aqueles que estudam história da psiquiatria brasileira do início do século XX e fornece elementos diversos daqueles presentes em vários estudos, principalmente ligados a ideologias críticas à psiquiatria. As diferenças nas representações da doença mental no início e no fim da obra provavelmente refletem elementos biográficos e psicológicos do autor. O achado de menor diversidade lexical presente na última obra pode ter relação com declínio cognitivo do autor. / INTRODUCTION: The medical humanities have been increasingly valued worldwide. However, there are few psychiatric studies in this area in Brazil. The work of Lima Barreto (1881-1922) is important to the psychiatrist because this writer had great contact with the psychiatric medium of his time, because of his father\'s mental illness and because of his picture of alcoholism, which led him to two hospitalizations in the National Asylum for the Insane in Rio de Janeiro. In this paper we try to relate the work of Lima Barreto to the historical context of psychiatry and his biography, testing also the hypothesis that the author had cognitive decline expressed in the preparation of his latest book, due to alcoholism. METHODS: The author\'s work was investigated, and the representations of mental illness analyzed. The theoretical framework used included Mikhail Bakhtin\'s concepts of chronotope and carnivalization. It was also used a software (WordSmith tools 3.0) to find linguistic variables that could be related to cognitive decline of the author. RESULTS: In early works, there is a tendency to characterize mental illness in an idealized or caricatured way, and in the late ones, in a more naturalistic and objective way. The paths of the characters in later novels have more fatalistic tendencies, with lower levels of psychological change of the characters, when compared to the early novels. With the reports of the author on his admission in the psychiatric hospital, we have access to a description of condition of the National Asylum for the Insane, its internals and the character of admissions. It was also observed that there was less lexical diversity in the last work of the author, as measured by the software. CONCLUSIONS: Lima Barreto\'s work is a representative source for those studying the history of psychiatry of the early twentieth century and provides different elements compared to those present in several studies, mainly linked to ideologies critical to psychiatry. The differences in the representations of mental illness at the beginning and the end of the work probably reflect biographical and psychological elements of the author. The finding of lower lexical diversity present in the latest work may be related to cognitive decline of the author.

However, there is still a great scarcity of information regarding the history of this field of medicine, which is fundamental for a better understanding of the transformations of the psychiatric medical thought in our milieu. It is essential a profound knowledge of the concepts related to the traditions, changes and permanences of the past for the adequate understanding of the specialty\'s present and the prognosis for its future. It is relevant, in order to be situated in the current days, to know psychiatry\'s history of concepts and medical practice in Brazil, by focusing the historical context, the political movements and the possible influences that are external to our culture, which have interfered, along the time, for psychiatry to present as such in the current days. The current work aimed at delineating the medical-psychiatric conception between the decades of 1920 and 1950 about the mental disorders and their treatments in the state of So Paulo, besides emphasizing its possible influences over the medical-psychiatric thought of our days. It was used the qualitative methodology, by integrating two different areas of knowledge, Psychiatry and History. It is an appropriate method to incorporating meanings and intentions to social acts, relations and structures. The data production was accomplished by the survey and analysis of historical documents produced in the decades of 1920 up to 1950 about the formation of the discipline and the cathedra of Psychiatry of the Medical School of the University of So Paulo (FMUSP), the formation of the So Paulo League of Mental Hygiene, subordinated to the Brazilian League of Mental Hygiene (LBHM), of the formation and action of the Institution of Social Assistance to Psychopaths of the State of So Paulo, of the institutional life of the Juquery Hospital, and of the conception and beginning of the activities of the Institute of Psychiatry (IPq) of the Clinical Hospital of the Medical School of the University of So Paulo. We conclude that the thought about mental health in São Paulo, within the period delimited by the study, was constituted based on great influence of other fields of knowledge determining the directions of psychiatry as a medical specialty in our milieu

Evidence of intense discussion on the rights of the mentally disturbed, curative as opposed to control measures in their treatment, and the drawbacks of congregating the afflicted in large institutions can be found as early as the middle of the 19th century. This discussion was provoked by dissemination of knowledge about the oldest community care program of all: the colony of mental patients in Gheel, Belgium. Based on document analysis of publications in the American Journal of Insanity from 1844 to 1921, this study attempts to trace how this discussion resulted in the first wave of deinstitutionalization in the American mental health care system, and the successful implementation of the alternative of hospital treatment. / ABSTRACT: My study further documents how the development of this program was inhibited by the need of psychiatry to attain professional legitimation. In its struggle to acquire public respect and occupational authority, the profession focused on somatic explanations of disease that could justify categorization of psychiatry as a branch of medical science. While this claim was not decisively supported by laboratory findings, or the ability to cure patients, psychiatry put forward genetic explanations of mental disorder. This took the profession to the extreme of the eugenics movement, and eventually positioned it as an institution of social control instead of medical authority. Having thus failed to achieve the ultimate professional legitimation in the medical field, psychiatry was exposed to a new wave of criticism in the 1960s, which led to the second wave of deinstitutionalization. History repeated itself with the same outcome. / ABSTRACT: In the absence of overall support within psychiatric circles, and a lack of appreciation of family care as a viable alternative to hospital treatment among social scientists, deinstitutionalization could not but fail again. Conclui-se que o pensamento acerca da saúde mental em São Paulo, no período delimitado pelo estudo, se constituiu a partir de grande influência de outras áreas do conhecimento, determinando os rumos da psiquiatria como especialidade médica em nosso meio / The history of psychiatry has been arising higher interest in the last years. However, there is still a great scarcity of information regarding the history of this field of medicine, which is fundamental for a better understanding of the transformations of the psychiatric medical thought in our milieu. It is essential a profound knowledge of the concepts related to the traditions, changes and permanences of the past for the adequate understanding of the specialty\'s present and the prognosis for its future. It is relevant, in order to be situated in the current days, to know psychiatry\'s history of concepts and medical practice in Brazil, by focusing the historical context, the political movements and the possible influences that are external to our culture, which have interfered, along the time, for psychiatry to present as such in the current days. The current work aimed at delineating the medical-psychiatric conception between the decades of 1920 and 1950 about the mental disorders and their treatments in the state of So Paulo, besides emphasizing its possible influences over the medical-psychiatric thought of our days. It was used the qualitative methodology, by integrating two different areas of knowledge, Psychiatry and History. It is an appropriate method to incorporating meanings and intentions to social acts, relations and structures. The data production was accomplished by the survey and analysis of historical documents produced in the decades of 1920 up to 1950 about the formation of the discipline and the cathedra of Psychiatry of the Medical School of the University of So Paulo (FMUSP), the formation of the So Paulo League of Mental Hygiene, subordinated to the Brazilian League of Mental Hygiene (LBHM), of the formation and action of the Institution of Social Assistance to Psychopaths of the State of So Paulo, of the institutional life of the Juquery Hospital, and of the conception and beginning of the activities of the Institute of Psychiatry (IPq) of the Clinical Hospital of the Medical School of the University of So Paulo. We conclude that the thought about mental health in São Paulo, within the period delimited by the study, was constituted based on great influence of other fields of knowledge determining the directions of psychiatry as a medical specialty in our milieu

Difference-makers in human affective distress: perspectives on causation and recovery gained from qualitative inquiry into lived experience

The biomedical conceptualisation of “depression” as a disease entity stems from biological science rooted in a 17th century paradigm, and is an inappropriately positivist idea which discounts individual agency, disregards social context underscored in the descriptive epidemiology, fails to accommodate inter-subjectivity and process, medicalizes suffering, and serves a managed care model of health administration. A qualitative study using in-depth interviews was undertaken to elicit perspectives from individuals who had lived experience of biomedical (pharmacological) treatment for affective distress and who self-reported having attained satisfactory recovery. Thematic analysis of interview data clustered around three main categories: (1) multifactorial conceptual understandings; (2) context pertinent to the experience of distress and recovery – including a variety of stressors – and (3) trajectories of recovery from acute distress to negotiation of ambivalence toward treatment, enlisting of supports, and reclaiming of agency. Relationships with service providers marked by trust, empathy, and hope were valued as primary difference-makers. / May 2017

A pesquisa busca analisar a concepção de menores anormais na produção científica do psiquiatra paulista Antônio Carlos Pacheco e Silva, na década de 30, no estado de São Paulo. A maior parte do material analisado encontra-se no acervo do Museu Histórico Prof. Carlos da Silva Lacaz, da FMUSP. Privilegiou-se, como recorte crítico de leitura, os escritos do psiquiatra que direta ou indiretamente discutem a infância. Porta-voz decisivo dos ideais da ciência médica do período, Pacheco e Silva teve grande importância para a institucionalização da psiquiatria no estado de São Paulo, baseando-se nos preceitos da higiene mental e da eugenia. Pode-se afirmar que o discurso sobre a anormalidade infantil, proveniente de Pacheco e Silva, estava em consonância com a preocupação política estadual de higienização da população, o que pressupunha vigiar e restringir a circulação social das crianças que, por sua condição econômica e/ou racial, não condiziam com o modelo desejado pelos higienistas. A expressão menores anormais aplicava-se às crianças de camadas econômicas mais baixas que se achavam em condições de inferioridade intelectual, moral, afetiva ou sensorial. A Escola Pacheco e Silva no Hospital do Juquery tornou-se destino institucional para parte dessas crianças / This research aims to assess the concept of abnormal underages in the scientific writings of Antônio Carlos Pacheco e Silva, an important psychiatrist and active public health policy maker at Sao Paulo State during the 1930s. Most part of the researched documentation belong to the collection of Museu Histórico Prof. Carlos da Silva Lacaz, da FMUSP. As a critical approach to this rich material, our research focused mostly Pacheco e Silvas writings related to childhood. The psychiatrist, a strong spreader of medical science ideology of that time, had a decisive role in the institutional setting of psychiatry within Sao Paulo State health system. His scientific principles concerned the ideas of mental hygiene and eugenics, and his discourse on childhood abnormality was in line with the local state political concerns of population sanitation. That implied supervision control and social restriction of children who, whether for economical or social conditions, didnt fit the desired hygienist model. Thus, the expression abnormal underages was used in order to designate children from lower economical layers who were considered to have lower intellectual, moral, affective or sensory capacities. The Pacheco e Silva school, at Hospital do Juquery, became a central institutional destination for most part of those children

We write here one of the possible histories of Adauto Botelho Psychiatric Hospital (since the 10th of May of 2011 this hospital is called Hospital Estadual de Atenção Clínica HEAC), located in Espírito Santo, Brazil. What matters here is not the accuracy of a timeline, but the flow of speeches and practices. Therefore, we have analysed 102 medical charts of Adauto Botelho Hospital and interviewed 4 hospital employees. We have researched the period from 1954 until 1990. During and after the research, we have discussed the following points: 1. The ways of referring patients to the hospital and how the police was part of this process. 2. The Hospital s Therapeutics, how moralizing they could be and the way they were mirrored on the patients bodies. 3. The struggles and resistances of people who lived and worked in the Hospital. Among the discussion of all these issues, we bring some short histories about the patients, employees and their lives in the hospital. There is no doubt that there are many other questions to discuss, but we write here about what seems to be more intense in the researched material / Não existe uma única história da loucura e da psiquiatria, nem uma única forma de contar cada história. Sabendo disso, o presente trabalho trata de uma história do Hospital Adauto Botelho (desde 10 de maio de 2010, Hospital Estadual de Atenção Clínica Heac), localizado em Cariacica município da Região Metropolitana da Grande Vitória no Espírito Santo. Trazemos essa história não a partir de uma linha do tempo que narre acontecimentos de cada época. Buscamos a fluidez dos discursos e das práticas em lugar da precisão das datas. E, sobre esses discursos e essas práticas, o próprio Hospital tem muito a apresentar, por meio dos prontuários médicos datados desde a inauguração em 1954 e de depoimentos de pessoas que lá tenham trabalhado. Foram analisados 102 prontuários e entrevistadas 4 pessoas. A pesquisa dos prontuários e a realização das entrevistas possibilitaram a discussão as seguintes questões: 1. As formas de encaminhamento dos pacientes para o hospital e o modo como a polícia se inseria nesse processo; 2. As terapêuticas do hospital, seu aspecto moralizante e o modo como elas incidiam sobre os corpos; 3. As lutas e resistências que se presentificavam no cotidiano do hospital. Todas essas questões são o tempo inteiro atravessadas por breves histórias de vidas dos pacientes ali internados e dos funcionários que narraram as vivências no hospital. Não há dúvida de que há inúmeras outras questões a serem debatidas, mas aqui falamos do que nos pareceu mais intenso no material pesquisado
